Záměr. Cílem studie bylo ověření psychometrických parametrů české verze Jeffersonské škály empatie (JSPE-S) na podkladě výpovědí českých studentů medicíny a testování diferencí z hlediska pohlaví, délky studia medicíny a místa studia. Soubor a procedura. Soubor tvořilo 725 studentů medicíny 3. lékařské fakulty UK a 871 studentů Fakulty medicíny UPOL. Studie byla průřezová a odpovědi byly získány od studentů všech šesti ročníků. Výsledky. Explorační faktorová analýza přinesla podporu pro existenci tří ortogonálních faktorů „Perspective taking“ (přebírání perspektivy), „Compassionate care“ (soucitná péče), „Empathetic understanding“ (empatické porozumění) a vnitřní konzistence stupnice byla pro celý soubor 0,76. Efekt pohlaví a místa studia nebyl statisticky významný. Výše JSPE-S skórů českých studentů statisticky i klinicky významně klesala v prvních dvou letech studia a zůstala beze změn do konce šestého ročníku. Nálezy obecně podpořily konstruktovou validitu i vnitřní konzistenci české verze nástroje. Omezení studie. Postoje k roli empatie se mohou výrazně lišit od skutečného chování, vzhledem k průřezovému plánu studie je zde možný kohortový efekt, data byla získána pouze na dvou lékařských fakultách., Objectives. The objective of the study was to examine the psychometric parameters of the Czech version of the Jefferson Scale of Physician Empathy-Student (JSPE-S), and to study differences in empathy scores between women and men, and students in different years of medical school. Sample and setting. The JSPE-S was administered to 725 students at the 3rd Medical School, CU Prague and to 871 students at the Faculty of Medicine UPOL. The design was cross-sectional and first to sixth year students were surveyed. Results. Exploratory factor analysis supported the existence of three components of “Perspective taking”, “Compassionate care”, “Empathetic understanding” and the Cronbach alpha coefficient was 0.76. The effects of gender and locality were not statistically significant. The JSPE-S scores of Czech students decreased in a statistically significantly and clinically meaningfully way over the first two years of study, staying low until the 6th year. The construct validity and internal consistency of the Czech version of the JSPE-S was generally supported. Study limitations. Firstly, attitude toward the role of the empathy in doctor-patient relation may differ substantially from actual behavior. Secondly, there is very strong possibility of cohort effects. Na podkladě odpovědí reprezentativního souboru českých lékařů (a) ověřit psychometrické parametry Dotazníku empatie DE14; (b) testovat předpoklad rozdílů mezi lékaři z hlediska pohlaví, délky praxe a způsobu výkonu profese; (c) ověřit vztah mezi generickou empatickou citlivostí a specifickou empatickou vnímavostí v léčebném kontextu; (d) konstruovat normy použitelné v experimentálním výzkumu. Soubor a procedura. Soubor tvořilo 1305 lékařů z celé České republiky vybraných náhodným kvótním výběrem dle regionu, pohlaví, věku a způsobu výkonu profese. Statistická analýza. Struktura metody byla zkoumána konfirmační faktorovou analýzou metodou maximální věrohodnosti. Simultánní faktorová analýza byla použita pro testování shody faktorového modelu pro muže a ženy z hlediska regresních vah, faktorové kovariance a strukturálních průměrů. Vztah mezi jednotlivými položkami a celkovým skórem byl testován Pearsonovým korelačním koeficientem, vnitřní konzistence stupnic Cronbachovým koeficientem alfa. Efekt pohlaví, délky praxe a profesní orientace multivariační analýzou rozptylu. Vztah mezi dotazníkem DE14 a JSPE-HP byl ověřován lineární multivariační regresní analýzou. Výsledky. Konfirmační faktorová analýza přinesla podporu pro existenci tří komponent 14položkového dotazníku DE14: Vnímavost, Respekt, Otevřenost, Cronbachův koeficient alfa 0,74, 0,73 a 0,69. Struktura nástroje byla z hlediska pohlaví invariantní a efekt pohlaví, ačkoli statisticky významný, byl vzhledem k velikosti účinku d = 0,2 bez praktického významu. Dotazník DE14 sdílel s nástrojem JSPE-HP cca 33 % variance. Faktory věk, délka praxe, profesní orientace a všechny interakce byly statisticky nevýznamné., a2_Psychometrické charakteristiky jsou uspokojivé a umožňují použití předběžných norem založených na reprezentativním souboru českých lékařů v experimentálním kontextu., Objectives. The aim of the study was to develop and psychometrically evaluate new general questionnaire for estimation of empathy. Sample and setting. The sample consisted of 1305 Czech physicians selected using random quota sampling according to region, gender, age, and employment status. Statistical analysis. The structure of questionnaire was verified using confirmatory factor analysis, method maximal likelihood. Simultaneous factor analysis was employed to test differences between men and women from the viewpoint of regression weights, factor covariance, and structural latent means. Cronbach alpha coefficient was calculated to assess internal consistency aspect of reliability. Effects of gender, age, professional orientation, and length of practice were tested using multivariate analysis of variance. Relationship between the JSPE-HP and the DE14 was examined by the linear multivariate regression analysis. Results. Confirmatory factor analysis results supported the presumption of the three components of the 14-item self-administered Empathy Questionnaire (DE14) Percipience, Openness, and Respect; Cronbach alpha coefficient was 0.67, 0.63, and 0.75, respectively. The structure of the questionnaire was invariant from the viewpoint of gender, regression weights, and factor covariance. The effect of gender, although statistically significant, was very small and of no practical consequence. Factors of age, length of practice, professional orientation, and all interactions were nonsignificant. Questionnaire DE14 shared cca 33 % of variance with the JSPE-HP. The DE14 psychometric characteristics are satisfactory and the provisional norms based on a representative sample of Czech physicians’s responses are valid for use in experimental setting. Cílem studie bylo testování dimensionality české verze Jeffersonské škály empatie pro poskytovatele léčebné péče (JSPE-HP) na podkladě údajů získaných od representativního souboru českých lékařů a testování vlivu věku, pohlaví, délky a typu praxe na jejich výpověď. Soubor a procedura. Soubor tvořilo 1305 respondentů vybraných náhodným kvótním výběrem, dle regionu, pohlaví, věku a způsobu výkonu profese. Výsledky. Struktura nástroje byla nejprve zkoumána EFA a nález následně testován na jiném souboru CFA. Byly identifikovány tři korelované faktory s marginálně akceptovatelnou mírou shody modelu a dat. EFA vyššího řádu se Schmid-Leimanovou ortogonalizací identifikovala obecný faktor „vysvětlující“ 73,3 % rozptylu a 26,7 % rozptylu připadajících na faktory prvního řádu. Cronbachův koeficient alfa byl 0,842 a nebyl zjištěn klinicky závažný efekt věku, pohlaví, typu praxe a profesionální orientace. Neočekávaným nálezem byla relativně nízká průměrná hodnota skóru empatie ve srovnání s americkými lékaři. Výsledky studie naznačují, že česká verze JSPE-HP je jednodimensionální stupnice, psychometricky srovnatelná s US verzí a použitelná pro odhad empatického chování českých lékařů. Omezení studie. Extrapolace nálezů je limitovaná charakteristikami souborů a skutečností, že autoreferenční data nemusí nutně odrážet skutečné chování respondentů., Objectives. The aim of this study was to examine the Jefferson Scale of Physician Empathy (JSPEHP) underlying dimensionality and the relation of empathy scores to age, gender, practice experience, and practice type using responses from a representative sample of Czech physicians. Subjects and setting. The JSPE-HP was administered to a sample of 1,305 respondents selected using the hierarchical random proportional stratified sampling technique representing Czech physicians from the viewpoint of region, practice type, gender, and age. Results. The structure of the instrument was examined using EFA and the findings were tested on different sample using CFA. Three intercorrelated primary factors were identified with a marginally acceptable fit. High-order EFA employing Schmid-Leiman orthgonalization identified a general factor explaining 73.3% of the common variance with 26.7% explained by the first-order factors beyond the general factor. Cronbach’s alpha for total scale was 0.842 and no clinically important effect of age, gender, practice type, and professional orientation on physicians’ empathy score was found. The substantially lower mean empathy score of Czech physicians in comparison to that reported for US physicians is worthy of consideration. The results of this study indicate that the Czech version of JSPE-HP is an unidimensional scale, psychometrically comparable to the US version, and a sound instrument for self-reported assessment of the empathetic behavior of Czech physicians. Study limitation. The extrapolation of findings may be limited as the sample was representative of Czech physicians only from the viewpoint of four variables: region, practice type, gender, and age. Soubor tvořilo 823 studentů medicíny (583 studentek a 240 studentů; průměrný věk 21,75; SD = 1,97; rozsah 19 – 29 let) 1. až 6. ročníku Lékařské fakulty Univerzity Palackého v Olomouci. Autoreferenční data byla od studujících shromážděna na konci zimního semestru. Účast studentů byla neanonymní, dobrovolná, podmíněná informovaným souhlasem. Statistická analýza. Struktura metody byla zkoumána explorační faktorovou analýzou a nálezy následně ověřovány konfirmační faktorovou analýzou. Simultánní faktorová analýza byla použita pro testování shody faktorového modelu pro muže a ženy z hlediska regresních vah, faktorové kovariance a strukturálních průměrů. Vztah mezi jednotlivými položkami a celkovým skórem byl testován Pearsonovým korelačním koeficientem a vnitřní konzistence stupnic Cronbachovým koeficientem alfa. Efekt délky studia byl zkoumán Jonckheer-Terpstra testem, vliv identifikace na odpovědi studentů GLM procedurou a vztah mezi dotazníkem DE14 a JSPE-S ověřován MRA. Výsledky. Explorační a následně i konfirmační faktorová analýza přinesla podporu pro existenci tří komponent 14položkového dotazníku DE14: Vnímavost, Respekt, Otevřenost, Cronbachův koeficient alfa resp. 0,67, 0,75, a 0,63. Struktura nástroje byla z hlediska pohlaví invariantní a efekt sebeidentifikace respondentů i délky studia, ačkoli statisticky významný, byl bez praktického dopadu.
