Omentin is a novel adipokine with insulin-sensitizing effects expressed predominantly in visceral fat. We investigated serum omentin levels and its mRNA expression in subcutaneous adipose tissue (SCAT) of 11 women with type 2 diabetes mellitus (T2DM), 37 obese non-diabetic women (OB) and 26 healthy lean women (C) before and after various weight loss interventions: 2-week very-low-calorie diet (VLCD), 3-month regular exercise and laparoscopic sleeve gastrectomy (LSG). At baseline, both T2DM and OB groups had decreased serum omentin concentrations compared with C group while omen tin mRNA expression in SCAT did not significantly differ among the groups. Neither VLCD nor exercise significantly affected serum omentin concentrations and its mRNA expression in SCAT of OB or T2DM group. LSG significantly increased serum omen tin levels in OB group. In contrast, omentin mRNA expression in SCAT was significantly reduced after LSG. Baseline fasting serum omentin levels in a combined group of the studied subjects (C, OB, T2DM) negatively correlated with BMI, CRP, insulin, LDL-cholesterol, triglycerides and leptin and we re positively related to HDL- cholesterol. Reduced circulating omentin levels could play a role in the etiopathogenesis of obesity and T2DM. The increase in circulating omentin levels and the decrease in omentin mRNA expression in SCAT of obese women after LSG might contribute to surgery-induced metabolic improvements and sustained reduction of body weight., M. Děložní myomy jsou nejčastějšími nádory dělohy u žen v reprodukčním věku. Ačkoli jsou děložní myomy ve většině případů asymptomatické, mohou být příčinou abnormálního děložního krvácení, bolesti, dyspareunie, těhotenských ztrát a infertility. Možnosti léčby děložních myomů zahrnují léčbu farmakologickou, chirurgickou, radiologickou a dále myolýzu či léčbu fokusovaným ultrazvukem. Volba léčebné modality by měla být u každé pacientky přísně individualizována v závislosti na řadě faktorů, jako jsou reprodukční plány pacientky, přání zachovat dělohu a přítomnost a závažnost symptomů. Optimální léčba by tedy měla být minimálně invazivní, bezpečná, fertilitu-šetřící, s dlouhodobým účinkem a minimálním rizikem rekurence., Uterine fibroids are the most common tumors of the uterus in premenopausal women. Despite being asymptomatic in the majority of cases, uterine fibroids may cause abnormal vaginal bleeding, pain, dyspareunia, pregnancy losses and infertility. The treatment options include pharmacological, surgical and radiological treatment as well as myolysis or focused ultrasound treatment. The therapeutic modality selection should be strictly individualized based on the reproductive plans of the patient, desire to preserve the uterus and the presence and severity of the symptoms. Pacienti s nezvladatelnou chronickou obstipací by měli být vyšetřeni fyziologickými testy poté, co se vyloučí strukturální a mimostřevní příčiny. Je nutné nejprve vyčerpat veškeré možnosti konzervativní léčby. Mělo by se zdůraznit, že zejména nové léky, jako jsou prucaloprid a linaclotid, se zdají být velkým krokem vpřed v léčbě pacientů s chronickou obstipací. Pokud je indikovaný chirurgický výkon, mnoho let byla operací volby subtotální kolektomie s IRA, i když segmentální resekce jsou také dobrou volbou u izolovaných megasigmoidů, sigmoidokél či rekurentních sigmoideálních volvulů. V dnešní době by se měly nejprve vyzkoušet méně invazivní postupy, jako například modulace sakrálního nervu (SNM). Pokud je neúspěšná, dá se pak uvažovat o kolektomii. Všeobecně by se pacientům s poruchou motility gastrointestinálního traktu (gastrointestinal dysmotility syndrome- GID) nemělo nabízet chirurgické řešení z důvodu očekávaných špatných výsledků. Navíc pacienti s psychiatrickou poruchou by měli být aktivně odrazováni od resekce, protože mívají horší prognózu. Pacientům musí být vysvětleno, že bolesti anebo nadýmání budou pravděpodobně přetrvávat, i když chirurgický výkon znormalizuje počet stolic. Pacienti se souvisejícími potížemi mohou profitovat ze stomie bez resekčního výkonu jakožto léčebného výkonu i diagnostického pokusu. Kolektomie není volbou léčby pro bolest anebo nadýmání. Ve většině případů se dají poruchy vyprazdňování úspěšně léčit konzervativně. Nicméně v dnešní době existuje také široký výběr chirurgických řešení. Každá metoda má své místo v armamentáriu kolorektálního chirurga, ale jejich přesná role ještě není definována. Cílem této práce je nabídnout stručný přehled, jak diagnostikovat a léčit chronickou obstipaci z pohledu kolorektálního chirurga. Chirurgická léčba chronické obstipace není rutinní a provádí se jen ve výjimečných případech. Ale nejprve je nutno si říci, že tzv. „příliš dlouhé střevo“ (dolichocolon) není nikdy indikované k chirurgickému výkonu. Cílem této práce je prezentovat stručný přehled možných mechanismů obstipace, diagnostických metod a nástrojů a různé možnosti konzervativní a chirurgické léčby. Dále je nutné si vždy pamatovat, že obstipace nemusí být jen příznakem, ale může být i jiným onemocněním!, Patients with intractable chronic constipation should be evaluated with physiological tests after structural disorders and extracolonic causes have been excluded. Conservative treatment options should be tried unstintingly. It should be pointed out that especially new drugs such as prucalopride and linaclotide seem to be a big step forward in treating patients with chronic constipation. If surgery is indicated, for many years subtotal colectomy with IRA was the treatment of choice, although segmental resections were also a good option for isolated megasigmoid, sigmoidocele or recurrent sigmoid volvulus. Nowadays, less invasive procedures like sacral nerve modulation (SNM) should be tried first. If unsuccessful, colectomy can still be considered. In general, patients with a gastrointestinal dysmotility syndrome (GID) should not be offered any surgical options because of their anticipated poor results. Moreover, patients with psychiatric disorders should be actively discouraged from resection, as they tend to have a poorer prognosis. Patients must be counseled that pain and/or bloating will likely persist even if surgery normalizes bowel frequency. Patients with associated problems may be better served by having a stoma without resection as both a therapeutic maneuver and a diagnostic trial. Colectomy is not an option for the treatment of pain and/or abdominal bloating. In most cases outlet obstruction can be treated successfully with a conservative approach. However, nowadays there are also a variety of surgical options on the market. Each technique has its special place in the armamentarium of a colorectal surgeon but its exact role is not defined yet. The aim of this article is to give a brief overview, how to diagnose and treat chronic constipation from the standpoint of a colorectal surgeon. Surgical treatment of chronic constipation is not routine and is performed only in exceptional cases. But one thing first: a “too long gut” (dolichocolon) per se is never an indication for surgery. The aim of this manuscript is to give a brief overview about possible mechanisms of constipation, diagnostic methods and tools and the various conservative and operative treatment options. Úvod: Tato práce referuje první zkušenosti s laparoskopicky asistovanou transanální totální excizí mezorekta u pacientů s karcinomem středního a dolního rekta. Metody: V období od října 2013 do května 2014 bylo na dvou pracovištích (CHK FN Brno, CHO KOC Nový Jičín) operováno 17 pacientů technikou transanální totální mezorektální excize. V souboru bylo 11 mužů a 6 žen ve věku 49–81 let. Vzdálenost dolního okraje tumoru od análního okraje byla 30–80 mm, medián 60 mm. U všech pacientů byla konstruována ručně šitá kolo-anální anastomóza. Výsledek: U všech operovaných byl dosažen negativní distální resekční okraj. Cirkumferentní resekční okraj byl pozitivní ve dvou případech. Kvalita totální mezorektální excize byla hodnocena v 8 případech jako kompletní a v 9 případech jako téměř kompletní. Dehiscence kolo-anální anastomózy byla zaznamenána ve dvou případech (12 %). Operační čas se pohyboval v rozsahu 212–375 minut (medián 280 minut). Medián pooperační hospitalizace byl 9 dnů v rozsahu 6 až 30 dnů, v případě řešení anastomotické a rané komplikace. Závěr: Na základě prvních získaných zkušeností hodnotíme výkon jako realizovatelný a bezpečný v podmínkách našich chirurgických pracovišť s uspokojivými patologickými výsledky. Přesné indikace zatím nelze jednoznačně doporučit. Předpoklad je, že k transanální totální mezorektální excizi by měli být indikováni pacienti s nízce uloženým karcinomem rekta, event. s úzkou pánví, objemným mezorektem a objemnou prostatou., Introduction: First experiences with laparoscopic low anterior resection with transanal mesorectal excision in patients with low and mid rectal cancer are referred. Methods: During a 9 month period (between October 2013 and May 2014), 17 laparoscopically assisted rectal resections with transanal total mesorectal excision, hand-sewn colo-anal anastomosis and protective ileostomy were performed at two institutions (CHK FN Brno, CHO KOC Nový Jičín). There were 11 males and 6 females, with a mean age 68 years (range 49–81 years) in our cohort. The tumor was located 30–80mm from the anal verge (median 60 mm). Results: A negative distal resection margin was achieved in all patients. The circumferential resection margin was positive in two cases. The TME quality was described as complete in 8 cases and nearly complete in 9 cases. Mean operative time was 280 min (range 212–375 min) with no intra-operative complications. Anastomotic leakage was observed in two patients (12%). Median length of postoperative hospital stay was 9 days with a range of 6–30 days, in case of anastomotic and wound complications. Conclusion: According to our first experience we evaluated this method as feasible and safe with satisfactory pathological outcomes. We cannot recommend exact indications yet.
