Psoriasis patients are often susceptible to cardiovascular diseases (CVD), including atherosclerosis. Traditional markers (biochemical and inflammatory) and diagnostic tools could detect occlusive but not subclinical atherosclerosis. Carotid intima-media thickness (CIMT), has recently been recognised as a non invasive diagnostic tool for identification of premature atherosclerosis. Therefore we evaluated 80 psoriasis patients and 80 age sex matched healthy controls for serum leptin levels and apolipoprotein B/apolipoprotein A-I ratio (apoB/apoA-I ratio) in relation with CIMT of carotid artery. Carotid intima-media thickness and carotid plaques were simultaneously measured by carotid sonography. Serum concentration of leptin and apolipoprotein were measured using enzyme-linked immuno sorbent assay (ELISA) and nephelometry respectively. Raised CIMT correlated to age of onset of the disease, serum leptin and apoB/apoA-I ratio in psoriasis patients. Taking into account, values that were above the 75 percentile of the three markers (leptin, apoB/apoA-I ratio and CIMT) the odds ratio was 4.26 (2.06-8.80 CI). Leptin and apoB/apoA-I ratio showed significant cumulative association with CIMT. Results of predictive analysis supports measurement of CIMT along with estimation of serum leptin and apoB/apoA-I ratio for prediction of premature atherosclerosis in psoriasis patients. and K. Cíl: Naším záměrem bylo zjistit diagnostické charakteristiky Montrealského kognitivního testu (MoCA) v tréninkové podobě pro lepší administraci u pacientů s Alzheimerovou nemocí (AN) poprvé v českém písemnictví. <p align="">Soubor a metodika: Záznamový formulář jsme obohatili o instrukce pro vyšetřovaného, instrukce k administraci a návod ke skórování. Obsah a skórování zůstaly zachovány a odpovídají předchozí české verzi. Upravili jsme návod k administraci. Zpětný překlad záznamového archu a návodu do angličtiny byl zkontrolován, schválen a nazván kanadským tvůrcem jako tzv. tréninková verze MoCA‑CZ1. Tímto testem jsme vyšetřili 30 pacientů s již diagnostikovanou mírnou demencí způsobenou Alzheimerovou nemocí podle nových doporučení NIA‑AA (78 ? 6 let, 13 ? 3 let vzdělání, 56 % žen) a 30 kognitivně normálních seniorů (NOS) podobného věku, vzdělání a pohlaví. <p align="">Výsledky: Oproti standardní verzi obsahují záznamové archy tréninkové české podoby MoCA‑CZ1 navíc další informace, s nimiž předpokládáme snadnější a spolehlivější provádění a vyhodnocování testu. Celkové skóry MoCA‑CZ1 byly významně nižší u pacientů s mírnou demencí způsobenou AN (18 ? 5 bodů) než u NOS (26 ? 3 body) (p < 0,00001). Celkové skóry NOS nezávisely na věku, pohlaví a vzdělání. Při porovnávání obou skupin podle analýzy receiver operating curve byl zjištěn optimální hraniční skór ≤ 23 bodů se senzitivitou 87 % a specificitou 84 %. Plocha pod křivkou byla 0,92. <p align="">Závěry: Tréninková MoCA‑CZ1 je volně dostupná na http://www.pcp.lf3.cuni.cz/adcentrum/testy.html. Výsledek testu může přispět k dřívějšímu záchytu mírné demence způsobené AN, a to časnou detekcí kognitivního deficitu. Při používání tréninkové verze MoCA‑CZ1 doporučujeme používat hraniční skór ≤ 23 bodů., Aim: Our aim was to determine diagnostic characteristics of the Montreal cognitive assessment (MoCA) using its training version for better administration in patients with Alzheimer’s disease (AD) for the first time in Czech literature. <p align="">Patients and methods: We complemented the test form with instructions for the patient, and with administration and scoring instructions. The content and the scoring were preserved from the previous Czech version. A separate Czech administration instruction manual was refined. English back translation of the data collection form and the manual was checked and approved by the Canadian author who also named it the training version. We examined 30 patients previously diagnosed with mild dementia due to Alzheimer’s disease according to the new NIA‑AA recommendations (78 ? 6 years, 13 ? 3 years of education, 56% women) and 30 cognitively normal seniors (NOS) of similar age, education and gender by MoCA‑CZ1. <p align="">Results: The training version of MoCA‑CZ1 contains additional information. We assume that this ensures reliability, and facilitates administration and scoring of the test. Total MoCA‑CZ1 scores were significantly lower in patients with AD (18 ? 5 points) than in NOS (26 ? 3 points) (p < 0.00001). Total scores of NOS did not correlate with age, gender or education. Analysis of receiver operating curve comparing the two groups found optimal cut off score of ≤ 23 points with sensitivity of 87% and specificity of 84%. BACKGROUND: The goal of this prospective study was to determine the frequency of micrometastases in patients with squamous cell carcinoma (SCC) of the oral cavity, pharynx and larynx in whom elective neck dissection was indicated (cN0). PATIENTS AND METHODS: A total of 12 patients (10 males and 2 females) were enrolled in the study. The age ranged 42-73 years (median 62 years). Elective neck dissection was performed in all patients (8 ipsilateral, 4 bilateral) and a total of 256 lymph nodes were removed and sent for microscopic examination. RESULTS: The presence of tumor cells in cervical lymph nodes was found in 5/12 (42%) patients. Micrometastases of SCC were found in two patients and isolated tumor cells (ITC) in two other patients. In the remaining one patient with oropharyngeal SCC, a micrometastasis of papillary thyroid carcinoma (PTC) was detected. Positive lymph nodes were localized in level II in three patients with SCC of larynx, hypopharynx and tongue base, respectively, in level I in one patient with SCC of oral tongue and in level III in one patient with PTC. CONCLUSION: Our results indicate that SCC of head and neck has a high potential for creating micrometastases which frequency is higher compared to clinically detected macrometastases. Úvod: Dynamické metody kvantitativního testování senzitivity (Quantitative Sensory Testing; QST) jsou relativně novou nadstavbovou aplikací QST umožňující objektivizovat funkci centrálních modulačních mechanizmů algické percepce, jejichž individuální nastavení může přispívat k rozvoji chronických bolestivých stavů. Testování těchto mechanizmů umožňuje např. predikci rozvoje pooperační bolesti. K nejčastěji hodnoceným dynamickým QST metodám patří tzv. podmíněná modulace bolesti (Conditioned Pain Modulation; CPM) a časová sumace percepce bolestivých podnětů (Temporal Summation; TS). Soubor a metodika: Vyšetření dQST (dynamic methods of Quantitative Sensory Testing) metod s využitím termoalgických podnětů a modulace bolestivým chladem bylo provedeno u 77 zdravých dobrovolníků (47 žen testovaných v ovulační fázi menstruačního cyklu, 30 mužů, věk 19–73 let). Hodnocen byl také vliv věku a pohlaví na výsledky obou dQST testů. Výsledky: Uspokojivá funkce podmíněné modulace bolesti byla prokázána u 84 % testovaných dobrovolníků. Časová sumace bolestivých podnětů byla zachycena u 92, resp. 98 % jedinců v závislosti na použité teplotě podnětů. Nefunkčnost mechanizmů u zbylé části souboru byla ve většině případů podmíněna nesprávně nastavenými technickými parametry testu. U žen testovaných v ovulační fázi menstruačního cyklu jsme prokázali signifikantně vyšší efektivitu CPM mechanizmu ve srovnání s muži. Funkce TS se mezi oběma pohlavími významně nelišila. Významný vliv věku na efektivitu CPM ani TS mechanizmu nebyl ve sledovaném souboru prokázán. Závěr: Provedená studie ověřila použitelnost zvolených algoritmů dynamického testování senzitivity a prokázala antinociceptivní nastavení žen v ovulační fázi menstruačního cyklu ve srovnání s muži, zatímco významný vliv věku na funkci centrálních modulačních mechanizmů algické percepce nebyl zachycen., Background: So called “dynamic quantitative sensory methods” (dQST) represent relatively new QST application which enables to objectify certain mechanisms of pain processing, e.g. central integration or descending control. Their dysfunction may contribute to the development of chronic painful conditions. dQST methods proved the potential to predict the development of chronic postoperative pain. Conditioned pain modulation (CPM) and temporal summation (TS) are the most commonly used dQST tests. Patients and methods: The CPM and TS testing was performed in a group of 77 healthy volunteers (47 women tested in the ovulatory phase of the menstrual cycle, 30 men, age range 19–73) using thermal pain stimuli. Efficiency of CPM and TS mechanism including age and gender influence was evaluated. Results: Efficient CPM was found in 84% of healthy volunteers. In remaining 16%, the inefficiency was mainly due to low intensity of testing or modulatory stimuli. Based on the temperature used, efficient TS mechanism was found in 92% or 98% of healthy individuals. Women in the ovulatory phase of the menstrual cycle showed significantly more efficient CPM comparing to men. No sex differences of TS testing were found. Age had no significant effect on the CPM or TS mechanism in our group of healthy voluteers. Conclusions: Our study has verified the applicability of CPM and TS algorithms used. Women in ovulatory phase of the menstrual cycle show increased anti-nociceptive setting comparing to men. No significant effect of age on central modulation of pain perception has been found in pain-free healthy individuals. Objective: The study aimed to evaluate plasma adiponectin (ADP), adipocyte fatty acid binding protein (A-FABP) and fibroblast growth factor 21 (FGF 21) levels as potential predictors of severity of acute pancreatitis (AP) in day 4 after admission. Simultaneously, the classical proinflammatory makers were analysed as well. Settings: Department of Clinical Biochemistry and 2nd Department of Internal Medicine, Faculty Hospital Olomouc, I. P. Study design: The study was conducted in subjects with acute pancreatitis (n = 84, 37 females, 47 males). The analyses were performed in the groups according to the mild/severe classification of AP, and partly in the computed-tomography severity index (CTSI) score subgroups. Results: From adipokines, only FGF 21 tended to be higher in the severe AP subgroup in day 4. The multiple regression analysis revealed a positive association of A-FABP with procalcitonin (PCT), whilst FGF 21 was associated positively with ADP and negatively with C-reactive protein (CRP) in patients with the severe AP. The receiver-operator characteristics (ROC) analysis confirmed that not adipokines, but only CRP and interleukin 6 (IL-6) were suitable as potential predictors of the disease severity. The cut-off values were established for both parameters: 100 mg/L for CRP and 37 ng/L for IL-6, with negative predictive values (NPV) 96 % and 92 %, and positive predictive values (PPV) 39 % and 29 %, respectively. Výsledky: Ze sledovaných adipokinů měl u pacientů s těžkou AP pouze FGF 21 tendenci k vyšším hladinám v den 4 po přijetí. Za pomoci mnohočetné regresní analýzy byla zjištěna pozitivní asociace A-FABP s prokalcitoninem (PCT), zatímco FGF 21 asocioval u těžké AP pozitivně s ADP a negativně s C-reaktivním proteinem (CRP). ROC (receiver-operator characteristics) analýza potvrdila, že pouze CRP a interleukin 6 (IL-6) jsou využitelné jako potenciální prediktory závažnosti onemocnění. Pro oba parametry byly stanoveny cut-off hodnoty: 100 mg/l pro CRP a 37 ng/l pro IL-6, s negativními prediktivními hodnotami (NPV) 96 % a 92 %, a s pozitivními prediktivními hodnotami (PPV) 39 % a 29 %. Úvod: Komplexní protokol kvantitativního testování senzitivity (QST), zahrnující hodnocení široké škály modalit termické, algické, taktilní a vibrační cítivosti, je často využívaným nástrojem pro kvantifikaci senzitivity a hodnocení tzv. senzitivního profilu, zejména u pacientů s neuropatickou bolestí. Protokol obsahuje složité slovní instrukce, které je pro dosažení standardizovaného výstupu nutné přesně následovat, což implikuje nutnost validace jeho jazykových mutací. Soubor a metodika: Pomocí české verze standardizovaných slovních instrukcí ke komplexnímu QST protokolu bylo vyšetřeno 75 zdravých dobrovolníků, 68 pacientů s periferní neuropatickou bolestí při diabetické neuropatii a 59 jedinců s centrální neuropatickou bolestí při roztroušené skleróze, a to vždy na HKK i DKK. Pro hodnocení spolehlivosti bylo u 26 jedinců testování provedeno opakovaně (4×) s přibližně 30denními rozestupy mezi jednotlivými vyšetřeními. Výsledky: Vyšetření pomocí české verze komplexního QST protokolu bylo pro pacienty nenáročné či málo náročné. V souboru zdravých dobrovolníků spadalo 94 % zjištěných hodnot jednotlivých senzitivních parametrů na HKK a 95 % na DKK do rozsahu fyziologických nálezů podle publikovaných normativních dat. Alespoň jedna abnormita z hodnocených senzitivních modalit byla zachycena u 90 % pacientů s neuropatickou bolestí. Shluková analýza umožnila identifikaci dvou symptomových profilů, z nichž jeden většinově odpovídal bolestivé diabetické neuropatii a druhý centrální bolesti při roztroušené skleróze. Reliabilita jednotlivých modalit hodnocená pomocí Cronbachova alfa se pohybovala v rozmezí 0,593–0,926. Závěr: Česká jazyková verze komplexního QST protokolu prokázala velmi dobrou praktickou použitelnost pro hodnocení širokého spektra senzitivních abnormit a nízkou subjektivní náročnost vyšetření. Jedná se o spolehlivý test s vysokou vnitřní konzistencí a dobrou opakovatelností všech hodnocených parametrů., Background: Quantitative sensory testing (QST) following the protocol of the German Research Network on Neuropathic Pain (DFNS) is frequently used for the evaluation of sensory profi les in neuropathic pain patients. It covers a broad spectrum of sensory modalities using both nociceptive and non-nociceptive thermal and mechanical stimuli. The protocol requires the use of standardized instructions and thus necessitates validation of each language version. Material and methods: The standardized instructions for DFNS-QST protocol were translated using the forward-backward translation method and validated in a group of patients suffering from neuropathic pain of peripheral (n = 68) or central origin (n = 59) due to painful diabetic polyneuropathy or multiple sclerosis, and in a group of healthy volunteers (n = 75). The testing was repeated four times (with one-month interval between examinations) in 26 individuals to determine test-retest reliability. Results: In a group of healthy volunteers, 95% of values across all the parameters were within the physiological range based on published normal data. At least one sensory abnormity was found in 90% of neuropathic pain patients. Cluster analysis identified two distinctive pain profiles, the first mostly consisted of patients with painful diabetic neuropathy and the second of patients with pain in multiple sclerosis. Repeated administration confirmed an acceptable test-retest reliability (Cronbach´s alpha of 0.59–0.93 for the different sensory modalities). Conclusion: The Czech version of DFNS-QST protocol instructions displayed a good practical applicability in evaluating sensory abnormities in neuropathic pain patients, acceptable reliability and high internal consistency of the findings. The protocol provides results comparable to the original language version and can be recommended as an easy-to-apply and suitable testing method. Cíle: Tato studie měla za cíl zjištění diagnostické přesnosti české verze souboru tří otázek ke stanovení diagnózy syndromu neklidných nohou (Restless Legs Syndrome, RLS), které se používají v epidemiologických studiích prováděných dotazníkovou cestou bez asistence investigátora. Originální anglická verze dotazníku byla publikována v roce 2003 Mezinárodní skupinou pro výzkum RLS (International RLS study group) společně s hlavními a vedlejšími diagnostickými kritérii pro klinické nebo telefonické stanovení diagnózy RLS. Soubor a metody: Sada otázek byla přeložena do češtiny a pak zaslepeně zpět do angličtiny, aby nedošlo k posunu významu. Ženy přicházející ke kontrole na Gynekologickou kliniku 1. LF UK a VFN v Praze v 34.–38. týdnu těhotenství byly požádány o vyplnění výše zmíněného dotazníku. Následně lékař vycvičený v diagnostice RLS pacientky kontaktoval na jimi zadaném telefonním čísle k standardnímu diagnostickému rozhovoru k ověření či vyloučení diagnózy RLS. Výsledky obou metod byly porovnávány a byla stanovena senzitivita, specificita, pozitivní a negativní prediktivní hodnoty. Výsledky: Dotazníkovou studii podstoupilo celkem 776 těhotných žen, ve věku 18–49 let. Celkem bylo validováno 199 dotazníků, u 65 osobní pohovor potvrdil diagnózu RLS. Pozitivita všech tří otázek má 89% specificitu, ale jen přibližně 78% pozitivní prediktivní hodnotu a senzitivitu. Za použití pouze první otázky je senzitivita 94 % a negativní prediktivní hodnota 96 %. Ze všech vyšetřených žen na všechny tři otázky odpovědělo pozitivně 28,0 % (95% konfidenční interval je 24,9–31,2 %). Závěr: Studie validovala českou verzi otázek a prokázala jejich relativně vysokou diagnostickou přesnost., Aims: The aim of this study was to estimate diagnostic accuracy of the Czech version of a self-administered minimum question set to diagnose the Restless Legs Syndrome (RLS). The wording of the three questions was published in 2003 by the International RLS study group together with the diagnostic criteria. Material and methods: The question set was translated into Czech, followed by blind translation back to English. Women in 36th–38th week of pregnancy were asked to complete the questionnaire. Subsequently, the women were contacted over a telephone by a physician trained in diagnosing RLS and a standard telephone interview to establish or exclude the diagnosis of RLS was performed. The results of both methods were compared to estimate sensitivity, specificity, positive and negative predictive values of the minimum question set. Results: We surveyed 776 pregnant women (18–49 years old) who came to a prenatal outpatient clinic to consult an obstetrician at the third trimester (36th–38th week of pregnancy). In total, 199 questionnaires were validated; personal interview confirmed the diagnosis of RLS in 65 of them. Positive response to all three questions yields specificity of 89% but only about 78% sensitivity and positive predictive value. Positive response to the first question only has sensitivity of 94% and negative predictive value of 96%. The prevalence of RLS during pregnancy estimated using the three-question questionnaire was 28% (95% confidence interval from 24.9 to 31.2%).
