Motivation. Our previous study showed differences in the atherosclerosis phenotype between Lithuanian and Swedish men that could be influenced by complementary factors, namely oxidation processes and/or oxidative stress. The goal of this study was to evaluate the mainstream biological pathways inducing and maintaining the atherosclerotic process by analyzing genetic biomarkers particularly in inflammatory and metabolic pathways where the main focus is laid on the oxidation process. Methods. There were 32 families recruited for the study and clinical as well as biochemical analyses were performed. For genetic analysis 150 SNPs in 89 genes were selected in order to construct a microarray based on Arrayed Primer Extension (APEX) genotyping technology. Genotyping was carried out in 28 families and transmission disequilibrium test (TDT), siblingTDT (STDT), and combined analysis were performed. Results. Clinical and biochemical analysis revealed that probands with premature CAD were more likely to have diabetes mellitus, arterial hypertension, dyslipidemia and were male with high body mass index. Genetic analysis showed six SNPs statistically significantly associated with the atherosclerosis phenotype in the candidate genes ITGA2, IL1B, ALOX5A, OR13G1, MMP9 and NFKB1. These genes belong to different biological pathways: trombocyte adhesion and vessel damage, inflammation response, cholesterol and lypoxygenase metabolic pathway and nutrition. Conclusions. Cíl: Prospektivní sledování pacientů po prodělané akutní myokarditidě s cílem posoudit přínos a bezpečnosti pravidelného sledování pacientů pomocí magnetické rezonance (MR), laboratorních hodnot kardiospecifických enzymů a bicyklové ergometrie s ohledem na možné zkrácení doporučovaného přísného klidového režimu. Metodika: U 21 pacientů s diagnostikovanou akutní myokarditidou (včetně přítomnosti pozdního sycení - LGE při vstupní MR) byla v intervalu 1 a 6 měsíců provedena kontrolní MR, stanovení hladiny hypersenzitivního troponinu I (hs-Tn) a bicyklová ergometrie. Obrazová analýza byla zaměřena na vývoj rozsahu a intenzity LGE lézí v MR, výsledky byly porovnány s vývojem hs-Tn a výsledku bicyklové ergometrie po 1 měsíci. Výsledky: Předběžné výsledky studie prokázaly, že u všech pacientů byla prokázána regrese rozsahu i intenzity LGE při kontrolních vyšetřeních a zároveň nebyl prokázán rozvoj systolické dysfunkce levé komory srdeční u žádného pacienta. Tento vývoj korespondoval s vývojem průměrné hodnoty hs-Tn (z 9214,2 µg/1 na 25,4 µg/1). Při bicyklové ergometru nebyly prokázány závažné arytmie a všichni pacienti prokázali dobrou toleranci fyzické zátěže (minimálně 150 W). Závěr: Předběžné výsledky naší studie ukazují, že kombinace magnetické rezonance, hodnoty hs-Tn a bicyklové ergometrie je bezpečná a přínosná pro sledování pacientů po prodělané akutní myokarditidě. Důkladná analýza provedených výše uvedených vyšetření v delších intervalech je nezbytná pro možnou stratifikaci rizika u pacientů i s ohledem na délku doporučeného klidového režimu., Aim: Prospective study of patients following an acute myocarditis in order to assess the benefits and safety of regular monitoring using MRI, laboratory values of cardiac enzymes and exercise ECG. Methods: 21 patients with acute myocarditis (diagnosis including the presence of late gadolinium enhancement - LGE at acute MR) underwent in 1 and 6 months interval a follow-up MRI, level of hypersensitive tro-ponin I (hs-Tn) and the exercise ECG. Image analysis was focused on the development of LGE extent and intensity, results were compared with the development of hs-Tn and exercise ECG in 1 month follow-up. Results: Preliminary results of the study prove regression of the extent and intensity of LGE in all patients and normal left ventricular systolic function in all patients in 1 and 6 months follow-up. This trend corresponds with the development of the average value of hs-Tn (from 9214.2 µg/1 to 25.4 µg/1). During exercise ECG severe arrhythmia was not identified and all patients showed good tolerance of physical burden (at least 150 W). Conclusion: Preliminary results of our study indicate that the combination of MR, the value of hs-Tn and exercise ECG is safe and beneficial for follow-up of patients after acute myocarditis. Diagnosis of nerve agent intoxication is based on anamnestic data, clinical signs and laboratory examination. For acute poisoning, cholinesterase activity in the blood (erythrocyte AChE, plasma/serum BuChE) is sensitive, simple and most frequent laboratory examination performed in biochemical laboratories. Specialized examinations to precise treatment (reactivation test) or to make retrospective diagnosis (fluoride induced reactivation etc.) can be conducted. Popisujeme případ multicentrické formy Castlemanovy choroby, s generalizovanou lymfadenopatií a splenomegalií, provázenou typickými B symptomy – úbytek hmotnosti 15 kg, horečky neinfekčního původu, noční pocení, symptomy anémie. Histologické vyšetření uzlin s nejvyšší akumulací fluorodeoxyglukózy, odebrané torakoskopicky z mediastina, prokázalo plazmocelulární typ Castlemanovy choroby. Vyšetření HIV a lidského herpesviru 8 (HHV-8) bylo negativní. Stanovení diagnózy nemoci předcházely 3 recidivy herpes zosteru, což svědčilo o alteraci imunity. Léčba byla zahájena kombinací talidomidu, dexametazonu a cyklofosfamidu. Po 2 měsících léčby nebyla zřetelná léčebná odpověď a pacient tuto léčbu špatně toleroval. Proto byl tento způsob léčby ukončen. Pro léčbu 2. linie byla zvolena léčba R-CHOP (Mabthera – rituximab, cyklofosfamid, adriamycin, vinkristin a prednison. V průběhu 2 cyklů této léčby se zmenšila lymfadenopatie a splenomegalie, ale léčbu provázely závažné infekční komplikace. Proto dále pokračovala pouze monoterapie přípravkem Mabthera v dávce 375 mg/m2 v 28denních intervalech. Tato léčba se ukázala jako účinná a dobře tolerovaná. Při kontrolním PET-CT vyšetření vymizela lymfadenopatie a splenomegalie a normalizovala se akumulace fluorodeoxyglukózy. Mabthera v monoterapii se v tomto případě ukázala jako účinný a velmi dobře tolerovaný lék. Pro multicentrickou formu Castlemanovy nemoci existuje v současnosti více účinných léčebných alternativ: léčba rituximabem v monoterapii či v kombinaci, léčba imunomodulačními léky (talidomid nebo lenalidomid), léčba protilátkami proti interleukinu 6 (siltuximab) nebo proti jeho receptoru (tocilizumab). V případně neúčinnosti jedné léčebné alternativy je nutné otestovat další alternativu. V tomto případně nebyla úspěšná léčba založená na talidomidu, zatímco léčba založená na přípravku Mabthera dosáhla vymizení projevů nemoci., We describe a case of multicentric Castleman disease with generalized lymphadenopathy and splenomegaly, accompanied by typical B symptoms - loss of 15 kg, fever of non-infectious origin, night sweats, symptoms of anemia. Histological examination of the nodes with the highest accumulation of fluorodeoxyglucose, taken from mediastinum by thoracoscopy, revealed plasmocellular type of Castleman disease. Tests for HIV and human herpesvirus 8 (HHV-8) were negative. Three recurrences of herpes zoster indicating an alteration of immunity preceded the diagnosis of disease. Treatment was initiated with combination of thalidomide, dexamethasone, and cyclophosphamide. The response after 2 months therapy was not clear and patient doesn´t tolerated the therapy well. Therefore, this treatment was terminated and R-CHOP (Mabthera - rituximab, cyclophosphamide, adriamycin, vincristine, and prednisone) was selected as a second-line therapy. Lymphadenopathy and splenomegaly were reduced during the 2 cycles of treatment, however, serious infectious complications accompanied the therapy. Therefore, only use of Mabthera monotherapy 375 mg /m2 was administered in 28-day intervals. This treatment has shown efficacy and tolerability. PET-CT scan has demonstrated disappearance of lymphadenopathy and splenomegaly, in addition, normalized accumulation of fluorodeoxyglucose. Monotherapy with Mabthera has proved to be effective and well tolerated drug in this case. Currently, there are more effective therapeutic alternatives in multicentric Castleman disease: treatment with monotherapy of rituximab or in combination therapy with immunomodulatory drugs (thalidomide or lenalidomide, treatment with anti-IL-6 (siltuximab) or against its receptor (tocilizumab). In the case of ineffectiveness of one treatment option must be tested other alternative. Nutriční nedostatečnost je zřídka příčinou polyneuropatie či jiných poruch nervového systému ve vyspělých zemích. Pokud se vyskytuje, tak obvykle u alkoholiků a u špatně živených osob (psychopatie, vyšší věk, nemoci gastrointestinálního traktu, extrémní diety). Paradoxem v těchto zemích je skutečnost, že k poruchám z nedostatku z výživy dochází u lidí, kteří původně trpí nadvýživou. Tu si léčí různými postupy, které mají výživu snížit. Mezi ně patří řada bariatrických chirurgických zákroků, jejichž cílem je snížit množství kalorií přijatých do těla. S tím současně dojde někdy k nepředvídatelné nerovnováze a nedostatku příjmu živin nutných k normální funkci nervového systému. Počet bariatrických zákroků ve světě rychle vzrůstá, i když pomaleji, než narůstá obezita. Jejich častou komplikací bývají polyneuropatie, plexopatie, radikulopatie, myelopatie, encefalopatie a optická neuropatie, které vznikají jak časně, tak i několik let či desítek let po zákroku. Etiologie těchto poruch zůstává nejasná, nejvíce je obviňován nedostatek vitaminů (thiamin, E, B12), poruchy glykemie a nedostatek mědi. Řada faktorů však zůstává skrytých a suplementace předpokládaných chybějících složek průběh někdy neovlivní. Mohla by se rovněž podílet změněná imunita a zánět. Jako rizikové faktory vzniku nervových komplikací se uvádějí celková ztráta tělesné hmotnosti, gastrointestinální potíže, pokles albuminu a transferinu a rehospitalizace pro komplikace zákroku. Jako preventivní i léčebné opatření se doporučují u nemocných po bariatrickém zákroku především pravidelné návštěvy nutričního centra., In developed countries, nutritional deficiency only rarely causes polyneuropathy or other disorders of the nervous system. When it does occur, it appears in alcoholics or in the malnourished – people with long-term psychiatric disorders, with gastro-intestinal system disorders or on extreme diets. Paradoxically, certain initially obese patients may become malnourished. Pathological obesity is addressed in various ways, with the shared target of restricting food intake. Bariatric surgery is one of the options. However, such a profound diminution of nutrition may induce unpredictable imbalance or insufficiency in nutrients important for normal functioning of the nervous system. The number of bariatric operations is steadily increasing, both in the Czech Republic and elsewhere; in the USA, they already outnumbered cholecystectomies. Complications associated with bariatric surgery include polyneuropathy, plexopathy, radiculopathy, myelopathy, encephalopathy and optic neuropathy, all of which may appear both early as well as many years later. The aetiology of these disorders remains unclear, although vitamin insufficiency (thiamine, B12, E), copper deficiency and hypoglycaemia are often held responsible. However, many factors remain hidden and supplementation of the “missing” factors sometimes fails to affect the course of the disease. Damage to the immune system or inflammation may contribute to the aetiology. Weight loss, gastro-intestinal complaints, albumin and transferrin decrease and rehospitalisation for digestive symptoms are considered risk factors.
