Úvod: Tři jednonukleotidové polymorfismy (SNP) v NOD2/CARD15 genu (3020insC, R702W, G908R) jsou pro Crohnovu chorobu významným rizikovým faktorem. Jejich diagnostické a terapeutické využití v klinické praxi je však doposud limitované. Cílem studie byla analýza možné asociace těchto polymorfismů s rizikem reoperací a posouzenísouvislosti s fenotypem onemocnění. Metody: U 76 pacientů s Crohnovou chorobou a minimální délkou trvání nemoci 5 let byly testovány tři sekvenční varianty NOD2/CARD15 genu (R702W, G908R, 3020insC). Anamnestická a klinická data, včetně chirurgické léčby byly retrospektivně získány ze zdravotnické dokumentace a prospektivně doplňovány podle protokolu studie. Analyzovány byly souvislosti mezi vybranými genovými polymorfismy, nutností reoperací a fenotypem nemoci. Výsledky: U 24 pacientů (32 %) se našla alespoň jedna sledovaná varianta v NOD2/CARD15 genu. 25 pacientů (33 %) bylo reoperováno, 51 (67 %) představovalo kontrolní skupinu. Trend vyšší četnosti reoperací ve skupině s genovou mutací nedosáhl statistické významnosti (p=0,2688). Dva ze čtyř homozygotů (50 %) nesoucích variantu 3020insC byli opakovaně operováni. Neprokázali jsme signifikantní rozdíly mezi soubory s wild formou a mutovanou formou NOD2/8CARD15 genu s ohledem na věk v době diagnózy (p=0,4356), formu onemocnění (p=0,6610) a lokalizaci (p=0,4747) podle Montrealské klasifikace. Závěr: Polymorfismy v NOD2/CARD genu signifikantně neovlivnily četnost reoperací, homozygoti nesoucí mutaci 3020insC genu však představovali vysoce rizikovou skupinu. Fenotyp onemocnění signifikantně nesouvisel s přítomností vyšetřovaných sekvenčních variant., Introduction: Three NOD2/CARD15 gene variants (3020insC, R702W, G908R) have been identified as genetic risk factors for Crohn’s disease patients. However the diagnostic and therapeutic relevance for clinical practice remains limited. The aim of this study was to evaluate the association between these variants, the risk of reoperation and disease phenotype. Methods: In 76 Crohn’s disease patients (41 female, 35 male) with a minimum 5 year follow-up, three polymorphisms of the NOD2/CARD15 gene (R702W, G908R, 3020insC) were tested. Detailed clinical and medical history including surgical procedures and reoperations were obtained by reviewing the medical charts and completed prospectively. Association between the need for reoperation, disease phenotypes and gene variants were analyzed. Results: 24 patients (32%) showed at least one NOD2/CARD15 mutation. 25 patients (33%) required reoperation, 51 (67%) represented the control group. The expected trend that patients with NOD2/CARD15 variants have a higher frequency of reoperations was not confirmed to a level of statistical significance (p=0.2688). Two of the four patients (50%) with the 3020insC variant required further surgery. We did not confirm any association between NOD2/CARD15 mutations and age at diagnosis (p=0.4356), behavior (p=0.6610), or localization (p=0.4747) according to the Montreal classification. Conclusion: NOD2/CARD15 polymorphisms did not significantly affect the reoperation rate. Homozygosity for the 3020insC variant in the NOD2/CARD15 gene is associated with a high risk of reoperation. Objective: To determine risk factors in different pathogenic subtypes of ischemic stroke in Tashkent hospital registry and to compare with the data of other stroke registers. Design: Prospective observational study. Participants: 300 adults aged 18 years or over who suffered first-ever acute ischemic stroke, of them, the data of 100 patients with atherothrombotic, lacunar and cardioembolic strokes were analyzed. Main outcome measures: Mathematically significant differences and not significant differences but interesting from other point of view. Categorical variables are expressed as frequencies and percentages. Differences were regarded as statistically significant when the P value was less than 0.05 (P<0.05). Results: Of 100 patients with acute first-ever ischemic stroke at the median age of 62.63±4.68 (interquartile range 25-92) years, 57% were men and 43% were women. Depending on aetiology and stroke subtype, all the patients were divided into three groups: 42 patients with atherothrombotic stroke, 41 – with lacunar stroke, and 17 patients with cardioembolic stroke. Atherothrombotic stroke developed often in people over 60 years old, while lacunar stroke prevailed at the younger age. Stroke cases increased in people over 51 years old. Although men were more likely to suffer strokes than women, stroke incidence was equal in both sexes with increasing of age (over 60 years old), especially in atherothrombotic stroke, testifying to that women live longer. Combination of cerebral atherosclerosis and hypertension was the most significant risk factor for stroke. Atherosclerosis was the main aetiological factor for ischemic stroke, independently of its pathogenic heterogeneity. Diabetes mellitus was an independent risk factor for all stroke subtypes, which in most cases worsened the European Medical, Health and Pharmaceutical Journal ISSN 1804-5804 course and outcome of stroke. Hypertension was the leader aetiological factor for lacunar stroke. In these patients, we Cardioembolic stroke developed mainly because of cardiac thrombosis and valve pathology, which were often associated with chronic diseases of other organs, worsening the course and outcome of stroke. Despite of high prevalence of myocardial infarction in men, women were more likely to develop cardioembolic stroke. Conclusions: This study found simultaneous influence of many risk factors (multifactorial disease) in the development of ischemic stroke. The more course and influence of the underlying disease and risk factor, the higher stroke risk and severity, especially in the absence or inadequate previous therapy of underlying disease before the stroke index. We have noted a high prevalence of under-diagnosis of vascular risk factors at levels of primary and secondary care. We recommend for improved public awareness of vascular risk factors and better diagnostic and treatment facilities aimed at addressing those factors at levels of primary and secondary healthcare. Cílem naší práce bylo přispět k hodnocení zdravotního rizika kombinované inhalační a dermální expozice PAU u osob pracujících v oboru silniční stavitelství. V rámci hodnocení expoziční cesty bylo v pracovním ovzduší a na kůži exponovaných osob stanoveno 15 vybraných PAU. Zhodnocení genotoxické expozice (chromozomové aberace periferních lymfocytů) bylo provedeno před/po sezoně (jaro/podzim) vždy jednorázovým odběrem krve. Skupina pracovníků v oboru silniční stavitelství byla inhalačně exponována PAU na hladinách výrazně nižších, než jsou pracovní limity v České republice. Rovněž dle hodnocení World Health Organization (WHO) lze námi zjištěnou inhalační expozici benzo(a) pyrenu považovat za přijatelnou. Dermální expozice byla na nízké úrovni. Nalezli jsme významný nárůst chromozomových aberací periferních lymfocytů po sezoně. Tento nález však byl stále v mezích normálních hodnot. Genotoxické riziko lze považovat za nízké., The aim of our study was to contribute to the evaluation of health risk assessments of combined inhalation and dermal exposure to PAHs amongst road construction crews. Within the framework of evaluating inhalation and dermal exposure of persons there have been determined 15 selected PAHs. Assessments of genotoxic exposure (chromosomal aberrations of peripheral lymphocytes) were performed in the spring and autumn seasons with single blood samples in each case. Road construction crews were exposed to inhaled PAHs at levels significantly lower than those stipulated by work limits in the Czech Republic. According to the World Health Organization (WHO) assessment, the detected inhalation exposure to benzo(a) pyrene is also acceptable. Dermal exposure was low. We detected a significant increase in chromosomal aberrations of peripheral lymphocytes post season. This finding, however, did not exceed limit values. Úvod: Cílem studie bylo hodnocení významu Pneumonia Severity Index (PSI) pro predikci 30denní mortality pacientů s komunitní pneumonií (CAP) hospitalizovaných na jednotce intenzivní pneumologické péče (JIP). U pacientů byly hodnoceny komorbidity, komplikace, fyzikální, laboratorní, RTG a mikrobiologické nálezy a jejich vztah k prognóze nemocných. Pacienti a metody: Do studie bylo zařazeno 197 pacientů hospitalizovaných pro CAP na JIP Kliniky plicních nemocí a TBC LF UP a FN Olomouc v letech 2008–2012. U všech pacientů byla hodnocena riziková kritéria dle systému PSI. Výsledky: V celém souboru pacientů s CAP byla průměrná hodnota PSI 115,4 ? SD 30,4 bodů. Z celkového počtu 197 pacientů zemřelo 29 nemocných (14,7 %). Při srovnání skupin zemřelých a přeživších pacientů byly prokázány statisticky významné rozdíly v PSI (průměr ? SD: 137,4; 26,1 vs 111,7 ? 29,6 bodů; p < 0,0001), věku pacientů (průměr ? SD: 76,3 ? 12,9 vs 65,5 ? 14,7 let; p < 0,0001) a výskytu onemocnění srdce (86,2 % vs 67,3 %; p = 0,04) s nejvyšším podílem ICHS (58,6 % vs 38,7 %; p = 0,04). Dále při hodnocení fyzikálních a laboratorních nálezů byla prokázána u zemřelých pacientů oproti přeživším významně vyšší frekvence výskytu tachykardie nad 90/min (51,7 % vs 27,4 %; p = 0,01), tachypnoe > 30/min (37,9 % vs 13,7 %; p = 0,001) a acidózy s pH < 7,35 (27,6 % vs 8,9 %; p = 0,004). Korelační analýza neprokázala statisticky významnou závislost mezi PSI a délkou hospitalizace u přeživších pacientů. U pacientů s infekcí Staphylococcus sp. a Klebsiella pneumoniae byla zjištěna statisticky signifikantně delší doba hospitalizace. Hodnocení souvislosti ostatních sledovaných komorbidit, fyzikálních, laboratorních nálezů a patogenů s mortalitou neprokázalo významné rozdíly. Závěr: Naše studie prokázala, že PSI je významným prediktorem 30denní mortality pacientů s CAP na JIP, ale nekoreluje s délkou hospitalizace ani s nálezem jednotlivých patogenů. Nezávislými negativními prognostickými faktory u pacientů s CAP jsou věk, výskyt nemocí srdce (zejména ICHS), tachykardie, tachypnoe a acidóza. Výskyt infekce Staphylococcus sp. a Klebsiella pneumoniae se významně podílejí na prodloužení doby hospitalizace. Všechny tyto faktory mohou upozorňovat na potřebu zvýšené péče o pacienty s CAP., Introduction: The aim of this study was to evaluate the prognostic value of Pneumonia Severity Index (PSI) for prediction of 30-day mortality in patients admitted to intensive care unit (ICU) for community-acquired pneumonia (CAP). In patients with CAP, comorbidities, complications, and physical, laboratory, radiological and microbiological findings were evaluated relative to their prognosis. Patient and methods: In the study, 197 patients with CAP, hospitalised at ICU of Department of Respiratory Medicine, University Hospital Olomouc between 2008 and 2012, were enrolled. Risk factors according to PSI were assessed in all patients. Results: In the studied cohort of patients with CAP, mean values of PSI were 115.4 ? SD 30.4 points. Overall, 29 patients (14.7 %) deceased. When comparing deceased and survived patients, statistically significant differences were found in PSI (mean ? SD: 137.4 ? 26.1 vs 111.7 ? 29.6 points, p < 0.0001), age (mean ? SD: 76.3 ? 12.9 vs 65.5 ? 14.7 years, p < 0.0001), incidence of heart diseases (86.2 % vs 67.3 %, p = 0.04) and ischaemic heart disease (58.6 % vs 38.7 %, p = 0.04). Assessment of physical and laboratory findings showed that deceased patients had significantly increased incidence of tachycardia above 90/min (51.7 % vs 27.4 %, p = 0.01), tachypnoe above 30/min (37.9 % vs 13.7 %, p = 0.001) and acidosis with pH < 7.35 (27.6 % vs 8.9 %, p = 0.004) comparing to survived patients. No significant correlation between PSI and the length of hospitalisation in survived patients was observed. In patients with Staphylococcus sp. and Klebsiella pneumoniae infection, longer hospitalisation period was observed. Comparison of other parameters such as comorbidities, physical and laboratory findings, and pathogens showed no significant differences when comparing deceased to survived patients. Conclusion: Our study showed that PSI represents an important predictor of 30-day mortality in patients with CAP at ICU, but does not correlate neither with the length of hospitalisation nor with particular pathogens. Independent negative prognostic factors in CAP were age, incidence of heart diseases (most importantly ischaemic heart disease), tachycardia, tachypnoe and acidosis. Staphylococcus sp. and Klebsiella pneumoniae infection led to longer hospitalisation period.
