Natalizumab (monoklonální protilátka) je v současné době indikován jako lék druhé volby u relabující-remitující formy roztroušené sklerózy (RR-RS). Závažným a potenciálně fatálním nežádoucím účinkem této léčby je progresivní multifokální leukoencefalopatie (PML), která patří k vzácným oportunním infekcím mozku. Je vyvolána JC virem (JCV). Dochází k progredujícím zánětům bílé hmoty v mnoho četných lokalizacích mozku. Vysoké riziko vzniku PML zahrnuje pozitivitu JCV v krvi, předchozí imunosupresivní léčbu a délku léčby natalizumabem více než 2 roky. Jedním z nejdůležitějších diagnostických metod je MRI, neboť může určit postižení mozku ve velmi raných fázích nemoci. Prezentujeme nálezy 47leté pacientky s RR-RS, která byla pro progresi choroby poslední 3 roky léčena natalizumabem s velmi dobrým klinickým efektem. V posledním roce léčby se u pacientky vyskytl epileptický záchvat, v pozdějších fázích pak rychle probíhající deteriorace kognitivních funkcí a těžká kvadruparéza. V likvoru byl pozitivní nález kopií JCV, na MR byla diagnostikována splývavá ložiska frontálně, charakteristická pro PML. I přes okamžité vysazení léčby natalizumabem a následné zaléčení opakovanými plazmaferézami onemocnění PML progredovalo a pacientka zemřela za 2 měsíce od definitivního stanovení diagnózy. PML je závažná komplikace léčby roztroušené sklerózy natalizumabem. Důležité je včasné rozpoznání příznaků této nemoci. Proto je nezbytné sledovat tyto pacienty podrobně klinicky a včas vyšetřit MR (zejména flair), aby se po stanovení diagnózy a zaléčení nemoci předešlo fatálním komplikacím., Natalizumab (monoclonal antibody) is currently indicated as a second line treatment for relapse-remitting multiple sclerosis (RR-MS). Serious and potential fatal side effect of this treatment is progressive multifocal leukoencefalopathy (PML), which belongs to rare brain infections caused by JC virus (JCV). There is a progressive inflammation of the brain white matter in multiple localizations. A high risk of developing PML includes serum JCV positivity, previous immunosuppressive treatment and duration of natalizumab treatment of more than two years. Magnetic resonance is one of the most important diagnostic methods because it can identify brain damage in the early stage of the disease. We present 47-years-old female with RR-MS which has been successfully treated by natalizumab for three years with very good clinical outcome. In last year of treatment, patient experienced a seizure and in later stages progressive deterioration of cognitive function and severe quadriplegia. Positive JCV was found in cerebrospinal fluid. MR presented diffuse hyperintensive lesions located in frontal lobes. Despite immediate discontinuation of natalizumab treatment and subsequent daily plasma-exchange PML disease progressed and patient died within two months of the final diagnosis. PML is a serious complication of natalizumab treatment in multiple sclerosis. It is important to recognize early signs and symptoms of this disease. In this paper, studies on zoonotic cutaneous leishmaniasis (ZCL) are reviewed that were performed during the last ten years largely by scientists of the Martsinovsky Institute of Medical Parasitology and Tropical Medicine. New data on the taxonomy of Leishmania circulating in populations of Rhombomys opimus, their main host, and the results of field and laboratory studies allowed revision of certain concepts generally accepted in epidemiology and epizootology of ZCL in Central Asia.

Over the last decade, C-reactive protein concentration analyzed by the high sensitivity method (hsCRP) has been proven as a marker of premature atherosclerosis. Concentration exceeding 2 mg/l represents an increased individual risk of myocardial infarction and stroke but strict application of this borderline is complicated by relations of CRP concentrations to other risk factors of cardiovascular diseases. In a large 1 % representative sample of the Czech population, a positive relation of hsCRP to BMI, a waist circumference and triglyceride concentration was documented. Substantial sex differences were found in its relationship to age. Whereas it is continuously increasing in men, this increase appears in women only after menopause. A substantial decrease of body weight and visceral fat volume by increased physical activity is accompanied by significant decrease of hsCRP in young obese women. This decrease was not related to a change of interleukin-6 concentration, although it is supposed to regulate CRP production. CRP concentration is partly under genetic control as a higher concentration in young siblings of probands with proved coronary atherosclerosis was documented. The participation of genes related to lipoprotein metabolism (genes for apolipoprotein CI and apolipoprotein E) influence hsCRP concentrations. We hypothesized that an increased concentration of hsCRP represents a certain marker of proinflammatory status related to central obesity and triglyceride metabolism and it might be related to individual properties of monocytes in atherogenesis., R. Underground mining hard coal in the area of Walbrzych ended in the late 90-ties of the last Century. Its effects on the state of heterogeneous rock mass (complicated geology and tectonics) is still evident and not known precisely. Analysis of levelling measurement results carried out after the end of mining activity (1997-2008) for part of the former “Thorez” mine indicates increased rock mass surface activity in tectonic zones. With the aim to continue monitoring of the rock mass surface movements on the area of former mining grounds a research satellite GPS network (7 points) has been established and supplemented with new precise levelling lines linked to the existing 2nd class levelling network. Location of these points takes into consideration, among other things geology and tectonics of the Walbrzych Basin and areas of underground mining activity. In this paper work on the design and construction of the research satellite GPS and levelling networks has been described and the first measurement campaign carried out in September 2009 has been presented.
