The aim of this study was to compare the isolation systems OptraDam® Plus and OptiDam™ with the conventional rubber dam in terms of objective and subjective parameters. The isolation systems were applied during the dental treatment of the patients. The time of preparation, placement, presence and removal were measured and the quality of isolation was evaluated. The median time of rubber dam placement was 76 s (Q1=62 s; Q3=111.25 s). The application time of OptraDam® Plus was significantly longer compared to the other systems (P ® plus. Dyskinetické extrapyramidové syndromy mohou vzácně vést k akutnímu zhoršení do život ohrožujícího stavu, který si vyžádá hospitalizaci na jednotce intenzivní péče, hlubokou analgosedaci a umělou plicní ventilaci. V případě neúspěšnosti symptomatické farmakologické léčby lze uvažovat o stereotaktickém neurochirurgickém výkonu (pallidotomii nebo hluboké mozkové stimulaci). V příspěvku uvádíme naše dosavadní zkušenosti se třemi pacienty, kteří v akutní fázi dyskinetického syndromu takový výkon podstoupili. U dvou dětských pacientů (12 a 6 let) pro status dystonicus s geneticky verifikovanou hereditární generalizovanou dystonií (DYT‑6 a DYT‑1) byla provedena hluboká mozková stimulace vnitřního pallida a u jedné pacientky (21 let) s Huntingtonovou nemocí byla provedena pallidotomie pro akutní zhoršení generalizované chorey s rozvojem balizmu. Přes počáteční kritický stav došlo u všech tří pacientů v odstupu 1–6 týdnů od výkonu k významnému potlačení mimovolných pohybů. Stereotaktická neurochirurgická léčba by proto měla být zvažována jako metoda volby v terapii akutních, život ohrožujících hyperkinetických stavů, kde farmakologická terapie selhala., Dyskinetic movement disorders may rarely progress to an acute life-threatening condition requiring admission to an intensive care unit, deep sedation and mechanical ventilation. Stereotactic neurosurgery (pallidotomy or deep brain stimulation) should be considered in all patients who did not respond to pharmacological treatment. We report three patients who underwent stereotactic neurosurgery due to acute dyskinetic syndrome. Two boys (12 and 6 years) with generalized hereditary dystonia (DYT-6 and DYT-1) developed status dystonicus and were treated with deep brain stimulation of the globus pallidus interna. The third patient was a 21-years-old woman with Huntington’s disease who underwent a pallidotomy due to acute worsening of generalized chorea with ballistic movements. Despite the initial critical state of all three patients, their abnormal movements significantly subsided within six weeks after the neurosurgery. The stereotactic neurosurgical procedure should be considered as the method of choice in patients with acute life-threatening hyperkinetic movement disorders that failed to improve with pharmacological treatment. Úvod: Existuje mnoho studií věnujících se problematice biliárních komplikací po jaterních resekcích. Přes zlepšení operační techniky a perioperační péče má incidence této komplikace spíše vzrůstající charakter. Byly analyzovány četné prediktivní faktory. Na jejich vliv na vznik biliárního leaku doposud nepanuje shoda. Za cíl analýzy jsme si stanovili zhodnotit incidenci biliárního leaku, jeho vliv na mortalitu a na dobu hospitalizace na našem pracovišti. Současně jsme provedli rozbor známých prediktivních faktorů. Metoda: Autoři retrospektivně analyzují soubor 146 nemocných, u kterých byla na Chirurgické klinice 2. LF Univerzity Karlovy a ÚVN, Praha v období 2010–2013 provedena jaterní resekce. K vyhodnocení biliárního leaku jsme užili současnou uznávanou klasifikaci ISGLS (International Study Group of Liver Surgery). Závažnost komplikace byla stanovena dle Clavien-Dindo. Statistickou významnost prediktivních faktorů jsme určili pomocí Fisherova exaktního testu a Studentova t-testu. Výsledky: Incidence biliárního leaku byla 21 %. Dle ISGLS bylo 6,5 % pacientů zařazeno do skupiny typu A, v 61,2 % se jednalo o leak B a v 32,3 % o leak typu C. Stupeň závažnosti dle Clavien-Dindo I-II, IIIa, IIIb, IV a V byl v poměru 19,3 %, 42 %, 9,7 %, 9,7 %, resp. 19,3 %. Jako statisticky významné jsme stanovili tyto faktory: operaci pro malignitu (p<0,001), velkou jaterní resekci (p=0,001), dobu operace (p<0,001), vyšší peroperační krevní ztrátu (p=0,02), konstrukci HJA (p=0,005), portální venózní embolizaci/two-stage chirurgii (p=0,009) a ASA skóre (p=0,02). Biliární leak významně prodloužil dobu hospitalizace (p<0,001). V souboru pacientů s biliárním leakem byla 23krát vyšší perioperační mortalita (p<0,001) než v souboru bez leaku. Závěr: Biliární leak je jednou z nejzávažnějších komplikací jaterních resekcí. Většina rizikových faktorů je těžko ovlivnitelných a na jejich vliv nepanuje jednoznačná shoda. Incidenci by mohly redukovat diskutované peroperační leak testy. V budoucnu bude třeba dále pracovat na zlepšení perioperačního managementu a techniky k prevenci vzniku této závažné komplikace. V léčbě se uplatňuje multidisciplinární přístup., Introduction: Many previous reports have focused on bile leakage after liver resection. Despite the improvements in surgical techniques and perioperative care the incidence of this complication rather keeps increasing. A number of predictive factors have been analyzed. There is still no consensus regarding their influence on the formation of bile leakage. The objective of our analysis was to evaluate the incidence of bile leakage, its impact on mortality and duration of hospitalization at our department. At the same time, we conducted an analysis of known predictive factors. Method: The authors present a retrospective review of the set of 146 patients who underwent liver resection at the Department of Surgery of the 2nd Faculty of Medicine of the Charles University and Central Military Hospital Prague, performed between 2010−2013. We used the current ISGLS (International Study Group of Liver Surgery) classification to evaluate the bile leakage. The severity of this complication was determined according to the Clavien-Dindo classification system. Statistical significance of the predictive factors was determined using Fisher‘s exact test and Student‘s t-test. Results: The incidence of bile leakage was 21%. According to ISGLS classification the A, B, and C rates were 6.5%, 61.2%, and 32.3%, respectively. The severity of bile leakage according to the Clavien-Dindo classification system – I-II, IIIa, IIIb, IV and V rates were 19.3%, 42%, 9.7%, 9.7%, and 19.3%, respectively. We determined the following predictive factors as statistically significant: surgery for malignancy (p<0.001), major hepatic resection (p=0.001), operative time (p<0.001), high intraoperative blood loss (p=0.02), construction of HJA (p=0.005), portal venous embolization/two-stage surgery (p=0.009) and ASA score (p=0.02). Bile leakage significantly prolonged hospitalization time (p<0.001). In the group of patients with bile leakage the perioperative mortality was 23 times higher (p<0.001) than in the group with no leakage. Conclusion: Bile leakage is one of the most serious complications of liver surgery. Most of the risk factors are not easily controllable and there is no clear consensus on their influence. Intraoperative leak tests could probably reduce the incidence of bile leakage. In the future, further studies will be required to improve the perioperative management and techniques to prevent such serious complications. Úvod: V roce 2016 to bude 60 let od doby, kdy Nissen poprvé publikoval svoji operační metodu 360° fundoplikace kolem distálního jícnu a její výsledný efekt. V současnosti je Nissenova fundoplikace pravděpodobně nejčastěji používanou metodou k chirurgické léčbě gastroezofageálního refluxu (GER) a hiátové hernie (HH). Incidence HH je v západní populaci mezi 25–50 %. Současná prevalence GER je v Evropě kolem 4–10 %, v Severní Americe 5–7 %, ale v některých studiích je tato prevalence udávána dokonce kolem 10–20 %. Metody: Na Chirurgické klinice 2. LF UK a ÚVN bylo v období 2010 až 2015 provedeno 157 laparoskopických operací pro hiátovou hernii nebo refluxní nemoc jícnu (GERD). Většina nemocných byla indikována pro symptomatickou HH s/bez GERD, jen minimum nemocných bylo indikováno pro GERD bez HH. V souboru bylo 73 mužů a 84 žen s průměrným věkem 53,9 roku v rozmezí 20–88 let. Z tohoto počtu mělo 11 nemocných syndrom hrudního žaludku. U 109 nemocných (69,4 %) byla provedena 360° fundoplikace dle Nissena (NFP) a u 48 nemocných (30,6 %) byla provedena 270° fundoplikace dle Toupeta (TFP). Po operaci byli nemocní ambulantně kontrolováni v intervalu 3 měsíce, 6 měsíců a 12 měsíců a dále po roce. Prospektivně byly sledovány příznaky pyrózy, dysfagie, možnosti či nemožnosti odříhnutí, meteorismu nebo výskyt recidivy, získaná data byla zkoumána retrospektivně. Pro vyhodnocení získaných dat byla použita metoda deskriptivní statistiky a explorační jednosměrné analýzy. Výsledky: V období 2010 až 2015 bylo provedeno 157 laparoskopických operací pro hiátovou hernii nebo GERD. V souboru se nevyskytlo úmrtí ve vztahu k provedené operaci. Tři měsíce po operaci došlo jak u pacientů po NFP, tak u pacientů po TFP k výraznému poklesu až vymizení symptomů refluxu, příznaků pyrózy a epigastrické bolesti. Pooperační dysfagické obtíže vymizely ve skupině po NFP do 4–7 týdnů, u nemocných po TFP do 3–5 týdnů. V intervalu 2–5 let po operaci došlo celkem u 36 (22,9 %) nemocných k recidivě obtíží. U 22 nemocných (14 po NFP, 8 po TFP), u kterých byla prokázána endoskopicky nebo RTG vyšetřením recidiva refluxu bez recidivy kýly, byla zahájena léčba inhibitory protonové pumpy s velmi dobrým efektem. U 14 nemocných s anatomickou recidivou hiátové hernie s/bez refluxu (8 po NFP, 6 po TFP) byla provedena laparoskopicky rehiátoplastika a refundoplikace dle Nissena. Závěr: Laparoskopická chirurgická antirefluxní léčba GERD a HH je bezpečná metoda s velmi nízkou perioperační morbiditou a s minimální mortalitou. NFP nebo TFP ve stejné míře dlouhodobě obnovují mechanickou bariéru proti regurgitaci žaludečního obsahu do jícnu se signifikantním efektem na vymizení příznaků GERD a HH a s výrazným zlepšením kvality života nemocných. U obézních nemocných je před operací nutná redukce hmotnosti. U morbidně obézních nemocných s HH indikovaných k bariatricko-metabolické operaci je doporučováno provést hiátoplastiku současně s bariatrickým výkonem., Introduction: In 2016 60 years will have elapsed from the first publication by Nissen about his surgical method of 360° fundoplication around distal esophagus and its final effect. Nissen’s fundoplication is currently the most common surgical method for the gastroesophageal reflux (GER) and hiatal hernia (HH) probably. Incidence of HH ranges between 25–50% in the western population. Current prevalence of GER ranges between 4–10% in Europe and 5–7% in North America, but some studies report even 10–20%. Methods: From 2010 to 2015, 157 laparoscopic procedures for HH or gastroesophageal reflux disease (GERD) were done at the Department of Surgery, the Second Faculty of Medicine, Charles University and Central Military Hospital in Prague. Most of the patients were operated because of symptomatic HH with/without GERD. Only very few patients were operated only for GERD without HH. The group of the patients included 73 men and 84 women with the mean age 53.9 years in the range 20–88 years. 11 patients of the group were operated because of the upside-down stomach syndrome. 109 (69.4%) patients underwent Nissen’s 360° fundoplication (NFP) and 48 (30.6%) patients underwent Toupet’s 270° fundoplication (TFP). The patients were followed after 3 month, 6 month, 12 month and then once a year after the surgery. Symptoms such as heartburn, dysphagia, belching, bloating or recurrence of HH or GERD were collected prospectively and analyzed retrospectively. Methods of descriptive statistics and explorative unidirectional analysis were used. Results: From 2010 to 2015, 157 laparoscopic procedures were done for HH or GERD. No death occurred in the group of the patients in connection with the surgery. Similar rates of considerable improvement or resolution of the symptoms such as the reflux, heartburn, and epigastric pain were observed after three months from the surgery among patients after NFP or TFP. Postoperative dysphagia resolved in 4–7 weeks in the group after NFP, and in 3–5 weeks in the group after TFP. Disease recurrence was observed in 36 (22.9%) patients in 2 to 5 years after the surgery. Recurrence of only the gastroesophageal reflux without HH was observed in 22 patients (14 after NFP; 8 after TFP), shown by endoscopy or x-ray assessment, with a satisfactory response to PPI medication. Laparoscopic rehiatoplasty and refundoplication according to Nissen were performed in 14 patients with HH recurrence with/without the reflux (8 after NFP; 6 after TFP). Conclusion: Laparoscopic antireflux surgery of GERD and HH is safe and is associated with very low perioperative morbidity and minimal morbidity. Both NFP and TFP have a similar effect on restoration of the mechanical gastroesophageal barrier against gastric reflux, with significant effect on resolution of GERD and HH symptoms and with a marked improvement of the patient’s quality of life. As regards morbid obese patients, weight reduction is needed before the surgery. BACKGROUND: Human immunodeficiency virus (HIV) continues to be a serious health issue and one of the world most devastating epidemics. An estimated 1.5 million people died from AIDS-related illnesses in 2013, and an estimated 37 million people with AIDS have died worldwide since the epidemic has begun. HIV infection is known for its oral manifestations which causes discomfort and pain for infected individuals. The objective of this study was to document oral conditions of HIV positive patients and the pattern and frequency of oral and dental lesions. METHODS: All patients with confirmed HIV infection who were treated at the Department of Dentistry, University Hospital in Hradec Králové, were examined. RESULTS: During the study period, 29 HIV positive patients were examined and treated--19 men, 10 women, with mean age of 32.9 years (range 22-58 years). 72.41% patients received ART. In total, all patients underwent 186 visits. The most frequent treatments were associated with teeth and periodontal lesions (71.80%), oral mucosal lesions were diagnosed and treated only in 3.96% cases. CONCLUSION: Since the introduction of ART, the frequency of oral mucosal lesions is minimal in patients with HIV infection. and M. Šembera, V. Radochová, R. Slezák
Předkládáme popis případu nemocného s familiární formou amyloidové polyneuropatie. Mladý nemocný s postupně progredujícími projevy léze periferních nervů byl zprvu diagnostikován a léčen pro chronickou zánětlivou demyelinizační polyneuropatii. Pro následné komplikace bylo provedeno endoskopické vyšetření zažívacího traktu s bioptickým průkazem amyloidových depozit. Následné komplexní vyšetření potvrdilo hereditární formu transtyretinové amyloidové polyneuropatie na podkladě raritní mutace v genu pro transtyretin na pozici Val50Ala. Anamnesticky, matka nemocného byla léčena pro amyloidózu s neuropatickými projevy. Léčba pacienta preparátem tafamidis meglumine vedla k zabránění další progrese onemocnění a následně byla provedena ortotopní transplantace jater. V současnosti dochází ke klinickému zlepšování stavu nemocného s postupnou regresí motorických a senzitivních neuropatických projevů., We present a case of a patient with familial amyloid polyneuropathy. A young patient with gradually progressing symptoms of peripheral neuropathy was initially diagnosed and treated for chronic inflammatory demyelinating polyneuropathy. Subsequent complications resulted in endoscopic examination of the digestive tract. Biopsy showed evidence of amyloid deposits. Subsequent comprehensive examination confirmed hereditary form of amyloid polyneuropathy on the basis of a rare mutation in the transthyretin gene in Val50Ala position. Family history revealed that the mother of the patient was treated for amyloidosis with neuropathic symptoms. Treatment with tafamidis meglumine prevented further disease progression, and was followed by orthotopic liver transplantation. At present, clinical condition of the patient gradually improves with regression of motor and sensory neuropathic symptoms. Cíl: Autoři prezentují první zkušenosti s hemihypoglossofaciální anastomózou při řešení postoperačních plegií lícního nervu u neurochirurgických pacientů. Soubor a metodika: V průběhu let 2009–2014 byli na našem oddělení operováni tři pacienti, u kterých byla provedena hemihypoglossofaciální anastomóza dle Cushimana a Sekhara. Důvodem plegie nervus facialis byl u dvou pacientů objemný vestibulární schwannom (v jednom případě operován po ozáření gama nožem) a u jednoho pacienta objemný schwannom nervus facialis pod bazí lební. Výsledky: U dvou pacientů bylo dosaženo reinervace ve stupni III a u jednoho pacienta bylo dosaženo reinervace ve stupni IV dle House a Brackmanna. U všech pacientů se rozvinula pouze lehká hemilinguální atrofie, která u žádného z nich neměla žádné funkční následky ve smyslu poruchy žvýkání, polykání či fonace. Průměrný follow‑up pacientů byl 39,3 měsíce. Závěry: Naše první zkušenosti ukázaly u tří pacientů dobrou reinervaci nervus facialis po hemihypoglossofaciání anastomóze. Metodu považujeme spolu s dalšími variantami hemihypoglossofaciálních spojek (Darrouzet, LeClerc) za efektivní způsob neurotizace plegického lícního nervu při malém riziku vzniku nežádoucích příznaků., Aim: Authors present their early experience with a hemihypoglossofacial anastomosis in the treatment of neurosurgical patients with complete postoperative facial palsy after removal of tumors in the posterior fossa or skull base. Patients and method: In three patients undergoing an open surgery for complete facial palsy between 2009 and 2014, the hemihypoglossofacial anastomosis according to Cushimano and Sekhar was performed. Facial palsy was due to a large vestibular schwannoma in two patients and a facial nerve schwannoma in one patient. Results: Functional reinnervation of grade III according to the House and Brackmann scale was achieved in two patients and reinervation of grade IV in one patient. All patients had mild hemilingual atrophy only that did not lead to any functional disturbances in chewing, swallowing or speech. The mean follow-up period was 39, 3 months. Conclusions: Our early experience showed very good functional reinervation in patients after hemihypoglosofacial anastomosis. We consider this method together with other modifications (Darrouzet, LeClerc) to be very effective in the treatment of facial plegia with low risk of side effects. BACKGROUND: In the cholesteatoma surgery ossicles can be replaced to reconstruct middle ear function. It is important that these ossicles are free of squamous epithelium, to prevent residual disease. This study focuses on the histological findings of the malleus and incus harvested during cholesteatoma surgery. MATERIALS AND METHODS: Eighty middle ears ossicles were examined in vivo and histologically to consider the relationship of cholesteatoma to ossicles, grade of bone destruction and invasion of cholesteatoma to deeper layers of bone. RESULTS: Serious ossicular destruction was observed more frequently in incus compared to malleus (p=0.0065). Difference of ossicles destruction between children and adults was not significant (p=0.3032). Deep invasion of cholesteatoma into the vascular spaces or inner core of the bone was not observed. CONCLUSIONS: Autograft ossicles from cholesteatomatous ears should not necessarily be rejected for reconstruction of the ossicular chain. Sledování expozice látkám, široce užívaným v průmyslu, a vyhodnocení jejich koncentrací v těle člověka, je jedním z hlavních cílů biologického monitorování obyvatel. V pilotní studii byla sledována hladina metabolitů ftalátu dietylhexylftalátu (DEHP), vybraných metabolitů polyaromatických uhlovodíků a bisfenolu A v moči ve vztahu k možným expozičním zdrojům sledovaných dotazníkovým šetřením. Do studie bylo zařazeno 95 studentů nekuřáků ve věkovém rozmezí 20 až 29 let (44 mužů a 51 žen) z Prahy a Ostravy. Analyty byly stanovovány v moči pomocí metody kapalinové chromatografie s tandemovou hmotnostně-spektrometrickou detekcí typu trojitý kvadrupól (LC-MS/MS). Hodnoty metabolitů DEHP u žádného jedince nepřekročily zdravotně významné limitní hodnoty. Za pomoci dotazníku, který byl součástí studie, nebyly zjištěny žádné skutečnosti, které by významně ovlivňovaly hladiny vybraných látek v moči. Byl zjištěn pouze signifikantní vliv pohlaví u 5-OH-MEHP a sumy 5-OH-MEHP a 5-oxo-MEHP, kdy ostravské ženy mají významně vyšší hladiny metabolitů než muži., The main target of human biomonitoring is monitoring of exposure and effects of substances used in industry and determination of their concentrations in the human body. In this pilot study the metabolite levels of phthalates, polycyclic aromatic hydrocarbons and bisphenol A were determined in urine. Their association with possible exposure sources was analyzed. The study comprised 95 adults (44 males and 51 females) aged 20 and 29 years from Prague and Ostrava. The analytes in urine samples were determined by liquid chromatography with triple quadrupole tandem mass spectrometry detection (LC-MS/MS). Metabolite levels did not exceed limit values for these biomarkers for any participant. The questionnaire survey, as part of the study, revealed no factors that might affect levels of the selected substances in urine. Cíl: Sémontův repoziční manévr se v praxi užívá k léčbě pacientů s benigním paroxyzmálním polohovým vertigem zadního polokruhového kanálku. Cílem studie bylo zhodnotit interindividuální a intraindividuální variabilitu při provádění tohoto manévru a definovat parametry, které jeho provedení nejvíce ovlivňují. Metody: Tři zkušení terapeuti provedli manévr oboustranně u 10 zdravých jedinců. Inerciální měřicí jednotka složená z akcelerometru a gyroskopu získávala informace o trajektorii pohybu hlavy v prostoru a její rychlosti. Úhlové změny byly zaznamenány v souřadnicovém referenčním systému. Z odchylek od uvažované roviny polokruhového kanálku byla vyhodnocena data o přesnosti provedení manévru. Výsledky: Parametry ovlivňujícími přesnost manévru jsou výška probanda (p = 0,0252), fáze pohybu (p < 0,0001) a dále terapeut a strana pohybu, přičemž tyto faktory vykazují vliv především ve své interakci. Byl hodnocen vztah fáze pohybu k výšce probandů (p = 0,0130), terapeuta k fázi pohybu (p = 0,0001), terapeuta k výšce probandů (p < 0,0252). Největší zaznamenaný rozsah odchylky od senzorické roviny byl v rozmezí –37,17° až 31° se standardní odchylkou 16,6°. Závěry: Získaná data poukazují na překvapivě vysokou variabilitu při provádění Sémontova manévru ať v porovnání mezi jednotlivými měřeními u jednoho terapeuta nebo v porovnání mezi terapeuty. Analýza vlivu těchto odchylek na úspěšnost terapie benigního polohového paroxyzmálního vertiga a bližší analýza ovlivňujících parametrů by měly být předmětem dalšího výzkumu., Aim: Sémont liberatory manoeuvre is used in the treatment of posterior semicircular canal in patients with benign paroxysmal positional vertigo. The aim of the study was to determine the inter-individual and intra-individual variability in the processing of the manoeuvre and to define the parameters that affect it the most. Methods: Three experienced therapists applied the manoeuvre bilaterally in 10 healthy probands. Inertial measurement unit consisting of the accelerometer and gyroscope recorded the trajectory and speed. Angle changes were placed in the coordinate reference system. Accuracy of the manoeuvre was evaluated based on the deviations from the projected sensory plane. Results: Parameters affecting the accuracy of the manoeuvre included height of a proband (p = 0.0252), phase of the movement (p < 0.0001), therapist and the side of the movement. The effect of these factors is the most pronounced when combined. We assessed interactions of the phase of the movement and the height of the probands (p = 0.0130), the therapist and the phase of movement (p = 0.0001), the therapist and the height of the probands (p < 0.0252). The largest magnitude of deviation from the sensory plane was in the range of –37.17° to 31° with a standard deviation of 16.6°. Conclusions: The data highlight high variability in the implementation of the Sémont liberatory manoeuvre, whether measurements by a single therapist or inter-individually between the therapists are compared. Analysis of the impact of these deviations on therapeutic efficacy in patients with benign paroxysmal positional vertigo and detailed analysis of influencing parameters should be the subject of further research.
