Cíl: Zjistit prevalenci celiakie u dospělých pacientů s diabetes mellitus 1. typu (DM1T). Vliv nově zahájené léčby celiakie na kompenzaci diabetu a body mass index (BMI) pacientů. Chování protilátek proti tkáňové transglutamináze (atTG) po roce od zahájení léčby. Metodika: Retrospektivní hodnocení cíleného screeningu na celiakii v období 1. 1. 2007–1. 7. 2011 u 465 dospělých pacientů s DM1T v Diabetologickém centru I. interní kliniky LF UK a FN Plzeň (80 % všech pacientů s DM1T). Při pozitivitě atTG-A (popř. atTG-G) byla indikována enterobiopsie. U pacientů s nově indikovanou bezlepkovou dietou byl porovnán HbA1c a BMI v průběhu roku před stanovením diagnózy celiakie vs období 1 roku po 6 měsících od zahájení léčby (3–4 kontroly). Po roce od zahájení léčby byla kontrolována hladina atTG. Ke statistice byl použit párový t-test. Výsledky: Prevalence všech forem celiakie v souboru byla 10,5 %. 1,1 % pacientů (5 ze 465) mělo diagnostikovanou celiakii v dětství. Nově byla pozitivita atTG zachycena u 9,5 % (44 ze 465) pacientů – indikováni k enterobiopsii. Z toho 3 pacienti s atTG > 300 kIU/l odmítli enterobiopsii, u těchto pacientů je celiakie vysoce pravděpodobná a pro malkomplianci nešlo hodnotit vliv bezlepkové diety na BMI a HbA1c. 22 pacientů mělo potenciální formu celiakie (negativní histologie). Pozitivní nález z enterobiopsie byl zjištěn u 19 pacientů (4,1 %). Z podsouboru 22 pacientů (nově indikována bezlepková dieta), bylo nutné vyřadit další 3 pacienty (vývoj HbA1c a BMI byl ovlivněn prvozáchytem DM1T). Charakteristika podsouboru: 9 žen a 7 mužů, průměrný věk 38 ? 12 let, trvání diabetu 21 ? 13 let, celiakie diagnostikována 20,7 ? 13 let od záchytu DM1T. Během sledovaného období nedošlo ke statisticky významné změně bazální hodnoty HbA1c 67 ? 11,4 vs 69 ? 13,9 mmol/mol po sledování a významně se zvýšil BMI z 25,4 ? 4,2 na 25,9 ? 4,3 (p < 0,01). Po roce přetrvávala pozitivita atTG u 47 % (9 z 19) pacientů. Závěr: V souboru dospělých pacientů s DM1T byla zjištěna celková prevalence celiakie 10,5 %. Po zahájení léčby nedošlo k významné změně HbA1c, došlo k významnému vzestupu BMI. Pozitivita atTG přetrvávala po roce u 47 % pacientů., Objective: To assess the prevalence of celiac disease in adult patients with type 1 diabetes mellitus (T1DM). Influence the new started treatment of celiac disease on glycemic control and body mass index (BMI) of the patients. Prevail the anti-transglutaminase antibody (atTG) positivity one year after commencement of the therapy. Methods: A retrospective assessment of celiac disease targeted screening in 465 adult T1DM patients at Diabetes Center, 1st Medical Department, University Hospital in Pilsen (80 % of all T1DM patients) from 1. 1. 2007 until 1. 7. 2011. Enterobiopsy was indicated in case of atTG-A (or atTG-G) positivity. In patients with newly started gluten-free diet, HbA1c and BMI within a year after diagnosis of celiac disease were compared to a year period six months after treatment commencement (3–4 visits), atTG was evaluated one year after treatment beginning. Paired T-test was used for statistical evaluation. Results: The prevalence of all forms of celiac disease in the studied group was 10.5 %. Celiac disease diagnosed in childhood was found in 1.1 % patients (5/465). Positivity of atTG was newly observed in 9.5 % (44/465) patients. Three patients with atTG > 300 kIU/l refused the enterobiopsy examination. Celiac disease is highly plausible. The influence of gluten-free diet on BMI and HbA1c could not be evaluated due to the lack of compliance. 22 patients had a potential form of celiac disease (negative histology). Positive enterobiopsy was found in 19 patients (4.1 %). Another 3 patients had to be excluded from the subgroup of 22 patients (newly indicated gluten-free diet) as the HbA1c values and BMI were affected by the primary diagnosis of T1DM. Subgroup characteristics: 9 women and 7 men, mean age 38 ? 12 years, diabetes duration 21 ? 13 years, celiac disease diagnosed 20.7 ? 13 years since first diagnosis of T1DM. No statistically significant change in HbA1c (67 ? 11.4 vs 69 ? 13.9 mmol/mol) was observed in the studied period, however and a significant change of BMI from 25.4 ? 4.2 to 25.9 ? 4.3 (p < 0.01) was found. The atTG positivity prevailed in 47 % (9/19) of patients after one year. Conclusion: A total prevalence of the celiac disease in the group of adult T1DM patients was 10.5 %. No significant change in HbA1c occurred following treatment, a significant change of BMI was observed. Retrospektivní studie u žen do 49 let analyzovala novotvary následující po primárních nádorech cervixu (CC), dělohy (CU), vaječníků (CO) a primární novotvary před těmito gynekologickými nádory (GC), hlášené v Národním onkologickém registru ČR v letech 1976–2010. Celkem 3 727 pacientek těchto tří diagnóz představovalo 12,1 % z 30 794 nově evidovaných GC, spojených s 4 160 dalšími novotvary. Jejich vývoj se týkal a) 1 339 žen s primárním CC, 715 s CU, 664 s CO (s následnými 1 509 a 831, respektive 758 dalšími novotvary), b) 263 žen s následným CC, 212 s CU, 534 s CO (s předcházejícími 274 a 223, respektive 565 primárními novotvary). Průměrný interval vzniku následných novotvarů byl 14,2 roku po CC, 13,6 roku po CU a 10,2 roku po CO. Nejvíce z 3 098 následných nádorů bylo 21 % trávicího traktu, 16 % prsů, 13 % kůže, 11,7 % rodidel, 9,7 % dýchacích a 7,8 % močových cest; z nich během prvního roku po primárním GC bylo nejvíce nádorů vaječníků, endometria, cervixu a kolorekta. Nejvíce z 1 063 primárních novotvarů před GC bylo 26,7 % rodidel, 24,3 % prsů a 14,8 % trávicího traktu. Během 35 let počet žen s primárními GC klesal, zejména v zastoupení jejich časných klinických stadií, žen s následnými GC rostl, zejména jejich pokročilých stadií. Časná stadia byla evidována u 70,4 % žen s primárními GC a 54 % s následnými GC, pokročilá stadia u 29,5 % žen s následnými GC a 13,3 % s primárními GC, neznámá stadia zahrnovala asi 16,5 %. Ze studie se odhaduje asi 1100 případů pokročilých stadií u žen do 49 let přežívajících s GC v letech 1976–2010. Většina z hodnocených kritérií byla nepříznivá u žen s karcinomy ovarií. Zejména těmto případům v pokročilých stadiích by měly zabránit dispenzární opatření a preventivní intervence. Omezené finanční zdroje a přetrvávající rizika životního stylu neumožňují zpomalit v blízké budoucnosti nárůst primárních a následných novotvarů u onkologicky přežívajících žen nejen v gynekologii., The neoplasms following primary cancers of cervix (CC), uterus (CU), ovary (CO), and primary neoplasms before these gynaecological cancers (GC), were analyzed in a retrospective study in survived females aged 0–49 years, notified in the National Czech Cancer Registry between 1976 and 2010. A total 3,727 patients of these three diagnoses, presented 12.1 % of 30,794 newly registered GC, were associated with 4,160 other neoplasms. Their development was concerned with a) 1,339 females with primary CC, 715 with CU and 664 with CO (followed by 1,509 and 831, respectively 758 other neoplasms) and b) subsequent 263 CC, 212 CU and 534 CO (preceded by 274 and 223, respectively 565 primary neoplasms). The average interval of occurrence of subsequent neoplasms was 14.2 years after CC, 13.6 years after CU and 10.2 years after CO. The most frequent of 3.098 subsequent neoplasms were 21 % of digestive tract, 16 % breast, 13 % skin, 11.7 % female genital, 9.7 % respiratory and 7.8 % urinary organs; the most frequent during the first year after primary GC were those of the ovarial, endometrial, cervical and colorectal cancers. The most frequent of 1,062 primary neoplasms before GC were 26.7 % cancers of female genital organs, 24.3 % breast and 14.8 % digestive tract. During 35 years the number of females with primary GC has decreased, especially of their representation at the early clinical stages, the females with subsequent GC has increased, especially at the advanced stages. There were recorded early stages in 70.4 % females with primary GC and 54 % with subsequent GC, advanced stages in 29.5 % females with subsequent GC and 13.3 % with primary GC, the unknown stages included about 16.5 %. In our study are estimated about 1100 cases of advanced stages in females aged 49 years survived with GC in 1976–2010. Most of the evaluated criteria were unfavorable among females with ovarian cancers. Mainly the cases at advanced stages must be prevented by the dispensary guidelines and preventive interventions. Úvod: Je uváděno, že prevalence neurodegenerativních onemocnění v malých evropských komunitách izolovaných od okolí je vyšší ve srovnání s běžnou populací. Tentýž jev jsme pozorovali v malém, relativně izolovaném regionu jihovýchodní Moravy. Cíl: Zjištění prevalence neurodegenerativního parkinsonizmu v izolovaném regionu jihovýchodní Moravy. Metodika: Byla použita metoda třífázového vyšetření. V první fázi byly do ordinací praktických lékařů distribuovány dotazníky, které byly vyplněny všemi pacienty, kteří se z jakéhokoliv důvodu dostavili v průběhu tří měsíců do ordinace praktického lékaře. Ve druhé fázi byly u pozitivních respondentů příznaky parkinsonizmu objektivizovány vyšetřením edukovaným ambulantním neurologem. Ve třetím stupni byli respondenti, u kterých byly při skríningovém vyšetření příznaky parkinsonizmu objektivizovány, přijati k detailnímu vyšetření za hospitalizace v terciárním centru pro diagnostiku a léčbu neurodegenerativních onemocnění. Výsledky: Celková prevalence parkinsonizmu v populaci starší 50 let byla 2,8 % (95% CI: 2,2-3,4); prevalence v populaci 50-64 let byla 1,9 % (95% CI: 1,2-2,5); a v populaci nad 65 let byla 4,06 % (95% CI: 2,9-5,1). Byly získány tři velké rodokmeny s patrnou autozomálně dominantní dědičností parkinsonizmu. Závěr: Zjištěná prevalence byla překvapivě vysoká a podstatně odlišná od hodnot prevalence publikovaných v jiných evropských zemích. Vzhledem k charakteristice regionu je předpokládán vliv genetických faktorů. Současný výzkum je zaměřen na genetické pozadí a DNA analýzu probandů z rodokmenů, ve kterých byla identifikována autozomálně dominantní dědičnost parkinsonizmu. We recently observed this phenomenon in a small specific region of south-eastern Moravia. Objective: To assess the prevalence of neurodegenerative parkinsonism in an isolated region with a rural population in south-eastern Moravia. Methods: A three-stage method of data collection was used. In the first phase, originally designed questionnaires were distributed to general practitioners and completed by all patients who visited them for any reason during a three-month period. In the second phase, positive responders were examined by trained primary care neurologists. Finally, the diagnosis was confirmed or excluded by a movement disorders specialist. Results: The overall prevalence in the population older than 50 years of age was 2.8% (95% CI: 2.2-3.4); the prevalence in the population from 50 to 64 years was 1.9% (95% CI: 1.2-2.5), and it was 4.06% (95% CI: 2.9-5.1) in the population over 65 years of age. Three large families with an autosomal-dominant inheritance patterns of parkinsonism were identified. Conclusions: The prevalence rates were surprisingly high; they substantially differed from the published prevalence rates in other European countries. Due to the characteristics of the region, we expected a particular impact of genetic factors, most probably the autosomal-dominant inheritance of parkinsonism. Z 20 855 nádorů hlavy a krku (HNC), uvedených v registru nádorů ČR v letech 1976–2005 bylo 4 912 vícečetných případů, z toho 3 679 (74,9 %) u mužů a 1 233 (25,1 %) u žen, tj. ze všech hlášených HNC bylo 17,6 % případů u mužů a 19,5 % u žen spojeno s výskytem dalšího nádoru. U mužů bylo 56,3 % primárních a 43,7 % následných HNC, u žen 43,8 % a 56,2 %. Počet primárních HNC rostl během první dekády a pak klesal, počet následných HNC trvale rostl. Ze 14 krajů se vyskytlo 51,6 % vícečetných HNC v pěti krajích (Moravskoslezský, Jihomoravský, Praha, Jihočeský, Středočeský). Nejvíce bylo 28,8 % vícečetných nádorů rtů, 11,9 % tonzil a 11,1 % příušní žlázy. Po primárních 2 070 HNC u mužů se vyskytlo 2 436 následných novotvarů za průměrnou dobu 6,7 roku, po primárních 540 HNC u žen 649 dalších novotvarů za 6,9 roku. Z 425 synchronních případů bylo nejvíc nádorů dýchacího systému, kůže a jiných HNC, z 2 660 metachronních případů nádory kůže, dýchacího a trávicího systému. Následným 2 302 HNC předcházelo 2 858 jiných novotarů, nejvíce kůže, dýchacího systému a jiných lokalizací HNC. Očekávané nižší zastoupení pokročilých klinických stadií se nepotvrdilo. Z 2 436 následných novotvarů u mužů a 649 u žen bylo hlášeno po primárních časných stadiích HNC 16,1 % mužů a 18,6 % žen v následném stadiu I, II, 10,7 % mužů a 7,8 % žen ve stadiu III, IV; po pokročilých primárních stadiích HNC bylo 8,2 % mužů a 6,5 % žen v následném stadiu I, II, 8,9 % mužů a 5,2 % žen ve stadiu III, IV; nehlášená stadia představovala u mužů 56,1 %, u žen 61,9 %. Všech 729 dalších pokročilých nádorů (606 mužů, 123 žen) představovalo 3,5 % nemocných s primárním HNC, evidovaných v registru za 30 let, což bylo asi 1 540 nemocných s HNC, postižených dalším pokročilým nádorem z 43 850 HNC za 50 let. K říjnu 2007 přežívalo z primárních HNC 14,7 % mužů a 20,6 % žen a zemřelo 85,3 % mužů a 79,4 % žen. V roce 2015 bude v ČR léčeno téměř 8 tisíc přežívajících s HNC, část z nich s vícečetnými nádory. The primary 2,070 HNC in males were followed by 2,436 other neoplasms in the average interval 6.7 years; the primary 540 HNC in females were followed by 649 other neoplasms in males in the average interval 6.9 years. The most frequent diagnoses of 425 synchronous cases were cancers of respiratory tract, skin and other HNC, of 2,660 metachronous cases were cancers of skin, respiratory and digestive tract. The subsequent 2.302 HNC were preceded by 2.858 other neoplasms with the most frequent cancers of skin, respiratory tract and other HNC. The expected low representation of advanced clinical stages was not confirmed. Of 2,436 subsequent neoplasms in males and 649 in females were registered after early HNC stages in 16.1 % males and 18.6 % females of stages I, II, in 10.7 % males and 7.8 % females of stages III, IV; after advanced HNC stages were in 8.2 % males and 6.5% females of stages I, II, in 8.9 % males and 5.2 % females of stages III, IV; the unknown stages represented 56.1 % in males and 61.9 % in females of subsequent neoplasms. All 729 subsequent advanced cancers (606 males, 123 females) represented 3.5 % of total registered cases of HNC during 30 years and about 1,540 patients with HNC affected by other subsequent cancer of 43,850 HNC based in the Czech Cancer Registry during half-century. Česká republika patří celosvětově mezi země s nejvyšším výskytem kolorektálního karcinomu (KRK), jehož incidence však mezi českými kraji není distribuována rovnoměrně. Ve všech epidemiologických ukazatelích (incidence, mortalita i prevalence) tohoto maligního onemocnění dominuje Plzeňský kraj. Vzhledem k prodlužování průměrné délky života vzrostl v posledním desetiletí podíl KRK jako druhého metachronního primárního maligního onemocnění. V roce 2000 byl zahájen Národní program screeningu KRK, který t. č. nabízí asymptomatickým jedincům starším 50 let možnost výběru ze dvou screeningových metod (testy na okultní krvácení a primární screeningová kolonoskopie). Od roku 2006 bylo v rámci screeningu kolonoskopicky vyšetřeno 57 255 osob, u 17 760 (31,0 %) z nich byly diagnostikovány a odstraněny adenomové polypy a u 2 665 (4,7 %) byl nalezen karcinom., The Czech Republic belongs to the countries with highest colorectal cancer (CRC) incidence worldwide which differs in regions of this country. The Pilsner region stands out in all epidemiologic features (incidence, mortality, prevalence). The proportion of CRC as the second metachronous primary malignant disease has increased in past ten years, due to the elongation of life expectancy. The National colorectal cancer screening programme was launched in year 2000. Nowadays two screening methods (fecal occult glood tests and primary screening colonoscopy) are offered to asymptomatic individuals aged over 57 255 people have been examined by colonoscopy since year 2006. The aim of this study was to determine the prevalence of musculoskeletal problems among Czech dentists and to analyze the factors that affect these disorders. Information was gathered through questionnaire completed by 581 physicians. The questionnaire ascertained general information about physicians including their work habits and the characteristics of their work environment along with the occurrence of musculoskeletal problems as well as their intensity. In the past year the occurrence of at least mild difficulties associated with the locomotive system was reported by 96.9% of dentists surveyed (n = 557), with 66.3% of respondents (n = 381) reporting difficulties ofa moderate or major nature. Most respondents of both sexes indicated neck pain. A statistically significant correlation with the occurrence of musculoskeletal complaints of medium and major intensity was demonstrated for the following factors: sex, age, running a private practice, past injury or musculoskeletal diseases, and the perception of work as psychologically demanding. Musculoskeletal disorders in dentists in Czech Republic are relatively frequent and serious health problem. The causes of these diseases must be identified and appropriate preventive interventions undertaken that will contribute to a reduction in the incidence of these problems. and Z. Sustová, L. V letech 1959–2008 počet zhoubných novotvarů centrální nervové soustavy (ZN CNS) z dat registru nádorů ČR dosáhl 26 548, z toho 14 263 (53,7 %) u mužů a 12 285 (46,3 %) u žen, v roce 2008 představující u mužů 412 (8,1/100 tis.), u žen 418 (7,9/100 tis.) onemocnění. Jejich prevalence v letech 1989–2005 vzrostla z 1169 na 2 565 žijících případů s očekávaným počtem 3 430 v roce 2015. Z 962 vícečetných ZN CNS, evidovaných v letech 1976–2005, bylo 150 primárních onemocnění u mužů a 162 u žen, 327 následných ZN CNS u mužů a 323 u žen tj. 5,1 % ze všech nově hlášených u mužů a 6,2 % u žen. Za toto období vzrostl v pětiletých intervalech počet primárních případů dvakrát, následných téměř třicetkrát. U primárních a následných ZN CNS převažovaly ve věku do 49 let ženy nad muži, v 50–69 letech muži nad ženami. Ze 14 krajů se vyskytlo 47 % ZN CNS ve čtyřech krajích (Moravskoslezský, Jihomoravský, Praha, Středočeský). Pouze 6,2 % multiplicit bylo evidováno v časných a 7,4 % v pokročilých klinických stadiích; 86,4 % stadií nebylo nahlášeno. Průměrný interval mezi 150 primárními ZN CNS a 170 jinými následnými novotvary u mužů byl pět let, mezi 162 primárními a 170 následnými u žen byl šest let, ze kterých byly u obou pohlaví nejpočetnější následné karcinomy trávicího ústrojí a kůže. Před 327 následnými ZN CNS bylo u mužů 390 primárních novotvarů, u žen před 323 následnými 362 primárních, ze kterých byly nejpočetnější karcinomy kůže, rodidel, prsů, prostaty a trávicího ústrojí.
