Sdělení podává současný pohled na úlohu jednotlivých zobrazovacích metod v diagnostice mnohočetného myelomu (MM) a jeho odlišení od monoklonální gamapatie nejistého významu (MGNV). Jsou rozvedeny možnosti i úskalí rozpoznání jednotlivých projevů myelomové kostní nemoci při použití konvenční radiografie skeletu, výpočetní tomografie (CT), magnetické rezonance (včetně celotělové formy MR), 99mTc-MIBI scintigrafie a FDG-PET/CT, sdružující vyšetření s pomocí pozitronové emisní tomografie a multidetektorové výpočetní tomografie. Je popsán přínos nových zobrazovacích metod pro stratifikaci MM (stážovací systém Durie-Salmon Plus), monitorování vývoje nemoci včetně hodnocení léčebné odezvy, rozpoznání relapsu/progrese a extramedulárního šíření nemoci. Jsou rovněž zhodnocena interpretační úskalí jednotlivých zobrazovacích technik a principy jejich individuální volby podle specifické klinické situace., The paper presents recent opinion on individual imaging methods in the diagnostics of multiple myeloma (MM), and its differentiation from monoclonal gammopathy of undetermined significance (MGUS). We assess the possibilities and difficulties in the differentiation of individual manifestations of myeloma bone disease using conventional radiography, computed tomography (CT), magnetic resonance imaging (including whole body MR), 99mTc-MIBI scintigraphy, and FDG-PET/CT which combines positron emission tomography and multidetector computed tomography. We describe the contribution of new imaging methods for MM stratification (Durie-Salmon Plus staging system), monitoring of the course of MM including therapeutics response, recognition of relapse/progression, and extramedular progression of the disease. Užití radiodiagnostických metod v diagnostice gynekologických onemocnění má delší tradici, avšak jeho význam byl dlouhá léta podceňován. Vedle klasických metod (vylučovací urografie, irrigoskopie, hysterosalpingografie) se ani výpočetní tomografie nestala metodou zlatého standardu. Její role přetrvává na poli zobrazování ovariálních tumorů a posuzování výsledků chirurgické či konzervativní léčby nebo recidiv gynekologických onemocnění. Výpočetní tomografie nicméně selhává v diagnostice nádorů dělohy. Teprve vstup magnetické rezonance zlepšil diagnostiku děložních tumorů. V současné době je magnetická rezonance užívána především k stagingu karcinomu cervixu, eventuálně k průkazu jeho recidiv. Klíčová role magnetické rezonance ženského vnitřního genitálu je v potvrzení invaze parametrií (rozlišení mezi stadiem TIIa a TIIb dle FIGO klasifikace). Pomáhá v rozhodování mezi chirurgickou léčbou nebo aktino- či chemoterapií. Ve snaze zlepšit tuto diagnostiku vstupuje vedle zobrazování v T1 nebo T2 obrazu užití difuzního vážení nebo dynamické sekvence (perfuzní zobrazení). Druhou nejčastější indikací magnetické rezonance v diagnostice gynekologických onemocnění je zobrazování endometriózy Magnetická rezonance prokáže různá stadia i lokalizace endometriózy. Hybridní metoda PET/CT je užívána k vyloučení generalizace nádorů dělohy. Vychází se z faktu zvýšeného metabolismu radioizotopem fluóru (18F) značené glukózy v buňkách nádoru. Aktuálně dochází k rychlému rozvoji jak vlastních vyšetřovacích metod v gynekologii, ale i gynekologického zobrazování., The use of radiodiagnostic methods in diagnosis of gynaecological diseases has longer tradition but their relevance has been for long time underestimated. Besides classical methods (excretion urography irrigoscopy hysterosalpingography) neither computed tomography did not gain acceptance as a method of golden standard. The role of computed tomography remained on the field of imaging of ovarian tumours and the assessment of results of surgical or conservative therapy and recurrence of female diseases. Nevertheless, computed tomography fails in diagnostic of uterine tumours. Only start of magnetic resonance improved the diagnostic of uterine tumours. Recently, magnetic resonance is used especially for staging of cervical carcinoma or to prove its recurrence. Key role of magnetic resonance of female inner genital organs is to confirm the invasion to parametria (the differentiation between the stadium T II a and T II b according to FIGO classification). It helps to decide between surgical therapy or actino- chemotherapy. In order to improve this diagnostic apart from T1 and T2 weighted images diffusion weighted images and dynamic imaging (perfusion) are used. The second most frequent indication of magnetic resonance in diagnostic of gynaecological diseases is imaging of endometriosis. Magnetic resonance depicts various stadia and localisations of endometriosis. The hybrid method PET/CT is used to confirme dissemination of malignant tumours of uterus. The method is based on the presumption of increased metabolism in tumour cells.
