I have hypothesized that mammalian offspring sex ratios (proportions male at birth) are partially controlled by the hormone levels of both parents around the time of conception. Recently data have been published (privately) which impugn suggestions of mine (based on that hypothesis) relating to the offspring sex ratios of people who are carriers of hepatitis B virus; and (publicly) to hormone profiles of women who are infected with the protozoan parasite Toxoplasma gondii. Here these data are reviewed in the light of data on offspring sex ratios of people infected with hepatitis C, and cytomegalovirus and T. gondii; and of mice with T. gondii and the trichostrongyline nematode Heligmosomoides polygyrus. Three proposals are made viz: 1. Women infected with T. gondii may have high oestrogen levels. This would potentially explain reports of their high offspring sex ratios, long gestations and (perhaps) behavioural traits. 2. Some of these infective organisms may 'prefer' a particular endocrine environment in their host (in the sense that it facilitates replication or transmission or both). 3. Some of these organisms apparently cause changes in that endocrine environment as a consequence of their presence. If both the latter two proposals were correct, it could explain recent puzzling data on the offspring sex ratios of hepatitis B carriers. And if this solution turns out to be correct, it would suggest new endocrine means of combating hepatitis B infection.

A sequence $\pi \in {\rm NS}_n$ is said to be graphic if it is the degree sequence of a simple graph $G$ on $n$ vertices, and such a graph $G$ is called a realization of $\pi $. The set of all graphic sequences in ${\rm NS}_n$ is denoted by ${\rm GS}_n$. A graphical sequence $\pi $ is potentially $H$-graphical if there is a realization of $\pi $ containing $H$ as a subgraph, while $\pi $ is forcibly $H$-graphical if every realization of $\pi $ contains $H$ as a subgraph. Let $K_k$ denote a complete graph on $k$ vertices. Let $K_m-H$ be the graph obtained from $K_m$ by removing the edges set $E(H)$ of the graph $H$ ($H$ is a subgraph of $K_m$). This paper summarizes briefly some recent results on potentially $K_m-G$-graphic sequences and give a useful classification for determining $\sigma (H,n)$.
Application of Cerebrolysin (0.1 /rg per 1 ml) by a fast microperfusion system induced an inward current of 0.2 to 1 nA in all neurones from newborn mouse hippocampi held at —30 mV membrane potential. Cerebrolysin- induced currents were reduced by the GABAa antagonist bicuculline (2 /¿M) by 65 %, by the NMDA antagonist aminophosphovaleric acid (APV, 10 /¿M) by 27 %, and by the non-NMDA antagonist cyanonitroquinoxalinedione (CNQX, 10 ¡utA) by 20 %. Cerebrolysin dialyzed through a 3.6 kD gut did not induce any transmembrane current but potentiated the response induced by GABA (10 /utA) to 135 %. We conclude that, in addition to amino acids which activate GABAa, NMDA and non-NMDA receptors, Cerebrolysin also contains a peptide which potentiates the GABAa receptor response.
Cíl: Hodnocení komunikačních a vybraných psychických oblastí (napětí, obavy, podráždění, deprimace) dispenzarizovaných nemocných se zhoubnými nádory hrtanu a slinných žláz, pomocí standardizovaných dotazníků kvality života. Východiska: Zhoubná nádorová onemocnění hlavy a krku se v četnosti výskytu řadí na šesté místo mezi zhoubnými nádory na celém světě. Otázka léčby a následné péče je velmi aktuální zvláště z hlediska rizika mutilujících léčebných výkonů a jejich vlivu na kvalitu života. Metody: Do výzkumu byli zařazeni aktivně dispenzarizovaní nemocní ORL kliniky se zhoubnými nádory hrtanu a slinných žláz. Vlastní šetření absolvovalo 113 respondentů, výzkum probíhal od ledna do prosince 2011. Pro sběr dat byly využity dva standardizované dotazníky kvality života (EQRTC QLQ-30, EQRTC QLQ-H&N35). Dále informace z nestandardizovaného dotazníku a zdravotnické dokumentace. Použité statistické metody – dvouvýběrový Wilcoxonův test a analýza rozptylu. Výsledky: U pacientů s nádorovým onemocněním hrtanu s tracheostomickou kanylou a bez ní byl statisticky významný rozdíl v komunikaci. Stádium nemoci dle klasifi kace TNM nemělo vliv na vybrané psychické oblasti u respondentů s nádorovým onemocněním hrtanu a slinných žláz. Závěry: Závažnost obtíží u dispenzarizovaných respondentů závisí na lokalizaci a velikosti nádoru, terapeutické strategii, věku pacienta, polymorbiditě a dalších faktorech. Trvalá tracheostomická kanyla má vliv na komunikaci respondentů s nádory hrtanu. V našem souboru respondentů jsme neprokázali vliv stádia nemoci dle klasifi kace zhoubných novotvarů TNM, na vybrané oblasti psychiky (napětí, obavy, podráždění, deprimace)., Aim: Assessment of communication and selected psychological areas (e. g. tension, concerns, irritation, depression) related to the dispensarized patients with malignant laryngeal and salivary glands tumors using standardized questionnaires assessing quality of life. Background: The head and neck malignant tumors rank sixth in frequency among malignant tumors worldwide. The question of treatment and follow-up care is very current especially in terms of the risk of mutilating surgical procedures and their impact on quality of life. Methods: In the research actively dispensarized patients of ENT clinic with malignant tumors of the larynx and salivary glands were included. 113 respondents took part in the research, which was conducted from January to December 2011. Standardized (EQRTC QLQ-30, EQRTC QLQ-H&N35) and non-standardized questionnaires assessing quality of life and medical records were used for data collection. Statistical methods such as double selection Wilcoxon test and analysis of dispersion were used. Results: There was a statistically signifi cant diff erence in communication between patients with tracheostomy cannula and without it. Stage of the illness according to TNM classifi cation did not infl uence selected psychological areas of the respondents with cancer of the larynx and salivary glands. Conclusions: The seriousness of the diffi culties of the dispensarized patients depends on localization and size of the tumor, therapeutic strategy, age of the patient, polymorbidity and other factors. Permanent tracheostomy cannula has an impact on communication of the respondents with laryngeal tumors. Cieľ práce: V našej práci uvádzame kazuistiku pacienta po orgánovej transplantácii obličky, u ktorého bolo diagnostikované zriedkavé potrasplantačné lymfoproliferatívne ochorenie typu mnohopočetného myelómu. Typ práce: Kazuistika. Názov a sídlo pracoviska: Klinika hematológie a onkohematológie FNsP L. Pasteura a LF UPJŠ, Košice, Slovenská republika. Výsledky: Potrasplantačné lymfoproliferatívne ochorenie typu mnohopočetného myelómu sa vyskytuje zriedkavo. U nášho pacienta sa vyvinulo 19 rokov po orgánovej transplantácii obličky. V porovnaní so zdravou populáciou majú pacienti s transplantovanou obličkou 20-násobne vyššie riziko vzniku. V súlade s literárnymi údajmi uvádzanými pri transplantácii obličky bola aj v tomto prípade zaznamenaná Epstein-Barr virus negativita a výskyt s dlhšou časovou latenciou po transplantácii. Terapia thalidomidom a dexamethasonom bola efektívna s akceptovateľnou toxicitou. Z hľadiska nefrológov však nebolo možné redukovať dávku dlhodobej imunosupresívnej liečby, ktoré je v prípade rozvoja PTLD indikované. Záver: Potrasplantačné lymfoproliferatívne ochorenie je lymfoidná proliferácia alebo lymfóm, ktorý vzniká ako následok imunosupresie u transplantovaného pacienta. Riziko vzniku potrasplantačného lymfoproliferatívneho ochorenia je závislé od typu allograftu a imunosupresie. Príčinou vzniku je potlačenie imunitného systému príjemcu a zároveň chronická antigénna stimulácia zo strany transplantovaného orgánu. Väčšina potrasplantačných lymfoproliferatívnych ochorení má spojitosť s Epstein-Barr virus infekciou. Celková mortalita u príjemcov orgánového štepu je približne 60% a u príjemcov krvotvorných kmeňových buniek až 80%. Skorá diagnostika, redukcia imunosupresie, starostlivé zváženie liečebných postupov a úzka spolupráca odborníkov môžu zlepšiť prognózu pacientov., Objective: We demonstrate the case report of patient receiving renal allograft and have developed rare Post-transplant lymphoproliferative disorder (PTLD) Plasma cell myeloma. Design: Case report. Settings: The Clinic of Hematology and Oncohematology, Faculty Hospital and Policlinic of L. Pasteur and Medical Faculty, University of Pavel Josel Šafarik, Košice, Slovakia. Resuslts: Transplant patients rarely develop plasma cell myeloma. We demonstrate the case report of a patient receiving renal allograft, who has developed it 19 years after the transplantation. The risk of incidence is estimated to be 20 times for renal allograft recipients as compared with the normal population. According to literature our patient had lateoccuring, Epstein-Barr virus (EBV) – negative monomorphic post-transplant lymphoproliferative disorder. Thalidomide and dexamethasone combined therapy was effective with acceptable toxicity. It was not possible to reduce the longterm immunosuppressive regimen because of graft rejection according to nephrologists. Conclusion: PTLD is a lymphoid proliferation or lymphoma that develops as a consequence of immunosuppression in a recipient of a solid organ or haematopoietic stem cells allograft. The risk of lymphoma varies depending on the type allograft and immunosuppressive regimen and has been linked to both defective immune surveillance and chronic antigenic stimulation from the engrafted organ. The majority of cases are associated with EBV infection. The mortality of PTLD in solid organ allograft recipients is approximately 60%, while that of haematopoietic stem cells recipients is 80%. Narůstající počet závislých osob seniorského věku si vyžaduje adekvátní reakci ze strany sociálně zdravotní péče o tyto osoby. Jde zejména o hledání optimálního nastavení spolupráce mezi formální a neformální péčí, a dále o rozvíjení vhodných forem péče - terénních, ambulantních i pobytových služeb a služeb zdravotních a sociálních tak, aby na sebe navazovaly a dokázaly pružně uspokojovat aktuální potřeby seniorů a jejich rodinných pečujících. Článek se zaměřil na situaci v oblasti sociálně-zdravotních služeb určených pro seniorskou populaci v hlavním městě Praha (HMP) ve srovnání s dalšími re - giony ČR a identifikoval nejvýznamnější neuralgické body současného systému dlouhodobé sociálně-zdravotní péče v HMP. Využívá kvantitativní i kvalitativní metody, na jejichž základě byla podrobně analyzována existující nabídka sociálně-zdravotních služeb v HMP a byla reflektována očima zúčastněných aktérů, tj. uživatelů služeb a jejich rodinných příslušníků, poskytovatelů a zadavatelů sociálních a sociálně zdravotních služeb. Výsledky kvantitativního i kvalitativního výzkumu ukazují na závažné deficity jak v nabídce těchto služeb, tak v jejich vzájemné propojenosti., The increasing number of dependent seniors requires an adequate response from long-term care system for these citizens. This mean a search for setting of cooperation between formal and informal care, and need for appropriate forms of long-term care (field care, outpatient care and residential services) There is an increasing need for health and social services to relate mutually and cooperate flexibly, so that they would appropriately meet the current needs of seniors and their family caregivers. The article is focused on long-term care for the senior population in the capital Prague in comparison with other Czech regions and identifies the most neuralgic points of long- term care in Prague. It uses both qualitative and quantitative methods to analyse in detail supply of long-term care services in Prague from the point of view of main stakeholders. V okamžiku, kdy dítě onemocní onkologickým onemocněním, potřeby a priority rodičů se jistým způsobem transformují – středem pozornosti se stává nemocné dítě. Cílem šetření byla identifikace potřeb rodičů dětí s onkologickým onemocněním a vnímání těchto potřeb zdravotními sestrami. Výzkumný soubor zahrnoval 100 rodičů onkologicky nemocných dětí, kteří byli hospitalizováni společně s dítětem ve Fakultní nemocnici v Ostravě nebo ve Fakultní nemocnici v Praze Motole. Druhý soubor byl tvořen 70 sestrami pracujícími ve stejných zařízeních s onkologicky nemocnými dětmi a jejich rodiči. Byl použit standardizovaný dotazník Needs of Parents Questionnaire (NPQ). Statistické vyhodnocení se uskutečnilo prostřednictvím mediánového testu, chí-testu a Fisherova exaktního testu. Ve výsledcích je patrná rozdílnost ve vnímání potřeb mezi rodiči onkologicky nemocných dětí a zdravotními sestrami. Na rozdíl od sester je pro rodiče důležitá psychická podpora rodičů ze strany sester, vyjádření naděje, včasná informovanost o vyšetřeních a kvalitní vzdělávání dítěte. Výsledky by měly vést sestry k dominantnímu uspokojování právě těchto potřeb rodičů onkologicky nemocných dětí., The moment a child becomes ill with cancer, the needs and priorities of parents are in some way transformed, and the ill child becomes the focus of the attention. The aim of this research was to identify the needs of parents of children with cancer and perception of these needs by nurses. The research sample included 100 parents of children with cancer who were hospitalized with the child in the Faculty Hospital Ostrava or in the Faculty Hospital in Prague Motol. The second sample consisted of 70 nurses working in the same facilities with cancer ill children and their parents. A standardized questionnaire Needs of Parents Questionnaire (NPQ) was used. Statistical evaluation was carried out by median test, chi-test and Fisher‘s exact test. In the results are evident differences in perception between the needs of parents of children with cancer and nurses. Unlike for nurses, for parents is important the psychological support of parents by nurses, expressing hope, timely information on screening and quality education of the child.
