Přehled problematiky: Magnézium je čtvrtým nejčastějším kationtem v lidském organizmu a po draslíku druhým kationtem v intracelulárním prostoru. Účastní se jako kofaktor enzymů metabolických reakcí, tvorby a utilizace energie a je nezbytný k udržení elektrolytové rovnováhy. Tělo dospělého člověka obsahuje asi 0,043 % (535-730 mmol) magnézia, což je v přepočtu asi 22-30 g (60 % v kostěném skeletu, 40 % intracelulárně ve svalovině a měkkých tkáních, 1 % (7,5-10 mmol) je obsaženo v extracelulární tekutině). Denní potřeba magnézia se odhaduje na 10-20 mmol, tj. 200-400 mg. Ve vztahu k podané dávce pak platí, že čím vyšší podaná dávka, tím nižší procento rezorbce ze střeva. Z doporučené denní dávky 15 mmol (365 mg) se rezorbuje pouze okolo 1/3. Význam magnézia v těhotenství: Důsledkem manifestního nedostatku magnézia může být: 1. zvýšené riziko potratu - předčasný porod (inkompetence hrdla děložního, předčasný odtok vody plodové), 2. placentární insuficience a hypotrofizace plodu, 3. rozvoj gestózy u matky, 4. zvýšené riziko astmatu a ekzému u dětí. Magnézium sulfát, podávaný těhotným ohroženým předčasným porodem (23+0 -30+0), snižuje riziko dětské mozkové obrny u přežívajících novorozenců. Souhrn: Nedoporučuje se používat intravenózní magnézium sulfát v indikaci léčby hrozícího předčasného porodu déle jak 5-7 dní. Tato omezení se nijak nevztahují na perorální užívání magnézia., Objectives: Magnesium is the fourth most common cation in the human body, and the second cation to potassium in the intracellular space. It participates as a cofactor of enzymes in the metabolic reactions, creation and utilization of energy and is necessary for the maintenance of electrolyte balance. The body of an adult contains about 0.043% (535-730 mmol) of magnesium, which is the equivalent of about 22-30 g (60% in the bony skeleton, 40% intracellular in muscles and soft tissue, 1% (7.5-10 mmol) in extracellular fluid). Magnesium sulfate given to pregnant women with threatened preterm labor (23+0 – 30+0) reduces the risk of cerebral palsy in surviving infants. Summary: In cases of threatened preterm labor it is not recommended to administer magnesium sulfate intravenously for more than 5-7 days. Competencies develop and changes throughout a persons life, they can gain or lose, going through various age stages. Their development does not end in youth, but continue on through the adult life. The ability to thing and reflect those thoughts specifically come forward in the center of structure of competency, which grows at the same time as the individual matures. One of the competency types is health competency. Health competency is a relatively new concept; it is not sufficiently researched. Aim of the study was to determine the factors of an adult individual health competency. 827 respondents participated in the study, in the processing of data was used SPSS. Was used factor analysis, analysis of variance with ANOVA and KruskalWallis test and Pearsons correlation. It was found that health competency is affected by several factors. These are: health education, health behavior, and the value of the environment. Each of the sets was distributed to key factors. Aim: The aim of this paper is to develop an evidence-based clinical protocol for the use of gum chewing in postoperative care to reduce the risk of paralytic postoperative ileus in patients who have undergone abdominal surgery. Methods: A clinical question (For adult patients who have undergone abdominal surgery, does the use of gum chewing postoperatively reduce the risk of paralytic postoperative ileus in comparison with the usual care regimen?) was formulated, a population and setting were defined, and the databases OVID Medline®, CINAHL, and PubMed were searched for relevant material. Six data-based research articles were chosen for review: four randomized controlled trials and two meta-analyses. Finally, the articles were critically appraised to generate evidence on which to base the clinical protocol. Results: The development team created a detailed description of the clinical protocol as well as a protocol algorithm to assist clinicians in determining patient eligibility. In addition, protocol implementation and evaluation plans were proposed. Cílem práce je hodnocení vývoje úhrad nemocniční péče v ČR po roce 1998 ve vztahu k alokační efektivitě finančních zdrojů, rozdělených mezi různé typy českých nemocnic podle jejich velikosti, funkcí a rozsahu poskytovaných služeb s využitím statistických dat o činnosti a hospodaření těchto nemocnic. Vývoj úhrad je hodnocen v kontextu tržně zaměřených reforem zdravotnictví v ČR. Malé a střední nemocnice se v ČR staly v posledních dvou letech cílem další redukce jejich činnosti (lůžka, pracovníci, oddělení, rušení celých nemocnic) v rámci aktivit veřejných zdravotních pojišťoven (nástrojem jsou smlouvy pojišťoven s nemocnicemi). Předchozí restrukturalizace lůžkové péče v 90. letech byla v souladu s platnou právní úpravou řízena veřejnou správou. Současný reformní proces probíhá bez výchozích analýz a hodnocení ze strany veřejné správy na centrální a regionální úrovni. Veřejná a odborná komunikace tohoto procesu jsou nedostačující, vede k různým protestům ze strany dotče - ných subjektů. Není naplněn zákonem stanovený postup. U velkých nemocnic, zajišťujících poskytování zdravotní péče, výzkum a výuku byl zjištěn rozdíl mezi jejich výnosy a náklady na lůžkovou péči, uvedených v jejich výkazech. Jde o poměrně vysoké finanční částky (ročně v mld Kč). Tento rozdíl je zapotřebí objasnit cestou nezávislé veřejné kontroly. Práce upozorňuje na potřebu rozšířit současnou technokratickou argumentaci organizačních změn nemocniční péče, založenou na primárním neoliberálně směrovaném záměru inančních úspor a omezení produkce ve veřejném sektoru služeb. Doporučeným východiskem je obnova demokratických mechanismů se zohledněním funkcí a cílů zdravotnického systému v souladu s veřejnými zájmy a lidskými právy se vztahem ke zdraví., The aim of the work is to assess the development of hospital payment in the Czech Republic after 1998 in relation to the allocative efficiency of financial resources distributed among different types of Czech hospitals according to their size, function, and scope of services, using statistical data of their activities. Small and medium-sized hospitals in the Czech Republic were during the last two years challenged to further reduction of their activities (beds, staff, departments, hospitals complete interference) in the framework of their contracts with public health insurance. Previous restructuring of inpatient care in the 90s, was in accordance with the valid legislation. Bor a jeho sloučeniny byly zařazeny díky Nařízení Evropské komise č. 1907/2006 (REACH) do skupiny látek negativně ovlivňující reprodukci a vývoj plodu. To vyvolalo potřebu shrnout současné poznatky o jeho toxicitě a užití v medicíně během těhotenství a laktace. Akutní i chronická toxicita sloučenin boru je nízká. Otravy byly popsány jen ve výjimečných případech. Reprodukční toxicita zahrnuje v pokuse na zvířeti poškození spermatogenese a možnost vzniku vrozených vývojových vad obvykle za situace, kdy matka vykazuje známky toxického poškození. Práce rovněž shrnuje publikované klinické studie o vlivu kyseliny borité na vývoj plodu. Dle publikovaných výsledků studií nedosahuje expozice u člověka takových hodnot, aby k poškození reprodukce a plodu došlo. Protože užití jakéhokoli léku v těhotenství a laktaci musí být opodstatněné a profit musí převládat nad rizikem, shrnuje práce i studie, které se zabývají účinností kyseliny borité ve srovnání s jinými antimykotiky. Pro léčbu mykóz lze užít lokálně působící nystatin, případně klotrimazol, nebo mikonazol, výjimečně perorálně flukonazol v nízkých dávkách. Účinnost kyseliny borité je srovnatelná, případně vyšší, při léčbě rezistentních kmenů. Výhodou je i její nízká cena. Na závěr je třeba zdůraznit, že užití jakéhokoli přípravku během 1. trimestru by mělo být podloženo naléhavou potřebou léčby., The European Union Regulation REACH (No. 1907/2006) classified of boron and its compounds in the group adversely affecting reproduction and fetal development raised the need to to summarize findings about its toxicity and use in medicine during pregnancy and lactation. Acute and chronic toxicity of boron compounds is low. Intoxications were reported only in exceptional circumstances. In animals, reproductive toxicity includes the damage to spermatogenesis and the possibility of congenital defects usually in doses causing maternal toxicity. The work also summarizes the published clinical studies on the effect of boric acid on fetal development. According to the published results of studies, the human exposure does not reach of such values that the reproduction and fetal damage occurred. Since the use of any drug in pregnancy and lactation must be justified and profit must prevail over the risk, the work summarizes studies on the effect of boric acid as compared to other antifungal agents. Nystatin, clotrimazole, or miconazole, fluconazole can be used topically for the treatment of fungal infections, applied p.o. exceptionally in low doses, only. The effectiveness of boric acid is comparable or higher in resistant strains. Low price is an advantage. Obezita a nadváha se stávají velkým medicínským problémem současnosti. Komplikují a zhoršují prognózu mnoha situací včetně těhotenství. Tuková tkáň není jen pasivní zásobárna energie, ale produkuje též hormony a cytokiny, kterými ovlivňuje činnost celého organizmu. Obezita negativně působí nejen přímo na matku a její reprodukční zdraví, ale působí i na zdravotní stav a vývoj dítěte. Kromě dobře známých komplikací se začínají diskutovat i další otázky jako například neuropsychický vývoj dítěte nebo vliv na vyšší riziko rozvoje astma, Obesity and overweight are becoming a major problem in medicine. Obesity impairs the prognosis of many conditions, including pregnancy. Adipose tissue is not only a passive reservoir of energy but also produces hormones and cytokines, which interfere with the regulation of the whole organism. Obesity adversely affects not only the mother and her reproductive health but also the health and the development of the child. In addition to well-known complication issues such as neuropsychological development of the child or a higher risk of astma are now discussed. Neexistuje jedna obecná defi nice seniorské křehkosti. Někteří autoři ji popisují jako symptom, jiní jako komplexní syndrom, nebo jako nahromadění funkčních defi citů u seniora. Dle odborných publikací se křehkost vyskytuje až u 25 procent seniorů (Holmerová, 2007, s. 100). Rozvoj seniorské křehkosti může být mimo jiné podmíněn multimorbiditou, nežádoucími účinky léků, pohybovou inaktivitou, malnutricí, osamělostí či depresí (Kalvach, 2008, s. 122). Cílem práce bylo vyhledání a analýza odborných zahraničních článků týkající se křehkosti seniorů a následně získané poznatky využít v diagnostice NANDA International. Pro získání relevantních dat byly využity elektronické databáze Pubmed, Medline, ProQuest a Springer a vyhledávač Google. Byla využita specifi cká vyhledávací strategie. Na základě diagnostického algoritmu dle NANDA International byly identifi kovány diagnostické prvky a stanoveny ošetřovatelské diagnózy. Ošetřovatelská diagnostika dle NANDA International se jeví jako vhodná při poskytování ošetřovatelské péče u křehkých seniorů., There is no general definition of frailty in the elderly. Some authors describe it as a symptom, others as a complex syndrome, or as an accumulation of functional defi cits in older adults. According to professional publications, frailty occurs in up to 25 percent of the elderly (Holmerová, 2007, p. 100). Developing of frailty in older adults may be subject to multimorbidity, undesired eff ects of medication, physical inactivity, malnutrition, loneliness or depression (Kalvach, 2008, p. 122). The aim of the article is to search for and analyse foreign scientifi c articles related to frailty in the elderly and to use gained knowledge in NANDA International diagnostics. To obtain the relevant data, electronic databases Pubmed, Medline, ProQuest, Springer and Google were used. Published articles concerning frailty in the elderly were found by a specifi c search strategy. Diagnostic elements were identifi ed and nursing diagnoses were determined according to the diagnostic algorithm by NANDA International. Cíl: Přesnost screeningových škál pro vyhledávání pacientů s rizikem vzniku dekubitů byla u nás dosud zkoumána jen okrajově. Cílem bylo navázat na pilotáž zabývající se inter-rater reliabilitou (shodou mezi posuzovateli) Škály Bradenové (ŠB) a zjistit shodu mezi dvěma posuzovateli u většího vzorku respondentů. Metodika: Dva posuzovatelé nezávisle na sobě hodnotili 60 pacientů hospitalizovaných s neurologickým onemocněním pomocí ŠB. Celková skóre (max. 23 bodů) byla dichotomizována (riziko versus absence rizika) dle doporučeného cut-off skóre (18 bodů). Inter-rater reliabilita byla vyjádřena pomocí koeficientu kappa (κ), a to pro dichotomizovaná celková skóre i pro skóre v jednotlivých položkách škály. Výsledky: Koeficient κ byl pro dichotomizovaná celková skóre 0,564 (95% konfidenční interval [KI] = 0,182; 0,946). V jednotlivých položkách škály byl κ následující: „smyslové vnímání“ 0,557 (95% KI = 0,306; 0,808); „vlhkost“ 0,454 (95% KI = 0,142; 0,766); „aktivita“ 0,613 (95% KI = 0,425; 0,801); „mobilita“ 0,091 (95% KI = – 0,127; 0,309); „výživa“ 0,283 (95% KI = 0,095; 0,471); a „tření a střih“ 0,554 (95% KI = 0,240; 0,868). Závěr: Inter-rater reliabilita dichotomizovaných celkových skóre byla střední úrovně (κ = 0,564). V položce „aktivita“ lze shodu mezi posuzovateli považovat za poměrně dobrou (κ = 0,613); v ostatních položkách však byla nižší. Jsou prezentovány faktory, které mohly tyto výsledky ovlivnit., Aim: The accuracy of the screening scales for identification of patients who are at risk of developing pressure sores has, so far, received only marginal attention in the Czech Republic. The aim was to extend a pilot study that focused on inter-rater reliability of the Braden Scale and to determine inter-rater reliability for a larger patient sample. Methods: Two raters performed independent assessments of 60 patients hospitalized with a neurological disease, using the Braden Scale. Total scores (max. 23 points) were dichotomized (at risk versus not at risk) using the recommended cut-off score (18 points). Using the kappa (κ) coefficient, inter-rater reliability was expressed for the dichotomized total scores as well as for scores obtained for each category of the scale. Results: For the dichotomized total results, the κ coefficient was 0.564 (95% confidence interval [CI] = 0.182; 0.946). For the individual categories, the κ coefficient was as follows: “sensory perception” 0.557 (95% CI = 0.306; 0.808); „moisture“ 0.454 (95% CI = 0.142; 0.766); „activity“ 0.613 (95% CI = 0.425; 0.801); „mobility“ 0.091 (95% CI = – 0.127; 0.309); „nutrition“ 0.283 (95% CI = 0.095; 0.471); and „friction and shear“ 0.554 (95% CI = 0.240; 0.868). Conclusion: For the dichotomized total scores, moderate inter-rater reliability (κ = 0.564) was obtained. For the category “activity”, the inter-rater reliability was quite good (κ = 0.613); for other categories, it was lower.
