Background: Few data are available on the specific care giving-related problems of stroke patient’s caregivers and factors that influence the burden of these caregivers. Aim: To study the influences of the active rehabilitation process on anxiety, depression, care burden and perceived social support level of stroke patients caregivers. Design: A prospective clinical trial. Setting: Patients and caregivers entering a rehabilitation program at a university hospital in Turkey. Populations: Ninety patients with a first episode of stroke and 90 caregivers responsible for their care were recruited for our study. Methods: Patients and caregivers were assessed before and after the active rehabilitation process. The functional disability level of the patients was assessed by Functional Independence Measure (FIM). The Beck Anxiety Scale (BAS) and the Beck Depression Scale (BDS) were used for anxiety and depression assessment, the Zarit Care Burden Scale (ZCBS) for care burden assessment and the Multi-Dimensional Scale of Perceived Social Support (MDSPSS) for perceived social support assessment. Results: A statistically significant rise is observed in the special person sub-assessment of MDSPSS in both female and male caregivers. Also, a significant decrease in care burden, anxiety and depression levels of caregivers was noted after the rehabilitation program (p < 0.05). Conclusion: Caregivers accept the rehabilitation period as important social support in addition to the support provided by family and friends. Also, our positive results were associated with an improvement in the patients’ functional level and an increase in the acquisition of knowledge and skill required of caregivers in order to provide care during rehabilitation. Clinical Rehabilitation Impact: The rehabilitation team should be aware of the fact that the perceived care burden may be greater due to the lack of knowledge concerning available resources and due to the inability to cope with stress effectively. and A. Cílem této studie bylo stanovit prognózu psychologického distresu u pacientek s karcinomem prsu po primární onkologické léčbě pomocí biologických a psychologických proměnných. Skupina hodnocených pacientek zahrnovala 98 starších pacientek po léčbě karcinomu prsu (ve středním věku 65 let), které vyplnily dotazník SVF 78 (detekce způsobů zvládání stresu), dotazník NEO-FFI (detekce osobnostních rysů), dotazník SCL-90 (zjišťování psychopatologie) po dokončení léčby, a další retrospektivně po diagnóze. Stupnice SAS (měření stupně úzkosti) byla vyplněna menším počtem pacientek. Údaje o faktorech souvisejících s karcinomem a léčbou byly získány z lékařských záznamů. V rámci této studie byl psychologický distres měřen metodou SCL-90 pomocí indexu celkové závažnosti Global Severity Index (GSI) a indexu zátěže pozitivních příznaků Positive Symptom Distress Index (PSDI). Kvantifikace vztahu mezi biologickými a psychologickými prediktory a GSI a PSDI jakožto závislými proměnnými byla vyhodnocena pomocí lineárních regresních modelů s jednou proměnnou i více proměnnými. Po diagnóze a po jednom roce primární léčby byly sledovány hladiny C-reaktivního proteinu. Nejlepší model predikce GSI po léčbě byl zjištěn lineární regresí s více proměnnými jakožto kombinace prediktorů GSI, hladiny CRP a přívětivosti (podškála NEO-FFI) v době diagnózy, v níž R2 = 76,6 %. Nejlepší model predikce PSDI po léčení sestával z hodnoty PSDI, prvku sebeobviňování SVF 78 a stadia nemoci (IV. oproti nižším) v době diagnózy při R2 = 53,9 %. Začlenění celkového hrubého skóre metody SAS do modelů s více proměnnými pro predikci GSI a PSDI způsobilo nárůst hodnoty R2 (ze 71,5 % na 85,0 % a 46,0 % na 65,1 %). Biologické i psychologické prediktory se prokázaly jako významné a vhodné pro predikci psychologického distresu u starších pacientek s karcinomem prsu po dlouhodobém onkologickém léčení. The test group consisted of 98 elderly breast cancer survivors (median age was 65 years) who completed the SVF 78 questionnaire (coping styles measures), NEO-FFI questionnaire (personality traits measures), SCL-90 questionnaire (psychopathology measures) completing treatment and another retrospectively at diagnosis. The SAS scale (anxiety measures) was completed by a lower number of patients. Data on tumour-related factors and treatment were obtained from medical records. Within the scope of this study, psychological distress was measured via the SCL-90 method using Global Severity Index (GSI) and Positive Symptom Distress Index (PSDI). Quantification of the relationship between biological and psychological predictors and GSI and PSDI as dependent variables was estimated using both univariate and multivariate linear regression models. C-reactive protein levels were monitored at diagnosis and one year after primary treatment. The best model for the prediction of GSI after treatment was identified by multivariate linear regression as the combination of GSI, CRP level and agreeableness (NEO-FFI subscale) predictors at the time of diagnosis in which R2 = 76.6 %. The best model for predicting PSDI after treatment consisted of PSDI, the self-accusation component of SVF 78 and the stage of the disease (IV vs lower) at the time of diagnosis with R2 = 53.9 %. Incorporating the total raw score of the SAS questionnaire into the multivariate models for prediction of GSI and PSDI caused an increase in R2 (71.5 % to 85.0 % and 46.0 % to 65.1 %), respectively. Both biological and psychological predictors proved significant and suitable for psychological distress prediction in elderly breast cancer survivors after long-term oncological treatment. Cílem pilotní studie bylo vyhodnotit kvalitu života (Quality of Life – QoL) u pacientek s karcinomem prsu ve vztahu k léčebným modalitám a jejich kombinacím. Ve studii QoL participovalo 71 pacientek s karcinomem prsu, QoL byla hodnocena pomocí Amerického dotazníku zdraví (SF36), Dotazníku životní spokojenosti a české výzkumné verze Functional Assessment of Breast Cancer Therapy (FACT-B). Průměrná doba od diagnózy do doby hodnocení QoL ve výzkumném souboru byla 4,5 roku. Statistická významnost rozdílů v kvalitě života mezi pacientkami s různou léčbou byla testována Mannovým-Whitneyovým testem pro jednotlivé typy terapie a Kruskalovým-Wallisovým testem pro kombinace terapie. Vztah mezi kvalitou života a léčebnou modalitou byl zjištěn pro dotazník SF36 – pro dimenzi emocionální funkční role a operační léčbu (vyšší kvalita života u operovaných, p = 0,049), pro dotazník FACT-B – fyzický stav a chemoterapii (vyšší kvalita života u pacientek bez chemoterapie, p = 0,012), pro FACT-B – sociální/rodinný stav a operační léčbu (vyšší kvalita života u operovaných, p = 0,036), pro FACT-G skóre a operační léčbu (vyšší kvalita života u operovaných, p = 0,049) a dále pro Dotazník životní spokojenosti – pro dimenzi zdraví a kombinaci léčby (nejvyšší kvalita života u kombinace operace, chemoterapie, radioterapie, biologické a hormonální terapie, nejnižší u kombinace předešlých bez biologické terapie, p = 0,027). Pro úplnost uvádíme typ a četnost vedlejších efektů léčebných modalit. Zjistili jsme vyšší QoL u operovaných pacientek a nižší QoL u pacientek vystavených chemoterapii, avšak naše závěry vyžadují další výzkumná ověření. A total of 71 patients with breast carcinoma participated in this study. QoL was assessed using the 36-item Short-Form Health Survey (SF-36), Life Satisfaction Questionnaire and Czech research version of Functional Assessment of Breast Cancer Therapy (FACT-B). The average time from diagnosis at the time of QoL assessment was 4.5 years. Mann-Whitney U test was used to test the differences in QoL between groups of patients with different type of therapy and Kruskal-Wallis test for combinations of therapies. A relation between quality of life and a treatment modality was found for questionnaire SF 36 – for the dimension of emotional functional role and operation treatment (higher quality of life in operated patients, p = 0.049), for questionnaire FACT-B – physical condition and chemotherapy (higher quality of life in patients without chemotherapy, p = 0.012), for FACT-B – social/family condition and operation treatment (higher quality of life in operated patients, p = 0.036), for FACT-G score and operation treatment (higher quality of life in operated patents, p = 0.049) and further for Life Satisfaction Questionnaire – for the dimension of health and treatment combination (the highest quality of life in case of the combination of operation, chemotherapy, radiotherapy, biological and hormonal therapy, the lowest quality of life in case of the combination of the previous without biological therapy, p = 0.027). Additionally, we described type and occurrence of side effects of treatment modalities. Generally we found higher QoL in operated patients and lower in patients who underwent chemotherapy, however our findings require further research verifications.
