- Stadium III nemalobuněčného karcinomu plic (NSCLC) má nízké 5leté přežití (méně než 30 %). Abychom měli reálnou šanci zlepšit přežití nemocných, je nutné dodržet kvalitní staging a použít kvalitní léčbu. V rámci stagingu je důležité přesně určit postižení regionálních mízních uzlin a vyloučit generalizaci. Pro staging regionálních uzlin je doporučeno použití PET/CT trupu (pozitronové emisní tomografie/výpočetní tomografie). Pozitivní nálezy, měnící stadium, je doporučeno biopticky verifikovat. Indikace léčby je vždy přísně individuální, ideálně v rámci multidisciplinárních týmů; používáme léčbu multimodální, s využitím více léčebných metod (chemoterapie, radioterapie, operace). Nadějným příslibem do budoucna je pro nemocné se stadiem III imunoterapie., Stage III non-small-cell lung cancer (NSCLC) has a low 5-year survival rate (less than 30%). In order to have a real chance of improving patient survival, both staging and treatment must be of appropriate quality. During staging, it is important to precisely determine regional lymph node involvement and rule out generalization. PET/CT (positron emission tomography/computed tomography) of the torso is recommended to be used for regional lymph node staging. Positive findings that alter the stage are to be confirmed bioptically. Treatment is always indicated strictly individually, ideally within multidisciplinary teams; it is multimodal, using multiple therapeutic methods (chemotherapy, radiotherapy, surgery). - Kolorektální karcinom z prstenčitých buněk je vzácné agresivní nádorové onemocnění. Incidence u mladých pacientů je velmi nízká a zaujímá do 1 % všech chirurgicky léčených kolorektálních karcinomů. Raritní výskyt u mladých pacientů koresponduje s diagnostikováním tohoto onemocnění v pokročilých stadiích a s nepříznivou prognózou. Pro verifikaci karcinomu z prstenčitých buněk je zásadní bioptické vyšetření a léčba je obdobná jako u ostatních kolorektálních karcinomů. Autoři popisují případ mladé ženy s diagnostikovaným kolorektálním karcinomem z prstenčitých buněk. Cílem sdělení je upozornit na raritní výskyt vzácného kolorektálního karcinomu u mladé nemocné., Signet ring cell colorectal carcinoma is a rare aggressive tumor. The incidence of this desease is very low in young patients and accounts up to 1% of all surgical patients with colorectal cancer. The infrequency of the disease among young patients makes the diagnosis more difficult and the prognosis less favourable. Biopsy results are crucial for verification of signet ring cell carcinoma. Treatment is similar as in other types of colorectal cancer. We report a case of signet ring cell colon cancer in a 22 years old female patient. - Karcinom penisu patří mezi řídce se vyskytující onemocnění, avšak s významnými psychosociálními následky. Postihuje muže převážně vyššího věku s maximem výskytu v šesté dekádě života. Etiologické a epidemiologické faktory jeho vzniku jsou poměrně dobře známy. Významnou roli při vzniku nádoru hraje infekce HPV, zejména typy HPV 16 a 18. Vzhledem k nízké incidenci karcinomu penisu kolem 1/100 000 osob jsou na většině urologických a onkologických pracovišť malé zkušenosti s jeho terapií. Neexistují žádné prospektivní randomizované studie o jeho léčbě. Literární soubory o chirurgické a další onkologické léčbě, jako je radioterapie a chemoterapie, se týkají obvykle jen několika desítek pacientů léčených během dlouhého období. Časná chirurgická léčba lokálního nálezu a případného uzlinového postižení hraje podstatnou roli ve vyléčení a prognóze pacienta. Nechirurgická léčba karcinomu penisu je v českém písemnictví málo publikována. V přehledovém článku je zdůrazněna nutnost přesné diagnózy a stagingu pro stratifi kaci pacientů do skupin ke vhodné léčbě podle rizika a prognostických faktorů. Dále je uveden pohled na současné možnosti a roli radioterapie a chemoterapie v léčbě karcinomu penisu. Stručně jsou také uvedeny vlastní zkušenosti s léčbou 25 pacientů ozářených pomocí HDR brachyterapie., Penile cancer is a rare tumour, however it has serious psychological and social consequences. The incidence of penile cancer increases with reaching with maximum in the sixth decade of life. The aetiological and epidemiological penile cancer risk factors are well recognized. Among them human papilloma virus infection, namely types HPV 16 and 18 is an important. Due to its low incidence about 1/100 000, there is limited experience with penile cancer therapy in most of urology or oncology departments. Randomised prospective studies are not available. Reports about surgical and other oncological therapy describe small studies with patients treated during long period. Early surgical treatment of penile lesions, and if possible of nodal metastases, plays an essential role in curing the disease and its prognosis. The review article highlights the importance of precise diagnosis and staging for the stratifi cation of the patients and selection appropriate therapy according to risk and prognostic factors. The role of contemporary radiotherapy and chemotherapy in multimodal treatment of penile cancer is discussed. - To improve the results of radiotherapy for cervical cancer used local and systemic radiomodification drugs. Materials and methods: 1984 patients with locally advanced cervical cancer (2b 3a stage), which at one stage of complex treatment by a course indoarterial of regional longterm polychemotherapy (EARDPHT) mode: methotrexate 50 mg/m2, the first 12 hours, fluorouracil 1000 mg/m2, over the next 12 hours and cisplatin 100 mg/m2, for 48 hours, 2 stage surgical treatment in the volume expanding hysterectomy 3 type (modification of Wertheim Meigs). Results: The results of treatment of locally cervical cancer using VADRPHT, depending on the unilateral and bilateral tumor invasion in parametrial tissue showed that under unilateral lesion of parametrium results of treatment were higher than for the bilateral involvement of parametrium. Medical pathomorphosis in tumors of different severity was observed in 86.9% of patients, metastasis to regional lymph nodes were detected in 36% of operated patients, and also studied the degree of therapeutic pathomorphosis in lymph nodes. There was revealed that the complete regression of tumors, 5 year survival rate is 98%, whereas in tumor regression of more than 50% of the original volume 78.5%, with stabilization of tumor no more than 58.4%. Conclusion: The study of molecularbiological parameters of the tumor cells show the effectiveness of conducted neoadvent indoarterial chemotherapy and play an important role in further treatment and prediction of disease., Navruzova V.
