Politika dlouhodobé péče o starší věkové skupiny chronicky nemocných a nesoběstačných osob je poměrně často ve zjednodušených modelech dávána do souvislosti s budoucími hrozbami pro veřejný sektor a veřejné finance. Navrhovaným „nezbytným“ řešením jsou pak obvykle změny v organizaci a financování dlouhodobé péče v rámci tržně orientovaných reforem, ovšem se zásadními důsledky vzniku nerovností v dostupnosti a ve zdraví, nekoordinovaným procesem poskytování služeb, v nichž dochází k výrazné asymetrii mezi příjemci a poskytovateli služeb. Předmětem práce je tržně orientovaný přístup případu věcného návrhu zákona o dlouhodobé péči v ČR v roce 2011, porovnávaný s rozdílným vývojem diskurzu této oblasti v EU/EC-OECD-WHO a také od vývoje poznání/výzkumu. Cílem práce je kritická reflexe příčin tak odlišného zjednodušujícího vývoje v ČR s využitím metody diskurzivní analýzy. Článek vznikl za finanční podpory projektu „PRVOUK P17 - Vědy o společnosti, politice, médiích ve výzvách doby", Long-term care (LTC) policy for chronically ill and disabled seniors is quite often simplified in models overestimating future threats to the public sector and public finance. The proposed solution is then formulated in changes of the organization and financing of LTC as a part of market-oriented reforms, without the evaluation of major consequences in terms of inequalities in access and in health. The subject of the work is marketoriented approach of the bill on LTC in the Czech Republic in 2011, in contrast to the discourse of LTC policy options in the EU / EC-OECD-WHO and the development of knowledge / research. V české společnosti a českém zdravotnictví došlo od roku 1989 k hlubokým změnám. Financování zdravotní péče z daní bylo nahrazeno veřejným zdravotním pojištěním s veřejně-soukromým mixem poskytovatelů zdravotní péče. Dvacet let reforem lze považovat za úspěch, i když se zdravotní systém potýká s mnoha problémy. Radikální a rychlá řešení reformy zdravotního systému neexistují; přednost by měl mít konzervativní přístup. Zdravotní reformy ukazují, že společnost potřebuje více času na přijetí společenských změn. Systém zdravotní péče se složitými komplexními vztahy, není laboratoř pro experimenty na lidech. V textu jsou diskutována některá vybraná reformní opatření, která by měla vést ke zlepšení zdraví a zdravotního systému., Since 1989 the Czech society and the Czech health system have undergone deep changes. Health care financing out of taxation was replaced by public health insurance, with public-private mix of health providers. The twenty years of reforms can be viewed as a success, although the health system faces many problems. The radical and quick solutions do not exist in health system reform; the conservative approach should be preferred. Health reforms show that the society needs more time to accept social changes. The health system with complex relations is not a laboratory for pro experiments on humans. Health performance and economic performance are interlinked. Good health is essential for economic and social development. This paper identifies and describes linkages between economic globalization and health. The aim is to analyze international professional discourse on ways how economic globaliza - tion affects health and to critical reflex development of the Czech health policy in the relation to economic globalization, financial crisis and health. Článek se zabývá vývojem a současnou situací v oblasti infastruktury podpory zdraví v ČR a nabídkou vzdělávacích programů pro zdravotnické profesionály, kteří by se podporou zdraví mohli zabývat a realizovat jí v praxi, a to jak na pregraduální tak na postgraduální úrovni. The article represents an overview of the development of Czech health promotion in the last two decades. Current “state of art” and health promotion in international context are mapped. Major behavioral risk factors are described and analybesity in comparison to EU countries. Their impact on the Czech population health is discussed in terms of relevant diseases.
