Autoři prezentují sestavu 76 nemocných radikálně operovaných pro karcinom hlavy pankreatu a analyzují pooperační komplikace a přežívání. Materiál a metodika: V období 1/2006–9/2010 podstoupilo 76 nemocných potenciálně kurativní hemipankreatoduodenektomii pro adenokarcinom hlavy pankreatu. Pacienti jsou prospektivně sledováni, diskutovány jsou komplikace a přežití. Výsledky: 30denní pooperační mortalita je 2,6 %. Na základě TNM klasifikace 11 % mělo stadium I, 78 % stadium II a 3 % stadium III. Nejzávažnější komplikací byla insuficience pankreatojejunální anastomózy (6,5 %) a opožděná evakuace žaludku (11,8 %). Ve stadiu I přežívá 1., 2., 3. a 4. rok 82 %, 52 %, 35 % a 35 % operovaných. Závěr: Resekce hlavy pankreatu je bezpečná operace s nízkou mortalitou, ale neuspokojivým dlouhodobým přežíváním. Toto je zřejmě způsobeno agresivní biologickou aktivitou nádoru a pozdní diagnostikou., The authors refer seventy six patients radically operated for pancreatic head carcinoma and analyzed postoperative complications and survival. Material and Methods: During 1/2006–9/2010 76 patients underwent potentially curative hemipancreato-duodenectomy for adenocarcinoma of the head of pancreas. The patients are prospectively followed, complications and survival are discussed. Results: The 30-days postoperative mortality was 2,6 %. According to TNM classification 11 % of patients had stage I, 78 % stage II and 3 % stage III. Most severe complications were leak from pancreato-jejunal anastomosis (6,5 %) and delayed gastric emptying (11.8 %). The survival analyses showed 1,2,3 and 4 years survival as follows: 82 %, 52 %, 35 %, 35 % respectively. Conclusion: Resection of the pancreatic head is a safe procedure with low mortality but disappointing long-term survival. Retrospektivní studie u žen do 49 let analyzovala novotvary následující po primárních nádorech cervixu (CC), dělohy (CU), vaječníků (CO) a primární novotvary před těmito gynekologickými nádory (GC), hlášené v Národním onkologickém registru ČR v letech 1976–2010. Celkem 3 727 pacientek těchto tří diagnóz představovalo 12,1 % z 30 794 nově evidovaných GC, spojených s 4 160 dalšími novotvary. Jejich vývoj se týkal a) 1 339 žen s primárním CC, 715 s CU, 664 s CO (s následnými 1 509 a 831, respektive 758 dalšími novotvary), b) 263 žen s následným CC, 212 s CU, 534 s CO (s předcházejícími 274 a 223, respektive 565 primárními novotvary). Průměrný interval vzniku následných novotvarů byl 14,2 roku po CC, 13,6 roku po CU a 10,2 roku po CO. Nejvíce z 3 098 následných nádorů bylo 21 % trávicího traktu, 16 % prsů, 13 % kůže, 11,7 % rodidel, 9,7 % dýchacích a 7,8 % močových cest; z nich během prvního roku po primárním GC bylo nejvíce nádorů vaječníků, endometria, cervixu a kolorekta. Nejvíce z 1 063 primárních novotvarů před GC bylo 26,7 % rodidel, 24,3 % prsů a 14,8 % trávicího traktu. Během 35 let počet žen s primárními GC klesal, zejména v zastoupení jejich časných klinických stadií, žen s následnými GC rostl, zejména jejich pokročilých stadií. Časná stadia byla evidována u 70,4 % žen s primárními GC a 54 % s následnými GC, pokročilá stadia u 29,5 % žen s následnými GC a 13,3 % s primárními GC, neznámá stadia zahrnovala asi 16,5 %. Ze studie se odhaduje asi 1100 případů pokročilých stadií u žen do 49 let přežívajících s GC v letech 1976–2010. Většina z hodnocených kritérií byla nepříznivá u žen s karcinomy ovarií. Zejména těmto případům v pokročilých stadiích by měly zabránit dispenzární opatření a preventivní intervence. Omezené finanční zdroje a přetrvávající rizika životního stylu neumožňují zpomalit v blízké budoucnosti nárůst primárních a následných novotvarů u onkologicky přežívajících žen nejen v gynekologii., The neoplasms following primary cancers of cervix (CC), uterus (CU), ovary (CO), and primary neoplasms before these gynaecological cancers (GC), were analyzed in a retrospective study in survived females aged 0–49 years, notified in the National Czech Cancer Registry between 1976 and 2010. A total 3,727 patients of these three diagnoses, presented 12.1 % of 30,794 newly registered GC, were associated with 4,160 other neoplasms. Their development was concerned with a) 1,339 females with primary CC, 715 with CU and 664 with CO (followed by 1,509 and 831, respectively 758 other neoplasms) and b) subsequent 263 CC, 212 CU and 534 CO (preceded by 274 and 223, respectively 565 primary neoplasms). The average interval of occurrence of subsequent neoplasms was 14.2 years after CC, 13.6 years after CU and 10.2 years after CO. The most frequent of 3.098 subsequent neoplasms were 21 % of digestive tract, 16 % breast, 13 % skin, 11.7 % female genital, 9.7 % respiratory and 7.8 % urinary organs; the most frequent during the first year after primary GC were those of the ovarial, endometrial, cervical and colorectal cancers. The most frequent of 1,062 primary neoplasms before GC were 26.7 % cancers of female genital organs, 24.3 % breast and 14.8 % digestive tract. During 35 years the number of females with primary GC has decreased, especially of their representation at the early clinical stages, the females with subsequent GC has increased, especially at the advanced stages. There were recorded early stages in 70.4 % females with primary GC and 54 % with subsequent GC, advanced stages in 29.5 % females with subsequent GC and 13.3 % with primary GC, the unknown stages included about 16.5 %. In our study are estimated about 1100 cases of advanced stages in females aged 49 years survived with GC in 1976–2010. Most of the evaluated criteria were unfavorable among females with ovarian cancers. Mainly the cases at advanced stages must be prevented by the dispensary guidelines and preventive interventions. Úvod: Každoročně jsou zveřejňována oficiální data, která popisují péči o pacienty s onemocněním diabetes mellitus v České republice. Je vždy uváděn celkový počet osob s diabetem, počet nově zachycených onemocnění a počet zemřelých. Z těchto dat však nelze zjistit, jaké jsou diference v mortalitě pro jednotlivé skupiny diabetiků ve vztahu k terapii. Cíl: Porovnat vývoj mortality v letech 2002 až 2006 s vývojem v letech 2010 až 2013 v reprezentativním vzorku populace pacientů s diabetes mellitus 2. typu léčených perorálními antidiabetiky v ČR vedených v databázi Všeobecné zdravotní pojišťovny České republiky, u níž bylo v roce 2013 pojištěno 63 % populace ČR. Metodika: Retrospektivní epidemiologická analýza. V databázi VZP jsme identifikovali všechny osoby, které měly v letech 2002–2008 a 2009–2013 záznam o diagnóze DM (E10-E16 podle MKN 10), nebo měly předepsanou jakoukoliv antidiabetickou terapii (ATC skupina A10). Pro vlastní analýzu jsme vybrali pouze pacienty, kteří byli léčeni perorálními antidiabetiky (v daném roce nebo v roce předcházejícím měli záznam o léčbě alespoň jedním preparátem ze skupiny A10B a současně neměli v obou letech záznam o léčbě preparáty ze skupiny A10A). Zvoleným kritériím odpovídalo v roce 2003 celkem 237 665 osob, a v roce 2013 celkem 315 418 osob. Výsledky: Mortalita se snížila napříč všemi věkovými skupinami (rok 2003–2013): věk 50–59 let 1,2 %-0,7 %; věk 60–69 let 2,6 %-1,6 %; věk 70–79 let 5,8 %-3,5 %. V roce 2013 se mortalita blížila obecné populaci, v níž byla pro tytéž věkové skupiny 0,6 %, 1,5 % a 3,4 %. Mortalita se snížila v relativním vyjádření oproti roku 2003 ve věkové skupině 50–59 let o 42 % (ČR o 25 %), ve věkové skupině 60–69 let o 39 % (ČR o 17 %) a ve věkové skupině 70–79 let o 40 % (ČR o 28 %). Pokles mortality u diabetiků léčených perorálními antidiabetiky byl v abslolutním i relativním vyjádření větší mezi roky 2003–2013, než v obecné populaci ČR. Závěr: Analýza mortality pacientů léčených perorálními antidiabetiky, kteří jsou v databázi VZP, prokázala jednoznačně příznivý trend snižování, který je rychlejší než v obecné populaci. Kriticky důležitým zjištěním je skutečnost, že se mortalita v tomto souboru přibližuje populaci obecné odpovídajícího věku. Současně lze oprávněně předpokládat, že při extenzivnějším využití dosavadních terapeutických postupů bude mortalita dále klesat. Klíčová slova: diabetes mellitus – mortalita – perorální antidiabetika, Introduction: Every year official data is published which describes the care of patients with diabetes mellitus in the Czech Republic. An overall number of individuals with diabetes, the number of newly reported cases and the number of patient deaths is always specified. However this data does not allow us to identify the differences in mortality between the individual cohorts of diabetic patients in relation to therapy. Goal: Comparison of the mortality development in the periods of 2002–2006 and 2010–2013 in a representative sample of the patient population with type 2 diabetes mellitus using oral antidiabetic drugs, kept in the database of the General Health Insurance Company of the Czech Republic (VZP) which provided health care coverage for 63% of Czech population in 2013. Methodology: A retrospective epidemiologic analysis. We identified all individuals in the VZP database who had a record of DM diagnosis (E10 – E16 based on ICD 10) or who had any antidiabetic therapy prescribed (ATC group A10) in the periods of 2002–2008 and 2009–2013. We only selected those patients for the analysis who were treated with oral antidiabetic medicines (in the given year or the preceding years they had a record of treatment with at least one medicine from A10B group, while having no record of treatment with medicines from A10A group within both years). 237 665 individuals met the selected criteria in 2003 and 315 418 individuals in 2013. Results: Mortality rates dropped for all age groups (from 2003–2013): for 50–59 year olds by 1.2%-0.7%; in 60–69 year olds by 2.6%-1.6%; for 70–79 year olds by 5.8%-3.5%. In 2013 mortality rates came close to the general population where for the same age groups they reached 0.6%, 1.5% and 3.4% respectively. When expressed in relative terms, the mortality among 50–59 year olds declined by 42% (Czechia by 25%), among 60–69 year olds by 39% (Czechia by 17%) and among 70–79 year olds by 40% (Czechia by 28%) from the year 2003. The decline in mortality among the patients with DM treated with oral antidiabetic medicines was greater in both absolute and relative terms in the period of 2003–2013 than among the general population in the Czech Republic. Conclusion: The analysis of mortality among the patients treated with oral antidiabetic medicines, registered in the VZP database, has shown a clearly favourable trend of mortality decline which is faster than among the general population. The fact that mortality among this cohort is getting closer to that among the general population of the corresponding age is a finding of critical importance. There is a justified expectation that mortality, with increasingly extensive utilization of the present therapeutic procedures, will continue to decrease. Česká republika patří celosvětově mezi země s nejvyšším výskytem kolorektálního karcinomu (KRK), jehož incidence však mezi českými kraji není distribuována rovnoměrně. Ve všech epidemiologických ukazatelích (incidence, mortalita i prevalence) tohoto maligního onemocnění dominuje Plzeňský kraj. Vzhledem k prodlužování průměrné délky života vzrostl v posledním desetiletí podíl KRK jako druhého metachronního primárního maligního onemocnění. V roce 2000 byl zahájen Národní program screeningu KRK, který t. č. nabízí asymptomatickým jedincům starším 50 let možnost výběru ze dvou screeningových metod (testy na okultní krvácení a primární screeningová kolonoskopie). Od roku 2006 bylo v rámci screeningu kolonoskopicky vyšetřeno 57 255 osob, u 17 760 (31,0 %) z nich byly diagnostikovány a odstraněny adenomové polypy a u 2 665 (4,7 %) byl nalezen karcinom., The Czech Republic belongs to the countries with highest colorectal cancer (CRC) incidence worldwide which differs in regions of this country. The Pilsner region stands out in all epidemiologic features (incidence, mortality, prevalence). The proportion of CRC as the second metachronous primary malignant disease has increased in past ten years, due to the elongation of life expectancy. The National colorectal cancer screening programme was launched in year 2000. Nowadays two screening methods (fecal occult glood tests and primary screening colonoscopy) are offered to asymptomatic individuals aged over 57 255 people have been examined by colonoscopy since year 2006.
