Cílem článku je prezentovat současná doporučení k léčbě karcinomu močového měchýře po iniciální transuretrální resekci tumoru. Adjuvantní intravezikální léčba chemoterapií a imunoterapií prokazatelně snižuje recidivy karcinomů a v případě imunoterapie při udržovací dlouhodobé léčbě dokonce i oddaluje progresi onemocnění. Proto má svoji nezastupitelnou roli v komplexní péči pacientů s karcinomy měchýře neinfiltrujícími svalovinu., The aim of this article is to present current recommendations for treatment of bladder cancer after initial transurethral resection of tumor. Adjuvant intravesical treatment with chemotherapy and immunotherapy unequivocally lowers cancer recurrence and in case of maintenance immunotherapy even more prolongs the time to bladder cancer progression. Léčba chronické lymfocytární leukemie (CLL) dosáhla v posledních letech mimořádných pokroků. Důležitým mezníkem v léčbě CLL bylo zavedení monoklonálních protilátek a kombinované imunochemoterapie, které vedlo k významnému zlepšení léčebných výsledků a poprvé i k prodloužení celkového přežití. Navzdory těmto léčebným úspěchům je CLL stále považována za nevyléčitelné onemocnění. Pouze alogenní transplantace má potenciálně kurativní potenciál. Ta je však proveditelná pouze u malé, selektované skupiny mladších nemocných. S rozšiřujícími se poznatky o CLL a novými léčebnými možnostmi se snažíme léčbu nemocných maximálně individualizovat s přihlédnutím k věku nemocných, biologickým faktorům, přidruženým onemocněním a našim terapeutickým cílům. Vývoj nových léků a léčebných kombinací u CLL postupuje rychle vpřed a poskytuje tak naději na další zlepšování prognózy našich pacientů., The treatment of chronic lymphocytic leukemia (CLL) has achieved extraordinary progress over the last years. An important milestone in the treatment of CLL was reached with the incorporation of monoclonal antibodies and combined chemoimmunotherapy which led to significant improvements in treatment outcomes and survival advantage for the first time. Despite these therapeutic successes, CLL is still considered to be an incurable disease. Only the allogenic transplantation is potentially curative but it is feasible only for selected group of younger patients. There is an effort to individualize the treatment of patients with regard to the age of patients, biological factors, comorbidities and our therapeutic goals. Condylomata acuminata představují významný epidemiologický problém, a to zejména v teenagerovské populaci. Podstatou onemocnění je infekce tzv. lidským papilomavirem – HPV. Lidský papilomavirus neumíme léčit, proto terapie kondylomat většinou probíhá pouze formou lokální destrukce bradavic a nijak neřeší kauzální podstatu onemocnění. I samotné odstranění kondylomat, zejména při větším rozsahu onemocnění, může být obtížné a zdlouhavé, stejně tak jako i následné hojení pacienta, nezřídka komplikované časnými recidivami. Jakýkoliv nový preparát, který rozšíří možnosti terapie tohoto nepříjemného onemocnění, je velkým přínosem. Celkově působící imunomodulační preparát Isoprinosine je z tohoto pohledu velká naděje pro pacienty, a to zejména pro jeho systémové působení s efektem na všechny kožní a slizniční povrchy, pro jeho šetrnost vůči pacientovi, ale především pro jeho předpokládanou schopnost snížit procento recidiv. Na ambulanci onkogynekologické prevence při porodnicko-gynekologickém oddělení Městské nemocnice Ostrava a jejím přidruženém laserovém pracovišti jsme sledovali efekt léčby Isoprinosinem v klinické praxi a výsledky jsou velmi nadějné. Na základě našich pozorování se rovněž snažíme dát návod, jak lék používat, v jaké dávce a jak dlouho. Pro zajímavost uvádíme 1 kazuistiku i s fotodokumentací, v níž jsme léčbu Isoprinosinem využili s velmi dobrým efektem., Condylomata acuminata present a considerable epidemiological problem, particularly in a teenage population. The illness develops through the infection by a human papillomavirus- HPV. We are unable to treat the human papillomavirus, therefore the treatment of anogenital warts largely only consists in local destruction of warts and does not treat the cause of the illness. The removing of warts itself, in particular when the affected site is larger-sized, can be difficult and protracted, as well as subsequent healing of the patient, often complicated by recurrences. Any new preparation that broadens the possibilities of treatment of this obtrusive illness means a great benefit. In this regard the overall effect of the immunomodulation preparation Isoprinosine is a great hope for patients, mainly due to its systemic effect on all cutaneous and mucosal surfaces, its patient friendliness, but mainly due to its assumed ability to reduce the percent of recurrence. At the outpatients‘ department of oncogynecological prevention affiliated to the Department of Obstetrics and Gynecology of the Ostrava City Hospital and its Laser centre, we followed the effect of the treatment with Isoprinosine in clinical practice and the results are very encouraging. Based on our observations we also try to provide guidance on how to use the medicine, on its dosing and length of application.
