Epilepsia je chronické ochorenie, ktoré je často sprevádzané psychiatrickou komorbiditou. Najčastejšou z nich je depresia, ktorá je sprevádzaná subsyndromálnymi, atypickými a chronifikovanými príznakmi depresie. Incidencia depresie je u pacientov s epilepsiou vyššia ako u iných neurologických alebo chronických ochorení. Psychopatológia spojená s epilepsiou obťažuje pacienta častokrát viac ako samotné ochorenie. Cieľom práce je prezentácia a analýza kazuistiky 37-ročného muža s diagnózou epilepsie so sprievodnou psychiatrickou symptomatikou, ktorá znížila kvalitu jeho života oveľa viac ako samotné neurologické ochorenie. V kazuistike popisujeme klinické symptómy neuropsychiatrického ochorenia – epilepsie ako aj životné udalosti, ktoré sú u každého pacienta jedinečné a majú závažný vplyv na priebeh a liečbu epilepsie., Epilepsy is a chronic disease that is often accompanied by psychiatric comorbidity. The most frequent of them is depression accompanied by subsyndromic, chronic and atypical symptoms of depression. The incidence of depression in patients with epilepsy is higher than in other neurological or chronic diseases. Psychopathology associated with epilepsy often bothers the patient more than the disease itself. The aim of this work is to present and analyze a case report of a 37‑year‑old man diagnosed with epilepsy and associated psychiatric symptomatology that reduced his quality of life much more than the neurological disease itself. In this case report, we describe clinical symptoms of neuropsychiatric disease – epilepsy as well as life events that are unique to each patient and have a significant impact on the course and treatment of epilepsy. Hlboká infiltrujúca endometrióza (DIE) je chronické progresívne estrogén dependentné ochorenie, ktoré môže postihovať aj štruktúry gastrointestinálneho traktu. Ochorenie je zvyčajne dlhodobo asymptomatické, respektíve sa prejavuje veľmi nešpecifickými príznakmi. Jeho neskorý záchyt môže viesť k ireverzibilným zmenám na jednotlivých štruktúrach. Táto kazuistika popisuje raritný prípad rozsiahlej hlbokej infiltrujúcej endometriózy zadného kompartmentu malej panvy, ktorá vyústila do stavu obštrukčného ilea. Protinádorová farmakoterapie je vedena dominantně parenterální formou, a proto je volba adekvátního žilního vstupu v onkologii zásadním tématem, nabývajícím na významu s rozšiřováním možností druhů žilních katétrů, které jsou k dispozici. Periferní žilní systém je navíc v některých případech při dlouhodobé léčbě výrazně zatěžován, neboť celá řada látek má nevyhovující vlastnosti pro periferní aplikaci – hlavně pH, osmolalitu a chemickou strukturu. Permanentní centrální venózní vstup nabízí šetření periferního žilního systému, je prevencí paravazace a lze jej využívat ve většině případů k odběrům krve. Kromě portů a tunelizovaných centrálních kanyl zažívá renesanci inzerce periferně implantovaných centrálních katétrů (PICC), které splňují kritéria permanentního centrálního vstupu. Uvádíme indikační kritéria pro implantaci jednotlivých druhů vstupů s přihlédnutím k vlastnostem pacienta, charakteru a délce podávané léčby. V rámci Společnosti pro porty a permanentní katétry jsme definovali několik oblastí onkologické péče, kdy můžeme preferovat inzerci PICC před ostatními druhy žilních katetrů., The choice of adequate venous access device is a key issue in oncology since anticancer drug therapy is administered via the parenteral route predominantly. An increasing importance of this topic has been observed with the expansion of various types of venous catheters available. In some cases, long-term therapy places a heavy burden on the peripheral venous system because many substances have unsatisfactory properties for peripheral administration – mainly the pH, osmolality and chemical structure. A long-term central venous access device offers a way to spare the peripheral venous system, prevents extravasation, and can be used for blood sampling in most cases. In addition to ports and tunneled venous access devices, there is a renaissance in insertion of peripherally inserted central catheters (PICCs) that meet the criteria for a permanent central venous access device. The indication criteria for the insertion of various kinds of devices are presented, taking into account the patient characteristics as well as the type and duration of the treatment administered. Autori prezentujú kazuistiku zriedkavej náhlej brušnej príhody. Ide o 39-ročnú polymorbídnu pacientku so systémovým lupus erythematosus, chronickou renálnou insufficienciou na podklade lupusovej nefritídy, v hemodialyzačnom programe, po transplantácii ľavej obličky v minulosti, prijatej na Traumatologickú kliniku za účelom totálnej endoprotézy (TEP) ľavej koxy pre bionekrózu hlavice femuru. V pooperačnom priebehu bol stav komplikovaný gangrénou kolonu, CT i peroperačne verifikovanou. Pacientka bola operovaná, bola vykonaná subtotálna kolektómia, terminálna ileostómia a ľavostranná ovarektómia. Pooperačný priebeh bol komplikovaný perforáciou jejuna, bola vykonaná sutura jejuna. Pacientka bola hospitalizovaná na OAIM, následne na Chirurgickej klinike, bola plne realimentovaná. Na Internej klinike bola doriešená pre rozvrat vnútorného prostredia. Po 60 dňoch bola prepustená z nemocnice v klinicky dobrom stave s obnovenou pasážou, plne per os alimentovaná, mobilná., The authors present a case report of a 39-year-old woman with acute abdomen – a comorbid patient with systemic lupus erythematosus, chronic renal insufficiency as a complication of lupus nephritis, included in a haemodialysis programme. The patient had also undergone transplantation of the left kidney in the past. She was initially admitted to the Department of Traumatology for a total endoprosthesis procedure due to bionecrosis of the head of the thigh bone. Postoperatively, the patient’s condition was complicated by gangrene of the colon confirmed by CT scan and during the operation. The patient was operated on – subtotal colectomy, terminal ileostomy and left-sided ovariectomy was performed. The postoperative course was complicated by perforation of the jejunum which was sutured. The patient was admitted to ICU and, after recovery, to our surgical department. Because of the metabolic disturbance she was treated in the internal medicine department. After 60 days she was discharged in a good condition, walking and with full per os realimentation., and S.
