The aim of this study was to compare the isolation systems OptraDam® Plus and OptiDam™ with the conventional rubber dam in terms of objective and subjective parameters. The isolation systems were applied during the dental treatment of the patients. The time of preparation, placement, presence and removal were measured and the quality of isolation was evaluated. The median time of rubber dam placement was 76 s (Q1=62 s; Q3=111.25 s). The application time of OptraDam® Plus was significantly longer compared to the other systems (P ® plus. In the last decade the health of young people in Ukraine as well as in many other countries of the world significantly deteriorated according to the results of screening tests. Because of this the main task of prophylactic medicine is development and introduction of new methods of early diagnostics of diseases. The aim of the work to assess the peculiarities of adaptation of students with different types of vegetative regulation. Material and methods: 415 second year students of Lugansk State Medical University were tested. The mechanisms of physiological functions of the body regulation were assessed by registration of cardiac rhythm variability (CRV). Analysis of CRV was performed by three methods: statistical, geometrical, and spectral analysis. The express method proposed by Shlyk, N.I. (2009) was later used to determine the prevailing type of vegetative regulation. To assess adaptability the method of R.M. Baevskij (1979) was used. Results: In the result of investigation 4 types of functional states of regulatory systems were determined: I type 295 (71 %) of tested, II in 14 (3 %), III in 96 (23 %) and IV only in 10 (2%). I type of functional state of regulatory systems in the wakeful state at rest was characterized by moderate prevalence of central regulation of heart rhythm (MPCR), decreased activity of autonomic regulation. It was noticed that some of the parameters of CRV (like RR, SDNN, RMSSD and pNN50) which characterize autonomic regulation, were statistically lowere (р≤0.01) compared to type III, and some (like AMo and SI) higher, in both female and male students. Summarized spectrum capacity and components of its wave structure (HF, LF, VLF) which characterize central regulation were statistically significantly lower (р≤0.01) for the I type of regulation compared to the III one which is characterized by moderate prevalence of autonomic regulation of the heart rhythm. In female students with type I autonomic regulation compared to male students such CRV parameters as RR and AМo were statistically significantly lower (р≤0.05), while the parameters of total potency (TP) and high frequency waves (HF) were higher (р≤0.05), which could point to more centralized heart rhythm regulation in male compared to female. The evaluation of regulatory systems overstress allowed to determine the state of adaptation and risk of overstress or breakdown in students with different types of autonomic regulation. Cíl práce: Analyzovat výsledky indukovaných porodů u primipar po ukončeném 41. týdnu těhotenství bez další zátěže. Typ studie: Retrospektivní klinická studie. Název a sídlo pracoviště: Gynekologicko-porodnická klinika 1. LF UK v Praze, Nemocnice Na Bulovce. Materiál: V letech 2011–2013 porodilo na Gynekologicko-porodnické klinice v Nemocnici Na Bulovce celkem 6 317 žen. U 203 prvorodiček byl porod indukován po ukončeném 41. týdnu těhotenství prostaglandinem dinoproston. Do kontrolní skupiny bylo zařazeno 291 primipar se spontánním začátkem porodu po termínu. Metodika: Porovnali jsme u obou skupin věk rodiček, celkovou délku porodu, celkovou krevní ztrátu, rozdíly ve způsobu ukončení porodu, výskyt hypotonie děložní a manuální vybavení placenty, porodní poranění, četnost epidurální analgezie a poporodní adaptaci novorozenců (Apgarové skóre a pH z pupečníkové krve novorozenců). Údaje byly retrospektivně získány ze zdravotnické dokumentace a statisticky zpracovány. Výsledky: Ve sledované skupině jsme zjistili signifikantně vyšší četnost ukončení porodu císařským řezem (21,2 % vs 11,0 %, p < 0,002), statisticky významně vyšší krevní ztrátu (341 ml vs 314 ml, p = 0,002) a četnost podání epidurální analgezie (25,1 % vs 11,7 %, p < 0,001). Další sledované parametry byly srovnatelné u obou skupin. Předpokládaný vliv věku nad 35 let na frekvenci císařského řezu nebyl v této studii prokázán. Závěr: U primipar s indukovaným porodem jsme zjistili výrazně vyšší frekvenci císařských řezů. Délka porodů a frekvence hypotonie děložní byly srovnatelné s kontrolní skupinou. Farmakologická indukce porodu prostaglandinem E2 z důvodu potermínové gravidity představuje u primipar silný rizikový faktor k ukončení císařským řezem., Objective: To analyse effect of induced labor in primiparous women after completed 41st gestation week without any other pathology. Design: Retrospective clinical study. Setting: Department of Gynaecology and Obstetrics, First Faculty of Medicine, Charles Univesity in Prague, Hospital Na Bulovce. Material: Altogether 6 317 women delivered at Hospital Na Bulovce in Prague between years 2011–2013. In 203 primiparous women after completed 41st gestation week, labor was induced by prostaglandin dinoproston. The control group consisted of 291 primiparae with spontaneus onset of labor after the term. Methods: We compared the groups in respect to patient´s age, parity, total duration of labor, total blood loss, frequency of different ways of delivery, frequency of analgesia, frequency of diverse delivery injuries, manual removal of placenta and uterine hypotonia, neonatal outcomes (Apgar score of newborns, and the pH of umbilical arterial and vein blood). Data were retrospectively obtained from the medical documentation and statistically processed. Results: We proved significantly higher frequency of Caesarean sections (21,2 % vs 11,0 %, p < 0,002), statistically significant higher blood loss (341 ml vs 314 ml, p = 0,002), and frequency of epidural analgesia applications (25,1 % vs 11,7 %, p < 0,001) in the study group. Other analysed parameters were similar in both groups. Expected effect of age more than 35 years on frequency of Caesarean section was not proved in the study group. Conclusion: We found a substantially higher frequency of Cesarean sections in primiparous women with induced labor. The duration of delivery and the frequency of uterine hypotonia were similar as in the control group. Antrochoanal polyps are benign lesions originating from the mucosa of the maxillary sinus. Nasal obstruction and rhinorrhea are their main symptoms. Their endoscopical and radiological appearance makes them relatively easy to diagnose. These polyps are usually presented unilaterally, although bilateral presentation is also possible. We described two cases of atypically giant antrochoanal polyps: in a 15-year-old child and in a 38-year-old man. In both cases, the diagnosis was done by nasal endoscopy and computed tomography (CT) of the paranasal sinuses and supported by histopathological analysis. In the first patient, the excised polyp had the histological characteristic of an angiomatous antrochoanal polyp. Because of their unusual dimension, the combined transoral and endonasal endoscopic approach was performed for complete polyp excision. We discussed the clinical, histopathological and immunohistochemical characteristics of choanal polyps in comparison to inflammatory nasal polyps, and the applicable surgical techniques for treatment of these polyps. and C. The aim of our study was to determine the significance of lymphocyte-platelet adhesion (LPA), interleukins, transforming included 139 pregnant women aged between 17 and 27 years (21.3±4.22 years). GH was diagnosed in 119 women after 20 weeks of pregnancy. 20 patients (control group) were with physiological course of pregnancy. The distribution of patients by groups was carried out according to the level of blood pressure (BP) in accordance with ICD-10 (Geneva, WHO, 2002). The survey was conducted at the moment of detection pregnancy from 7 to 10 weeks and in dynamics of I, II and III trimesters of gestation. In the dynamics of gestation, were studied the number of desquamated endothelial cells circulating in the systemic circulation (CECs), nitrates levels, the adhesion of platelets by estimation their ability to form co-aggregates with lymphocytes by determining the percentage of lymphocytes aggregates with thrombocytes (lymphocyte-platelet plugs), at risk of hypertensive disorders, especially after 20-22 weeks of gestation and later, lymphocytes ability to platelets adhesion is rose, the concentrations of pro-inflammatory cytokines and NO level are increased. The target of research was the implementation of the comparative analysis of conservative therapy of preparation of basic therapy and also new methods of treatment (anticytokine therapy). Appointment of salofalk in twos tablets (0, 5) 34 times a day per os, and also 2,0 gr in the form of a rectal enema unitary for a period of seven days, then had transferd the patients to reception 2,0 gr salofalk per os, allowed to achieve substantial improvement patients cliniclaboratory and endoscopic symptoms with a heavy intensification of ulcer colitis variation, providing full cliniclabaratory remissions achievements of disease at 8 (34 %) patients. In the Minister of the Uzbek Public Health of the Republican Science Coloproctological Center at 10 patients with the IDI have spent the therapy remicade, 7 of them had NUC and 3 patients had the Crohn's disease. Therefore, all patients no longer than on the first week after introduction of remicade had improvement of a course disease which was expressed in defering stool, extinction or reduction of pathological impurity in excrement and subsidence bellyaches. During colonoscopical inspection, in 1218 weeks after the first infusion at 80% of the patients had observed disappearance of ulcers and erosion. In terms of 6 months of supervision of relapse of disease was not noted. All new directions in treatment of the IDI, apparently, are perspective, but while are proved only theoretically and had no practical development. INTRODUCTION: Cellulitis remains a very serious disease even today. Mortality, which varied between 10-40%, has been reduced owing to the standard securing of airway patency and use of an appropriate surgical treatment approach. MATERIALS AND METHODS: A total of 195 patients were hospitalised for cellulitis at the University Hospital in Hradec Králové during 2007-2011. The following parameters were evaluated: age, gender, dependence of incidence of the disease on the season of the year, frequency of attacks of the particular areas and their clinical characteristics, aetiology of the inflammation, types of patient complaints, prevalence of current systemic diseases, results of microbiological and selected laboratory analyses, socio-economic status of the patients, and duration of patient stay at the hospital. Statistical analysis was performed by using Pearson's correlation coefficient, the statistical significance level was p < 0.05. RESULTS: The mean age of the patients was 39.8 years. The group of 195 patients included 108 (55%) males and 87 (45%) females. The mean time between the first symptoms of the disease and admission to the Department was 5 days. From among the 195 patients, 116 (59.5%) were working persons, 79 (40.5%) were non-working (children, students, unemployed persons, women on maternity leave, retired people). The odontogenic origin of the disease was verified in 173 (88.7%) patients. In total, 65 (33.3%) patients had no coinciding complicating systemic disease, 22 (11.3%) patients had diabetes mellitus. The most frequent symptom of cellulitis was painful swelling, found in 194 (99.5%) patients, followed by jaw contracture, found in 153 (78.5%) patients. CONCLUSION: The results are largely very similar to those of previous studies performed in other countries, except that we found no correlation between the prevalence of cellulitis and the socio-economic status, nor have we confirmed Klebsiella pneumoniae sp. as the cause of cellulitis in patients with diabetes mellitus. and H.
