Úvod: Fyloidní tumor je velmi zřídka se vyskytující nádor prsu. Vyskytuje se pouze v 1 % případů všech nádorů prsu. Diagnostika fyloidního tumoru může být obtížná vzhledem k malému počtu případů. Léčba fyloidního tumoru je vždy chirurgická. Metody: V letech 2004−2013 jsme na Chirurgické klinice FN Plzeň operovali 12 pacientek s fyloidním nádorem prsu z celkového počtu 1564 operací pro nádory prsu (0,8 %). Hodnotili jsme věk, biologické chování nádoru v závislosti na velikosti nádoru a délce trvání, vzdálené metastázy, terapii a přežití. Výsledky: Průměrný věk v době operace byl 50 let (26−84), délka trvání nemoci do operačního řešení byla 1 měsíc až 10 let. Velikost tumoru se pohybovala v rozmezí 2−29 cm, tumory menší než 5 cm byly vždy benigní. 7x pro benigní fyloidní nádor byla provedena excize tumoru. 5x byl diagnostikován maligní fyloidní tumor a byla vždy provedena mastektomie, ve 3 případech s exenterací axily, kdy uzliny byly vždy benigní. V jednom případě po mastektomii následovala radioterapie pro dosahování nádoru do okraje resekátu, ostatní pacientky byly pouze dispenzarizovány. 2 pacientky měly generalizaci do plic diagnostikovanou 5 a 10 měsíců po operaci prsu. 1 pacientka s generalizací zemřela, ostatní žijí bez lokální recidivy onemocnění. Medián přežití je 52 měsíců, interval bez nemoci je 50 měsíců. Závěr: Z našeho souboru vyplývá, že pokud je fyloidní nádor diagnostikován včas, jedná se téměř výhradně o benigní variantu, při déle trvající anamnéze a s rostoucí velikostí tumoru nádory malignizují. Chirurgická léčba je dostačující i v případě maligních forem. Výkon na prsu není třeba doplňovat o exenteraci axily., Introduction: Phyllodes tumour is a breast tumour occurring very rarely. It accounts for only in 1% of all cases of breast tumour. The diagnosis of phyllodes tumours can be difficult in consideration of the small number of cases. Treatment of phyllodes tumours is always surgical. Methods: In 2004–2013, we operated on twelve female patients with phyllodes tumours out of the total number of 1564 surgeries for breast tumours (0.8%) at the Department of Surgery at Teaching Hospital in Pilsen. We evaluated the age, the biological behaviour of the tumour depending on the tumour size and duration, the distant metastases, therapy and survival. Results: The average age at the time of surgery was fifty years (26−84), the duration of disease to the surgical solution ranged from one month to ten years. Tumour size was in the range of two to twenty-nine centimetres, tumours measuring less than five centimetres were always benign. Tumour excision for benign phyllodes tumour was performed seven times. Malignant phyllodes tumour was diagnosed five times with mastectomy performed in each case, and the axilla was exenterated in three cases where nodes were benign in each of them. In one case, mastectomy was followed by radiotherapy because the tumour reached the edge of the resected part; the other patients were only monitored. In two patients, tumour spreading into the lungs was diagnosed at five to ten months after breast surgery. One patient with generalized disease died, the other ones live with no local recurrence of this disease. Median survival is fifty-two months; the disease-free interval is fifty months. Conclusion: The results show that if phyllodes tumour is diagnosed in time, it is almost exclusively benign. If the case history is longer and the tumour is growing, the likelihood of malignancy increases. Surgical treatment is also sufficient in the case of malignant forms. Úvod: Z klinického hlediska je zjevné, že karcinom rekta a karcinom kolon jsou ve svém průběhu a léčbě odlišné nosologické jednotky. Cílem bylo analyzovat a objasnit rozdíly mezi chováním jaterních metastáz karcinomu kolon a karcinomu rekta. Studie těchto faktorů je důležitá pro stanovení přesné prognózy a indikace jejich co nejefektivnější terapie a léčby karcinomu kolon a karcinomu rekta jako systémového onemocnění. Metoda: Do našeho souboru bylo zařazeno 223 pacientů s metastatickým postižením jater kolorektálním karcinomem, kteří byli operováni na Chirurgické klinice LF UK a FN Plzeň od 1. 1. 2006 do 31. 1. 2012. Z celkového počtu 223 jedinců bylo 145 (65 %) mužů a 78 (35 %) žen. Jednalo se celkem o 275 výkonů. Resekční výkon byl proveden u 177 pacientů a ošetření pomocí radiofrekvenční ablace (RFA) v celkem 98 případech. Soubor byl rozdělen do kategorií podle lokalizace primárního tumoru na C (kolon) čítající 58 pacientů, S (c. sigmoideum) čítající 61 pacientů a R (rektum) čítající celkem 101 pacientů. Analýza významnosti jednotlivých studovaných parametrů (věk, pohlaví, TNM klasifikace, grading, typ operačního výkonu) byla provedena pomocí ANOVA testu. Celkové přežití (OS), bezpříznakové přežití (DFI) nebo období bez známek onemocnění (NED) bylo hodnoceno za pomoci Kaplan-Meierových křivek, které byly porovnávány pomocí Log-Rank a Wilcoxon testů. Výsledky: Z hlediska porovnávání primárního origa jaterních metastáz kolorektálního karcinomu bez ohledu na jejich ošetření (resekce i RFA) z naší studie vyplývá, že metastázy karcinomu rekta mají statisticky významně časnější recidivu (kratší NED/DFI). V případě ostatních faktorů byl pro prognózu časné recidivy statisticky významný lokálně pokročilý nález u primárního nádoru kolon a sigmoidea, dále podstoupená R2 resekce jaterních metastáz a pozitivita uzlinových metastáz v případě primárního nádoru kolon a sigmoidea. Dále jsme prokázali, že u nemocných s primárním postižením rekta nemá na DFI po resekci metastáz jater vliv pozitivita uzlinových metastáz ani lokální pokročilost primárního nádoru. Další studované faktory (časový odstup diagnózy orgánových metastáz od primární operace, grading, pohlaví či věk) nebyly pro prognózu OS i DFI (souhrnně u kolorektálního karcinomu) prokázány jako statisticky významné. Závěr: Z naší studie je zřejmé, že u karcinomu rekta a karcinomu kolon lze ve vztahu k jejich jaterním metastázám předpokládat různé chování se specifickými prognostickými faktory. Tyto rozdíly nejsou dosud plně objasněny a vyžadují další zkoumání a rozdělení, a to nejen na základě histopatologických, imunohistochemických a klinických faktorů, ale i molekulárně biologických parametrů. Klíčová slova: metastázy karcinomu kolon − metastázy karcinomu rekta − prognostické faktory − celkové přežití – jaterní metastázy, Introduction: From the clinical point of view, rectal cancer and colon cancer are clearly different nosological units in their progress and treatment. The aim of this study was to analyse and clarify the differences between the behaviour of liver metastases from colon and rectal cancer. The study of these factors is important for determining an accurate prognosis and indication of the most effective surgical therapy and oncologic treatment of colon and rectal cancer as a systemic disease. Method: 223 patients with metastatic disease of colorectal carcinoma operated at the Department of Surgery, University Hospital in Pilsen between January 1, 2006 and January 31, 2012 were included in our study. The group of patients comprised 145 men (65%) and 117 women (35%). 275 operations were performed. Resection was done in 177 patients and radiofrequency ablation (RFA) in the total of 98 cases. Our sample was divided into 3 categories according to the location of the primary tumor to C (colon), comprising 58 patients, S (c. sigmoideum) in 61 patients, and R (rectum), comprising 101 patients. Significance analysis of the studied factors (age, gender, staging [TNM classification], grading, presence of mucinous carcinoma, type of operation) was performed using ANOVA test. Overall survival (OS), disease-free interval (DFI) or no evidence of disease (NED) were estimated using Kaplan-Meier curves, which were compared with the log-rank and Wilcoxon tests. Results: As regards the comparison of primary origin of colorectal metastases in liver regardless of their treatment (resection and RFA), our study indicated that rectal liver metastases showed a significantly earlier recurrence than colon liver metastases (shorter NED/DFI). Among other factors, a locally advanced finding, further R2 resection of liver metastases and positivity of lymph node metastases were statistically significant for the prognosis of an early recurrence of the primary colon and sigmoid tumor. Furthermore, we proved that in patients with primary rectal carcinoma, DFI (after the resection of liver metastases) was not influenced by the positivity of lymph node metastases of primary tumor or locally advanced primary tumor. The other factors studied (time from diagnosis of organ metastases to primary operation, grading, sex or age) were not shown to be statistically significant for the prognosis of OS and DFI (colorectal cancer in total). Conclusion: As proven by our study, rectal cancer and colon cancer are two different nosological units with specific prognostic factors with respect to their liver metastases. These differences have not been fully understood yet and require further exploration and classification based not only on histopathological, immunohistochemical and clinical factors, but also on molecular biological parameters. Melanom je vzácný typ malignity v dětské populaci, ale jeho incidence prudce narůstá, především u adolescentních dívek. Předkládáme přehled současných klinických a biologických rysů pediatrického melanomu a problémy spojené s jeho diagnostikou a léčbou. I přes rozdíly mezi pediatrickým melanomem a melanomem dospělého věku je přežívání obdobné. Stran léčby metastatického pediatrického melanomu jsou nutné klinické studie projektované pro dětský věk. Velmi důležitá je rovněž prevence, včasná detekce., Melanoma is a rare malignancy in the pediatric population, but its incidence has risen very rapidly, specially in adolescent girls. We are presenting review of the current clinical and biological features of pediatric melanoma and problems with its diagnosis and management. Despite the differences between pediatric and adult melanoma survival rates are very similar. Age specific pediatric clinical studies are essential for management of metastatic pediatric malignant melanoma. Aim of investigation: to study immediate and remote results of standard and extended lymphodissection in patients with rectal cancer. Material and methods: 132 patients with rectal cancer were performed radical surgery with lymphodissection in D2 and D3 volume according to a height of tumor location. In immediate postoperated period there were noted complications in 4 patients that made up 3,03%. Observation time was 5 years. Tumor recurrence was reported in 5 patients (3,8%). In remote period metastases were revealed in 8 patients (6,6%). 5year total survival rate was 78,5±4,2% at stage II. 5year total survival rate was 56,4±3,7% at stage III. Comparing the results of standard and extended lymphodissection it was noted statistically authentic increase of 5 year survival rate in patients at stage III. To improve the results of radiotherapy for cervical cancer used local and systemic radiomodification drugs. Materials and methods: 1984 patients with locally advanced cervical cancer (2b 3a stage), which at one stage of complex treatment by a course indoarterial of regional longterm polychemotherapy (EARDPHT) mode: methotrexate 50 mg/m2, the first 12 hours, fluorouracil 1000 mg/m2, over the next 12 hours and cisplatin 100 mg/m2, for 48 hours, 2 stage surgical treatment in the volume expanding hysterectomy 3 type (modification of Wertheim Meigs). Results: The results of treatment of locally cervical cancer using VADRPHT, depending on the unilateral and bilateral tumor invasion in parametrial tissue showed that under unilateral lesion of parametrium results of treatment were higher than for the bilateral involvement of parametrium. Medical pathomorphosis in tumors of different severity was observed in 86.9% of patients, metastasis to regional lymph nodes were detected in 36% of operated patients, and also studied the degree of therapeutic pathomorphosis in lymph nodes. There was revealed that the complete regression of tumors, 5 year survival rate is 98%, whereas in tumor regression of more than 50% of the original volume 78.5%, with stabilization of tumor no more than 58.4%. Conclusion: The study of molecularbiological parameters of the tumor cells show the effectiveness of conducted neoadvent indoarterial chemotherapy and play an important role in further treatment and prediction of disease., Navruzova V.
