Objective: to analyze main clinical-statistical indices of spine and spinal cord injuries (SCI) and system of organization of medical aid to the victims. We conducted a cohort retrospective study of materials of medical institutions of Tashkent city. Medical records of patients and records of forensic examinations were analyzed. 242 cases of spinal cord injuries were studied. Over the last 10 years in Tashkent the frequency of SCI have been tend to increase, males and working-age people are dominated. In 60.9% of cases the injury was associated, in 36.2% - isolated, and in 2.9% - combined. Fallings from height and road traffic accidents were the main reasons for getting SCI. Lesions at cervical level registered in 45% of cases, thoracic level - 27.3%, at lumbar level - 27.7%, respectively. According to ASIA/IMSOP, full injuries were noted in 40.1% of cases, incomplete injures were in 59.9%. Cíl studie: Cílem studie bylo zjištění vztahů mezi manifestací tinnitu, nálezem na sluchových evokovaných potenciálech a genetickým pozadím u receptoru pro gamaaminomáselnou kyselinu typu A (GABA(A) receptor), podporující desinhibiční hypotézu vzniku tinnitu. Soubor a metodika: Bylo vyšetřeno 131 pacientů z hlediska sluchové ztráty, provedena kvantifikace tinnitu, sluchové evokované potenciály střední latence (MLR) a kmenové sluchové evokované potenciály (BAEP) a dále stanovení genotypu (CA)n repetitivní sekvence pro beta‑3 podjednotku GABA(A) receptoru. Následně byly hledány vztahy mezi jednotlivými výsledky a manifestací tinnitu. Výsledky: Byla nalezena korelace tinnitus skóre s amplitudovým poměrem vln V/III v BAEP (R = 0,22, p < 0,001) a s průměrným sluchovým prahem (R = 0,22, p = 0,17). Rovněž byla nalezena korelace tinnitus skóre s amplitudou vlny PA v MLR (R = 0,31–0,37; p < 0,001). Výsledky MLR neukázaly žádný vztah k průměrnému sluchovému prahu. U skupiny s kratší anamnézou tinnitu (méně než devět měsíců) byl prokázán rozdíl v manifestaci tinnitu na genotypu pro (CA)n repetitivní sekvenci genu pro beta‑3 podjednotku GABA(A) receptoru (p = 0,002). Tento výsledek byl rovněž konzistentní s rozložením amplitudy vlny PA v dané subpopulaci. Závěr: Tyto výsledky svědčí o existenci dvou hlavních regulačních mechanizmů vzniku tinnitu: první, který je závislý na velikosti sluchové ztráty, je na úrovni mozkového kmene, zatímco druhý je na úrovni korové s možnou souvislostí s genotypem (CA)n repetitivní sekvence pro beta‑3 podjednotku GABA(A) receptoru., Study aim:Study objective was to explore associations between manifestation of tinnitus, auditory evoked potentials and genetic background of gamma‑aminobutyric acid type A (GABA(A) receptors) to support the disinhibited feedback hypothesis of tinnitus generation. Materials and methods: A population of 131 patients was assessed for severity of hearing loss, quantification of tinnitus, mid‑latency responses (MLR) and brainstem auditory evoked potentials (BAEP), and (CA)n tandem repeat polymorphism in GABA(A) Beta‑3 subunit gene to establish any correlation with manifestation of tinnitus. Results: It was observed that tinnitus score correlates with V/III amplitude ratio in BAEP (R = 0.22, p < 0.001) and with mean pure tone audiometry (PTA) threshold (R = 0.22, p = 0.017). Analysis of the MLR results showed a significant correlation between the PA wave amplitude and the tinnitus score (R = 0.31–0.37; p < 0.001). MLR result analysis showed no statistically significant correlation between the wave amplitudes and the mean auditory threshold. An analysis of a subgroup with shorter clinical history (less than nine months) revealed a statistically significant difference in the tinnitus score in relation to the genotype of (CA)n tandem repeat of the GABRß3 receptor subunit gene (p = 0.002). This result was also consistent with the distribution of the PA wave amplitude in the given subpopulation. Conclusion: Our findings indicate existence of two main regulatory mechanisms of tinnitus generation: first, the brainstem mechanism is dependent on the severity of the hearing loss; second, the cortical mechanism is likely to be dependent on the genotype of (CA)n tandem repeat in GABA(A) beta‑3 subunit gene., and J. Introduction: Ankylosing spondylitis (AS) is an inflammatory rheumatic disease characterized by the development of osteoproductive changes in the spine which could possibly result in ankylosis. Treatment with tumour necrosis factor alpha (TNFα) inhibitors has proved to be an important step forward in the treatment of this disease, but for the time being it is not clear whether it favourably influences radiographic progression of the disease. Vascular endothelial growth factor most probably plays a role in the development of osteoproductive changes and recently its predictive influence on radiographic progression has been demonstrated. Bone morphogenic protein 2 (BMP-2) participates in the regulation of bone proliferation and its increased serum level has been demonstrated in patients with advanced AS and correlated with the degree of radiographic changes. Aim: The study aims to evaluate the VEGF and BMP-2 levels in patients with ankylosing spondylitis and how these levels relate to the concurrent treatment with TNFα inhibitors. Methods: Sera were evaluated from patients at the Rheumatologic Clinic of the Hradec Králové Faculty Hospital who fulfilled the modified New York Criteria for AS (n = 55). In these patients, the parameters of the activity of the disease (BASDAI = Bath Ankylosing Spondylitis Disease Activity Index, CRP = C-reactive protein) and the concurrent therapy (TNFα inhibitors, n = 21, vs. non-anti TNFα, n = 34) were recorded. The levels of VEGF and BMP-2 were analyzed using the ELISA method. Results: In patients treated with TNFα inhibitors, a significantly lower VEGF level was found when compared to untreated patients (140.3 (109.4; 262.2) vs. 261 (172.4; 396.6) pg/ml; p = 0.02). No difference was found between BMP-2 levels in both groups (treated vs. untreated patients) (254.8 (2301; 267.3) vs. 261.1 (248.6; 273.5) pg/ml; p = 0.24). A correlation analysis did not reveal any relationship between VEG F and BMP-2 (r = 0.057; p = 0.68). Serum levels of VEGF correlated with serum levels of CRP (r = 0.56; p = 0.00001) and the BASDAI value (r = 0.33; p = 0.015). Conclusion: Significantly lower VEGF levels were found in patients treated with TNFα inhibitors versus the untreated patients. These findings are in harmony with some hitherto published analyses and may give evidence of a favourable effect of TNFα inhibitors on radiographic progression. Kontinuální tvorba výpotků jakékoliv etiologie zásadním způsobem ovlivňuje kvalitu života nemocných. Akumulace tekutiny v pleurální dutině postupně komprimuje plicní parenchym. Prvním příznakem bývá progrese námahové dušnosti, později se přidává i klidová dušnost. S nárůstem výpotku dochází ke kompresi plicního parenchymu s rozvojem plicní atelektázy. Nemocný je ohrožen vznikem zánětového onemocnění plic v nevzdušném plicním parenchymu. U diagnózy maligního onemocnění je riziko zánětových komplikací výrazně vyšší vzhledem k předchozí chemoterapii či radioterapii, která zásadním způsobem snižuje imunitu. Opakovaná tvorba výpotku se výrazně podílí na rozvoji terminálního stavu nádorové kachexie. Efektivní metodou léčby je chemická pleurodéza, která se užívá u vybraného spektra nemocných. Principem je navození aseptické pleuritidy. Nejefektivnější cestou aplikace léčiva je videotorakoskopie. V souboru bylo 47 nemocných, 29 (61,7 %) mužů a 18 (38,3 %) žen, s cytologicky prokázaným recidivujícím maligním pleurálním výpotkem. Věkové rozmezí 42–80 let, Ø věk 65 let. Cílem práce bylo objektivizovat průběh celkových a lokálních zánětových změn po talkáži. Efekt léčby nebyl ovlivněn základním typem nádorového onemocnění. U obou skupin nebyly významně odlišné pooperační komplikace, což dokládá bezpečnost zvolené metody a podporuje správně zvolený algoritmus léčby., Continuous pleural effusion production of any aetiology can significantly affect the quality of patients life. Chronic effusion accumulation in the pleural cavity can lead to lung parenchyma compression. The first symptom of this entity is a progressive exertional dyspnea, which can later turn in dyspnea at rest. With the increase of the pleural effusion volume, pulmonary atelectasis can develop due to chronic parenchycha compression. The patient is at risk of inflamatory complications from the territory of non-ventilated parenchyma. Patients with a diagnosis of malignant disease have this risk considerably higher due to their previous chemotherapy or radiation, which substantially affect the immunity system. Repeated pleural effusion formation can significantly take part in the development of the terminal status of cancer cachexia. An effective method of palliative treatment is a chemical pleurodesis, which is used in selected patients. The principle is inducing aseptic inflammation. The most effective way of application is via videothoracoscopy. In a group of 47 patients, there were 29 (61.7 %) men and 18 (38.3 %) women with cytologically diagnosed recurrent malignant pleural. The age ranged between 42 and 80 with average age of 65 years. The aim was to assess the course of local and systemic inflammatory changes after talc application. The effect of treatment was not influenced by the type of malignancy . In both groups there was no significant difference in postoperative complications. Cil: Klasická neuralgie trigeminu (CTN) je charakterizována epizodickou ostrou neuropatickou obličejovou bolestí a s výjimkou přítomnosti neurovaskulárního konfliktu (NVC) nebyla v našem souboru podmíněna jinou známou abnormitou či patologií. V poslední době je diskutovaná atrofie trigeminu na straně bolesti. Našim cílem bylo ověřit tyto nálezy a vztáhnout je k dalším klinickým datům. <p align="">Soubor a metodika: Pomocí magnetické rezonance (MR) jsme změřili objemy trojklaného nervu na bolestí postižené a nepostižené straně a vyhodnotili výskyt a typ NVC u 45 nemocných s CTN. Následně jsme provedli korelace mezi objemy postiženého nervu, NVC, klinickými a demografickými daty (trvání nemoci, věk nemocného). <p align="">Výsledky: NVC jsme zjistili u 69 % postižených nervů, 51 % nepostižených nervů a u 42 % nemocných oboustranně. Signifikantně snížený objem trojklaného nervu byl zjištěn na bolestí postižené straně (p < 0,001). Žádné korelace jsme nezaznamenali mezi objemem postiženého nervu, přítomností a typem NVC ani mezi klinickými a demografickými daty. <p align="">Závěr: Naše výsledky naznačují, že NVC může být sice spoušťovým mechanizmem CTN u vnímavých subjektů, ale není spolehlivým ukazatelem nemoci. Snížený objem trojklaného nervu se objevuje nezávisle na NVC a odráží spíše predisponující asymetrii trigeminu než vzniklou atrofii., Aim: Classic trigeminal neuralgia (CTN) is characterized by episodic neuropathic facial pain that, with exception of neurovascular conflict (NVC), was not attributed to any underlying disorder in our sample. Using magnetic resonance imaging, we evaluated volumes of affected and non‑affected nerves and presence and type of NVC in a large group of CTN subjects prior to gamma knife treatment. <p align="">Design: Correlation between affected nerve volume and NVC, clinical data and demographic characteristics (duration of pain, age) were explored in 45 subjects diagnosed with CTN. <p align="">Results: NVC was detected in 69% of affected nerves, 51% of non‑affected nerves, and in 42% of subjects bilaterally. Lower trigeminal nerve volume was detected on the affected side (p < 0.001), however, no correlation between affected nerve volume and the presence and type NVC or demographic data was detected. <p align="">Conclusion: Our results suggest that NVC may trigger CTN in susceptible subjects but is not a reliable disease marker. Východiska: Pohybový projev zdravého organismu je charakterizován vysokou mírou variability provedení pohybového úkonu. Za situace vzniklé poruchy řízení, zejména v důsledku neurologických onemocnění, dochází v různé míře k fixnímu pohybového projevu, projevujícímu se omezením schopnosti pružné reakce na změnu zevních či vnitřních podmínek, působících v průběhu provádění cíleně zaměřené pohybové akce. Cíl: Zjistit rozdíly ve svalové aktivitě a variabilitě svalových souher akrálních a proximálních svalů během dosahu a úchopu u zdravých jedinců a u pacientů s hemiparézou. Metody: Experimentální soubor tvořilo 24 probandů po prodělané ischemické cévní mozkové příhodě (iktu) s pravostrannou hemiparézou. Kontrolní soubor tvořilo 30 zdravých probandů. Svalová aktivita byla zaznamenána pomocí povrchové elektromyografie v průběhu dosahu, úchopu a oddálení válcového a kulového předmětu. Rozdíly v hodnotách plochy pod křivkou záznamu jednotlivých svalů mezi experimentálním a kontrolním souborem byly statisticky vyhodnoceny pomocí T-testu pro nezávislé skupiny. Vztah mezi aktivitou svalů předloktí a svalů pletence ramenního byl hodnocen pomocí Spearmanova korelačního koeficientu pro neparametrické hodnoty. Výsledky: Byly zjištěny významné rozdíly v míře a počtu statisticky významných korelací akrálních a pletencových svalů. U souboru probandů s hemiparézou byla zjištěna statisticky významně nižší aktivita m. pectoralis major a naopak vyšší aktivita m. trapezius. Současně převažovala vysoká korelace m. pectoralis major a extenzorů zápěstí a prstů, na rozdíl od souboru zdravých probandů, kdy se vzájemné korelace akrálních a proximálních svalových skupin objevovaly nahodile, v menší míře a četnosti. Závěry: Výsledky ukazují na snížení variability svalové aktivity u pacientů s hemiparézou v průběhu úchopové funkce. Náhradou nedostatečné funkčnosti akrálních svalových skupin se stává relativně fixní „ko-aktivace“ s vybranými proximálními svalovými skupinami., Background: Movement of a healthy organism is characterized by a high degree of variability performance of a movement task. In a situation of a motor control lesion, mainly due to neurological disease, occurs in varying degrees to a fixed musculoskeletal manifestation. ere are manifesting in a limiting responsiveness to changing external or internal conditions during the task oriented physical action. Objective: To determine differences in muscle activity and variability between a muscle synergy of acral and proximal muscles during reach and grip in healthy subjects and stroke patients. Methods: e experimental group consisted of 24 probands aer ischemic stroke (ictus) with right-sided hemiparesis. e control group consisted of 30 healthy subjects. Muscle activity was recorded using surface electromyography during the performation of selected types of grips of cylindric and spheric objects. e differences in the area under the curve record of individual muscles between the experimental and control group were statistically evaluated using the t-test for independent groups. e relationship between the activity of the muscles of the forearm and shoulder girdle muscles was assessed using Spearman‘s correlation coefficient for non-parametric values . Results: ere were the significant differences in the size and number of statistically significant correlations distal and proximal arm muscles. In stroke group was found significantly lower activity m. pectoralis major and higher activity m. trapezius. At the same time the prevailing high correlation m. pectoralis major and extensor wrist and fingers, unlike the group of healthy subjectss, when the correlation distal and proximal muscle groups appeared at random, in a lesser extent and frequency. Conclusions: e results indicate a reduction in variability of muscle activity during grasp function of stroke patients. Cíl: Cílem práce je poukázal na možnosLi diagnosLiky sakroileitidy jako jedné z prvotních známek chronického zánětlivého onemocnění skeletu, typu ankylozující spondylitidy (AS) či jiného onemocnění z kategorie séronegativních spondylartritid (SpA). Nezpochybnitelnou roli v diagnostice časných stadií hraje magnetická rezonance (MR), která je schopna časně zobrazit přítomnost revmatoidního zánětu v oblasti sakroiliakálních kloubů (SI), a to v tzv. preradiologickém období, kdy ostatní diagnostické metody (CT a RTG) zatím žádné změny nedetekují. Předpokládáme, že v určitých stadiích onemocnění a také v závislosti na věku vyšetřovaných pacientů ztrácí MR svou výsadní roli a může být zdrojem falešně negativních nálezů, a proto se snažíme navrhnout optimálnější protokol vyšetření v závislostí na věku. Metodika: Zkoumali jsme 82 MR vyšetření SI kloubů, zaměřili jsme se na přítomnost edému kostní dřeně v subchondrální kosti jako známky aktivního zánětu a zároveň na přítomnost kostních změn. V případě pozitivního nálezu edému kostní dřeně jsme dále hodnotili rozsah edému, rozdělili jsme do čtyř kategorií: O - žádný edém, 1 - edém postihující méně než 1/3 rozsahu subchondrální kosti, 2 - edém postihující 1/3-2/3 rozsahu subchondrální kostí, 3 - rozsáhlý edém postihující více než 2/3 rozsahu subchondrální kosti. Hodnotili jsme oba SI klouby, každý samostatně a započítávali jsme vždy vyšší hodnotu. Výsledky: Naše zkušenosti ukazují, že MR je suverénní diagnostickou metodou potvrzující přítomnost revmatoidního zánětu v oblasti SI kloubů v časných stadiích zánětlivého postižení a především v nižších věkových kategoriích. Potvrdili jsme náš předpoklad, že MR ve věkové kategorii nad 35 let (35+) již ztrácí svou suverénní roli v diagnostice SpA a může být zdrojem falešně negativního nálezu, protože přítomnost kostních změn při absenci edému kostní dřeně může vést hodnotícího radiologa falešně k diagnóze artrózy. Prokázali jsme, že ve věkové kategorii 35+ je edém subchondrální kosti málo častým nálezem, a pokud se vyskytuje, je spíše menšího rozsahu. Naopak pravidelně se objevují kostní změny a navíc se v této věkové kategorii již běžně objevují artrotické změny na kloubech, které působí diferenciálně diagnostické rozpaky. Závěr: Na základě našich zkušeností navrhujeme u pacientů s podezřením na sakroileitidu ve věkové kategorii 35+ používat jako metodu první volby nativní CT SI kloubů., Aim: To refer about the posibility of diagnosis of sacroiliitis, which is one of the first signs of chronical inflammatory disease of the skeleton, the type ankylosing spondylitis (AS) or other diseases from the group of seronegative spondyloarthritis (SpA). An important role in the diagnosis plays magnetics resonance (MR), which can assess early the presence of rheumatoid inflammations in the sacroiliac joints (SI), primarily in the so called preradiological period, even when other metods (CT, RTG) do not detect any changes. We supposed, that in some stages of disease and according to the age of examined people MR loses its sovereign role in diagnosis and may cause false-negative findings and therefore we try to propose more optimal protocol testing depending on the age of the pacient. Methods: We evaluated 82 MR examinations of SI, focused on the presence of bone marrow edema in subchondral bone as evidence of active inflammation and also the presence of bone changes. In the case of a positive finding of bone marrow edema we evaluated its extent. For this purpose we devided findings into 4 categories : 0 - no edema, 1 - edema affecting less than 1/3 extent of subchondral bone, 2 - edema affecting 1/3 till 2/3 extent of subchondral bone, 3 - edema affecting more than 2/3 of subchondral bone. We evaluated both SI joints separately and we always counted the higher value. Results: Our experience shows that MR is a sovereign diagnosis method which confirms the presence of rheumatoid inflammation in the SI in early stage of inflammatory diseases and especially at lower ages. We confirmed our assumption that MR in the age categories over 35 years loses its sovereign role in diagnosis SpA and can be a source of false-negative results, because the presence of bone changes in the absence of bone marrow edema can falsely lead the radiologist to diagnose arthrosis. We have shown that in the age category over 35 years subchondral bone edema is a rather rare finding, and if present at all, it is of minor extent. On the contrary, bone changes appear regularly and moreover in this age group arthritic changes commonly occur at the joints, causing differential diagnostic embarrassment. Cíle: Tato studie měla za cíl zjištění diagnostické přesnosti české verze souboru tří otázek ke stanovení diagnózy syndromu neklidných nohou (Restless Legs Syndrome, RLS), které se používají v epidemiologických studiích prováděných dotazníkovou cestou bez asistence investigátora. Originální anglická verze dotazníku byla publikována v roce 2003 Mezinárodní skupinou pro výzkum RLS (International RLS study group) společně s hlavními a vedlejšími diagnostickými kritérii pro klinické nebo telefonické stanovení diagnózy RLS. Soubor a metody: Sada otázek byla přeložena do češtiny a pak zaslepeně zpět do angličtiny, aby nedošlo k posunu významu. Ženy přicházející ke kontrole na Gynekologickou kliniku 1. LF UK a VFN v Praze v 34.–38. týdnu těhotenství byly požádány o vyplnění výše zmíněného dotazníku. Následně lékař vycvičený v diagnostice RLS pacientky kontaktoval na jimi zadaném telefonním čísle k standardnímu diagnostickému rozhovoru k ověření či vyloučení diagnózy RLS. Výsledky obou metod byly porovnávány a byla stanovena senzitivita, specificita, pozitivní a negativní prediktivní hodnoty. Výsledky: Dotazníkovou studii podstoupilo celkem 776 těhotných žen, ve věku 18–49 let. Celkem bylo validováno 199 dotazníků, u 65 osobní pohovor potvrdil diagnózu RLS. Pozitivita všech tří otázek má 89% specificitu, ale jen přibližně 78% pozitivní prediktivní hodnotu a senzitivitu. Za použití pouze první otázky je senzitivita 94 % a negativní prediktivní hodnota 96 %. Ze všech vyšetřených žen na všechny tři otázky odpovědělo pozitivně 28,0 % (95% konfidenční interval je 24,9–31,2 %). Závěr: Studie validovala českou verzi otázek a prokázala jejich relativně vysokou diagnostickou přesnost., Aims: The aim of this study was to estimate diagnostic accuracy of the Czech version of a self-administered minimum question set to diagnose the Restless Legs Syndrome (RLS). The wording of the three questions was published in 2003 by the International RLS study group together with the diagnostic criteria. Material and methods: The question set was translated into Czech, followed by blind translation back to English. Women in 36th–38th week of pregnancy were asked to complete the questionnaire. Subsequently, the women were contacted over a telephone by a physician trained in diagnosing RLS and a standard telephone interview to establish or exclude the diagnosis of RLS was performed. The results of both methods were compared to estimate sensitivity, specificity, positive and negative predictive values of the minimum question set. Results: We surveyed 776 pregnant women (18–49 years old) who came to a prenatal outpatient clinic to consult an obstetrician at the third trimester (36th–38th week of pregnancy). In total, 199 questionnaires were validated; personal interview confirmed the diagnosis of RLS in 65 of them. Positive response to all three questions yields specificity of 89% but only about 78% sensitivity and positive predictive value. Positive response to the first question only has sensitivity of 94% and negative predictive value of 96%. The prevalence of RLS during pregnancy estimated using the three-question questionnaire was 28% (95% confidence interval from 24.9 to 31.2%). Úvod: Sexuální dysfunkce jsou běžným následkem spinálního traumatu. Léčba sexuálních dysfunkcí je důležitou součástí rehabilitace a resocializace u těchto pacientů a napomáhá ke zvyšování kvality života. Existence erektilní dysfunkce (ED) závisí na výšce poraněného segmentu míchy a rozsahu léze. Terapie ED poskytuje v současnosti široké spektrum možností a závisí na preferenci pacienta. Perorální inhibitory fosfodiesterázy 5. typu (I‑PDE5) jsou zlatým standardem v terapii ED u mužů po spinálním traumatu zejména pro svou jednoduchou aplikaci a prokázaný efekt. Materiál a metodika: Zjišťovali jsme vliv užívání I‑PDE5 na kvalitu erekce, frekvenci pohlavního styku a nekoitálních sexuálních aktivit u mužů po spinálním traumatu. Soubor tvořilo 67 pacientů po spinálním traumatu ve věku od 15 do 60 let (průměrný věk 36,5 let) dispenzarizovaných v androurologické ambulanci FN u sv. Anny v Brně. K získání dat byl použit standardizovaný dotazník IIEF‑5 a nevalidizovaný dotazník vlastní konstrukce, k vyhodnocení deskriptivní a induktivní statistika s využitím Wilcoxonova párového testu (p < 0,05). Návratnost dotazníků byla 44 % (30 dotazníků). Výsledky a závěr: Téměř všichni muži po spinálním traumatu našeho souboru (93 %) trpí erektilní dysfunkcí. Pouze necelá polovina z nich (43 %) užívala I‑PDE5. Prokázali jsme signifikantní závislost mezi užíváním I‑PDE5 a kvalitou erekce (p < 0,01). Uživatelé I‑PDE5 vykazují signifikantně vyšší frekvenci pohlavního styku (p < 0,01) a více praktikují nekoitální sexuální aktivitu. Treatment of sexual dysfunction is an important part of rehabilitation and resocialization of these patients and helps to improve their quality of life. Development of erectile dysfunction (ED) depends on the height of the injured segment of the spinal cord and the extent of the lesion. ED therapy currently provides a wide range of options, depending on the preference of the patient. Oral PDE5 inhibitors (PDE5-I) are the gold standard in the treatment of ED in men after spinal trauma, especially for its easy application and proven effect. Material and methods: We examined the influence of the use of PDE5-I on erection quality and frequency of sexual intercourse and non‑coital sexual activity in men after spinal trauma. The group consisted of 67 patients after spinal trauma aged 15–60 years (mean age 36.5 years) followed up in the androurologic ambulance of University Hospital St. Anne. IIEF‑5 questionnaire and custom design were used. The autors used descriptive and inductive statistics using the Wilcoxon paired test (p < 0.05). Rate of return was 44% (30 questionnaires). We found a significant correlation between the use of PDE5-I and erection quality (p < 0.01). Mezi nejnáročnější součásti klinického neurologického vyšetření patří vyšetření citlivosti vyžadující detailní edukaci pacienta a jeho následnou spolupráci. Při správné metodice vyšetření a jeho exaktním hodnocení lze často odhalit i diskrétní poruchy citlivosti, kterých si pacient nemusí být vědom nebo není schopen je validně popsat. Našim cílem je podat přehled nejčastěji používaných metod vyšetření běžně hodnocených senzitivních a algických modalit (taktilní, vibrační a termické čití, termoalgické čití, hodnocení hluboké tlakové bolesti a dále mechanická algická citlivost a hodnocení dynamické mechanické alodynie), včetně interpretace jejich nálezů a využitelných normativních dat (pokud jsou dostupná), a to od nejjednodušších kvalitativních a semikvantitativních metod po složitější metody kvantitativní. Kvalitativní či semikvantitativní metody jsou v běžné klinické praxi využívány podstatně častěji. Jejich výhodou je rychlost a metodická nenáročnost testování a široká dostupnost používaných pomůcek. Nevýhodou je naopak skutečnost, že poskytují pouze orientační informaci o citlivosti pro danou modalitu a spolehlivě detekují většinou až pokročilé poruchy senzitivity. Absence kvantifikace limituje možnosti těchto metod v longitudinálním sledování pacientů i při srovnání nálezů mezi pacienty či jejich skupinami. Kvantitativní metody souhrnně označované termínem kvantitativní testování senzitivity (Quantitative Sensory Testing, QST) jsou časově a metodicky náročnější a vyžadují použití specializovaných pomůcek. Jejich předností jsou kvantifikované výstupy s lepší diagnostickou validitou, které potenciálně diagnostikují i klinicky méně vyjádřené abnormity a jsou lépe využitelné např. pro longitudinální sledování. Tyto metody se častěji uplatňují v klinických studiích a jsou nezastupitelné v diagnostice neuropatie tenkých vláken., Sensory examination represents one of the most complicated parts of clinical neurological examination that requires detailed instructions to patients and their attentive cooperation. Using correct methodology of testing and precise evaluation of the results, sensory tests frequently disclose even very fine sensory abnormities which patients may not be able to describe or even may not be aware of. The aim of this paper is to provide an overview of methods most frequently used to evaluate common sensory and pain modalities (touch, vibration and thermal sensation, thermoalgic sensation, deep pain pressure, mechanical detection threshold and sensitivity, evaluation of dynamic mechanical allodynia), including interpretation of the findings and recommended normal values (if applicable). In each of the tested modalities, most commonly used methods are described from the easiest qualitative and semiquantitative methods (usually more frequently used in clinical practice) up to more complicated quantitative tests. Main advantages of qualitative and semiquantitative methods include their low methodological and time burden and high availability of the equipment used. Quantitative sensory testing (QST) is more demanding and requires the use of specialized, not widely available, equipment. These methods, however, provide quantified results with better diagnostic validity, may disclose even less pronounced sensory abnormities and are more useful in long-term follow-up as well as in inter-individual comparisons, both at individual and group level.
