Studiu vlastností karcinomu prsu je tradičně věnována velká pozornost. V minulosti byl nejdůležitějším faktorem rozhodujícím o indikaci adjuvantní terapie stav axilárních lymfatických uzlin. V současné době je však větší důraz kladen na vlastnosti primárního tumoru. Základní léčebná strategie se stanovuje již v době stanovení diagnózy maligního onemocnění. Proto je velmi důležité znát výpovědní hodnotu, spolehlivost a úskalí vyšetření jednotlivých prognostických a prediktivních markerů karcinomu prsu. Tato studie se zabývá srovnáním vlastností nádoru stanovených před operací s těmi, které jsou k dispozici po operaci. Jedná se o retrospektivní analýzu údajů z lékařské dokumentace 411 pacientek s časným primárním karcinomem prsu léčených primárním chirurgickým výkonem. Statistickou analýzou byla prokázána dobrá nebo velmi dobrá shoda mezi předoperačním a pooperačním stanovením u invazivity nádoru, histologického typu, gradingu, proliferační aktivity a zastoupení hormonálních receptorů a receptorů HER-2/neu. Naopak měření velikosti tumoru a stanovení počtu ložisek tumoru vykazovalo špatnou korelaci. Největší rozdíly jsme popsali u žen s předoperačně histologicky diagnostikovanými karcinomy in situ, a to jak pro určení velikosti nádoru a počtu nádorových fokusů, tak i pro samotné stanovení invazivity. Jako velmi nespolehlivé bylo třeba označit předoperační vyšetření stavu axilárních lymfatických uzlin – falešná negativita tohoto vyšetření přesahovala 92 procent., Great attention is traditionally given to the investigation of breast cancer characteristics. In the past, axillary lymph node status was the most important factor for the decision on adjuvant therapy. At present, higher accent is being laid on the characteristics of the primary tumor. Basic treatment strategy is being set just at the time of setting the diagnosis of malignant disease. For this reason it is very important to know the validity, reliability and pitfalls of the examinations of breast cancer prognostic and predictive markers. This study deals with the comparison of breast cancer characteristics set at the time of diagnosis with those verified histologically after surgery. This retrospective trial analyses data from medical records of 411 patients with early primary breast cancer treated with primary surgical procedure. Statistical analysis proved good or very good agreement between preoperative and postoperative results in following cancer characteristics: invasivity, histological type, grading, proliferative activity and percentage of steroid hormone receptors and HER-2/neu receptors. On the contrary, measurements of tumor size and settings of the numbers of tumor foci exhibited poor correlation. Biggest disagreements were described in women with preoperative histological diagnosis “carcinoma in situ”: the differences were proven both for the tumor size and the number of tumor foci, and also for setting of invasiveness itself. Cíl studie: Sledování a vyhodnocení přínosu výpočtu ROMA indexů u ovarialního karcinomu a porovnání s hodnotami sérových nádorových markerů HE4 a CA125. Typ studie: Prospektivní studie Materiál a metody: V období od června 2010 do prosince 2011 bylo v Imunoanalytické laboratoři FN Plzeň vyšetřeno sérum 553 pacientek gynekologicko-porodnické kliniky. Pacientky byly rozděleny do skupin podle diagnóz (benigní, maligní) a dále pak do dalších dvou skupin podle menopauzálního statusu. Hodnoceny byly tedy celkem čtyři skupiny. U všech pacientek bylo stanoveno HE4 a CA125 chemiluminiscenční metodou na přístroji Architect i1000 (Abbott, USA). Dále byly vypočítány ROMA indexy a jejich hodnoty porovnány se sérovými hodnotami HE4 a CA125. Pro veškeré další statistické výpočty byl použit statistický software SAS 9.2. Výsledky: ROMA indexy, HE4 a CA125 byly významně zvýšeny ve skupině premenopauzálních i postmenopauzálních pacientek s histologicky potvrzeným karcinomem ovarií oproti skupinám s benigním tumorem ovarií. Pro diferenciální diagnostiku benigních a maligních nádorů ovarií je možné použít ROMA indexy, HE4 a CA125 (shodně p-Value <0,0001). Podle hodnot AUC jsme sestavili pořadí užitečnosti jednotlivých parametrů v premenopauzálním a postmenopauzálním období. Nejvyšší přínos měl v našem souboru žen výpočet ROMA2 indexu, který se kalkuluje v postmenopuzálním období (AUC=0,9777), následuje pak CA125 postmenopauzálně (AUC=0.9715) a CA125 premenopauzálně (AUC=0,9534). Nejnižší výtěžnost měl HE4 premenopauzálně (AUC=0,8702). Závěr: Hodnoty ROMA1 indexu, ROMA2 indexu, HE4 a CA125 byly významně zvýšené ve skupinách ovariálních karcinomů oproti skupinám benigních nádorů ovarií. Výše uvedené parametry lze tudíž použít pro diferenciální diagnostiku ovariálního karcinomu. Pokud bychom měli hovořit o možném využití při prvozáchytu karcinomu ovarií, v našem souboru se jevil nejlépe ROMA2 index. ROMA1 index nedosáhl takové efektivity. Z hodnot AUC je možné dále vysledovat, že HE4 nedosáhl tak dobrých výsledků jako CA125. Jejich vzájemná kombinace a vypočet zejména ROMA2 indexu však ukazuje na oprávněnost zařazení HE4 do rutinního klinického využití., Objective: Monitoring and evaluation of the benefit of ROMA indexes calculation in ovarian cancer and comparison with values of serum tumor markers HE4 and CA125 Design: Prospective study Material and Methods: In the period from June 2010 to December 2011 was in the Immunoanalytical laboratory, University Hospital Pilsen serum of 553 patients from department of Gynecology and Obstetrics examined. Patients were divided into groups by diagnosis (benign, malignant) and then into the other two groups according to menopausal status. Total of four groups were evaluated. For all patients HE4 and CA125 were determined using the chemiluminescent Architect i1000 instrument (Abbott, USA). In addition, ROMA indexes were calculated. In the next step ROMA values were compared with the serum HE4 and CA125 values. Statistical software SAS 9.2 was used for all other statistical calculations. Results: ROMA indexes, HE4 and CA125 were significantly increased in the group of premenopausal and postmenopausal patients with histologically confirmed ovarian cancer compared with groups of benign ovarian tumors. For differential diagnosis of benign and malignant ovarian tumors can be used ROMA indexes, HE4 and CA125 (both p-value <0.0001). According to the AUC, we compiled a ranking usefulness of the various parameters in pre-menopausal and postmenopausal period. The highest contribution had in our group of women the calculation of ROMA2 index that is calculated in postmenopusal period (AUC = 0.9777), followed by the CA125 postmenopausal (AUC = 0.9715) and CA125 premenopausal (AUC = 0.9534). The lowest yield was HE4 premenopausal (AUC = 0.8702). Conclusion: Values of ROMA1 index, ROMA2 index, HE4 and CA125 were significantly increased in groups of ovarian cancer compared with benign ovarian tumors groups. All the parameters can be used for differential diagnosis of ovarian cancer. If we were talking about a possible use in primary detection of ovarian cancer ROMA2 index was the best one. ROMA1 index did not reach such efficiency. According AUC can be observed that HE4 didn‘t achieve such good results as CA125. Cílem neoadjuvantní chemoterapie je zlepšení operačních možností a kosmetického výsledku operace. Chemoterapie by měla být indikována u nádorů, u kterých lze očekávat léčebnou odpověď, tedy nádorů s negativními estrogenovými receptory, vyšší proliferací, vyššího gradu a non lobulární histologie. Kazuistika popisuje případ neoadjuvantní léčby chemoterapií u pacientky, u které byla podle fenotypu nádoru získaného trucut biopsií jen malá pravděpodobnost odpovědi na chemoterapii. Biologické chování nádoru však neodpovídalo fenotypu nádoru. Pacientka měla výbornou odpověď na chemoterapii. Více prací udává diskordanci histologie z trucut biopsie a definitivní histologie hlavně v parametru proliferace nádoru., The goal of neoadjuvant chemotherapy is to improve surgical options and cosmetic outcome of surgery. Chemotherapy should be indicated in tumours in which a therapeutic response can be expected, i.e. those with negative oestrogen receptors, higher proliferation, higher grade and nonlobular histology. A case is reported of neoadjuvant chemotherapy in a female patient in whom the likelihood of response to chemotherapy was low according to tumour phenotype obtained from Tru-Cut biopsy. However, the biological behaviour of the tumour did not correspond to the tumour phenotype. The patient had an excellent response to chemotherapy. Trastuzumab is a humanized monoclonal antibody directed against the HER-2 receptor. Trastuzumab-based therapy significantly improves response rate (RR), time to progression (TTP) and overall survival (OS) for women with HER-2 positive metastatic breast cancer. Despite its initial efficacy, acquired resistance to trastuzumab develops in a majority of patients with MBC, and a large subset never responds, demonstrating primary resistance. The purpose of this retrospective study was to determine prognostic factors applicable to clinical practice. METHODS: We enrolled 112 women with metastatic breast cancer, who started the trastuzumab-based therapy at Masaryk Memorial Cancer Institute until January 2007. Clinical and laboratory factors, such as: patients conditions, character ofmetastatic spread, histology, estrogen, progesterone and Her-2 receptor status, Her-2/neu gene amplification, and serum tumor markers CEA, CA 15-3 and extracellular domain of Her-2 receptor (S-HER-2 ECD) were monitored. The association of all factors to response to therapy, time to progression (TTP) and overall survival (OS) was assessed. RESULTS: In 95% patients, the trastuzumab was combined with cytostatics (83% taxanes), 88,4% of patients started the trastuzumab as the first or second-line anticancer treatment. The median TTP was 284 days (9,3 months) and the median OS was 612 days (20,1 months) for all patients, RR was 54,5%. The highest RR was associated with the first-line treatment (p<0.0001) and with HER-2 gene/Chromosome 17 ratio > 2,2 (p=0,0092). Eleven patients (9,8%) discontinued the treatment because of toxicity, 7 patients did it as a result of cardiotoxicity (6,2%). CNS metastases occurred in 31 patients (27,7%). The S-HER-2 ECD was the most frequently elevated serum marker at the time of the treatment initialization (72,5%) and at the time of the progression (55,9%). Cox regression analysis identified S-HER-2 ECD levels at the beginning and between day 90 and 130 of the trastuzumab therapy as the best predictors of TTP. On the other hand the best predictor of OS was level of CEA before the treatment started and level of S-HER-2 ECD between day 90 and 130 of the trastuzumab therapy.
