ONSTEP (Open New Simplified Totally Extraperitoneal) technika reparace tříselné/femorální kýly je inovativní metoda, jejímiž autory jsou portugalští chirurgové Dr. Augusto Lourenço a Dr. Rui Soares da Costa. Prezentace formou posteru proběhla na EHS/AHS kongresu v březnu 2012. Na území České republiky jsou prvními pracovišti provádějícími tento typ operace Chirurgické oddělení nemocnice Prostějov a Chirurgické oddělení Nemocnice ve Frýdlantě. ONSTEP technika představuje minimálně traumatizující přední přístup k operaci tříselné kýly s použitím samoexpandibilní síťky (oválná, anatomicky tvarovaná polypropylenová síťka s obvodovým paměťovým prstencem z polyetylen tereftalátu). Hlavní výhody této metody spočívají v krátké době operačního výkonu, možnosti provedení v lokální anestezii, krátké době rekonvalescence, nízkém výskytu pooperační chronické bolesti, nízkém počtu recidiv, krátké době hospitalizace a dobrém kosmetickému efektu. Je vhodná pro pracoviště zabývající se jednodenní chirurgií. Klíčová slova: tříselná kýla – operace – ONSTEP, The ONSTEP (Open New Simplified Totally Extraperitoneal) inguinal/femoral hernia repair technique is an innovative procedure presented by Portuguese surgeons Augusto Lourenço and Rui Soares da Costa within the EHS/AHS Congress in March 2012. The first institutions to perform this kind of hernia surgery in the Czech Republic are the Department of Surgery at Prostejov Hospital and the Department of Surgery at Frydlant Hospital. The ONSTEP technique represents a minimally traumatizing frontal approach to inguinal hernia repair using a special self-expanding mesh patch (an oval, anatomically-shaped moderate-pore polypropylene mesh with the circumferential memory-ring consisting of polyethylene terephthalate). The main advantages of this method are: short duration of surgery, the possibility of performing the procedure under local anaesthesia, short recovery time and rapid return to daily activities, low incidence of chronic post-operative groin pain, low recurrence rate, short hospital stay, good cosmetic effect and a low price. Úvod: Problematika náhlých příhod břišních (NPB) u gravidních pacientek představuje specifickou oblast na rozhraní chirurgie a gynekologie. I když zdravotní péče o těhotnou pacientku spadá do péče gynekologa-porodníka, indikace operačního výkonu pro NPB je požadována od chirurga, který má obvykle znalosti a zkušenosti s vyšetřováním těhotných pacientek. Tato skutečnost nabízí prostor ke vzniku pochybení v poskytování zdravotní péče s možnými velmi závažnými důsledky pro matku i plod. Metoda: Byla provedena literární rešerše studií publikovaných za posledních 10 let v databázi PubMed týkajících se apendektomie, cholecystektomie a laparoskopie v těhotenství v kontextu náhlých příhod břišních. Vylučovacím kritériem byly operace provedené z porodnické indikace. Výsledky: Těhotenství je doprovázeno řadou anatomických a fyziologických změn, které je nutné brát v úvahu při klinickém vyšetření pacientky, analýze výsledků laboratorních vyšetření i v rámci operační taktiky. Využití zobrazovacích technik v diagnostice NPB u těhotných je výrazně omezeno prokázaným škodlivým efektem ionizujícího záření, jakož i nejasným dopadem silného magnetického pole na plod. Timing akutního operačního výkonu pro NPB u těhotných by neměl být odlišný od časování operační intervence u ostatních pacientek s NPB. Miniinvazivní chirurgie, resp. laparoskopické řešení vybraných NPB u těhotných představuje v současnosti již standardní operační techniku. Mnohé z obecných výhod laparoskopického přístupu (nižší riziko infekcí a dehiscencí operačních ran, rychlejší rekonvalescence atd.) se uplatňují i u gravidních pacientek. Závěr: Náhlé příhody břišní v těhotenství je nutné řešit v rámci multidisciplinární spolupráce mezi chirurgem, gynekologem, anesteziologem a neonatologem. S ohledem na dostupné výsledky publikovaných studií je laparoskopické řešení NPB u těhotných pacientek považováno za bezpečné a efektivní., Introduction: Acute abdomen during pregnancy presents very specific problem on the boundary of surgery and gynaecology. Although gynaecologists provide healthcare to pregnant women, surgery indication because of acute abdomen during pregnancy should be done by surgeon (who has usually little knowledge and experience regarding pregnant women examination and management). There is therefore real opportunity for possible mistakes origin with serious consequences for mother and foetus. Methods: Literature search in PubMed was done aimed at studies within the last 10 years dealing with appendectomy, cholecystectomy and laparoscopy during pregnancy in the context of acute abdomen. Surgical interventions done from obstetrics indications were excluded. Results: Pregnancy is associated with many anatomical and physiological changes which have to be considered not only during clinical examination, but also when analysing laboratory findings and considering operative tactics. Imaging modalities employment during pregnancy is very limited due to harmful effect of ionising radiation and not clear impact of strong magnetic field on the foetus. Timing of surgical interventions because of acute abdomen during pregnancy should be the same as timing of acute abdomen interventions in non-pregnant patients. Minimally invasive surgery in acute abdomen during pregnancy presents standard technique nowadays. Many advantages of laparoscopic approach (lower rate of surgical site infections, quicker convalescence etc.) are valid also during pregnancy. Conclusion: Acute abdomen during pregnancy should be managed within a multidisciplinary cooperation between gynaecologist, surgeon, anaesthesiologist and neonatologist. Varikokéla je nejčastější chirurgicky ovlivnitelná příčina mužské infertility. Dilatované a varikózně změněné žíly plexus pampiniformis testis vytváří varikokélu. Regurgitace krve při insuficienci chlopní vnitřních spermatických žil způsobuje poškození testikulární funkce zvýšením intraskrotální teploty. Autoři sledují efekt a dopad mikrochirurgické operace varikokély na spermiogram, Varicocele is the most common cause of male infertility, which can be repaired surgically. Dilated and varicose changes of the venous pampiniform plexus of the testis cause varicocele. Valvular insufficiency of the spermatic veins allows regurgitation of blood which causes damage to the testicular function by increasing the intrascrotal temperature. Úvod: Zlomeniny proximálneho humeru predstavujú asi 4−5 % zo všetkých zlomenín. Terapia dislokovaných 3- a 4-časťových zlomenín je náročná a optimálna liečebná metóda je diskutabilná. Cieľom práce bolo porovnanie operačnej a neoperačnej terapie 3- a 4-časťových zlomenín proximálneho humeru u dospelých pacientov. Metoda: Od septembra 2010 do septembra 2013 sme na II. Ortopedicko-traumatologickej klinike Univerzitnej nemocnice Bratislava prospektívne vyhodnocovali operačnú verzus neoperačnú liečbu 3- a 4-časťových zlomenín proximálneho humeru. Pacienti zaradení do štúdie boli dospelí pacienti s 3- a 4-časťovými zlomeninami proximálneho humeru typu Neer IV, V a VI. V operačnej skupine bolo spolu 20 pacientov liečených pomocou uhlovo-stabilných dláh (Philos, Synthes) a intramedulárnych klincov (Multilock, Synthes). Skupinu tvorilo 13 žien (65 %) a 7 mužov (35 %). Priemerný vek bol 60,9?7,67 (52–80) rokov. Konzervatívna skupina obsahovala 19 pacientov. Dvanásť žien (63,2 %) a sedem mužov (36,8 %). Priemerný vek bol 65,1?9,4 (52−88) rokov. Poranená končatina sa fixovala použitím Gilchristovej bandáže na maximálne 3 týždne. Všetci pacienti boli hodnotení po 12 mesiacoch použitím skórovacieho systému podľa Constant- Murleyho a Oxford Shoulder Score. Druhotne boli hodnotené komplikácie liečby. Výsledky: Relatívne individuálne Constant-Murley Score v celej skupine liečenej operačne bolo priemerne 57,8?23,5 % (11−88 %). Oxford Shoulder Score priemerne 34,8?10,6 (10−46) bodov. Komplikácie boli zaznamenané u 12 pacientov (60 %). V skupine konzervatívnej bolo relatívne individuálne Constant-Murley Score po12 mesiacoch po úraze priemerne 60,9?20,9 % (16−90 %). Oxford Shoulder Score bolo priemerne 36,1?8,9 (15−48) bodov. Komplikácie boli prítomné u 4 pacientov (21,1 %). Záver: V tejto štúdii dislokovaných 3- a 4-časťových zlomenín proximálneho humeru bola operačná skupina pacientov, liečená zväčša použitím uhlovo-stabilných dláh, spojená so značným počtom komplikácií. Horšie funkčné výsledky operačnej skupiny boli porovnateľné s výsledkami skupiny neoperačnej. Pre dosiahnutie lepších výsledkov by sa starostlivo mala posudzovať indikácia operácie, výber pacienta, metóda operácie, operačné skúsenosti ako aj následná rehabilitácia. Treatment of 3- and 4-part fractures is difficult, and the optimal treatment option remains controversial. The aim of the study was to compare surgical and non-surgical treatments of 3- and 4-part fractures of the proximal humerus in adults. Method: A prospective study of surgical and non-surgical treatments of 3- and 4-part fractures of the proximal humerus was conducted at the 2nd Department of Orthopedics and Traumatology, University Hospital Bratislava, from September 2010 until September 2013. Patients included in the study were compliant adults patients with 3- and 4-part proximal humerus fractures Neer type IV, V and VI. Twenty patients were treated surgically. The group included 13 women (65%) and 7 men (35%). Mean age was 60.9?7.67 (52–80) years. Angle-stable plates (Philos, Synthes) and intramedullary nails (Multilock, Synthes) were used. The non-surgical group included 19 patients, consisting of 12 (63.2%) women and 7 (36.8%) men. Mean age of both sexes was 66.3?9.5 (52−88) years. Gilchrist bandage was applied for the maximum of 3 weeks. All patients were evaluated at 12 months from treatment using the Constant-Murley scoring system and the Oxford Shoulder Score. Treatment complications were evaluated as a secondary step. Results: The mean individual relative Constant-Murley score was 57.8?23.5% (11−88%) in the surgical group. The mean Oxford Shoulder score was 34.8?10.6 (10−46) points. Complications were recorded in 12 patients (60%). In the non-surgical group, the mean individual relative Constant-Murley score at 12 months from injury was 60.9?20.9% (16−90%). The mean Oxford Shoulder score was 36.1?8.9 (15−48) points. Complications were recorded in 4 patients (21.1%). Conclusions: In this study, surgical treatment patients with displaced 3- and 4-part proximal humerus fractures, mostly treated by angle-stable plates, showed a number of complications. Poor functional results of the surgical group were comparable to those of the non-surgical group. The indication for surgery, patient selection, type of surgery, surgical experience and subsequent rehabilitation should be carefully assessed for better results. We developed a novel behavioral task in which rats learn to recognize the configuration of objects in an animated scene displayed on a computer screen. The scene consisted of a moving bar and a stationary rectangle. Rats deprived of food were trained to press a lever for reward in a small chamber located in front of the screen. Lever presses were rewarded
only when the bar was at the rectangle. Rats anticipated the reward by gradually increasing frequency of lever pressing as the bar approached the rectangle. Control experiments showed that neither the timing nor the discrimination of rewarded and non-rewarded periods as two discrete conditions explain behavior of the rat. Because the changes in the
scene were generated by movement of the object, the presented task could be used for studying neural structures involved in spatial behavior of rats using virtual reality technology.
