The aim of our study was to determine the significance of lymphocyte-platelet adhesion (LPA), interleukins, transforming included 139 pregnant women aged between 17 and 27 years (21.3±4.22 years). GH was diagnosed in 119 women after 20 weeks of pregnancy. 20 patients (control group) were with physiological course of pregnancy. The distribution of patients by groups was carried out according to the level of blood pressure (BP) in accordance with ICD-10 (Geneva, WHO, 2002). The survey was conducted at the moment of detection pregnancy from 7 to 10 weeks and in dynamics of I, II and III trimesters of gestation. In the dynamics of gestation, were studied the number of desquamated endothelial cells circulating in the systemic circulation (CECs), nitrates levels, the adhesion of platelets by estimation their ability to form co-aggregates with lymphocytes by determining the percentage of lymphocytes aggregates with thrombocytes (lymphocyte-platelet plugs), at risk of hypertensive disorders, especially after 20-22 weeks of gestation and later, lymphocytes ability to platelets adhesion is rose, the concentrations of pro-inflammatory cytokines and NO level are increased. Comparative assessment of clinical efficiency of omeprazole and rabeprazole in treating gastroesophageal reflux disease (GERD). 65 patients with a verified diagnosis GERD were examined. Comparative clinical investigations of using of omeprazole and rabeprazole have revealed effectiveness of both drugs in the therapy of GERD. The target of research was the implementation of the comparative analysis of conservative therapy of preparation of basic therapy and also new methods of treatment (anticytokine therapy). Appointment of salofalk in twos tablets (0, 5) 34 times a day per os, and also 2,0 gr in the form of a rectal enema unitary for a period of seven days, then had transferd the patients to reception 2,0 gr salofalk per os, allowed to achieve substantial improvement patients cliniclaboratory and endoscopic symptoms with a heavy intensification of ulcer colitis variation, providing full cliniclabaratory remissions achievements of disease at 8 (34 %) patients. In the Minister of the Uzbek Public Health of the Republican Science Coloproctological Center at 10 patients with the IDI have spent the therapy remicade, 7 of them had NUC and 3 patients had the Crohn's disease. Therefore, all patients no longer than on the first week after introduction of remicade had improvement of a course disease which was expressed in defering stool, extinction or reduction of pathological impurity in excrement and subsidence bellyaches. During colonoscopical inspection, in 1218 weeks after the first infusion at 80% of the patients had observed disappearance of ulcers and erosion. In terms of 6 months of supervision of relapse of disease was not noted. All new directions in treatment of the IDI, apparently, are perspective, but while are proved only theoretically and had no practical development. Cíl: Cílem studie bylo vyhodnotit vztah mezi hodnotami indexu IgG a počtem oligoklonálních IgG pásů (OCB) v mozkomíšním moku (CSF) pacientů s roztroušenou sklerózou (SM). Materiál a metodika: Soubor 150 pacientů sestával ze 41 mužů a 109 žen (ve věku od 18 do 68 let, průměrný věk byl 36,6 ±10,1 let). U mozkomíšního moku odebraněho lumbální punkcí byla posouzena intratekální syntéza s využitím indexu IgG a stanovením OCB. Počet alkalických OCB v CSF byl stanoven metodou izoelektrické fokusace. K vyhodnocení statistické významnosti výsledků byla rovněž použita Pearsonova korelační analýza, x2 test homogenity, Mannův-Whitneyův test, párový t-test (parametrický) a jednovýběrový Wilcoxonův test (neparametrický). Výsledky: Pozitivní korelace mezi IgG indexem a počtem OCB nebyla nalezena. Ani Mannův-Whitneyův test neprokázal podstatný rozdíl v hodnotách indexu IgG u pacientů s OCB > 2 a < 2. Závěr Studie nepotvrdila korelaci mezi hodnotami IgG indexu a počtem OCB v CSF u pacientů s roztroušenou sklerózou., Objective: The aim of this study was to assess the correlation between IgG index values and the number of the oligoclonal IgG bands (OCB) in the cerebrospinal fluid (CSF) of patients with multiple sclerosis (MS). Material and Methods: The set of 150 patients consisted of 41 males and 109 females (aged 18–68, mean 36.6 ± 10.1 years). The CSF collected by a lumbar puncture was examined evaluating intrathecal synthesis using the IgG index and determining OCB. The number of alkaline OCB in the CSF was assessed using the method of isoelectric focusation. Pearson's correlation analysis, and homogeneity χ2 test , Mann-Whitney test, paired-sample t-test (parametric) and Wilcoxon signed-ranks test (nonparametric) were used to evaluate the statistical significance of the results. Results: No positive correlation between the IgG index and the number of OCB was found. Mann-Whitney test also did not demonstrate any significant difference of the IgG index values in patients both with the OCB number ≥ 2 and < 2. Úvod: Česká republika zaujímá v incidenci kolorektálního karcinomu (KRK) čelní místo ve světě. Radikální odstranění nádoru představuje nejúčinnější část v rámci současné multimodální terapie. Autoři prezentují průběžné výsledky roboticky asistované léčby u nemocných s karcinomem rekta (KR). Metoda: Vlastní soubor obsahuje 61 roboticky operovaných pacientů pro KR. U posledních 31 pacientů byla data získávána prospektivně. Analyzovány byly epidemiologické údaje, perioperační ukazatele, komplikace a onkologické parametry. Výsledky: Roboticky asistovanou operační léčbu pro karcinom rekta na našem pracovišti podstoupilo 61 nemocných: 34 mužů a 27 žen průměrného věku 62 let (33–80 let). Neoadjuvantní onkologická léčba byla indikována u 46 % pacientů. Průměrná krevní ztráta činila 187 ml, u tří nemocných byly podány transfuzní přípravky. Šest operací jsme byli nuceni konvertovat, u 16 pacientů jsme zaznamenali komplikace. Insuficience anastomózy se vyskytla u 10 % pacientů, u 4 nemocných si stav vyžádal operační léčbu. Žádný nemocný nezemřel. Rekurenci maligního onemocnění jsme diagnostikovali u 3 (5 %) pacientů. U 27 nemocných byla (od roku 2013) stanovena kvalita mezorektální excize (ME) a metrické stanovení cirkumferentního resekčního okraje – (y)pCRO. Ve dvou případech byla potvrzena pozitivita (y)pCRO a přibližně u čtvrtiny operantů nebyla ME kompletní. Závěr: Chirurgická léčba je v interdisciplinárním terapeutickém procesu u KR klíčová. Naše průběžné výsledky jsou srovnatelné se závěry některých literárně uveřejněných sdělení. Da Vinci systém je bezpečným manipulátorem v léčbě KR a operatérovi přináší nesporné výhody při preparaci ve stísněném pánevním prostoru. V současnosti chybí zhodnocení přínosu robotické operativy pro pacienta v oblasti břišní chirurgie z dlouhodobého hlediska a s dostatečným počtem operantů, resp. vysokým stupněm evidence based medicine (EBM). Vysoká pořizovací cena přístroje, jednotlivých instrumentů s vybavením a nesystémová úhrada představují významnou překážku v širším využití robotického systému v léčbě KR i jiných abdominálních malignit v ČR., Introduction: The incidence of colorectal cancer (CRC) in the Czech Republic is reported to be one of the highest on the global scale. Radical tumor removal has been observed to be the most effective part in the context of current multimodal therapy. The authors present their preliminary results of robotic assisted treatment of rectal carcinoma (RC). Method: The observed group includes 61 patients who underwent robotic assisted treatment for rectal cancer. The data were collected prospectively in the last 31 patients. Analyses were conducted on epidemiological data, perioperative outcomes, complications and oncological results. Results: Robotic assisted treatment of RC was performed in 61 patients: 34 men and 27 women, mean age of 62 years (33–80). Neoadjuvant oncological treatment was indicated in 46% of the patients. Average blood loss was 187 ml, transfusions were administered in three cases. Conversion to open procedure was performed 6 times, and 16 patients had postoperative complications. Anastomotic leak was observed in 10% of the patients, and 4 patients undewent surgical treatment. No patient died. Local recurrence of the cancer was diagnosed in 3 (5%) patients. The quality of mesorectal excision (ME) and the circumferential resection margin [(y)pCRM] have been determined in 27 patients since 2013. Positive (y)pCRM was recorded in two cases and incomplete ME was observed in 25.8% of the patients. Conclusion: Surgical treatment for RC is pivotal in multimodal therapy. Our preliminary results are similar to the conclusions in other published studies. The da Vinci robotic system is a safe manipulator in the treatment of RC and provides indisputable benefits to the surgeon when operating in the narrow pelvic space. However, the benefits of robotic treatment in abdominal surgery are yet to be evaluated in patients (with respect to long-term results, sufficient number of patients or a high EBM level of evidence). The high purchase price of the robotic device, individual instruments with equipment and non-systemic compensation constitute a significant hindrance that prevents wider use of the robotic system in the treatment of RC and other abdominal malignancies in the Czech Republic., and D. Retrospektivní studie u žen do 49 let analyzovala novotvary následující po primárních nádorech cervixu (CC), dělohy (CU), vaječníků (CO) a primární novotvary před těmito gynekologickými nádory (GC), hlášené v Národním onkologickém registru ČR v letech 1976–2010. Celkem 3 727 pacientek těchto tří diagnóz představovalo 12,1 % z 30 794 nově evidovaných GC, spojených s 4 160 dalšími novotvary. Jejich vývoj se týkal a) 1 339 žen s primárním CC, 715 s CU, 664 s CO (s následnými 1 509 a 831, respektive 758 dalšími novotvary), b) 263 žen s následným CC, 212 s CU, 534 s CO (s předcházejícími 274 a 223, respektive 565 primárními novotvary). Průměrný interval vzniku následných novotvarů byl 14,2 roku po CC, 13,6 roku po CU a 10,2 roku po CO. Nejvíce z 3 098 následných nádorů bylo 21 % trávicího traktu, 16 % prsů, 13 % kůže, 11,7 % rodidel, 9,7 % dýchacích a 7,8 % močových cest; z nich během prvního roku po primárním GC bylo nejvíce nádorů vaječníků, endometria, cervixu a kolorekta. Nejvíce z 1 063 primárních novotvarů před GC bylo 26,7 % rodidel, 24,3 % prsů a 14,8 % trávicího traktu. Během 35 let počet žen s primárními GC klesal, zejména v zastoupení jejich časných klinických stadií, žen s následnými GC rostl, zejména jejich pokročilých stadií. Časná stadia byla evidována u 70,4 % žen s primárními GC a 54 % s následnými GC, pokročilá stadia u 29,5 % žen s následnými GC a 13,3 % s primárními GC, neznámá stadia zahrnovala asi 16,5 %. Ze studie se odhaduje asi 1100 případů pokročilých stadií u žen do 49 let přežívajících s GC v letech 1976–2010. Většina z hodnocených kritérií byla nepříznivá u žen s karcinomy ovarií. Zejména těmto případům v pokročilých stadiích by měly zabránit dispenzární opatření a preventivní intervence. Omezené finanční zdroje a přetrvávající rizika životního stylu neumožňují zpomalit v blízké budoucnosti nárůst primárních a následných novotvarů u onkologicky přežívajících žen nejen v gynekologii., The neoplasms following primary cancers of cervix (CC), uterus (CU), ovary (CO), and primary neoplasms before these gynaecological cancers (GC), were analyzed in a retrospective study in survived females aged 0–49 years, notified in the National Czech Cancer Registry between 1976 and 2010. A total 3,727 patients of these three diagnoses, presented 12.1 % of 30,794 newly registered GC, were associated with 4,160 other neoplasms. Their development was concerned with a) 1,339 females with primary CC, 715 with CU and 664 with CO (followed by 1,509 and 831, respectively 758 other neoplasms) and b) subsequent 263 CC, 212 CU and 534 CO (preceded by 274 and 223, respectively 565 primary neoplasms). The average interval of occurrence of subsequent neoplasms was 14.2 years after CC, 13.6 years after CU and 10.2 years after CO. The most frequent of 3.098 subsequent neoplasms were 21 % of digestive tract, 16 % breast, 13 % skin, 11.7 % female genital, 9.7 % respiratory and 7.8 % urinary organs; the most frequent during the first year after primary GC were those of the ovarial, endometrial, cervical and colorectal cancers. The most frequent of 1,062 primary neoplasms before GC were 26.7 % cancers of female genital organs, 24.3 % breast and 14.8 % digestive tract. During 35 years the number of females with primary GC has decreased, especially of their representation at the early clinical stages, the females with subsequent GC has increased, especially at the advanced stages. There were recorded early stages in 70.4 % females with primary GC and 54 % with subsequent GC, advanced stages in 29.5 % females with subsequent GC and 13.3 % with primary GC, the unknown stages included about 16.5 %. In our study are estimated about 1100 cases of advanced stages in females aged 49 years survived with GC in 1976–2010. Most of the evaluated criteria were unfavorable among females with ovarian cancers. Mainly the cases at advanced stages must be prevented by the dispensary guidelines and preventive interventions. INTRODUCTION: Cellulitis remains a very serious disease even today. Mortality, which varied between 10-40%, has been reduced owing to the standard securing of airway patency and use of an appropriate surgical treatment approach. MATERIALS AND METHODS: A total of 195 patients were hospitalised for cellulitis at the University Hospital in Hradec Králové during 2007-2011. The following parameters were evaluated: age, gender, dependence of incidence of the disease on the season of the year, frequency of attacks of the particular areas and their clinical characteristics, aetiology of the inflammation, types of patient complaints, prevalence of current systemic diseases, results of microbiological and selected laboratory analyses, socio-economic status of the patients, and duration of patient stay at the hospital. Statistical analysis was performed by using Pearson's correlation coefficient, the statistical significance level was p < 0.05. RESULTS: The mean age of the patients was 39.8 years. The group of 195 patients included 108 (55%) males and 87 (45%) females. The mean time between the first symptoms of the disease and admission to the Department was 5 days. From among the 195 patients, 116 (59.5%) were working persons, 79 (40.5%) were non-working (children, students, unemployed persons, women on maternity leave, retired people). The odontogenic origin of the disease was verified in 173 (88.7%) patients. In total, 65 (33.3%) patients had no coinciding complicating systemic disease, 22 (11.3%) patients had diabetes mellitus. The most frequent symptom of cellulitis was painful swelling, found in 194 (99.5%) patients, followed by jaw contracture, found in 153 (78.5%) patients. CONCLUSION: The results are largely very similar to those of previous studies performed in other countries, except that we found no correlation between the prevalence of cellulitis and the socio-economic status, nor have we confirmed Klebsiella pneumoniae sp. as the cause of cellulitis in patients with diabetes mellitus. and H. Úvod: Dekubity u paraplegiků představují náročný problém vedoucí k opakovaným hospitalizacím, mnohonásobným operacím a potenciálně devastujícím komplikacím Metody: Přehledový článek. Výsledky: Uvádíme možnosti rekonstrukčních operací ischiadických, sakrálních a trochanterických dekubitů a vlastní zkušenost s lalokovými plastikami takových dekubitů u pacientů operovaných v průběhu tří let. Závěr: Zdůrazňujeme univerzálnost a spolehlivost gluteálního laloku, nezbytnost kvalitní předoperační přípravy a pooperační péče., Introduction: Pressure ulcers in spinal cord injury represent a challenging problem that often leads to recurrent hospitalizations, multiple surgeries, and potentially devastating complications. Methods: Review article. Results: The paper presents possible types of reconstructive surgery of ischial, sacral and trochanteric pressure ulcers and our own experience with procedures of such pressure ulcers done in three years.
