Hepatoblastom je nejčastější primární nádor jater dětského věku, u dospělých jedinců jde o mimořádně vzácné onemocnění se špatnou prognózou. Hepatoblastom u dospělých je v literatuře popisován pouze formou kazuistických sdělení. Popisujeme případ 26letého muže s inoperabilním hepatoblastomem, který byl na našem pracovišti léčen chemoterapií, chemoembolizacemi a dalšími metodami intervenční radiologie 11 let., Hepatoblastoma is the most common primary hepatic malignant tumor in children. Léčba gastrointestinálních stromálních nádorů (GIST) může být příkladem úspěšnosti multidisciplinárního přístupu k léčbě maligních onemocnění. Významného pokroku v léčbě metastatického GIST bylo dosaženo molekulárně cílenou léčbou imatinibem. Dostupnost účinné systémové léčby vedla i k rozšíření možností chirurgické léčby a v současné době je dokumentována účinnost imatinibu v adjuvantní i neadjuvantní indikaci. Klíčovým pro stanovení správného léčebného postupu a optimální výsledek léčby je úzká spolupráce chirurgického a interního onkologa i soustředění nemocných do center specializovaných na léčbu tohoto vzácného nádorového onemocnění., The therapy of gastrointestinal stromal tumors may serve as an example of success of multidisciplinary approach in the therapy of malignant disorders. A significant progress in the therapy of metastatic GIST was obtained with the use of molecular targeted therapy (imatinib). The advent of targeted therapy also resulted in improved possibilities of surgical intervention and imatinib is currently used in the adjuvant as well as neoadjuvant indication. Osteosarkom (OS) je nejčastější zhoubný nádor skeletu, představuje asi 35 % primárních kostních nádorů. Úspěšná léčba osteosarkomu vyžaduje úzkou spolupráci odborníků multidisciplinárního týmu, kam patří onkolog, chirurg, patolog a radiolog. Nejlepší péči zajišťují specializovaná centra s odpovídajícími zkušenostmi a možností komplexní péče. Léčebný režim musí zahrnovat radikální odstranění primárního nádoru, případně metastáz v kombinaci se systémovou chemoterapií. Z cytostatik je to zejména doxorubicin, vysoko-dávkovaný metotrexát, cisplatina a ifosfamid. Cílem rozdělení chemoterapie na část předoperační a pooperační je redukce nádorové masy a lepší ohraničení tumoru vůči okolním strukturám. Tím je usnadněna chirurgická radikalita s možností náhrady postižené části skeletu bez mutilujícího výkonu. Důležitá je také znalost histopatologické odpovědi na předoperační chemoterapii, která ovlivňuje intenzitu pooperační léčby. Dosud není dostatečně vyřešena léčba metastatického onemocnění a případné recidivy. Zlepšení výsledků je možno očekávat až po zavedení nových léčebných přípravků a modalit., Osteosarcoma (OS) is the most common malignant tumour of the skeleton accounting for approximately 35 % of primary bone tumours. Successful treatment of osteosarcoma requires a close cooperation between the specialists of a multidisciplinary team consisting of an oncologist, surgeon, pathologist and radiologist. The best care is provided by specialized centres with appropriate experience in which comprehensive care is available. The therapeutic regimen must include radical removal of the primary tumour and/or metastases in combination with systemic chemotherapy. The cytostatic drugs primarily used include doxorubicin, high-dose methotrexate, cisplatin and ifosfamide. The goal of dividing chemotherapy into preoperative and postoperative parts is a reduction in the tumour mass and improved delineation of the tumour from the surrounding structures. This improves the surgical radicality with the affected part of the skeleton being replaced without the need for a mutilating procedure. Also of importance is the knowledge of the histopathological response to preoperative chemotherapy which affects the intensity of postoperative treatment. The management of metastatic disease and possible recurrence has not been addressed adequately as yet. Pacienti s metastatickým kolorektálním karcinomem (mKRK) by měli v průběhu své léčby obdržet všech pět látek účinných u tohoto onemocnění. Tyto látky se skládají ze tří chemoterapeutik: 5-fluorouracil/leucovorin (5-FU/LV), irinotecan, oxaliplatina a dvou monoklonálních protilátek: bevacizumab namířený proti vaskulárnímu endoteliálnímu růstovému faktoru a inhibitor epidermálního růstového faktoru cetuximab nebo panitumumab. Léčebné možnosti třetí linie závisí na tom, jaké látky byly použity v předchozí terapii. U pacientů předléčených 5-FU, oxaliplatinou a irinotecanem představuje léčba kombinací cetuximab a irinotecan atraktivní možnost., Patients with metastatic colorectal cancer (mCRC) should receive all five drugs active in this disease during the overall course of their treatment. These drugs consist of three chemotherapeutic agents: 5-fluorouracil/leucovorin (5-FU/LV), irinotecan and oxaliplatin and two monoclonal antibody: bevacizumab directed against the vascular endothelial growth factor and the epidermal growth factor receptor inhibitor cetuximab or panitumumab. Aim: to improve treatment results of colorectal cancer, complicated carcinomatosis of abdominal cavity, by associated using of endolymphatic chemotherapy (ELCT) and local extremely high frequency (EHF) therapy. Igroup 21 people (33.3%) performed colostomy, then neoadjuvant systemic chemotherapy by the scheme FOLFOX4. It was held two cycles, after two cycles their were received cytoreductive surgery; II group 26 people (41,3%) received cytoreductive operations with early postoperative intraabdominal chemotherapy by oxaliplatin (200 mg/m(2) at day 1 and 5FU 650 mg/m(2) from days 1 to 4. III group 16 people (25,4%) also performed colostomy, then used 2 courses of ELCT with local EHF therapy and all patients were received cytoreductive operations. ELCT was carried out injecting oxaliplatin100 mg/m(2) a day during 12 hours and then 5 FU in doze of 600 mg/m2 a day during 72 hours by the instrumentality of the dozer and LV200 mg/m(2) (2h intravenous infusion). During ELCT patients received local EHF therapy in abdominal cavity for an hour. After chemotherapy the partial regress of tumor observed in 1st group in 4,8 % and in 3rd 37,5 %, stabilization of process noted in 28,6 and 50% cases, progressing 66,7 and 12,5% cases correspondingly to groups. Histomorphologic study of malignant foci after ELCT+EHF therapy showed reduction of specific area of parenchyma cancer cell from 57 till 39% simultaneous growth of stroma from 40 till 58% and in necrosis area from 1,8 till 2,5%. Mitotic activity of tumor cells in the 1st group decreased (average in 22 un.), but in 3rd group it decreased until 11 units. Analysis of life span showed that in 1st group of patients 2year survival rate was 66,7%. In 2nd group of patients 2year survival rate was 42,3%. ELCH+EHFtherapy has high clinic efficacy and promotes to decrease terms of progression of tumor process, increases life span from 0 till 24% and quality of life of the patients. ELCH+EHFtherapy brings to reduction of specific area of parenchyma in cancer cells from 57 till 39% with simultaneous growth of stroma area from 40 till 58% and necrosis area from 1,8 till 2,5%.
