INTRODUCTION: Autologous stem cell transplantation (ASCT) became standard of care for patients with multiple myeloma (MM) under the age of 65 years. We routinely perform ASCT for newly diagnosed MM since 1996 in our department. PATIENTS AND METHODS: We retrospectively analyzed all 285 transplants in 185 patients done for MM from January 1996 till December 2010. We analyzed overall survival (OS) and progression-free survival (PFS) regarding conditioning, stage, complete or very good partial remission (CR, VGPR) achievement, renal impairment, single vs. double transplant. RESULTS: Estimated 10-years survival of the whole set of patients is 39% (median survival 95 months). Patients with renal impairment show same OS as those without (p = 0.22). Patients show similar overall survival and event free survival regardless of type of transplant. We observed better outcome in terms of overall survival in patients treated with new drugs (p = 0.0014). Reaching CR or VGPR was not translated into better OS (p = 0.30) and EFS (p = 0.10). Also stage of the disease and whether single or double transplant was used did not make any significant difference in the outcome. CONCLUSION: Stem cell transplantation greatly improved outcome of patients with MM. Poor outcome of allogeneic transplantation in our group of patients is related to high transplant related mortality (20% vs. 0%) and unexpected high relapse rate. There is a trend towards better survival, when new drugs are incorporated at any time in the course of the disease. High grade gliomas are some of the deadliest human tumours. Conventional treatments such as surgery, radiotherapy and chemotherapy have only a limited effect. Nowadays, resection is the common treatment of choice and although new approaches, such as perioperative magnetic resonance imaging or fluorescent microscopy have been developed, the survival rate of diagnosed patients is still very low. The inefficacy of conventional methods has led to the development of new strategies and the significant progress of nanotechnology in recent years. These platforms can be used either as novel imaging tools or to improve anticancer drug delivery into tumours while minimizing its distribution and toxicity in healthy tissues. Amongst the new nanotechnology platforms used for delivery into the brain tissue are: polymeric nanoparticles, liposomes, dendrimers, nanoshells, carbon nanotubes, superparamagnetic nanoparticles and nucleic acid based nanoparticles (DNA, RNA interference [RNAi] and antisense oligonucleotides [ASO]). These nanoparticles have been applied in the delivery of small molecular weight drugs as well as macromolecules - proteins, peptides and genes. The unique properties of these nanoparticles, such as surface charge, particle size, composition and ability to modify their surface with tissue recognition ligands and antibodies, improve their biodistribution and pharmacokinetics. All of the above mentioned characteristics make of nanoplatforms a very suitable tool for its use in targeted, personalized medicine, where they could possibly carry large doses of therapeutic agents specifically into malignant cells while avoiding healthy cells. Akutní lymfoblastická leukemie dospělých s pozitivitou filadelfského chromozomu a fúzního genu BCR/ABL je agresivní hematoonkologické onemocnění, typické pro pacienty vyššího věku. Léčba spočívá v podávání chemoterapie a inhibitoru tyrozinkinázy. Maximálního efektu lze dosáhnout pomocí alogenní transplantace krvetvorných buněk, tu je však schopna podstoupit jen malá část pacientů. V případě relapsu je prognóza onemocnění extrémně nepříznivá., Philadelphia chromosome positive adult acute lymphoblastic leukemia with BCR/ABL fusion gene is an aggressive hematological malignancy most common in the elderly. Both chemotherapy and a tyrosine kinase inhibitor are engaged in the treatment. The best outcome can eventually be achieved using allogeneic hematopoietic stem cell transplantation. However, only a minority of patients can undergo such intensive therapy. Pomocí moderních léčebných strategií má šanci na vyléčení více než 80 % pacientů s Hodgkinovým lymfomem (HL) ve věku do 60 let. Cílem současných léčebných postupů je zachování vysokého počtu vyléčených pacientů a omezení toxicity. U pacientů v pokročilém stadiu HL je celkové přežití o 10 % vyšší po léčbě eskalovaným BEACOPP oproti ABVD, proto je 6 cyklů eskalovaného BEACOPP standardem léčby u pacientů do 60 let. U pacientů nad 60 let zůstává standardem léčby 6 cyklů ABVD. Po chemoterapii je možné vynechat radioterapii u pacientů s negativním nálezem podle pozitronové emisní tomografie (PET). Čeká se na dlouhodobé výsledky studií se stratifikací léčby podle interim PET a studií s brentuximab vedotinem v kombinaci s chemoterapií v první linii., More than 80 % of patients with Hodgkin lymphoma (HL) younger than 60 years are likely to be cured with modern treatment strategies. The goal of current treatment approaches is to maintain the high treatment efficacy and to reduce the toxicity. BEACOPP escalated has a 10 % overall survival advantage over ABVD in patients with advanced-stage HL, therefore 6 cycles of BEACOPP escalated has become the standard of care for patients younger than 60 years. Six cycles of ABVD remain the standard of care for patients over 60 years. Radiotherapy could be omitted in patients with a negative positrone emission tomography (PET) finding after chemotherapy. Diabetes mellitus zůstává nejčastější příčinou chronického selhání ledvin, slepoty získané v dospělosti, netraumatických amputací a těžkých forem neuropatie. Je proto nezbytné hledat nové formy terapie, které by dokázaly dosáhnout dlouhodobé normalizace hladin krevního cukru. Jedinou standardní metodu zatím představuje orgánová transplantace pankreatu. Provádí se nejčastěji v kombinaci s transplantací ledviny a jen výjimečně jako samostatný výkon. Novou, podstatně méně invazivní možnost dnes již představuje transplantace izolovaných Langerhansových ostrůvků. Zatímco počet transplantací pankreatu v IKEM již přesáhl 300, program ostrůvkové transplantace se teprve rozvíjí a do května 2007 bylo provedeno 10 klinických výkonů. Počet vhodných pacientů, kteří by z tohoto způsobu léčby mohli profitovat, však mnohonásobně převyšuje dostupnost vhodných orgánů k transplantaci. Hledají se proto nové možnosti získání buněčných linií produkujících inzulin, a to buď z tkáně zvířecí, ale zejména z embryonálních či dospělých kmenových buněk. Nově se studuje také možnost in vivo regenerace endogenních či transplantovaných ? buněk pankreatu. Kombinovaná transplantace ledviny a pankreatu představuje stále optimální možnost léčby uremických diabetiků 1. typu a její dlouhodobé výsledky jsou nyní velmi dobré, i když se dále zlepšují jen málo. Izolovaná transplantace pankreatu zůstává vyhrazena pro úzkou skupinu pacientů s velmi labilním diabetem. Transplantace izolovaných Langerhansových ostrůvků představuje alternativu, jež je pro pacienta podstatně bezpečnější, ale dlouhodobé výsledky jsou zatím horší. V Institutu klinické a experimentální medicíny (IKEM) je v současné době zavedena metoda orgánové transplantace pankreatu, byl zahájen program transplantace izolovaných ostrůvků a studuje se možnost propagace buněčných linií produkujících inzulin., František Saudek, and Lit. 16
Mezi pojmem cílené léčby a individualizací léčby na míru mohou být často velké rozdíly. V naší kazuistice naopak prezentujeme si tuaci, kdy jedině léčba cílená byla u pacienta s kolorektálním karcinomem jako jediná spojena s klinickým benefitem. Kromě extrémní důležitosti chirurgie i u metastatického onemocnění by tak mohla být dokumentována potřeba vyzkoušet efekt cílené léčby u většiny pacientů, přestože se v její indikaci často nemůžeme opírat o žádné prediktivní faktory., There might be big differences between the terms of targeted therapy and treatment tailoring. In our case report we describe situation when targeted therapy as the only method was associated with a clinical benefit in a patient with colorectal cancer. In addition to extrem importance of surgery also in treatment of metastatic disease this could be a clear documentation of need to explore the effect of targeted therapy in the majority of patients although we are often unable to define any predictive markers for it´s indication, and Tomáš Svoboda
