The authors present the case of a young woman with newly diagnosed Takayasu’s arteritis. This woman, with arterial hypertension, was investigated for the unspecific symptoms at the beginning. Afterwards, the transthoracic echocardiography showed dysfunction of the left ventricle and the abdominal sonography showed a stenosis of the right renal artery. PET/CT scan showed chronic modification after inflammatory processes on the wall of the thoracic and abdominal aorta. Rychlý rozvoj zobrazovacích metod a ozařovací techniky vedl k rozvoji stereotaktických metod. Stereotaktická radiochirurgie (aplikace jedné vysoké dávky záření) a stereotaktická radioterapie (užití frakcionačních režimů) významně rozšířily léčebné metody pro benigní i maligní mozkové nádory. Díky vysoké přesnosti je možné aplikovat vysoké dávky záření do přesně definovaných cílů při maximálním šetření okolní zdravé tkáně. Nejvíce se v současné době uplatňují v léčbě meningiomů, nádorů hypofýzy, neurinomů akustiku, mozkových metastáz a rozšiřují možnosti v léčbě recidiv nebo reziduí (gliomů, ependymomů, kraniofaryngeomů, meduloblastomů a maligních procesů na bázi lební). Dlouhodobá lokální kontrola nad nádorovým růstem je pro benigní nádory okolo 90–95 %, pro mozkové metastázy okolo 70 %, pozdní komplikace se pohybují v rozmezí 5–10 %., The fast development of imaging method (especially magnetic resonance) and of radiotherapy technique was followed by using of stereotactic methods. Radiosurgery (application of high dose in single session) and stereotactic radiotherapy (using fractionated scheme) widely extend treatment methods for benign and malignant brain tumors. Nowadays they play important role in treatment of meningiomas, pituitary adenomas, acoustic neuromas, brain metastases and also in the treatment of reoccurrence (gliomas, ependymomas, craniopharyngiomas, medulloblastomas, malignant skull base tumors). Stiff-person syndrome (SPS) is a rare disorder characterized by muscle stiffness and painful spasms. Misdiagnosis may occur due to the fact that the clinical picture of SPS is often atypical. The main pathophysiologic mechanism underlying the development of SPS is insufficient inhibition at the cortical and spinal levels. There is good evidence for a primary autoimmune etiology. A 61-year-old man was admitted to a neurological department due to muscle hypertonia with episodic attacks of painful spasms predominantly affecting axial muscles. The symptoms developed shortly after tickborne meningoencephalitis. Electromyography (EMG) revealed signs of continuous motor unit activity. Antibodies against glutamate decarboxylase (anti-GAD) were highly elevated. We present a case of a man who developed clinically severe anti-GAD positive SPS, provoked by tick-borne encephalitis. After therapeutic plasma exchange (TPE) a rapid, temporary improvement of the clinical and neurophysiological findings was noted. Only after being placed on long-term immunosuppression did the patient achieve stable recovery. Účel studie: Histologická verifikace je podmínkou adekvátní terapie mozkových tumorů. Sdělení prezentuje zkušenosti se stereotaktickou biopsií nádoru mozku s důrazem na diagnostickou výtěžnost, korelaci nálezů s resekčními výkony a bezpečnost. Použité metody: U 106 nemocných byla provedena biopsie předpokládaného tumoru mozku pomocí rámového systému Zamorano Dujovny (hluboko umístěné a méně rozsáhlé léze) a u 20 nemocných byl využit bezrámový neuronavigační systém BrainLab (povrchové léze). Výsledky: Diagnostická výtěžnost dosahovala 96,3 % u rámového systému a 100 % u systému bezrámového. Četnost komplikací byla 1,8 % po biopsii s využitím rámového systému. Komplikací bezrámové biopsie byl jeden asymptomatický intracerebrální hematom. Předpokládaný tumor mozku nebyl prokázán u 12,8 % nemocných. Histologické nálezy při resekci tumoru a stereobiopsii (10 nemocných) s jedinou výjimkou (grading maligního gliomu) korelovaly. Závěry: Biopsie tumoru mozku s využitím rámové i bezrámové techniky je bezpečným výkonem s přínosem pro diagnostiku u nemocných s tumorem mozku. Využití jednotlivých systémů záleží na lokalizaci léze (rámová technika u hlubokých lézí), jejím rozsahu (bezrámový systém u rozsáhlých subkortikálních procesů) a plánovaném dalším postupu., Study aim: Histological verification is a prerequisite for adequate therapy of brain tumors. Study presents the experience with stereotactic biopsy of brain tumors paying attention to diagnostic utility, correlation of findings with tumor resection and surgical safety. Methods: Brain tumor biopsy using stereotactic frame based system (deep and small lesions) was performed in 106 patients and frameless neuronavigation system BrainLab (superficially located lesions) was used in 20 patients. Results: Diagnostic efficiency was 96,3 % in frame based system and 100 % in frameless system. The incidence of complications was 1,8 % in frame based biopsies. There was one asymptomatic haematoma in frameless biopsy patients. Suspected brain tumor was not proven in 12,8 % of patient. There was correlation of histological findings in specimens taken during stereotactic biopsy and open tumor resection (10 patients) in all except one patient (malignant glioma grading). Conclusions: Frameless and neuronavigation based tumor biopsy is a safe procedure beneficial for the treatment of brain tumor patiens. Radiologické metody a postupy se staly neodmyslitelnou součástí screeningu, diagnostiky, terapie a managementu onemocnění pacientů s kolorektálním karcinomem. Do popředí zájmu se dostávají CT kolonografie (virtuální kolonoskopie) v rámci screeningu, metody kontrastního ultrazvuku ke zrychlení diagnostického postupu a magnetická rezonance zejména v precizní diagnostice jaterního postižení a lokálního stagingu karcinomu rekta. V posledním desetiletí se etablovaly radiologické intervenční metody určené pro ty nemocné, u kterých standardní řešení selhává. Intervenční radiologie tak rozšiřuje možnosti léčby u pacientů s diseminovaným kolorektálním karcinomem (ve stadiu IV). Terapeutické invazivní metody při terapii metastatického postižení jater mohou být buď intravaskulární (chemoembolizace a regionální chemoterapie, předoperační embolizace portální žíly), nebo perkutánní (především radiofrekvenční termoablace). Tyto metody je možné využít jak v rámci downstagingu onemocnění se záměrem umožnění kurativní resekce, tak v rámci paliace onemocnění., Radiological methods and procedures have become an inseparable part of the screening, diagnosis, therapy and disease management of patients with colorectal cancer. Major areas of interest are CT colonography (virtual colonoscopy) during screening, contrast-enhanced ultrasound in speeding up diagnostic procedures and magnetic resonance imaging, especially in the precise diagnosis of liver disease and local staging of rectal cancer. Radiological interventional methods have been more established in the last decade, as they are suitable for those patients where a standard solution fails. Interventional radiology extends the treatment options for patients with disseminated colorectal cancer (at stage IV). Invasive therapeutic methods in the treatment of liver metastases can be either intravascular (chemoembolization and regional chemotherapy, preoperative portal vein embolization) or percutaneous (particularly radiofrequency thermoablation).
