Dienogest, progestin syntetizovaný poprvé v roce 1979 v Jeně v Německu, si zachovává stále významné postavení mezi kontraceptivy. To vyplývá z jeho významných příznivých účinků – progestagenního a antiandrogenního. Další terapeutické využití stejně jako příznivý farmakologický a metabolický profil, zajišťují dienogestu, zvláště v kombinovaných formách, další perspektivu mezi perorálními kontraceptivy., Dienogest, progestin was synthesized for the first time in Jena, Germany in 1979 and still has an important role among contraceptives. This fact is based on its marked positive effects – progestagenic and antiandrogenic. Other therapeutic application as well as favorable pharmacological and metabolic profile of agent provides that dienogest, especially in combined form, has a good perspective among oral contraceptives., and Miloslav Maule
Cílem práce je ukázat v průběhu 18letého sledování kognitivní, emoční a behaviorální změny u pacientky s roztroušenou sklerózou mozkomíšní (RS). Předkládaná kazuistika dokumentuje maladaptaci na diagnózu RS a dlouhodobé maladaptivní zvládání, prolínání a vliv afektivních složek na inhibici kognitivních schopností a psychického stavu, vliv stresu na vznik „pseudoatak“ RS, psychosociální problematiku a kvalitu života. Diagnóza RS měla negativní vliv na pacientku a rodinu. Nepředvídatelný průběh ovlivňoval potíže v adaptaci a v plánování podpory. Vliv stresu souvisejícího s onemocněním a zároveň vedlejší negativní efekt léčby vedli k mnoha psychosociálním potížím, které snížily kvalitu života pacientky a měly negativní vliv na její spolupráci při léčbě a rehabilitaci. Součástí komprehensivní diagnostiky i léčby RS byla opakovaná neuropsychologická vyšetření a v závěru sledování, ve fázi chronicko‑progresivní RS, i kognitivně‑behaviorální terapie cílená na snížení zátěže způsobené chronicitou nemoci a zlepšení kvality života., The aim of the paper is to show cognitive, emotional and behavioural changes in a multiple sclerosis (MS) patient over eighteen years of observation. Case study documents maladaptation to MS diagnosis and prolonged maladaptive coping, diffusion and influence of affective components on inhibition of cognitive abilities and mental health status, the contribution of stress to development of „pseudo-relapses“ of MS, psychosocial problems and quality of life. The diagnosis of MS was devastating for the patient herself and her family. Its unpredictable course complicated adjustment and created difficulties in planning for support. Effects of disease-related stress as well as a negative side-effect of treatment led to many psychosocial problems that impaired the patient’s quality of life and affected compliance with treatment and rehabilitation. Repeated neuropsychological assessment was a part of a comprehensive diagnostic process and treatment of MS and, at the end of observation, in chronic-progressive phase of MS, was complemented with cognitive-behavioural therapy aimed at reducing burden of disease and improving the quality of life., and A. Cíl: Správný pooperační režim je důležitou součástí rekonvalescence po dekompresi karpálního tunelu. Cílem této prospektivní randomizované studie je zjistit, zda pooperační dočasné dlahování má klinicky významný efekt, nebo zda není naopak zbytečné. Výsledky dosud publikovaných prací nedávají jasné, prokazatelné a jednoznačné doporučení. Soubor a metodika: Do souboru bylo zařazeno celkem 72 pacientů s klinicky a elektromyograficky prokázaným syndromem karpálního tunelu, kteří byli randomizováni do dvou skupin. V první skupině byla pacientům po operaci na dva týdny přiložena dorzální dlaha fixující zápěstí v neutrální poloze a v druhé skupině dlaha použita nebyla. Před operací i po operaci byla hodnocena distální motorická latence, síla stisku a míra subjektivních obtíží pomocí specializovaných dotazníků DASH (Disabilities of Arm, Shoulder and Hand Questionnaire) a Bostonského dotazníku (Boston Carpal Tunnel Questionnaire). Na druhou stranu dočasné dlahování po operaci výsledky operační léčby nezhoršuje., Aim: Correct post-operative care is an important part of recovery after carpal tunnel decompression. The aim of this prospective randomized study was to determine whether postoperative temporary splinting has a clinically significant effect, or whether it is unnecessary. The results of published data do not give clear, demonstrable and unambiguous recommendations. Material and methods: A total of 72 patients with clinically and electrodiagnostically established diagnosis of carpal tunnel syndrome were randomized into two groups. The first group included patients with a dorsal splint, fixing the wrist in a neutral position for two weeks after surgery, whereas the second group included patients, in whom the splint was not used after surgery. Distal motor latency, grip strength and degree of subjective symptoms were assessed before and after surgery using specific questionnaires, namely the DASH questionnaire (Disabilities of Arm, Shoulder and Hand) and the Boston questionnaire (Boston Carpal Tunnel Questionnaire). Results: After surgery, there was a reduction of distal motor latency and a moderate reduction of grip strength, but there were no statistically significant differences (p > 0.05) between the two groups. After surgery, the anticipated improvement of subjective symptoms was achieved, objectified by questionnaire coefficients. However, there was no statistically significant difference (p > 0.05) between the two groups either. Conclusion: According to our findings, temporary splinting after open carpal tunnel decompression does not improve the results of surgical treatment compared to the non-splinted group. Using the splint in this indication seems to be redundant from the medical and economic point of view. On the other hand, temporary splinting after surgery does not cause worsening of the results of surgical treatment. Key words: carpal tunnel syndrome – surgical decompression – immobilisation – splints – postoperative care – electromyography The authors declare they have no potential conflicts of interest concerning drugs, products, or services used in the study. The Editorial Board declares that the manuscript met the ICMJE “uniform requirements” for biomedical papers., and A. Fibír, R. Čáp, J. Vaněk
Aldosterone blocker Spironolactone has antiinflammatory, antiproliferative and antioxidative effects, that is why pathogenetically it is expedient to use it in complex therapy of rheumatoid arthritis. Material and methods: 46 patients with RA took 2550 mg/day of Spironolactone during 12 months as an addition to standard therapy, the comparison group consisted of 47 patients that got only standard therapy, all the patients were fully examined prior and post the treatment. Results: complex RA therapy leads to improved VAS, HAQ, the antioxidative potential index F; decreased blood concentrations of TNFα, ICAM1, FGF and VEGF in contrast to standard therapy. Complex therapy made reduced the DAS 28 more > 0.6. Conclusions: applying of Spironolactone in complex therapy of rheumatoid arthritis contributes to more pronounced improvement in indices of articular syndrome and patients life quality, reduce of antiinflammatory, antiproliferative and angiogenic cytokines, and more effectively reduces the activity of the disease comparing to standard therapy., Elena Komarova, Borys Rebrov, and Literatura
Background: Few data are available on the specific care giving-related problems of stroke patient’s caregivers and factors that influence the burden of these caregivers. Aim: To study the influences of the active rehabilitation process on anxiety, depression, care burden and perceived social support level of stroke patients caregivers. Design: A prospective clinical trial. Setting: Patients and caregivers entering a rehabilitation program at a university hospital in Turkey. Populations: Ninety patients with a first episode of stroke and 90 caregivers responsible for their care were recruited for our study. Methods: Patients and caregivers were assessed before and after the active rehabilitation process. The functional disability level of the patients was assessed by Functional Independence Measure (FIM). The Beck Anxiety Scale (BAS) and the Beck Depression Scale (BDS) were used for anxiety and depression assessment, the Zarit Care Burden Scale (ZCBS) for care burden assessment and the Multi-Dimensional Scale of Perceived Social Support (MDSPSS) for perceived social support assessment. Results: A statistically significant rise is observed in the special person sub-assessment of MDSPSS in both female and male caregivers. Also, a significant decrease in care burden, anxiety and depression levels of caregivers was noted after the rehabilitation program (p < 0.05). Conclusion: Caregivers accept the rehabilitation period as important social support in addition to the support provided by family and friends. Also, our positive results were associated with an improvement in the patients’ functional level and an increase in the acquisition of knowledge and skill required of caregivers in order to provide care during rehabilitation. Clinical Rehabilitation Impact: The rehabilitation team should be aware of the fact that the perceived care burden may be greater due to the lack of knowledge concerning available resources and due to the inability to cope with stress effectively. and A.
