Gliomy limbického a paralimbického systému představují anatomicky a tím pádem i operačními přístupy unikátní skupinu mozkových nádorů. Mikrochirurgie v kombinaci s moderními perioperačními postupy zvyšuje šance na radikální odstranění těchto gliomů. Z hlediska lokalizace je dělíme do tří základních skupin: 1. gliomy inzuly, 2. amygdalohippokampálního komplexu a 3. cingula. V minimonografii se věnujeme anatomii i funkci limbického a paralimbického systému a v návaznosti na ni představujeme ucelený pohled na jednotlivé neurochirurgické přístupy k těmto oblastem. Maximální resekce za zachování funkční integrity mozku je předpokladem následné smysluplné onkologické léčby. Taktéž prezentujeme výsledky operací našeho pracoviště ve smyslu morbidity a radikality za posledních sedm let., Limbic and paralimbic gliomas represent a unique group of brain tumors both from an anatomic and surgical point of view. Microsurgery, together with modern perioperative procedures, enables a wider extent of resection of these gliomas. Based on localization, we divide gliomas into three basic groups: 1. insular, 2. amygdalohippocampal complex and 3. cingulum gliomas. In our review, we describe the anatomy and function of the limbic and paralimbic system and in relation to that, we present a description of the specific neurosurgical approaches to these regions. Maximal resection with brain function preservation represents the basis for subsequent oncologic therapy. We also present the surgical results of our department in the last seven years with regard to morbidity and radicality. Celosvětové počet onemocnění zhoubným nádorem vzrůs tá. Stejná situace je i v České republice, přičemž ta patří z hlediska epidemiologi e zhoubných nádorů mezi nejzatíženější státy Evropy. Sdělení diagnózy nádor ového onemocnění je pro pacienta těžko pochopitelné a může u něj spustit silné emočn í reakce. V tomto náročném období je sociální opora – přátelé, rodina či partner/ka – dů ležitější a stávají se zdrojem jistoty a spokojenosti. Výzkum se zaměřuje na zjištění výsk ytu úzkosti a deprese u pacientů s kolorektálním karcinomem a zjištění vlivu sociáln í opory na výskyt deprese a úzkosti u těchto pacientů. Byl použit kvantitativní výzkum využívající ex-post facto výzkumný design. Ke sběru dat byly použity tři standardizova né dotazníky: PSSS, BDI-II., BIA. Soubor respondentů tvořilo 113 respondentů – 55 pac ientů proktologické ambulance a 58 respondentů kontrolního souboru. Získávání dat prob ěhlo v roce 2012–2013. Z výsledků vyplynulo, že se vzrůstající mírou sociální opory k lesá míra úzkosti (r = − 0,899) i míra deprese (r = − 0,910). Sociální opora tak byla potv rzena jako významný protektivní faktor zdraví. Dobrá sociální opora je významný fak tor vedoucí ke snížení úzkosti a deprese u onkologických pacientů, a tím přispívá ke zlepšení průběhu nemoci., Number of malignant diseases has been increasing wo rldwide. The situation is similar in the Czech Republic, which seen from the epidemiolog ic perspective belongs among countries with the highest incidence in Europe. For the patients, it is difficult to grasp the diagnosis of malignant disease and may trigger stro ng emotional reactions. The aim focuses on establishing the rate of anxiety and dep ression in patients with colorectal cancer and to determine the effect of social suppor t on the prevalence of anxiety and depression in these patients. A quantitative resear ch was used, using ex-post facto research design. Three standardised questionnaires were used to collect data: PSSS, BDI-II. The research group comprised of 113 respond ents – 55 clients of proctological outpatient surgery and 58 respondents in the contro l group. The data were gathered in 2012-2013. The results indicate that with the highe r level of social support, the lower degree of anxiety (r = − 0,899) as well as depressi on. (r = − 0,910). Thus, social support was confirmed as a significant health-protective fa ctor. Health Technology Assessment (HTA) denotes a process (resp. a study), the objective of which is to get an idea about clinical, economic and ethical aspects of a health technology, including its effect to the quality of life, and a consequent assessment. The output of this process is compiled under strict methodological conditions, and it has only a character of recommendation. Abroad, HTA is intensely used above all to complex comparing two (or several) technologies. When the origin of HTA goes back to 1960 ́s and 1970 ́s, the first information appeared in the Czech Republic in the beginning of this millennium. No Czech institution is a member of international organizations INEHTA and HTAi, which makes any access to foreign know- how more difficult. Although no complex study has been probably produced in the Czech Republic, some efforts to a larger utilization of HTA methods can be seen, above all in pharmacoeconomics. What discourages their higher utilization is a lack of interest in results of HTA studies from the side of health care providers, decision makers (the Ministry, regional administrations, health insurance companies), missing methodology, absence of methodological guidance (non-existence of a national agency), pending funding, and (non)-participation in international networks., Vladimír Rogalewicz, Ivana Juřičková, and Literatura
Isidor Soyka (1850–1889) byl první průkopník vědecké hygieny v českých zemích. Vystudoval lékařství a krátce se věnoval patologické anatomii. Jako první popsal primární amyloid. Po stáži u Maxe von Pettenkofera v Mnichově byl r. 1884 jmenován prvním profesorem hygieny a založil první hygienický ústav (při německé lékařské fakultě v Praze) v Čechách. Je autorem mnoha odborných prací z oblasti komunální hygieny a bakteriologie, výrazně se zasloužil o asanaci Prahy i dalších měst. Národnostní a náboženská netolerance své doby dovedla tohoto průkopníka naší vědecké hygieny k dobrovolnému odchodu ze života. He studied medicine and then shortly worked in the field of pathological anatomy. He was the first person to describe primary amyloidosis. After his studies under Max von Pettenkofer in Munich, he was appointed professor of hygiene and founded the first Hygiene Institute (as a part of the German Faculty of Medicine in Prague) in Bohemia. He published many works specializing in hygiene and bacteriology and greatly contributed to sanitation projects in Prague and other cities. Základní ideou dekompresivní kraniektomie (DK) je chirurgické řešení konfliktu vzrůstajícího nitrolebečního obsahu s pevným objemem kostní schránky. Zvětšený nitrolebeční obsah je většinou představován edémem mozku při nejrůznějších patologických stavech. Přestože DK zažívá v posledních letech renesanci a objevily se důkazy o její účinnosti, její použití není dosud paušálně akceptováno. Nejčastější indikací DK je kraniocerebrální poranění. Důkazy o její efektivitě jsou však limitované. Jsou očekávána data o účinnosti sekundární DK u nemocných s refrakterní nitrolebeční hypertenzí po selhání konzervativní léčby. Provedení bifrontální sekundární dekomprese ještě před vyčerpáním konzervativní léčby nepřináší nemocným s difuzním poraněním benefit. Postavení primární DK u akutního subdurálního hematomu je testováno v randomizované studii. Nejsilnější důkazy o efektivitě DK jsou u nemocných s maligním hemisferálním infarktem, přesto frekvence operace stále neodpovídá incidenci maligního hemisferálního infarktu. DK by měla být zvážena u nemocných s trombózou mozkových splavů a refrakterní nitrolebeční hypertenzí, kteří mohou z operace profitovat i při oboustranné mydriáze. DK s evakuací nebo bez evakuace hematomu může snížit mortalitu nemocných s expanzivním supratentoriálním intracerebrálním krvácením, kteří jsou v bezvědomí nebo mají refrakterní nitrolebeční hypertenzi. Dekomprese může přinést benefit selektované podskupině nemocných se subarachnoidálním krvácením, její použití však není v současných doporučeních specifikováno., The fundamental idea behind decompressive craniectomy (DC) is a surgical solution to the conflict of an increasing intracranial content within the fixed-volume intracranial cavity. The expanding intracranial content is caused by brain edema caused by a variety of pathologies. In spite of DC renaissance over the recent years and some evidence of its effectiveness, its use is not yet generally accepted. Traumatic brain injury is by far the most frequent indication for DC. However, the evidence of its effectiveness is limited. Data on the role of secondary decompression in patients with refractory intracranial hypertension after conservative treatment failure are expected to be released soon. Bifrontal early DC is not superior to medical management in patients with diffuse traumatic injury. Primary decompression in patients with acute subdural haematoma has been investigated in a randomized trial. The most conclusive evidence is in patients with malignant middle cerebral artery infarction. In spite of this, the surgery is still underutilized and the frequency of its use does not correspond with the incidence of malignant infarction. When decompression is performed within 48 hours of stroke onset in patients younger than 60 years, it reduces mortality and improves functional outcome. DC is also a lifesaving procedure in patients over 60 years of age for whom it improves chances of survival without total dependency. Decompression should be considered in patients with cerebral venous thrombosis that causes intractable intracranial hypertension. Furthermore, patients with bilateral mydriasis can also profit from the procedure. DC with or without hematoma evacuation might reduce mortality in patients with large supratentorial intracerebral hemorrhage who are in coma or have refractory intracranial hypertension. Even though the DC can be effective in selected subgroups of patients with subarachnoid hemorrhage, current guidelines, do not specify the role of decompression in these patients.
