Metastatický melanom je agresivní onemocnění. Dosud využívané léčebné možnosti jako aplikace dakarbazinu, temozolomidu, high dose interleukinu-2 byly relativně omezené. Vemurafenib je malá molekula, která má schopnost inhibovat mutovanou BRAF proteinkinázu. Na základě provedených klinických studií BRIM-2 a BRIM-3, kdy vemurafenib dokázal prodloužit celkové přežití (OS) a dobu do progrese onemocnění (PFS), byl zaregistrována v EU k léčbě v monoterapii u dospělých pacientů s neresekovatelným nebo metastazujícím melanomem s pozitivní mutací V600 genu BRAF., Metastatic melanoma is an aggressive disease. Angiofibromas are rare vascular tumors which originate predominantly in the nasopharynx and occur typically in male adolescents. Extranasopharyngeal sites such as nasal cavity and paranasal sinuses are less frequent. This review article was undertaken to evaluate the incidence, clinical features and management of extranasopharyngeal angiofibromas originating exclusivelly from nasal cavity structures. Our focus of interest was to evaluate the significance of immunohistochemical analysis in diagnosis of such extremely rare neoplasms. In the PubMed and Google Search, we found only 39 cases of nasal angifibroma, 27 males and 12 females from 1980 to 2012. The most prevalent site of origin was nasal septum, followed by inferior and middle turbinate. The commonest symptoms were nasal obstruction and epistaxis. Nasal angiofibromas are clinically distinct from nasopharyneal angiofibromas and can therefore be misdiagnosed. The differential diagnosis includes other vascular lesions, such as lobular capillary hemangioma and sinonasal-type hemangiopericytoma. Although immunohistochemistry is not necessary for differentiation between angiofibroma and capillary hemangioma, that diagnostic procedure may be helpful in distinction from sinonasal hemangiopericytoma. As an ilustration for immunohistochemical analysis, we presented a case of an elderly woman with tumor arising from the middle turbinate, diagnosed as angiofibroma. Úvod: Nedostatečný zbytkový objem jater po resekci (FLRV) je hlavní příčinou primární neresekability jaterních nádorů. Embolizace větve portální žíly (PVE) s aplikací hematopoietických kmenových buněk (HSC) může být způsob, jak významnou měrou zvýšit počet nemocných s dostatečným FLRV, a tím rozšířit resekabilitu primárně chirurgicky neřešitelných nádorů jater. Metodika: U dvanácti nemocných s FLRV < 30 % byla provedena PVE na straně nádoru s následnou aplikací HSC do druhostranné neembolizované větve vena portae. Výsledky: PVE s aplikací HSC proběhly bez komplikací. U všech nemocných došlo do 4 týdnů k dostatečnému nárůstu FLRV. Radikální resekce jater byla provedena u osmi nemocných s metastázami kolorektálního karcinomu. U dvou nemocných současně se zvýšením FLRV došlo i k významnému nárůstu objemu nádoru, resekce jater nebyla možná. U jednoho nemocného těžké nitrobřišní srůsty zabránily jaterní resekci. Jeden nemocný je čerstvě po aplikaci HSC. Celkové přežívání nemocných po dobu jednoho roku a tří let je 66,7 %, resp. 33,3 %. Medián přežití bez recidivy je 6 měsíců. Závěr: Kombinace PVE s aplikací HSC se jeví jako nadějná metoda pro zvýšení operability jaterních nádorů., Introduction: Insufficient future liver remnant volume (FLRV) is the main cause of liver tumors unresectability. Portal vein embolization (PVE) with application of hematopoietic stem cells (HSC) can be a matter how to increase resectability of liver tumors. Method: PVE with application of HSC were performed in 12 patients with primary unresectable liver tumors. Results: The procedures were without complications. After 4 weeks the sufficient FRLV grew. Radical liver resection was performed in 8 patients. Tumor progression was pattern in 2 patients and in one patients the sever intraabdominal adhesions were cause of inoperability. One patient is just after PVE and HSC application. Overall survival in one and three years is 66.7 %, resp. 33.3 %. Median of progression free survival is 6 months.
