Background Patent ductus arteriosus (PDA) is common in very premature infants. Pharmacological closure of PDA with indomethacin, a prostaglandin inhibitor, has remained the mainstay of treatment in premature infants over the last three decades. Intravenous ibuprofen was recently shown to be as effective and to have fewer adverse reaction in preterm infants. If equally effective, then oral ibuprofen for PDA closure would have several important advantages over the intravenous route. This study was designed to assess the efficacy and safety of oral ibuprofen and intravenous ibuprofen for the early pharmacological treatment of PDA in LBW preterm infants with respiratory distress syndrome. Methods A randomized, single-blinded, controlled study was performed on premature neonates at the neonatal care unit of the University Hospital for Obstetrics and Gynecology”Koco Gliozheni”, Tirana, Albania, from January 2010 to December 2012. The study enrolled 68 preterm infants with gestational age between 28-32 weeks, birth weight ≤ 2000 g, postnatal age 48-96 h, and had echocardiographically confirmed significant PDA. The preterm infants received either intravenous or oral ibuprofen randomly as an initial dose of 10 mg/kg, followed by 5 mg/kg at 24 and 48 h. After the first dose of treatment in both groups, echocardiographic evaluation was performed, to determine the need for a second or third dose. The rate of ductal closure, adverse effects, complications, and the patient’s clinical course were recorded. Results All patients were born after 28 until 32 weeks’ gestation. 36 patients were treated with oral ibuprofen and 32 with intravenous ibuprofen in this period. After the first course of the treatment, the PDA closed in 30 (83.3%) of the patients assigned to the oral ibuprofen group versus 23 (71.8%) of those enrolled in the intravenous ibuprofen group (p = 0.355). There was no difference between treatment groups in demographics or baseline renal function. In the evaluation of renal tolerance, none of the patients had oliguria. There were no significant differences with respect to complications during the stay. Conclusions In low birth weight infants, the rate of early ductal closure with oral ibuprofen is at least as good as with the intravenous route. Osteochondritis dissecans(OCD) of the knee is identified with increasing frequency in the adolescent patient. Left untreated, OCD can cause significant impairment and restriction in physical activity and development of osteoarthritis at an early age. The diagnosis of lesions of OCD can be confirmed on plain radiographs. MRI has emerged as the gold standard to evaluate the stability of the lesion and the integrity of the overlying articular cartilage. Treatment of OCD lesions depend on the stability of the lesion. Stable lesions can be treated conservatively by physical activity modification and immobilization. Unstable lesions and stable lesions not responding to conservative measures should be treated surgically. Surgical options range from arthroscopic drilling, either transarticular or extra-articular drilling for stable lesions or salvage procedures such as autologous chondrocyte transplantation (ACT), mosaicplasty to restore joint and cartilage congruency. and H. BACKGROUND: Though mid-term survival rates of over 95% in several series have been published, there is still a paucity of related literature regarding the role of vertical stem instability in the osteointegration of fluted tapered stems. This paper presents a comprehensive and prospective assessment on short-term experiences with uncemented modular femoral stem in the treatment of defective femur during revision surgery of total hip replacement. MATERIALS AND METHODS: Clinical and radiological monitoring of 20 consecutive patients with implanted tapered fluted revision stem (Lima Corporate, Udine, Italy) was of 27 months in average (20-35 months). The average pre-operative Merle d'Aubigné and Postel method score was 6.3 points (3-10 points). The frequency of femur defects, classified according to Paprosky, was IIIA = 9 and IIIB = 11. RESULTS: During last follow-up, the Merle d'Aubigné and Postel hip score was on average 11.7 (6-16 points). Compared to post-operation radiograph, stem migration of 1.9 mm (0-11 mm) on average was found. This vertical stem migration was observed only when comparing hip radiographs immediately after surgery, and at 6 weeks post-surgery. The Paprosky IIIA defects group, presented a subsided stem by an average of 1.5 mm. In the group of Paprosky IIIB defects, the stem subsidence was on average 2.3 mm. All 20 patients in the study showed excellent osteointegration of the uncemented revision modular stem. CONCLUSIONS: This study found and excellent osteointegration of the Lima uncemented tapered fluted revision modular stem in defective femur with a cortical bone segment present in the diaphyseal isthmus area. Osteosarkom (OS) je nejčastější zhoubný nádor skeletu, představuje asi 35 % primárních kostních nádorů. Úspěšná léčba osteosarkomu vyžaduje úzkou spolupráci odborníků multidisciplinárního týmu, kam patří onkolog, chirurg, patolog a radiolog. Nejlepší péči zajišťují specializovaná centra s odpovídajícími zkušenostmi a možností komplexní péče. Léčebný režim musí zahrnovat radikální odstranění primárního nádoru, případně metastáz v kombinaci se systémovou chemoterapií. Z cytostatik je to zejména doxorubicin, vysoko-dávkovaný metotrexát, cisplatina a ifosfamid. Cílem rozdělení chemoterapie na část předoperační a pooperační je redukce nádorové masy a lepší ohraničení tumoru vůči okolním strukturám. Tím je usnadněna chirurgická radikalita s možností náhrady postižené části skeletu bez mutilujícího výkonu. Důležitá je také znalost histopatologické odpovědi na předoperační chemoterapii, která ovlivňuje intenzitu pooperační léčby. Dosud není dostatečně vyřešena léčba metastatického onemocnění a případné recidivy. Zlepšení výsledků je možno očekávat až po zavedení nových léčebných přípravků a modalit., Osteosarcoma (OS) is the most common malignant tumour of the skeleton accounting for approximately 35 % of primary bone tumours. Successful treatment of osteosarcoma requires a close cooperation between the specialists of a multidisciplinary team consisting of an oncologist, surgeon, pathologist and radiologist. The best care is provided by specialized centres with appropriate experience in which comprehensive care is available. The therapeutic regimen must include radical removal of the primary tumour and/or metastases in combination with systemic chemotherapy. The cytostatic drugs primarily used include doxorubicin, high-dose methotrexate, cisplatin and ifosfamide. The goal of dividing chemotherapy into preoperative and postoperative parts is a reduction in the tumour mass and improved delineation of the tumour from the surrounding structures. This improves the surgical radicality with the affected part of the skeleton being replaced without the need for a mutilating procedure. Also of importance is the knowledge of the histopathological response to preoperative chemotherapy which affects the intensity of postoperative treatment. The management of metastatic disease and possible recurrence has not been addressed adequately as yet. Článek popisuje případ pacienta s náhle vzniklou obstipací, bolestmi břicha a normocytární anémií. Endoskopické vyšetření horní a dolní části trávicího traktu vyloučilo organickou příčinu obtíží. Vyšetření nátěru krve a aspirátu kostní dřeně s nálezem hrubého bazofilního tečkování v erytrocytech a erytroblastech upozornilo na možnost otravy těžkými kovy. Hodnota plumbemie 8,4násobně překročila limit přípustný pro běžnou populaci. Jako zdroj intoxikace byla identifikována olovnatá glazura keramického džbánku, ze kterého pacient pil zhruba 3 měsíce čaj s citronem. Koncentrace olova v čajovém výluhu dosahovala hodnoty 227 mg/kg. Otrava olovem v souvislosti s neprofesionální expozicí tomuto kovu je ve vyspělých zemích výjimečnou situací provázenou nespecifickou symptomatologií, a tak může zůstat nerozpoznána. Mikroskopické vyšetření nátěru krve s nálezem převážně hrubého bazofilního tečkování erytrocytů však může na tuto vzácnou diagnózu upozornit., This article reports on patient that has been presented with sudden onset of constipation, abdominal pain and normocytic anemia. Gastroscopy and colonoscopy ruled out an organic diseases. In peripheral blood and bone marrow aspirates mears, coarse basophilic stippling of erythrocyte (and erythroblasts) point out a possibility of heavy metal poisoning. The level of plumbemia exceeded 8.4 times the maximal permitted value for common (non-professional) population. A source of poisoning was indentified from a glaze on a ceramic jug, from which the patient had drank tea with lemon for three months. A lead concentration in the tea extract was 227 mg/kg. In developed countries, lead poisoning is a rare diagnosis. As the symptoms are nonspecific, missed diagnoses could occur, especially in sporadic, non-occupational exposure. Cíl: Cílem pilotní studie bylo zjistit, zda dochází v průběhu klokánkování ke snížení srdeční a dechové frekvence u nedonošených novorozenců, zjistit zda klokánkování ovlivní periferní saturaci krve kyslíkem a délku hospitalizace nedonošených novorozenců na Jednotce intenzivní a resuscitační péče o novorozence. Dále zjistit, zda klokánkování ovlivňuje interakci matka - dítě. Metodika: Metodou studie byl experiment. Technikou sběru dat k posouzení efektu klokánkování bylo měření fyziologických funkcí. Interakce mezi matkami a jejich dětmi byly měřeny dotazníkem - Maternal Postnatal Attachment Scale (MPAS). Ke statistickému zpracování byl využit t-test pro dva výběry, neparametrický Wilcoxonův test, chí test a Fisherův exaktní test. Statistické testy byly hodnoceny na hladině významnosti 5 %. Výsledky: Z fyziologických funkcí byl signifikantně potvrzen vzestup tělesné teploty. Při porovnání intervenční a kontrolní skupiny před klokánkováním nebyl v interakcích matek k jejich dětem zaznamenán žádný signifikantní rozdíl. Před propuštěním (resp. po klokánkování) byl zájem o dítě u matek klokánkovaných dětí významně vyšší (p = 0,0354) oproti matkám, které své dítě neklokánkovaly. Ve všech položkách MPAS byla úroveň připoutání ke klokánkovanému dítěti před propuštěním vyšší než u neklokánkovaných novorozenců. Závěr: Klokánkování prokázalo pozitivní efekt na nedonošené novorozence v oblasti vzestupu tělesné teploty, zvýšeného zájmu o potřeby novorozence a silnějšího připoutání matky k novorozenci., Objective: The pilot study aimed at determining whether kangaroo mother care decreases heart and respiration rates in preterm newborns, affects saturation of peripheral oxygen and length of newborns’ hospital stay at a neonatal intensive care unit, and influences mother-child interaction. Methods: The method of study was an experiment. To assess the effect of kangaroo mother care, physiological functions were measured. Mother-child interaction was assessed using the Maternal Postnatal Attachment Scale (MPAS). Statistical analyses were carried out with a two-sample t-test, non-parametric Wilcoxon test, chisquare test and Fischer’s exact test. The statistical tests were evaluated at a significance level of 5%. Results: Of the physiological functions studied, only body temperature was confirmed to be significantly increased. There was no significant difference in mother-child interaction prior to kangaroo care between the study and control groups. After kangaroo care (i.e. prior to hospital discharge), interest in their babies was significantly higher (p=0.0354) in mothers providing kangaroo care than in the controls. In all MPAS items, pre-discharge levels of maternal bonding were higher in the group of children provided with kangaroo care than in the controls. Ozon (03) je složen ze tří atomů kyslíku a je silným oxidantem v důsledku značného dipólového momentu, který je velmi nestabilní. Ozon může být potencionálně nebezpečný, přesto mnohé studie dokazují taktéž jeho terapeutické účinky. Možné terapeutické využití ozonu jsou studovány již více než jedno století. Při správném dávkování nebyly zjištěny závažné nežádoucí účinky této léčby a v případě minimálních nežádoucích účinků se jedná o účinky preventabilní. Ozon se používá jako prostředek k dezinfekci a mnoha dalším léčebným účelům. Do skupiny onemocnění čelistního kloubu se řadí poměrně často se vyskytující stavy zejména u dospělých, které jsou charakterizovány především bolestmi a zvukovými fenomény provázejícími pohyb čelisti a změnou hybnosti čelisti. Zeny jsou postiženy častěji než muži, etiologie tohoto onemocnění zatím není plně objasněna. V současné době se uvažuje především o multifaktoriální teorii. V této kazuistice jsou popsány první zkušenosti s intraartikulární aplikací ozonu při léčbě onemocnění temporomandibulárního kloubu., Ozone (03) gas is a molecule consisting of three atoms of oxygen and powerful oxidant with dynamically unstable structure due to the presence of mesomeric states. Although 03 has potential dangerous effects, yet many studies confirm that it has also many therapeutic effects. Ozone therapy has been utilized and heavily studied for more than a century. Its effects are proven, consistent, safe and with minimal and preventable side effects. Temporomandibular joint disorders are common in adults; temporomandibular joint (TMJ) disorder refers to a cluster of conditions characterized by pain in the TMJ or its surrounding tissues, functional limitations of the mandible, or clicking in the TMJ during motion. The etiology of TMJ disorders remains unclear, but it is likely multifactorial. U 68letého pacienta s metastatickým nádorem ledviny, současně léčeného kortikoidy pro idiopatickou trombocytopenickou purpuru (ITP), bylo dosaženo podáním sunitinibu stabilizace stavu po dobu zatím 70 měsíců, bez nutnosti hospitalizace či ATB terapie, nebo krevních substitučních derivátů. Toxicita systémové léčby může být u pacientů s nádorem ledviny závažným a někdy i limitujícím problémem. U tohoto pacienta i při závažné komorbiditě podání sunitinibu vedlo k významnému klinickému účinku bez závažné toxicity., In a patient with metastatic renal cell carcinoma, concomitantly treated with corticoids because of idiopathic thrombocytopenic purpura (ITP), an unusually long survival of 70 months with minimal toxicity has been achieved using sunitinib. In patients with renal cell cancer toxicity of systemic therapy could be a serious and limiting factor. Východiska: Názor na přidání bevacizumabu do algoritmu léčby metastatického karcinomu prsu (MBC) je nejednoznačný, i když studie fáze III (E2100, RIBBON-1) prokázaly přínos kombinace proti standardní chemoterapii 1. linie. V těchto studiích bylo dosaženo zvýšení četnosti odpovědí a prodloužení přežití bez progrese. Kazuistika: Uvádíme případ nemocné léčené bevacizumabem v kombinaci s paclitaxelem v paliativní léčbě metastatického karcinomu prsu, u které bylo dosaženo výborného léčebného efektu – kompletní remise při léčbě symptomatického onemocnění v první linii. Šestapadesátiletá nemocná měla zjištěn nádor prsu metastazující do jater, plic a skeletu, histologicky invazivní duktální triple negativní karcinom s mucinózní grade 3, klinicky s výraznou symptomatikou bolesti, nevolnosti a hubnutí. Byla léčena chemoterapií v kombinaci paclitaxel/bevacizumab celkem po dobu 26 měsíců. Léčbou bylo dosaženo úplné odpovědi na játrech a plicích a zlepšení nálezu ve skeletu. V souvislosti s léčbou bevacizumabem nebyly zaznamenány jiné nežádoucí příhody než flebotrombóza bérce po 26 měsících léčby, bez závažných komplikací, dobře zvládnutá konzervativní léčbou. Kompletní remise potvrzena CT/PET vyšetřením byla po 11 měsících léčby a trvala 48 měsíců. Závěr: Celkově dosažený interval přežití naší pacientky byl 53 měsíců ve velmi dobré kvalitě života. Z důvodu toxicity nebyla nutná hospitalizace ani podání transfuze., Background: The role of the bevacizumab in the treatment metastatic breast cancer (MBC) is not clear. The studies fase III (E2100, RIBBON- 1) can declare profit for groups of patients treated with bevacizumab and standart first line of chemotherapy (high number of responses and longer overal survival). Case: We present the case of a patient with MBC triplet negative high grade with imperative symptomas (nauzea, anorexia, weight loss and pain) The patient was treated with chemotherapy bevacizumab and paclitaxel within 26 months. Complet remission was declared by CT/PET scan and first progression we detected 48 months later. Conclusion: A good quality of life and long-term persistent complete remission (16 months) was achieved, no transfusion, no hospitalization. Úvod, cíle: Poruchy ejakulace jsou po míšním poranění velmi časté. U nemocných s přáním vlastního potomka je většinou nutný asistovaný odběr spermií. Penilní vibrostimulace (PVS) je neinvazivní metoda k získání ejakulátu u pacientů s lézí nad úrovní T10. Cílem práce je zhodnocení efektivity a bezpečnosti PVS, vyhodnocení kvality a dalšího využití ejakulátu. Metodika: Od 2010 do 2014 jsme provedli PVS u 20 pacientů. Poranění krční (C) míchy bylo u 13 pacientů, hrudní (T) míchy u sedmi pacientů. Průměrný věk byl 30,8 let. Průměrná doba od úrazu do PVS byla 64 měsíců. První PVS mělo sedm nemocných déle než 3,5 roku a 13 do 3,5 roku od úrazu. K PVS jsme užili přístroj FERTI CARE Multicept. Výsledky: Ejakulace jsme dosáhli u 11 pacientů (55 %), z toho devět mělo poranění C míchy a dva T míchy. U nemocných se začátkem PVS do 3,5 roku od poranění byla úspěšnost 77 % a nad 3,5 roku 14 %, p = 0,027. Při srovnání ejakulátu z druhé PVS došlo ke zvýšení celkového počtu spermií (p = 0,03), koncentrace spermií (p = 0,025) a počtu progresivně pohyblivých spermií (p = 0,027) oproti první PVS. Získaný ejakulát je v 72 % pouze kryoprezervován k pozdějšímu využití. U dvou pacientů byl ejakulát použit k fertilizaci. Jeden nemocný měl azoospermii. Autonomní dysreflexie se vyskytla v průběhu PVS u sedmi pacientů, příznaky odezněly do 3 min. Závěr: PVS je bezpečná metoda asistovaného odběru ejakulátu. Při opakování PVS jsme zjistili zlepšení kvality ejakulátu. In men who wish to father children, assisted sperm retrieval is often necessary. Penile vibratory stimulation (PVS) is a non-invasive method for sperm retrieval in patients with a lesion above T10. The aim of our research was to evaluate the effectiveness and safety of PVS, semen quality and further utilization of the ejaculate. Methods: Between 2010 and 2014, we performed PVS in 20 patients. An injury of the cervical (C) and thoracic (T) spinal cord was present in 13 and seven patients, respectively. The mean age of patients was 30.8 years. The mean time from the injury to PVS was 64 months. Seven patients had the first PVS more than 3.5 years after the injury and 13 patients less than 3.5 years. FERTI CARE Multicept was used for PVS. Results: Ejaculation was achieved in 11 patients (55%) – in nine with C and in two with T SCI. Success rate of PVS in patients less than 3.5 years after the injury was 77%, compared to 14% in patients more than 3.5 years after the injury, p = 0.027. Total sperm count (p = 0.03), sperm concentration (p = 0.025) and the number of sperm with progressive motility (p = 0.027) increased in the semen from the second PVS. Obtained ejaculate is mostly (72%) cryopreserved only for further use. The ejaculate was used for fertilisation in two patients. One patient had azoospermia. Autonomic dysreflexia during PVS occurred in seven patients, symptoms resolved within 3 mins. Conclusion: PVS is a safe method for sperm retrieval. Semen quality improved with repeated PVS. The ejaculate was successfully used for fertilisation in two patients.
