Jako velmi nespolehlivé bylo třeba označit předoperační vyšetření stavu axilárních lymfatických uzlin – falešná negativita tohoto vyšetření přesahovala 92 procent., Great attention is traditionally given to the investigation of breast cancer characteristics. In the past, axillary lymph node status was the most important factor for the decision on adjuvant therapy. At present, higher accent is being laid on the characteristics of the primary tumor. Basic treatment strategy is being set just at the time of setting the diagnosis of malignant disease. For this reason it is very important to know the validity, reliability and pitfalls of the examinations of breast cancer prognostic and predictive markers. This study deals with the comparison of breast cancer characteristics set at the time of diagnosis with those verified histologically after surgery. This retrospective trial analyses data from medical records of 411 patients with early primary breast cancer treated with primary surgical procedure. Statistical analysis proved good or very good agreement between preoperative and postoperative results in following cancer characteristics: invasivity, histological type, grading, proliferative activity and percentage of steroid hormone receptors and HER-2/neu receptors. On the contrary, measurements of tumor size and settings of the numbers of tumor foci exhibited poor correlation. Biggest disagreements were described in women with preoperative histological diagnosis “carcinoma in situ”: the differences were proven both for the tumor size and the number of tumor foci, and also for setting of invasiveness itself. Preoperative investigation of the axillary lymph node status has to be considered very unreliable – the false negative rates of this investigation exceeded 92 percent., Dušan Kolařík, Jelena Vitalijevna Levá, Petr Křepelka, Miroslav Verner, Markéta Trnková, Hana Kolářová, and Lit.:
Cíl studie: Sledování a vyhodnocení přínosu stanovení solubilních cytokeratinových fragmentů pro diagnostiku jaterních metastáz. Typ studie: Kombinovaná retrospektivní a prospektivní studie. Výsledky: U jaterních metastáz jsme zjistili výrazně vyšší hladiny všech sledovaných markerů než u primárního plicního nádoru a než u nenádorových diagnóz (shodně p-Value <0,0001). ROC křivky ukazují, že schopnost odlišit spojenou nádo - rovou skupinu C34 a C787 od kontrolní není u hodnocených markerů příliš výrazná, což potvrzují vypočítané AUC: Mono - Total=0.6924, CYFRA 21-1=0,6398 a CEA=0,5955. Pokud hodnotíme nádorové skupiny odděleně podle diagnóz, ROC křivky jasně ukazují, že schopnost odlišení jaterních metastáz od kontrolní skupiny pomocí hodnocených markerů je výrazně vyšší než odlišení plicního nádoru. Vypočítané AUC pro jaterní metastázy jsou: MonoTotal=0,9497, CYFRA 21-1=0,8758 a CEA=0,7514. Závěr: U jaterních metastáz jsou hladiny všech sledovaných markerů významně vyšší než u primárního plicního nádoru a než u nenádorových diagnóz. Cytokeratinové markery umožňují časnější záchyt jaterních metastáz a s využitím moderní onkologické a chirurgické léčby umožňují prodloužení doby přežití a zvýšení kvality života pacientů, Objective: Monitoring and evaluation of benefit of the soluble cytokeratin fragments determination for the liver metastases diagnostics. Design: Combined retrospective and prospective study. Material and Methods: In the period from January 2010 to December 2014 was in the Laboratory of immunoanalysis examined 1616 serum samples of patients from lung and surgery clinic. Patients were divided into groups according to the diagnoses. C34 consisted of the patients with lung cancer in stage I, II and III. C787 group consisted of patients with metastatic liver disease without distinguishing of origin of these metastases. The control group consisted of patients treated for inflammatory lung diseases and then from patients who were treated for thyroid disorders or metabolic disorders. At the time of sample collection all the patients were in the compensated status. MonoTotal, CYFRA 21-1 and CEA were determi - ned in each sample. MonoTotal was determined using an immunoradiometric kit MonoTotal IRMA (IDL Biotech, Sweden), CYFRA 21-1 using immunoradiometric CYFRA 21-1 IRMA kit (Cisbio Bioassays, France) and CEA was determined using chemiluminescent kit and the measurement was performed using the Architect i1000 instrument (Abbott Laboratories, USA) . Statistical software (StatSoft, Inc., USA) was used for all statistical calculations. Results: For liver metastases, we found significantly higher levels of all studied markers than for the primary lung tumor and non-tumor diagnoses (both p-value <0.0001). ROC curves show that the ability of assessed markers to distinguish between the tumor group (C34 + C787) and the control is not too strong. Calculated AUC values confirm this fact: Mono - Total=0.6924, CYFRA 21-1=0.6398 and CEA=0.5955. When evaluating the cancer groups separately according to the diagnose ROC curves clearly show that the ability to distinguish hepatic metastases from control group using the evaluated markers is significantly higher than the distinguishing of pulmonary tumor. AUC calculated for liver metastases are: Mono - Total=0.9497, CYFRA 21-1=0.8758 and CEA=0.7514. Conclusion: For liver metastases are the levels of all monitored markers significantly higher than in the patients with the primary lung tumors and in the patients with non-tumor diagnoses. ROC curve and AUC clearly show that cytokeratin markers MonoTotal and CYFRA 21-1are highly sensitive markers of liver metastases. CEA, which was used as a marker of metastatic process in the past, did not match so good results as both cytokeratins did. Cytokeratin markers allow for earlier detection of liver metastases and the use of modern oncology and surgical treatments allow prolonged survival and improved quality of life of patients., Špišáková M., Kučera R, Topolčan O., Šafanda M., Slouka D., Kinkorová J, Třeška V., and Literatura
