Cíl: Převod Testu paměti pro záměry (Memory for Intentions (Screening) Test; MIST, 2010) do české verze zahrnoval standardizaci pomůcek, převod administrace a skórování, validační studii na české populaci, aby MIST bylo možné použít na české populaci. Úvod: Konstrukt prospektivní paměti (PP), tj. schopnosti si zapamatovat a realizovat záměry, je nezbytný pro uchování soběstačnosti, vykonávání aktivit denního života. V posledních letech hraje čím dál významnější roli pro diagnostiku poruch paměti. Standardizovaná a validovaná měřítka PP však v české diagnostice chybí. Metodika: Český převod MIST jsme administrovali 30 zdravým osobám. Výsledky: Celkový skór PP v testu MIST koreloval na střední úrovni s měřítky z neuropsychologické baterie zejména s retencí v paměti, mentální flexibilitou, odolností k interferenci a premorbidní inteligenční úrovní (všechna rho = 0,37–0,42, všechna p < 0,05). Reliabilita testu MIST ve smyslu vnitřní konzistence byla u základních osmi subškál nedostatečná (α = 0,50) i ve smyslu reliability obou polovin testu (split‑half reliabilita = 0,56), avšak reliabilita šesti subškál tříděných dle typu (oddálení, vodítka a modality odpovědi) byla vysoká (α = 0,88; split‑half = 0,95). Závěr: Výsledky validační studie naznačují, že český převod MIST je srovnatelný s verzí originální. Studie zpřístupňuje standardní vyšetření PP a umožňuje měření PP na klinických populacích. Introduction: Prospective memory (PM), i.e., the ability to remember and implement intentions after a delay, is essential as a subsystem of episodic memory for the maintenance of independence and execution of activities of daily living. PM assessment thus plays an important role in the diagnosis of episodic memory disorders. However, there are currently no standardized and validated PM tools in Czech language. Methods: The Czech version of MIST was administered to 30 healthy persons. Results: The MIST Summary score correlated at a medium level with a range of neuropsychological measures including memory retention, mental flexibility, and resistance to interference (all rho = 0.37–0.42; all p < 0.05). The reliability of MIST in terms of internal consistency was insufficient when analyzing the eight individual MIST trials (α = 0.50), as was split‑half reliability (split‑half reliability = 0.56). In contrast, there was a high degree of reliability between six subscales classified by type (delay, cue and mode of response; α = 0.88, split‑half = 0.95). Conclusion: The reliability and validity of the Czech version of MIST is comparable to the original English version. Cíl: Naším záměrem bylo zjistit diagnostické charakteristiky Montrealského kognitivního testu (MoCA) v tréninkové podobě pro lepší administraci u pacientů s Alzheimerovou nemocí (AN) poprvé v českém písemnictví. <p align="">Soubor a metodika: Záznamový formulář jsme obohatili o instrukce pro vyšetřovaného, instrukce k administraci a návod ke skórování. Obsah a skórování zůstaly zachovány a odpovídají předchozí české verzi. Upravili jsme návod k administraci. Zpětný překlad záznamového archu a návodu do angličtiny byl zkontrolován, schválen a nazván kanadským tvůrcem jako tzv. tréninková verze MoCA‑CZ1. Tímto testem jsme vyšetřili 30 pacientů s již diagnostikovanou mírnou demencí způsobenou Alzheimerovou nemocí podle nových doporučení NIA‑AA (78 ? 6 let, 13 ? 3 let vzdělání, 56 % žen) a 30 kognitivně normálních seniorů (NOS) podobného věku, vzdělání a pohlaví. <p align="">Výsledky: Oproti standardní verzi obsahují záznamové archy tréninkové české podoby MoCA‑CZ1 navíc další informace, s nimiž předpokládáme snadnější a spolehlivější provádění a vyhodnocování testu. Celkové skóry MoCA‑CZ1 byly významně nižší u pacientů s mírnou demencí způsobenou AN (18 ? 5 bodů) než u NOS (26 ? 3 body) (p < 0,00001). Celkové skóry NOS nezávisely na věku, pohlaví a vzdělání. Při porovnávání obou skupin podle analýzy receiver operating curve byl zjištěn optimální hraniční skór ≤ 23 bodů se senzitivitou 87 % a specificitou 84 %. Plocha pod křivkou byla 0,92. <p align="">Závěry: Tréninková MoCA‑CZ1 je volně dostupná na http://www.pcp.lf3.cuni.cz/adcentrum/testy.html. Výsledek testu může přispět k dřívějšímu záchytu mírné demence způsobené AN, a to časnou detekcí kognitivního deficitu. Při používání tréninkové verze MoCA‑CZ1 doporučujeme používat hraniční skór ≤ 23 bodů., Aim: Our aim was to determine diagnostic characteristics of the Montreal cognitive assessment (MoCA) using its training version for better administration in patients with Alzheimer’s disease (AD) for the first time in Czech literature. <p align="">Patients and methods: We complemented the test form with instructions for the patient, and with administration and scoring instructions. The content and the scoring were preserved from the previous Czech version. A separate Czech administration instruction manual was refined. English back translation of the data collection form and the manual was checked and approved by the Canadian author who also named it the training version. We examined 30 patients previously diagnosed with mild dementia due to Alzheimer’s disease according to the new NIA‑AA recommendations (78 ? 6 years, 13 ? 3 years of education, 56% women) and 30 cognitively normal seniors (NOS) of similar age, education and gender by MoCA‑CZ1. <p align="">Results: The training version of MoCA‑CZ1 contains additional information. We assume that this ensures reliability, and facilitates administration and scoring of the test. Total MoCA‑CZ1 scores were significantly lower in patients with AD (18 ? 5 points) than in NOS (26 ? 3 points) (p < 0.00001). Total scores of NOS did not correlate with age, gender or education. Analysis of receiver operating curve comparing the two groups found optimal cut off score of ≤ 23 points with sensitivity of 87% and specificity of 84%. BACKGROUND: The goal of this prospective study was to determine the frequency of micrometastases in patients with squamous cell carcinoma (SCC) of the oral cavity, pharynx and larynx in whom elective neck dissection was indicated (cN0). PATIENTS AND METHODS: A total of 12 patients (10 males and 2 females) were enrolled in the study. The age ranged 42-73 years (median 62 years). Elective neck dissection was performed in all patients (8 ipsilateral, 4 bilateral) and a total of 256 lymph nodes were removed and sent for microscopic examination. RESULTS: The presence of tumor cells in cervical lymph nodes was found in 5/12 (42%) patients. Micrometastases of SCC were found in two patients and isolated tumor cells (ITC) in two other patients. In the remaining one patient with oropharyngeal SCC, a micrometastasis of papillary thyroid carcinoma (PTC) was detected. Positive lymph nodes were localized in level II in three patients with SCC of larynx, hypopharynx and tongue base, respectively, in level I in one patient with SCC of oral tongue and in level III in one patient with PTC. CONCLUSION: Our results indicate that SCC of head and neck has a high potential for creating micrometastases which frequency is higher compared to clinically detected macrometastases. AIM: The purpose of this study was to develop a revised version of the Brief Bedside Dysphagia Screening Test for determining penetration/aspiration risk in patients prone to dysphagia. The priority was to achieve high sensitivity and negative predictive value. METHODS: The study screeners conducted bedside assessment of the swallowing function in 157 patients with a neurological (mainly stroke) or an ear, nose, and throat diagnosis (mainly head and neck cancer). The results were compared with a gold standard, flexible endoscopic examination of swallowing. RESULTS: For the neurological subgroup (N = 106), eight statistically significant bedside assessment items were combined into the Brief Bedside Dysphagia Screening Test-Revised (BBDST-R). Cut-off score 1 produced the highest sensitivity (95.5%; 95% confidence interval CI [CI]: 84.9-98.7%) and negative predictive value (88.9%; 95% CI 67.2-96.9%). Cíl: Na základě existujících cizojazyčných testů sestavit a validovat vlastní, v českém jazyce dosud chybějící test funkcionální komunikace u pacientů s afázií. Metodika: Byl vytvořen Dotazník funkcionální komunikace (DFK), který hodnotí stav komunikace v reálných situacích pomocí 20 položek v oblastech: I. bazální komunikace, II. sociální komunikace, III. čtení a psaní a IV. čísla a orientace; každá položka je hodnocena šestistupňovou škálou (0–5 bodů). Maximální možná hodnota Indexu funkcionální komunikace (Index FK) dosahuje 100 bodů. DFK byl validován na souborech zdravých dobrovolníků (n = 110, medián věku 63 let), pacientů s afázií (n = 38, medián věku 62 let) a s Alzheimerovou demencí (n = 8, medián věku 81,5 let). Výsledky: Hodnoty Indexu FK korelovaly významně s věkem (Spearman r = –0,354; p = 0,000148), proto byly stanoveny normativní hodnoty Indexu FK pro věk: 50–59 let: > 90; 60–69 let: > 85; 70 a více let: > 76. Pomocí ROC analýzy byla potvrzena vysoká diagnostická validita DFK v diskriminaci mezi normálními jedinci a pacienty s afázií (senzitivita při cut‑off hodnotě Indexu FK 86,5 je 89,5 % a specificita je 93,6 %; AUC = 0,974; p < 0,001). Hodnoty Indexu FK u pacientů s afázií významně korelovaly se stupněm jazykového deficitu vyjádřeného testem MASTcz a nezjistili jsme rozdíl oproti hodnotám Indexu FK u pacientů s poruchami komunikace u Alzheimerovy demence. Potvrdili jsme test‑retest reliabilitu opakovaným vyšetřením 10 pacientů s afázií. Byl vytvořen terapeutický materiál, který navazuje na položky DFK. Závěr: DFK je nový validovaný test v českém jazyce, který doplňuje paletu diagnostických nástrojů pro pacienty s afázií. Výzkum naznačil jeho výborné psychometrické vlastnosti, jež je třeba ověřit na větších souborech., Aim: To develop and validate a new, in the Czech language still missing test of functional communication for patients with aphasia. Methods: Functional Communication Questionnaire (FCQ) comprises 20 items that evaluate communication in four areas of real situations: I. Basal communication, II. Social communication, III. Reading and writing, and IV. Calculation and orientation. Every item is evaluated on 6–degree scale (0–5 points), and the sum (Functional Communication Index – FCI) of 100 points represents maximum possible value. FCQ was validated in groups of healthy volunteers (n = 110, median age 63 years), patients with aphasia (n = 38, median age 62 years), and patients with Alzheimer dementia (AD) (n = 8, median age 81.5 years). Results: Values of FCI correlated significantly with age (Spearman r = –0.354, p = 0.000148); and different normal limits of FCI were established for decades: 50–59 years: > 90; 60–69 years: > 85; 70–79 years: > 76. Using ROC analysis we confirmed high diagnostic validity of FCQ in discrimination between healthy controls and patients with aphasia (sensitivity of 89.5% and specificity of 93.6% for cut‑off value 86.5; AUC = 0.974; p < 0.001). Degree of functional communication impairment quantified with FCI in patients with aphasia correlated with the degree of language deficit quantified with MASTcz, while there was no significant difference in FCI values between patients with aphasia and AD. Repeated evaluation in 10 patients with aphasia proved very high test‑retest reliability of FCQ. New therapeutic material based on FCQ was finally introduced into clinical practice. Conclusion: FCQ extends the repertoire of diagnostic tests for patients with aphasia available in the Czech language. Our results showed very good psychometric characteristics of FCQ that should be confirmed by further research. Úvod: Dynamické metody kvantitativního testování senzitivity (Quantitative Sensory Testing; QST) jsou relativně novou nadstavbovou aplikací QST umožňující objektivizovat funkci centrálních modulačních mechanizmů algické percepce, jejichž individuální nastavení může přispívat k rozvoji chronických bolestivých stavů. Testování těchto mechanizmů umožňuje např. predikci rozvoje pooperační bolesti. K nejčastěji hodnoceným dynamickým QST metodám patří tzv. podmíněná modulace bolesti (Conditioned Pain Modulation; CPM) a časová sumace percepce bolestivých podnětů (Temporal Summation; TS). Soubor a metodika: Vyšetření dQST (dynamic methods of Quantitative Sensory Testing) metod s využitím termoalgických podnětů a modulace bolestivým chladem bylo provedeno u 77 zdravých dobrovolníků (47 žen testovaných v ovulační fázi menstruačního cyklu, 30 mužů, věk 19–73 let). Hodnocen byl také vliv věku a pohlaví na výsledky obou dQST testů. Výsledky: Uspokojivá funkce podmíněné modulace bolesti byla prokázána u 84 % testovaných dobrovolníků. Časová sumace bolestivých podnětů byla zachycena u 92, resp. 98 % jedinců v závislosti na použité teplotě podnětů. Nefunkčnost mechanizmů u zbylé části souboru byla ve většině případů podmíněna nesprávně nastavenými technickými parametry testu. U žen testovaných v ovulační fázi menstruačního cyklu jsme prokázali signifikantně vyšší efektivitu CPM mechanizmu ve srovnání s muži. Funkce TS se mezi oběma pohlavími významně nelišila. Významný vliv věku na efektivitu CPM ani TS mechanizmu nebyl ve sledovaném souboru prokázán. Závěr: Provedená studie ověřila použitelnost zvolených algoritmů dynamického testování senzitivity a prokázala antinociceptivní nastavení žen v ovulační fázi menstruačního cyklu ve srovnání s muži, zatímco významný vliv věku na funkci centrálních modulačních mechanizmů algické percepce nebyl zachycen., Background: So called “dynamic quantitative sensory methods” (dQST) represent relatively new QST application which enables to objectify certain mechanisms of pain processing, e.g. central integration or descending control. Their dysfunction may contribute to the development of chronic painful conditions. dQST methods proved the potential to predict the development of chronic postoperative pain. Conditioned pain modulation (CPM) and temporal summation (TS) are the most commonly used dQST tests. Patients and methods: The CPM and TS testing was performed in a group of 77 healthy volunteers (47 women tested in the ovulatory phase of the menstrual cycle, 30 men, age range 19–73) using thermal pain stimuli. Efficiency of CPM and TS mechanism including age and gender influence was evaluated. Results: Efficient CPM was found in 84% of healthy volunteers. In remaining 16%, the inefficiency was mainly due to low intensity of testing or modulatory stimuli. Based on the temperature used, efficient TS mechanism was found in 92% or 98% of healthy individuals. Women in the ovulatory phase of the menstrual cycle showed significantly more efficient CPM comparing to men. No sex differences of TS testing were found. Age had no significant effect on the CPM or TS mechanism in our group of healthy voluteers. Conclusions: Our study has verified the applicability of CPM and TS algorithms used. Women in ovulatory phase of the menstrual cycle show increased anti-nociceptive setting comparing to men. No significant effect of age on central modulation of pain perception has been found in pain-free healthy individuals. Výsledky: Za hlavné defi nujúce charakteristiky boli označené: „slovné vyjadrenie problému“ a „chybné, nepresné dodržiavanie inštrukcií“. Záver: Nie všetky defi nujúce charakteristiky ošetrovateľskej diagnózy Defi cit vedomostí uvedené v klasifi kačnom systéme NANDA-International sú pre vybraný súbor sestier rovnako významné pre jej stanovenie., Aim: The aim of the study was to validate the nursing diagnosis Deficient Knowledge by a chosen sample of Slovak nurses – experts, i. e. to find out which defining characteristics are considered major and minor. Methods: The Fehring’s Diagnostic Content Validity Model was used to evaluate the defi ning characteristics of nursing diagnosis Deficient Knowledge. Results: Th e following defi ning characteristics were marked as major: “verbalization of the problem”, “inaccurate compliance with instructions”. The following characteristics were marked as minor: “poor and inaccurate performance in a test” and “inappropriate behaviour”. The defining characteristic “exaggerated behaviour” had the weighted ratio less than 0.5. Ztráta elasticity arteriální stěny provázená její zvýšenou tuhostí byla identifikována jako nezávislý prognostický faktor u pacientů s kardiovaskulárními chorobami. Standardním neinvazivním způsobem vyšetřování arteriální tuhosti je měření rychlosti pulzové vlny (pulse‑wave velocity – PWV), avšak jeho přesnost je ovlivněna změnami krevního tlaku (TK) v okamžiku měření. Protože mezi intravaskulárním tlakem a průměrem arterie existuje exponenciální vztah, lze definovat pomocí přirozeného logaritmu poměru systolického a diastolického tlaku a elasticity arteriální stěny tzv. parametr tuhosti tepenné stěny β. Parametr β je základem pro výpočet CAVI (Cardio‑Ankle Vascular Index), který je nezávislý na změnách TK. To představuje velkou výhodu CAVI, který byl v uplynulé dekádě předmětem intenzivního klinického výzkumu zaměřeného na hodnocení tuhosti tepen u pacientů s již existujícími kardiovaskulárními chorobami a také u osob se zvýšeným rizikem, např. s hypertenzí, diabetem nebo obezitou. Prvořadým cílem diagnostického využití CAVI je posouzení tepenné tuhosti a včasného záchytu aterosklerózy. CAVI je možné s úspěchem využít při monitorování případné progrese nemoci a také účinnosti nastavené léčby. Při konečném hodnocení CAVI se však doporučuje jistá opatrnost, neboť jeho zvýšené hodnoty odrážejí jak cévní tuhost vyvolanou patologickými mechanizmy v arteriální stěně, tak i stav zvýšeného tepenného napětí následkem kontrakce hladké svaloviny., Loss of elasticity of the arterial wall accompanied by its stiffening has been identified as an independent prognostic factor in patients with cardiovascular diseases. The standard non‑invasive investigation of arterial stiffness is the measurement of pulse‑wave velocity (PWV), but its accuracy is affected by changes in blood pressure (BP) at the time of measurement. Because an exponential relationship exists between intravascular pressure and diameter of the artery, an arterial wall stiffness parameter β can be defined using the natural logarithm of the ratio of systolic and diastolic blood pressure and the elasticity of the arterial wall. Parameter β is the basis for the calculation of CAVI (Cardio‑Ankle Vascular Index), which is independent of changes in BP. This is a big advantage of CAVI, which has been the subject of intense clinical research in the past decade, focusing on the evaluation of arterial stiffness in patients with existing cardiovascular disease and also in patients at increased risk, such as those with hypertension, diabetes or obesity. The primary objective of the diagnostic use of CAVI is the assessment of arterial stiffness and early detection of atherosclerosis. CAVI can be successfully used for monitoring the possible progression of the disease and effectiveness of treatment. Popisujeme dva případy léčených kompenzovaných diabetiků s prokázanou přítomností paraproteinů v krevním séru. U obou z nich byly nalezeny výsledky HbA1c, neodpovídající jejich klinickému stavu a hodnotám plazmatické glukózy. Výsledky HbA1c byly porovnány s použitím dvou metod (obou na principu HPLC) měřením ve dvou různých laboratořích. U prvního pacienta s přítomností IgA kappa paraproteinu (48 g/l) poskytlo měření na přístroji DC-5 výsledky odpovídající dekompenzovanému diabetikovi a pacientovi bez diabetu v případě přístroje Variant II Turbo. U druhého pacienta s přítomností IgM lambda paraproteinu (27 g/l) byly oběma metodami opakovaně zjištěny neadekvátně nízké hodnoty HbA1c. Protože byla popsána řada interferencí paraproteinů při stanovení několika analytů krevního séra, považujeme za vhodné na tyto případy upozornit a pokračovat v podrobnějším studiu v budoucnosti na větším souboru diabetiků s prokázanou přítomností některého paraproteinu., We describe two causes of treated diabetic patients with paraproteinemia. In both cases the results of HbA1c show strong discrepancies with clinical statement and with plasma glucose concentrations. HbA1c results were controlled by repeated measurements by other method in other laboratory. Different results were obtained. In case of fi rst patient with IgA kappa paraprotein (48 g/l) show results of HbA1c obtained by method DC-5 picture of decompensated diabetes, results of Variant II Turbo picture of non-diabetes person, while glucose and clinical state indicated well compensation of diabetes. In case of the second patient with IgM lambda paraprotein (27 g/l) there were very low and improbable results in both HbA1c methods, while clinical statement and plasma glucose were in entire concordance with picture of well compensated diabetes. Interferences of paraproteins are very often observed in some biochemical analyses, but interference of paraprotein in HbA1c measurement is unusual. PURPOSE: Taking into account the fact that Goldmann applanation tonometry (GAT) geometrically deforms the corneal apex and displaces volume from the anterior segment whereas Dynamic Contour Tonometry (DCT) does not, we aimed at developing an algorithm for the calculation of ocular rigidity (OR) based on the differences in pressure and volume between deformed and non-deformed status according to the general Friedenwald principle of differential tonometry. METHODS: To avoid deviations of GAT IOP from true IOP in eyes with corneas different from the "calibration cornea" we applied the previously described Orssengo-Pye algorithm to calculate an error coefficient "C/B". To test the feasibility of the proposed model, we calculated the OR coefficient (r) in 17 cataract surgery candidates (9 males and 8 females). RESULTS: The calculated r according to our model (mean ± SD, range) was 0.0174 ± 0.010 (0.0123-0.022) mmHg/μL. A negative statistically significant correlation between axial length and r was detected whereas correlations between r and other biometric parameters examined were statistically not significant. CONCLUSIONS: The proposed method may prove a valid non-invasive tool for the measurement method of OR, which could help in introducing OR in the decision-making of the routine clinical practice. and E.
