Autor v článku popisuje šest případů emfyzematózní pyelonefritidy. Průměrný věk nemocných byl 63,5 roku, onemocnění se vyskytovalo častěji u mužů a ani v jednom případě nebyl příčinou konkrement. Dvě třetiny nemocných byli diabetici a nejčastěji byla prokázána jako etiologické agens Escherichia coli a v jednom případě Klebsiella pneumonie. Autor uvádí způsoby léčby jednotlivých případů onemocnění. Pouze v jednom případě vedla emfyzematózní pyelonefritida ke smrti pacienta., The author of this communication describes six cases of emphysematous pyelonephritis. The mean age of patients was 63.5, with the disease seen most frequently in men. None of the cases were associated with stone disease. Two thirds of patients had diabetes mellitus. Escherichia coli was confi rmed in the urine most frequently, and in one case, Klebsiella pneumoniae was identifi ed. The author presents the methods of treatment used in each case. Úvod: Klivus je centrální struktura baze lební v těsné blízkosti mozkového kmene a životně důležitých cév. K frakturám klivu dochází většinou při vysokoenergetických úrazech, jednou z nejčastějších komplikací bývá nazální likvorea. Kazuistika: Představujeme případ muže ve středním věku, u kterého se po běžném úrazu hlavy rozvinula nazální likvorea. CT vyšetření ukázalo masivní pneumocefalus a frakturu klivu v oblasti sinus sphenoidalis. Provedli jsme endoskopický endonazální výkon a defekt úspěšně uzavřeli. Diskuze: Pneumatizace sfenoidální dutiny vykazuje velkou interindividuální variabilitu. V našem případě se jedná o extrémně vzdušný sinus v kombinaci s gracilním klivem, což bylo predisponujícím faktorem pro zlomeninu. Traumatická likvorea představuje riziko likvorové hypotenze a vzniku meningitidy. Klasický chirurgický přístup je mikroskopický transnazální. Endoskopie je moderní miniinvazivní variantou. Závěr: Endoskopické endonazální ošetření traumatické likvorey u zlomenin baze lební v oblasti klivu je metodou volby. Klíčová slova: zlomenina baze lební – neurochirurgie – endoskopie, Introduction: Clivus is a central structure of the skull base located in the vicinity of the brainstem and vital brain vessels. Clival fractures are usually caused by a high-energy trauma. Cerebrospinal fluid leak is one of the most common complications. Case report: A middle-aged male sustained a mild head trauma, followed by a nasal cerebrospinal fluid leak. CT scan revealed the massive pneumocephalus and the fracture of the clivus in the posterior wall of the sphenoidal sinus. We performed an endoscopic endonasal surgery to seal the defect. Discussion: Pneumatization of a sphenoidal sinus shows high variability. In the presented case, extreme pneumatization of the sinuses was combined with the gracile clivus, which was the predisposing factor for fracture. Traumatic cerebrospinal fluid leak carries the risk of intracranial hypotension and meningitis. Microscopic transseptal management is the classical surgical approach, while endoscopy provides the modern miniinvasive option. Conclusion: Endoscopic endonasal treatment of traumatic cerebrospinal fluid leak is the treatment of choice for the clival fractures. Key words: skullbase fracture – neurosurgery – endoscopy, and M. Hypotalamický hamartom může způsobovat rozvoj katastrofické a extrémně farmakorezistentní epilepsie s epileptickou encefalopatií. První epileptický záchvat je typicky gelastický, posléze následují další typy záchvatů. Diagnostika malého hypotalamického hamartomu pomocí magnetické rezonance mozku může být obtížná. Neurochirurgická léčba včetně radiochirurgie je vzhledem k uložení komplikovaná s rizikem vedlejších účinků. Odstranění hypotalamického hamartomu však velmi účinně ovlivňuje epileptické záchvaty a brání rozvoji encefalopatie. V léčbě uveálních melanomů se v některých případech používá radiochirurgie CyberKnifem. Histologickými a imunohistologickými změnami v nádorové tkáni po terapii se zabývá poměrně málo autorů. V tomto sdělení uvádíme výsledky vyšetření dvou takových případů. U obou pacientů byla provedena enukleace bulbu pro bolestivý glaukom. U prvého pacienta šlo o recidivující melanom, který byl ošetřen gama nožem před deseti lety. Pro lehkou progresi ložiska melanomu bylo provedeno ošetření CyberKnifem celkovými dávkami 45Gy ve 3 frakcích. Po necelých pěti měsících po ozáření bulbu došlo k regresi recidivujícího nádoru z původní prominence 1,4 mm v ploše 6 mm × 0,6 mm na prominenci 0,3 mm v ploše 3 mm × 0,3 mm. Histologicky byla zaznamenána rezidua řídce celulárního ložiska regresivně změněných nádorových buněk a přítomnost reaktivních změn optického nervu. Druhý pacient byl měsíc po radioterapii CyberKnifem s následnou komplikovanou operací katarakty a s enukleací bulbu bez regrese – tedy zániku nádorových buněk. Velikost tumoru 11,1 mm × 15mm s prominencí 9,3mm přetrvávala i po ozáření dávkou 40Gy/1Fr. V histologickém vyšetření byly nádorové buňky bez patologických změn. V imunohistochemickém vyšetření jsme pozorovali u prvého pacienta vysokou difuzní expresi proteinu bcl-2 až do výše 75 % a méně než 1% pozitivitu ve vyšetření s proliferačním markerem Ki-67. U druhého případu byla situace odlišná, s výraznou až 75% pozitivní expresí bcl-2 proteinu v nádorových buňkách v povrchových vrstvách tumoru a s klesajícím počtem pozitivní exprese v hlubokých vrstvách nádoru, kde dosahovala maxima do 25 %. Proliferační aktivita s pozitivní jadernou expresí v Ki-67 vzrůstala od povrchu, kde se pohybovala v rozmezí 1–3 % do hloubky nádoru a k jeho bázi, kde dosahovala až 5 %. Naše pozorování ukazuje na výraznou redistribuci genetické aktivity uvnitř nádoru. Ta se projevuje „overexpresí“ bcl-2 proteinu a u druhého případu je patrná rozdílná pozitivita bcl-2 proteinu v různých vrstvách nádoru. Vztah tohoto fenomenu k radioterapii není dosud jasný., Radiosurgery with CyberKnife is used in the treatment of uveal melanomas in some cases. Histological and immunohistological changes in tumour tissue following treatment have been addressed by relatively few authors. This paper presents the results of investigations of two such cases. Enucleation of the eye was performed for painful glaucoma in both patients. Patient 1 had a recurrent melanoma treated with a Gamma Knife ten years ago. Due to slight progression in the melanoma site, treatment with CyberKnife was used with total doses of 45 Gy in 3 fractions. Some five months following eye irradiation, there was regression of the recurrent tumour from the original height of 1.4 mm in an area of 6 mm x 0.6 mm to that of 0.3 mm in an area of 3 mm x 0.3 mm. Histologically, residua of a sparsely cellular locus of regressively altered tumour cells and the presence of reactive changes in the optic nerve were noted. Patient 2 underwent radiotherapy with CyberKnife one month prior, with subsequent complicated cataract surgery and enucleation of the eye without regression, i.e. destruction of tumour cells. The size of the tumour of 11.1 mm x 15 mm with a height of 9.3 mm remained unchanged even after irradiation with a dose of 40 Gy/1 fr. No pathological changes in the tumour cells were seen on histological examination. In patient 1, immunohistochemistry showed high diffusion expression of the bcl-2 protein of up to 75 % and less than 1 % positivity on examination with the proliferation marker Ki-67. In patient 2, the situation was different, with significant, as high as 75 % positive expression of the bcl-2 protein in tumour cells in the superficial tumour layers and a decreasing rate of positive expression in the deep tumour layers where it reached no more than 25 %. The proliferation activity with positive nuclear expression of Ki-67 increased from the surface, where it ranged from 1 % to 3 %, to the depth of the tumour and toward its base, where it reached up to 5 %. Our observation suggests significant redistribution of genetic activity within the tumour. It is manifested by overexpression of the bcl-2 protein and, in patient 2, different bcl-2 protein positivity in the various tumour layers is apparent. We report the case of a 52year old man with severe acute pancreatitis. In this case report we discuss the undesirable effects of parenteral nutrition and the importance of nutritional support at patients with severe necrotizing pancreatitis. Severe acute pancreatitis is usually accompanied by systemic inflammatory response syndrome, which results in hypermetabolism with prominent protein catabolism. Providing nutrition to these patients is of paramount importance. An adequate nutritional support is crucial in patients with severe and complicated pancreatitis. Kalcifikující uremická arteriolopatie, neboli kalcifylaxe, je raritní onemocnění vyznačující se systémovou kalcifikací médie arteriol vedoucí k ischemii a podkožním nekrózám. Nejčastěji se vyskytuje u pacientů s konečným selháním ledvin v chronickém hemodialyzačním programu (incidence se udává 1–4 %) a po transplantaci ledviny. Kalcifylaxe je zrádná svou rychlou progresí v nekrózu tkáně, obtížným hojením a vysokým rizikem sekundární infekce, která je nejčastější příčinou úmrtí při tomto onemocnění. Úmrtnost se udává v rozmezí 60–80 % v průběhu několika měsíců od stanovení diagnózy. Základem léčebného úspěchu je včasné stanovení diagnózy a správně a trpělivě vedená léčba. Na příkladu 3 pacientů zařazených v našem chronickém hemodialyzačním programu demonstrujeme typické klinické projevy a léčbu kožních defektů při kalcifylaxi, jež vyžaduje těsnou spolupráci pacienta a ošetřujícího personálu. Základem léčby u všech našich pacientů byla korekce kalciofosfátového metabolizmu a sekundární hyperparatyreózy. Na konci dialýz byl všem aplikován tiosulfát sodný. Další důležitou součástí byla péče o ránu, citlivý debridement a intenzivní lokální péče. Po 5–6 měsících došlo k úplnému zhojení kožních defektů u první pacientky, výraznému zlepšení v případě druhého pacienta a progresi u třetí pacientky. Nebyly pozorovány žádné nežádoucí účinky aplikace tiosulfátu sodného., Calcific uremic arteriolopathy or calciphylaxis is a rare disorder characterized by systemic medial calcification of arterioles that leads to ischemia and subcutaneous necrosis. It most commonly occurs in patients with end-stage renal disease who are on haemodialysis or who have received a renal transplant. Calciphylaxis is dangerous by its fast progression into tissue necrosis, difficult healing process and a great risk of secondary infection which is the most common cause of death in this condition. The reported mortality rates are as high as 60–80 % in a couple of months once it is diagnosed. The key to successful treatment of calciphylaxis is fast diagnosing of the disease and appropriate treatment management. On the examples of three patients from our haemodialysis centre we demonstrate typical clinical manifestation of calciphylaxis and its treatment, which requires close patient-medical staff cooperation. The basic principle of treatment of all our patients was normalization of calcium-phosphate metabolism and secondary hyperparathyroidism. Sodium thiosulfate had been administered to all patients at the end of haemodialysis session. The wound care played another major role with gentle debridement and intensive local care. After five to six months the skin defects resolved in the first patient, partially resolved in the second patient and deteriorated in the third patient. Nádory hlavy a krku jsou celosvětově 6. nejčastějším zhoubným nádorem. Histologicky se ve většině případů jedná o spinocelulární karcinomy s lokalizací dutiny ústní, orofaryngu, laryngu, hypofaryngu a dutiny nosní. Hlavním rizikovým faktorem je kouření a alkohol. Nově se diskutuje souvislost s infekcí lidského papilomaviru (HPV 16), zejména v oblasti orofaryngu. Pacienti mající infekci HPV 16 mají 15–50× vyšší riziko pro vznik karcinomu. Dle výsledků zkoumání se jedná o rozdílnou formu karcinomu s výskytem u mladších pacientů, lepšího sociálního stavu, nejčastěji v oblasti orofaryngu a báze jazyka, s pokročilým postižením uzlin, s lepší prognózou proti HPV negativním tumorům. Základem léčby je chirurgie a radioterapie, chemoterapie a nově cílená léčba. Ve většině případů (nad 50 %) se jedná o lokálně pokročilé onemocnění zasahující oblast polykání, mluvení – výrazně omezující denní aktivity. Chemoradioterapie s cisplatinou je standardem pro pacienty s pokročilým neresekabilním karcinomem hlavy a krku. Ve studiích fáze III byla srovnávána samostatná radioterapie proti přidání cetuximabu k radioterapii. Výsledky odpovědi pro kombinaci s cetuximabem 74 % vs. 64 %, s mediánem přežití 49 vs. 29,3 měs. (Bonner, 2006). Cetuximab je monoklonální protilátka třídy IgG1, blokující vazbu endogenních EGFR ligand, což vede k inhibici funkce receptoru. EGFR je exprimován u nádorů epiteliálního původu. U nádorů hlavy a krku se vyskytuje téměř u všech – nad 90 %., Worldwide, head and neck tumours are the sixth leading malignant tumour. Histologically, the majority of cases are squamous cell carcinomas of the oral cavity, the oropharynx, the larynx, the hypopharynx, and the nasal cavity. Smoking and alcohol are the major risk factors. Recently, an association with the human papillomavirus (HPV 16) infection, particularly in the area of the oropharynx, has been discussed. Patients with the HPV 16 infection have a 15–50-fold higher risk of developing carcinoma. According to research results, it is a distinct form of carcinoma occurring in younger patients, of a higher social status, most frequently in the area of the oropharynx and the base of tongue, with advanced lymph involvement, and with a better prognosis compared to HPV-negative tumours. The mainstay of treatment is surgery and radiotherapy, chemotherapy, and, more recently, targeted therapy. In most cases (over 50%), there is locally advanced disease involving the area of swallowing and speaking, thus significantly limiting daily activities. Chemoradiotherapy with cisplatin is the standard treatment for patients with advanced unresectable head and neck cancer. Radiotherapy alone versus adding cetuximab to radiotherapy was compared in phase III trials. The response results for the combination with cetuximab were 74% vs. 64%, with a median survival of 49 vs. 29.3 months (Bonner 2006). Cetuximab is a monoclonal IgG1 antibody that blocks the binding of endogenous EGFR ligands, resulting in inhibition of receptor function. EGFR is expressed in tumours of epithelial origin. Jedinou naději na dlouhodobé přežití pacientů s kolorektálním karcinomem metastazujícím do jater je radikální chirurgická resekce jaterních metastáz. Systémová léčba zvyšuje pravděpodobnost úspěšného chirurgického odstránění jaterních metastáz. Cílem systémové léčby je dosažení co nějvětší léčebné odpovědi. V současné době se využívá kombinace systémové chemoterapie s cílenou léčbou. Kazuistika prezentuje pacienta, u kterého po chemoterapii FOLFOX v kombinaci s cetuximabem došlo ke kompletní klinické remisi onemocnění. The probability of the radical resection could be achieved by the aplication of the systemic treatment. The main of systemic treatment is to achieve the greatest possible overall response rate. The combination of chemotherapy and targeted treatment is a standard now. Our case presents the patient with metastatic colorectal cancer with the complete response in liver metastasis after FOLFOX regimen with cetuximab.
