Pro indikaci spinocelulárních karcinomů hlavy a krku je v rámci cílené léčby zaregistrován pouze jediný přípravek a sice cetuximab, jehož nižší toxicita je nespornou výhodou oproti chemoterapii. Díky vysokému počtu dosažených kompletních remisí u lokoregionálně pokročilých onemocnění v našem souboru máme zkušenosti s toutu léčbou velmi dobré. Nádory tlustého střeva a konečníku patří v České republice k nejčastějším zhoubným nádorům jak v populaci mužů, tak v populaci žen. Více než 50 % nově diagnostikovaných kolorektálních nádorů je zjištěno v pokročilém stadiu vývoje a zhruba 20 % má v době stanovení diagnózy založeny vzdálené metastázy. Játra jsou nejčastější orgánovou lokalizací metastáz kolorektálních karcinomů. Kurativní resekce metastáz je primárně možná pouze u malé části pacientů. Proto systémová léčba, chemoterapie a v posledních letech biologická léčba hrají významnou úlohu v léčbě metastazujícího kolorektálního karcinomu. Cetuximab (Erbitux) je monoklonální protilátka specificky namířená proti EGFR receptoru nádorových buněk, včetně buněk kolorektálního karcinomu. Inhibicí aktivity EGFR receptoru a jeho signální kaskády může Erbitux zastavit další růst nádorových ložisek a navodit jejich regresi. Přestože je cetuximab účinný i jako monoterapie, jeho účinek se zvyšuje v kombinaci s chemoterapií, opírající se jak o irinotekan, tak o oxaliplatinu. Přidání cetuximabu k chemoterapii navíc pomáhá zvrátit refrakternost nádorových buněk vůči chemoterapii samotné. V této kazuistice popisujeme případ staršího pacienta mužského pohlaví, u kterého byl cetuximab nasazen v době, kdy předcházející chemoterapie (režimy FOLFOX4 i FOLFIRI) nedokázala zabránit progresi onemocnění. Léčba cetuximabem v kombinaci s režimem FOLFIRI dosáhla významné regrese metastatického postižení a při pokračující udržovací léčbě cetuximabem samotným zůstává pacient v klinické remisi, která již trvá déle než 15 měsíců. Pacient dlouhodobou léčbu cetuximabem vcelku dobře toleruje, hlavním nežádoucím účinkem léčby je kožní akneiformní reakce, která však nebrání v pokračování terapie., In the Czech Republic, colorectal cancer is one of the most common malignancies both among men and women. More than 50 % of newly diagnosed colorectal cancers are found in an advanced stage and about 20 % have distant metastases at diagnosis. The liver is the most common site of metastasis from colorectal cancer. Curative resection of metastases is primarily feasible only in a small proportion of patients. Thus, systemic therapy, chemotherapy and, more recently, biological therapy play a significant role in treating metastatic colorectal cancer. Cetuximab (Erbitux) is a monoclonal antibody targeted specifically at the EGFR receptor of tumour cells, including those of colorectal cancer. By inhibiting the activity of the EGFR receptor and its signal cascade, Erbitux can stop further growth of tumour foci and induce their regression. Although cetuximab is also effective as monotherapy, its effect increases in combination with chemotherapy involving both irinotecan and oxaliplatin. Moreover, adding cetuximab to chemotherapy helps to reverse the refractoriness of tumour cells to chemotherapy alone. The present case report describes an elderly male patient who was started on cetuximab after previous chemotherapy regimens (FOLFOX4 and FOLFIRI) had failed to prevent the progression of disease. Treatment with cetuximab in combination with the FOLFIRI regimen resulted in a significant regression of metastatic disease and, while being continued on maintenance therapy with cetuximab alone, the patient has remained in clinical remission for more than 15 months now. Panitumumab je první plně humánní anti-EGFR monoklonální protilátka, jež vykazuje velmi slibnou účinnost u nemocných s nemutovaným (wild) K-ras genem. Léčebný účinek panitumumabu v monoterapii byl prezentován ve stěžejních studiích III. fáze, ve kterých byl u nemocných léčených panitumumabem v monoterapii výrazně prodloužen čas do progrese ve srovnání s pacienty, jejichž onemocnění bylo rezistentní ke standardním chemoterapeutickým režimům a kteří dostávali podpůrnou léčbu. Pacienti s metastatickým kolorektálním karcinomem (mKRK) by měli v průběhu své léčby obdržet všech pět látek účinných u tohoto onemocnění. Tyto látky se skládají ze tří chemoterapeutik: 5-fluorouracil/leucovorin (5-FU/LV), irinotecan, oxaliplatina a dvou monoklonálních protilátek: bevacizumab namířený proti vaskulárnímu endoteliálnímu růstovému faktoru a inhibitor epidermálního růstového faktoru cetuximab nebo panitumumab. Léčebné možnosti třetí linie závisí na tom, jaké látky byly použity v předchozí terapii. V klinické kazuistice je prezentována role cetuximabu ve 2. linii terapie metastatického kolorektálního karcinomu s následnou udržovací monoterapií, kdy byla u pacienta dosažena kompletní remise nádorového onemocnění. Uvedený případ potvrzuje účinnost léčby cetuximabem u předléčených pacientů s metastazujícím kolorektálním karcinomem za předpokladu nemutovaného genu K-RAS. Bisfosfonáty jsou součástí komplexní léčebné strategie u všech onkologických diagnóz, které způsobují nádorovou kostní chorobu. Bisfosfonáty se používají od 80. let minulého století a neustále mají pro pacienty velký přínos. Ve studii, která je zde prezentována se hodnotí přínos první generace bisfosfonátů tj. klodronátu, nejen z hlediska jeho účinku na kostní příhody, ale hodnotí se zde i výsledky dlouhodobého přežití pacientů. Data ve zmiňované studii ukazují, že u skupiny pacientů užívajících klodronát bylo prodloužení přežití prokázáno., Bisphosphonates are part of comprehensive therapeutic strategy in all oncological diagnoses resulting in tumorous bone disease. Mezi pojmem cílené léčby a individualizací léčby na míru mohou být často velké rozdíly. V naší kazuistice naopak prezentujeme si tuaci, kdy jedině léčba cílená byla u pacienta s kolorektálním karcinomem jako jediná spojena s klinickým benefitem. Kromě extrémní důležitosti chirurgie i u metastatického onemocnění by tak mohla být dokumentována potřeba vyzkoušet efekt cílené léčby u většiny pacientů, přestože se v její indikaci často nemůžeme opírat o žádné prediktivní faktory., There might be big differences between the terms of targeted therapy and treatment tailoring. In our case report we describe situation when targeted therapy as the only method was associated with a clinical benefit in a patient with colorectal cancer. In addition to extrem importance of surgery also in treatment of metastatic disease this could be a clear documentation of need to explore the effect of targeted therapy in the majority of patients although we are often unable to define any predictive markers for it´s indication, and Tomáš Svoboda
