V těhotenství je anafylaktická reakce naštěstí sice relativně vzácná, její důsledky ale mohou vést až k závažnému postižení matky i plodu. Ve většině publikovaných případů těžké anafylaxe v těhotenství bylo těžké postižení dokonce spíše na straně plodu než matky. Vznik anafylaxe se většinou nedá předvídat, první symptomy jsou někdy netypické, pouze lokální, ale rychle mohou přecházet v generalizované postižení celého organismu. Ať už jsou zprostředkovány protilátkami typu IgE (pravá anafylaxe), nebo vznikají na jiném podkladě (anafylaktoidní reakce), klinický obraz bývá obdobný. Je třeba myslet na to, že i klinicky nenápadné počáteční symptomy mohou mít průběh závažný a život ohrožující. Každé opomenutí a prodleva v diagnostice a léčbě anafylaxe může vést k fatálnímu konci. O úspěšném výsledku léčby rozhoduje velkou měrou medicínská zkušenost a včasnost zásahu. Protože je toto téma v současné literatuře zmiňováno spíše jen výjimečně, je cílem tohoto článku shrnout problematiku anafylaxe v těhotenství z hlediska patofyziologie, klinického průběhu, diagnostiky i léčby., Anaphylaxis in pregnancy is a relatively rare reaction but it can lead to serious maternal and fetal impairment. In published cases of severe anaphylaxis in pregnancy there were even more severe disabilities in the fetus than the mother. Development of anaphylaxis is usually unpredictable, the first symptoms are sometimes atypical and only local, but can move quickly in a generalized impairment of the whole organism. Whatever they are mediated by IgE antibodies (right anaphylaxis), or are caused on another basis (anaphylactoid reactions), the clinical picture is similar. It should be kept in mind that even clinically inconspicuous initial symptoms may progress into serious and life-threatening state. Any omission and delay in diagnosis and treatment of anaphylaxis can lead to fatal end. A successful outcome is largely dependent on experience and timeliness of medical intervention. Based on survey of 257 women with nondeveloping pregnancies of various geneses, it was shown that the level of apoptosis marker P53 in the decidual tissue is increased, and antiapoptotic protein Bcl2 simultaneously is reduced, compared with similar data of women with physiological pregnancies. Jednu ze vzácných indikací k císařskému řezu představuje oční onemocnění. Článek popisuje oční změny v těhotenství a rozebírá vliv těhotenství a porodu na tři vybrané choroby, především na myopii, která je udávána jako nejčastější oční indikace k císařskému řezu. Dále je popsán soubor pacientek z let 2010 až 2013 s oční indikací k císařskému řezu a proveden podrobnější rozbor indikací v letech 2012 a 2013. Tyto indikace bývají nejčastěji zdůvodněny zvýšením nitroočního tlaku při porodu, ačkoliv žádné dostupné zdroje nepotvrzují při tomto ději riziko vzniku či progrese amoce sítnice. The article describes ocular changes during pregnancy and discusses the influence of pregnancy and delivery on three chosen diseases, especially on myopia, which is reported as the most common indication for caesarean section. The article also describes patients diagnosed from 2010 to 2013 with ocular indication for caesarean section and analyses in detail indications in the year 2012 and 2013. These indications are often based on the increase in intraocular pressure during the labor, even though no available sources confirm the risk of developing or progression of retinal detachment. The article presents results of peculiarities of the hepatobiliary system in pregnant women with preeclampsia. It is set in the pregnant with preeclampsia of mild severity the frequency and combination of two and more complaints was reliably lower comparing to the frequency and combination of complaints in the pregnant with preeclampsia of mean severity. It must be mentioned that the received laboratory data positively correlated with the clinical manifestations of hepatobiliary dysfunction and the level of severity of preeclampsia that makes the further scientific researches necessary in future. Cíl práce: Upozornit na problematiku apendicitidy v graviditě. Typ práce: kazuistika. Vlastní pozorování: Formou kazuistiky prezentovaný výskyt perforované apendicitidy s difuzní peritonitidou ve 27. týdnu gravidity. Následný porod císařským řezem ve 36. týdnu a komplikované pooperační období a infekční komplikace v šestinedělí. Závěr: Apendicitida v těhotenství je spojována se zvýšenou mateřskou a fetální morbiditou. Perforace apendixu přispívá ke zvýšenému riziku dalších komplikací, které zahrnují např. předčasný porod, abort a mateřské komplikace, zejména infekční. Včasná diagnóza s následnou operací může snížit mateřskou i fetální morbiditu., Objective: To draw attention to the issue of appendicitis in pregnancy. Design: Case report. Case report: We present a case study in the form of a case of occurrence of appendicitis with peritonitis in 27 weeks pregnancy. And describes the birth and childbed, which is complicated by infectious complications. Conslusion: Appendicitis in pregnancy is associated with increased maternal and fetal morbidity and even more in case of peritonitis. Delay surgical intervention correlates with increased risk of perforation, which contributes to the increase of other complications, including premature birth, abortion and maternal complications. Hormony štítné žlázy hrají zásadní roli při početí i v průběhu těhotenství, jsou podstatné pro vývoj plodu, dítěte i pro zdraví dospělého člověka. Četné studie prokázaly vazbu mezi onemocněními štítné žlázy v graviditě, zejména hypotyreózy, s porodními komplikacemi anebo psychomotorickými negativními dopady na novorozence. Prevalence nediagnostikované snížené funkce štítné žlázy je 4–8 % u těhotných žen, eufunkčních žen s autoimunitním postižením štítné žlázy je dalších 6–8 %, a i ty jsou v těhotenství kandidátkami dystyreózy. K zachování eutyreózy a hladiny TSH v 1. trimestru ideálně ≤ 2,5 mIU/l a ve 2. a 3. trimestru TSH ≤ 3,0 mIU/l, musí štítná žláza zvýšit produkci tyroxinu o 50 %. Jakmile je v graviditě potvrzena diagnóza subklinické anebo plně rozvinuté hypotyreózy, je třeba ihned zahájit substituční terapii. To platí i pro většinu eutyreózních žen s chronickou autoimunitní tyreoiditidou. Ženy léčené levotyroxinem ještě před těhotenstvím musí v těhotenství zpravidla zvýšit dávkování o 30–80 %. Vzhledem k tomu, že onemocnění štítné žlázy v těhotenství je poměrně časté a většinou asymptomatické, je však nejen snadno diagnostikovatelné, ale i bezpečně a levně léčitelné, navrhuje dnes většina světových endokrinologických společností včetně České endokrinologické společnosti České lékařské společnosti J. E. Purkyně realizovat screening dysfunkcí štítné žlázy rámci prevence. K hodnocení výsledků TSH, antiTPO protilátek v začátcích gravidity by se měly používat trimestr specifické referenční normy. Musíme zdůraznit, že přiměřené zásobování jodem v těhotenství je velmi důležité. Pokud je to možné, měl by být příjem zvýšen ještě před otěhotněním., Thyroid hormones play fundamental role in conception and pregnancy and are essential for normal adult health, fetus and childhood development. Many studies have shown an association between maternal thyroid diseases esp. hypothyroidism with obstetric problems and/or psychomotoric impairment in the offspring. The prevalence of undiagnosed lower thyroid function in pregnancy is present in about 4–8 % of pregnant women, and euthyroid women with thyroid autoimmunity (6–8 %) are further candidates for thyroid disorders in pregnancy. The thyroid gland needs to produce 50 % more thyroxine in pregnancy to maintain an euthyroid state to keep TSH ideally ≤ 2.5 mIU/l in the first trimester of pregnancy and TSH ≤ 3.0 mIU/l in the second and third trimester. Consequently, there is a need to start the substitution therapy as soon as diagnosis of subclinical and /or overt hypotyroidism is established, and in majority of euthyroid women with autoimmune thyroid disease there is a need to start therapy as well. Most women on levothyroxine therapy before pregnancy require an increase in dose when pregnant. As maternal thyroid disease is a quite prevalent condition and often asymptomatic, but easily diagnosed and for which an effective, safe and cheap treatment is available, endocrinological societies including ČES ČLS JEP worldwide are suggesting the need of thyroid dysfunction screening as a simple prevention attitude. Hormone determination of TSH and TPOab antibodies should be performed early during the first trimester, using trimester-specific reference values.
