Typically, there is an extensive infiltration of abdominal cavity, peritoneal effusion with no evident primary origin of tumor. Prognosis is unfavourable with 5-years overall survival described in 15 % of cases even though multimodal treatment strategy is performed. We introduce case report of 34-years old patient with DSRCT treated in our Oncology center. Reasonable schema of differential diagnosis of soft-tissue expansion in abdominal cavity is mentioned and we emphasize crucial role of molecular genetic analysis. We focus on therapeutic approaches of this rarely occuring disease., Viktor Maňásek, Renata Soumarová, and Lit.: 19
Onemocnění z depozice lehkých řetězců imunoglobulinu postihuje především ledviny a vede k progresivní poruše funkce s konečným orgánovým selháním. Kromě etiopatogeneze, odlišení idiopatické formy od formy provázející mnohočetný myelom a vzácně i jiné B-lymfoproliferativní stavy, je nastíněna problematika klinických projevů, diagnostiky, diferenciální diagnostiky a terapie včetně přínosu biologické léčby bortezomibem a vysokodávkované chemoterapie s podporou autologních kmenových buněk, vedoucí k podstatnému zlepšení kvality i celkové délky života nemocných s touto prognosticky stále závažnou nemocí., The aim of the paper within the background of the diagnostics and treatment of three patients is the summary of recent knowledge about a relatively rare disease which is characterized by the deposition of immunoglobulin light chains in vital tissues and organs, especially the kidneys, leading to progressive deterioration of their function with end stage organ failure. We demonstrate the etiopathogenesis a differentiation of idiopathic form of the disease from the form accompanying multiple myeloma or other B-lymphoproliferative diseases, the clinical manifestation, diagnostics and differential diagnostics and therapy as well as the contribution of biological treatment using bortezomib and high dosed chemotherapy with the support of autologous stem cell transplantation, leading to a substantial improvement of the quality of life and overall survival in patients with this prognostically unfavorable disease., Ščudla V., Tichý T., Minařík J., Pika T., Krejčí K., Zadražil J., and Literatura 38
