Dyskinetické extrapyramidové syndromy mohou vzácně vést k akutnímu zhoršení do život ohrožujícího stavu, který si vyžádá hospitalizaci na jednotce intenzivní péče, hlubokou analgosedaci a umělou plicní ventilaci. V případě neúspěšnosti symptomatické farmakologické léčby lze uvažovat o stereotaktickém neurochirurgickém výkonu (pallidotomii nebo hluboké mozkové stimulaci). V příspěvku uvádíme naše dosavadní zkušenosti se třemi pacienty, kteří v akutní fázi dyskinetického syndromu takový výkon podstoupili. U dvou dětských pacientů (12 a 6 let) pro status dystonicus s geneticky verifikovanou hereditární generalizovanou dystonií (DYT‑6 a DYT‑1) byla provedena hluboká mozková stimulace vnitřního pallida a u jedné pacientky (21 let) s Huntingtonovou nemocí byla provedena pallidotomie pro akutní zhoršení generalizované chorey s rozvojem balizmu. Přes počáteční kritický stav došlo u všech tří pacientů v odstupu 1–6 týdnů od výkonu k významnému potlačení mimovolných pohybů. Stereotaktická neurochirurgická léčba by proto měla být zvažována jako metoda volby v terapii akutních, život ohrožujících hyperkinetických stavů, kde farmakologická terapie selhala., Dyskinetic movement disorders may rarely progress to an acute life-threatening condition requiring admission to an intensive care unit, deep sedation and mechanical ventilation. Stereotactic neurosurgery (pallidotomy or deep brain stimulation) should be considered in all patients who did not respond to pharmacological treatment. We report three patients who underwent stereotactic neurosurgery due to acute dyskinetic syndrome. Two boys (12 and 6 years) with generalized hereditary dystonia (DYT-6 and DYT-1) developed status dystonicus and were treated with deep brain stimulation of the globus pallidus interna. The third patient was a 21-years-old woman with Huntington’s disease who underwent a pallidotomy due to acute worsening of generalized chorea with ballistic movements. Despite the initial critical state of all three patients, their abnormal movements significantly subsided within six weeks after the neurosurgery. The stereotactic neurosurgical procedure should be considered as the method of choice in patients with acute life-threatening hyperkinetic movement disorders that failed to improve with pharmacological treatment. Cíl: Podle literatury dosahuje četnost komplikací kranioplastiky po dekompresivní kraniektomii i více než 30 %. Cílem studie je hodnocení četnosti, příčin a léčby komplikací kranioplastik po dekompresivní kraniektomii pro expanzivně se chovající supratentoriální ischemii nebo kraniocerebrální poranění a analýza důvodů neprovedení kranioplastiky u části nemocných po dekompresivní kraniektomii. Soubor a metodika: V retrospektivní studii nemocných po dekompresivní kraniektomii (2006–2014) byla kranioplastika provedena u 25 z 35 nemocných s expanzivní malacií (71,4 %) a u 33 z 68 zraněných s poúrazovou nitrolební hypertenzí (48,5 %). Výsledky: Nejčastější příčinou neprovedení kranioplastiky bylo úmrtí. U 20 % nemocných s mozkovou ischemií a 61,3 % zraněných byla kranioplastika provedena časně (do 10 týdnů). Četnost neurochirurgických komplikací byla u kranioplastik u nemocných s expanzivní malacií 12 % (explantace plastiky u 8 %) a u kranioplastik po kraniocerebrálním poranění dosahovala 9,1 % (explantace plastiky u 3 %). Hydrocefalus a subdurální kolekce byly před kranioplastikou přítomny u 6 a 9,1 % zraněných. Četnost hydrocefalu po kranioplastice byla u kraniotraumat 6 % a u mozkové ischemie 4 %. Závěry: Četnost komplikací kranioplastik, byť nižší než uvádí literatura, převyšuje hodnoty očekávané u elektivních operací. Subdurální kolekce a hydrocefalus nelze jednoznačně hodnotit jako komplikaci kranioplastiky, protože mohou být přítomny již před kranioplastikou v důsledku základního onemocnění., Aim: Reported incidence of cranioplasty complications after decompressive craniectomy may exceed 30%. The aim of the study was to analyse the incidence, causes and treatment options for neurosurgical complications of cranioplasty after decompressive craniectomy for expansive supratentorial ischaemic stroke and craniocerebral injury and to analyse the reasons for not performing cranioplasty in a subgroup of patients after decompressive craniectomy. Patients and methods: During a retrospective study (2006–2014) cranioplasty was performed in 25 of 35 patients after decompressive craniectomy for expansive supratentorial ischaemia (71.4%) and in 33 of 68 (48.5%) patients after decompressive craniectomy for posttraumatic intracranial hypertension. Results: Death was the most frequent reason for not performing cranioplasty after decompressive craniectomy. The frequency of early cranioplasties (< 10 weeks after decompressive craniectomy) was 20% in patients with ischaemic stroke and 61.3% in head-injured patients. The incidence of neurosurgical complications was 12% in patients after expansive brain ischaemia (cranioplasty explantation rate 8%). The incidence of cranioplasty complications in patients after craniocerebral injury was 9.1% (cranioplasty explantation rate 3%). Subdural collections and hydrocephalus were observed in 9.1% and 6% of patients after craniocerebral injury before cranioplasty, resp. Hydrocephalus requiring neurosurgical treatment was found in 6% of patients after craniocerebral injury and 4% of patients with brain ischaemia during the postcranioplasty period. Conclusions: Although the incidence of cranioplasty complications after decompressive craniectomy is lower than that reported in literature, it exceeds the rates expected for elective surgeries. Subdural collections and hydrocephalus cannot be unequivocally considered a complication of cranioplasty, because they are frequently observed before cranioplasty and may be caused by the disease requiring decompressive craniectomy. Cíl: Autoři prezentují výsledky operační léčby tumorů periferních nervů. Operační výkony byly provedeny na Neurochirurgické klinice LF MU a FN Brno v letech 2003–2013. Ve sdělení je prezentován současný přehled histologické klasifikace a její vliv na možnosti chirurgické léčby. Součástí práce je i kazuistika dokumentující operační postup u raritního objemného tumoru n. ischiadicus v hloubce hýžďové krajiny. Soubor a metodika: Operační revizi podstoupilo celkem 24 pacientů, z toho 13 mužů a 11 žen. Průměrný věk pacientů v souboru byl 46,5 roku (min. 22, max. 74 let). Výkony byly provedeny standardní mikrochirurgickou operační technikou, od roku 2007 s užitím peroperační elektrofyziologické monitorace. Retrospektivní analýzou je soubor definován dle postiženého nervu, rozsahu resekce a histologické klasifikace. Je zhodnocen výsledný pooperační neurologický stav. Výsledky: Nejčastěji postiženým periferním nervem byl n. tibialis. V souladu s literárními zdroji se histologicky nejčastěji jednalo o benigní schwannom, a to celkem ve 20 případech. Jednou šlo o neurofibrom, jednou o intraneurální ganglionovou cystu, jedenkrát byla histologie uzavřena jako raritní extraoseální Ewingův sarkom a u jednoho pacienta byl tumor způsoben chronickým zánětlivým procesem. Závěr: V našem souboru bylo dosaženo totální resekce u všech operovaných pacientů, kromě jednoho pacienta. U něj byla provedena pouze revize s otevřenou biopsií. U pacienta s maligním Ewingovým sarkomem byla nutná kompletní rozšířená resekce postižených nervů s příslušným totálním motorickým a senzitivním deficitem. Ve dvou dalších případech došlo po operaci ke zhoršení neurologického stavu, jednou se jednalo o lehkou parézu, jednou o trvalou hypestezii. V ostatních případech klinické obtíže prakticky vymizely., Aim: The authors present surgical treatment results of peripheral nerve tumors. All the surgical procedures were performed between 2003 and 2013 at the Department of Neurosurgery, The University Hospital Brno. The paper provides an overview of current histological classification and its impact on surgical treatment. The paper also includes a case report describing a surgical procedure used for a rare, bulky tumor of the sciatic nerve in the depth of the gluteal region. Methods: In total, 24 patients, 13 men and 11 women, underwent surgical revision. The mean age of the group was 46.5 years (min. 22, max. 74 years). All the surgical procedures were completed using standard microsurgical tools. Perioperative electrophysiological monitoring has been used since 2007. A retrospective analysis according to the localization of the affected nerve, extent of resection and histological classification of the tumor was performed and the final outcome in terms of neurological status was evaluated in the study group. Results: A tumor of the tibial nerve was the most frequent target of the surgery in our study group. Benign schwannoma was the most common tumor in our study group (20 cases) and this is in accordance with previously published data. Other tumors were classified as neurofibroma (one case) and intraneural ganglion cyst (one case). Histology confirmed the presence of a rare extraosseous Ewing’s sarcoma in one case. Finally, one patient suffered from chronic inflammation. Conclusion: In our study group, total resection was performed in all patients except one. Revision with open biopsy was performed in a patient with nerve inflammation. Complete extended resection of the affected nerves was necessary in a patient with malignant Ewing’s sarcoma. This resulted in total motor and sensitive deficit of the corresponding area. Neurological status has also worsened after the surgery in two other cases, one had light paresis and the other had permanent hypoesthesia. The clinical symptoms disappeared after surgery in all other cases. Dermoidální cysta s dermálním sinusem v oblasti zadní jámy lební je málo častým mozkovým nádorem. Autoři uvádí kazuistiku sedmiměsíční pacientky léčené pro dermoidální cystu s dermálním sinusem v oblasti zadní jámy lební spolu s mozečkovým abscesem, detekovaným CT a MR vyšetřením. Pacientka byla operována den po zavedení zevní komorové drenáže; byla provedena totální excise dermoidální cysty a mozečkového abscesu. Včasná diagnóza a správný postup ošetření těchto benigních mozkových tumorů mohou zabránit závažným komplikacím, jak je dále rozvedeno v diskuzi. Authors recently treated a 7-<span class=""></span>month‑<span class=""></span>old girl who suffered from a posterior fossa dermoid cyst with dermal sinus. In addition, cerebellar abscess in the left cerebellar hemisphere was detected by CT and MRI. Surgery was performed the day after external ventricular drainage insertion, and the dermoid cyst and cerebellar abscess were fully excised. Po dva roky trvajících bolestech bederní páteře s narůstajícími klaudikacemi dolních končetin byl u 61leté ženy diagnostikován rozsáhlý tumor v oblasti těla L3 a retroperitonea. Nádor byl totálně odstraněn a provedena náhrada těla L3 a stabilizace. Po operaci přetrvávaly výrazné bolesti, oslabení dolních končetin a instabilita stoje a chůze. Po léčebné rehabilitaci došlo k vymizení bolesti, ústupu nestability i parézy levé dolní končetiny. Vývoj pooperačního stavu dokumentuje stabilometrické vyšetření., After 2-year period of persistent pain of lumbar spine with increasing intensity of lower extremities claudication a vast tumor of L3 vertebra extending to the retroperitoneum was diagnosed in a 61-year old woman. The total L3 vertebrectomy as well as complete tumor removal has been performed and situation was reconstructed and stabilized. Pronounced pain, lower extremities paresis and instability persisted after the surgery. The rehabilitation therapy led to the relief of the pain improvement of standing and gait instability and paresis. Periferní primitivní neuroektodermální tumor (pPNET) je vysoce maligní sarkom měkkých tkání vycházející pravděpodobně z primitivních buněk zárodečné neurální lišty. Pro některé společné cytologické, imunohistochemické a cytogenetické rysy s kostním Ewingovým sarkomem bývá také někdy nepřesně označován jako Ewingův sarkom měkkých tkání, kdy se může jednat o různou morfologickou expresi stejného nádorového typu. Objevuje se převážně u mladých nemocných v různých lokalizacích, vždy v intimním spojení s periferním nervem. Jeho výskyt v průběhu spinálního kořene je velmi vzácný a literární výčet je v tomto případě omezen pouze na několik kazuistických sdělení. V naší práci představujeme případ 35leté pacientky, u které byl odhalen pPNET v oblasti extraforminálního výstupu kořene L5. Díky časné diagnostice a léčbě zahrnující radikální resekci v kombinaci s agresivní chemoterapií a ozářením lůžka nádoru nebyla dosud zaznamenána recidiva nádorového růstu, přičemž doba sledování činí více než pět let., Peripheral primitive neuroectodermal tumor (pPNET) is a highly malignant soft tissue sarcoma probably originating from embryonic neural crest cells. For some common cytological, immunohistochemical and cytogenetic features with bone Ewing’s sarcoma, pPNET is also sometimes inaccurately referred to as Ewing’s sarcoma of soft tissues. pPNET probably is a different morphological expression of the same tumor type. pPNET mainly occurs in younger patients in various locations, always in close proximity to a peripheral nerve. However, its occurrence within a spinal nerve root is very rare and, so far, only a few cases have been described in the literature. In our report, we present a case of 35-year-old woman with pPNET in extraforaminal projection of L5 root. As a result of early diagnosis and treatment, including radical surgery combined with aggressive chemotherapy and irradiation of the tumor bed, no recurrence has been reported so far. The follow-up period is more than five years.
