Úvod: Cílem práce je zhodnocení našich zkušeností a výsledků s použitím hybridních rekonstrukcí v terapii multietážového postižení tepenného systému dolních končetin a vyhodnocení vlivu indikace a typu hybridní rekonstrukce na jejich průchodnost. Metoda: Retrospektivní analýza souboru pacientů s prospektivním sběrem dat. Na základě typu hybridní rekonstrukce rozdělení pacientů do 5 skupin. Skupina pacientů „1“ podstoupila transluminální angioplastiku (TA) ? implantaci stentu do pánevního řečiště v kombinaci s plastikou tepen v třísle (35 pacientů; 27,6 %). Skupina pacientů „2“ podstoupila TA ? implantaci stentu do pánevního řečiště v kombinaci s infrainguinálním bypassem (15 pacientů; 11,8 %). Skupina pacientů „3“ podstoupila plastiku tepen v třísle v kombinaci s TA výtokového traktu končetiny: AFS ? AP ? bércové tepny (52 pacientů; 40,9 %). Skupina pacientů „4“ podstoupila provedení infrainguinálního bypassu v kombinaci s TA distálně od místa distální anastomózy rekonstrukce (3 pacienti; 2,4 %). Skupina pacientů „5“ představuje heterogenní skupinu hybridních rekonstrukcí, které nespadaly do některé ze 4 výše uvedených skupin (22 pacientů; 17,3 %). Na základě indikace k výkonu rozdělení souboru do 3 skupin: skupina „ALI“ pacienti s akutní končetinovou ischemií, skupina „CLI“ pacienti s kritickou končetinovou ischemií, skupina „II“ pacienti s klaudikačními obtížemi. Analýza průchodností provedena s použitím Kaplan-Meier life tables. Porovnání průchodnosti mezi skupinami provedeno pomocí log-rank testu. Statistická analýza provedena s použitím softwaru IBM SPSS Statistics 21.0 . Výsledky: Soubor prezentuje 127 pacientů, kteří podstoupili 127 hybridních intervencí. Technický a klinický úspěch hybridní rekonstrukce byl 96,9 % a 98,4 %. 30denní perioperační mortalita byla 3,1 %. Výsledky primární (PP), primární asistované (PAP) a sekundární (SP) průchodnosti ve 2 letech byly nejlepší u pacientů s hybridním výkonem typu „1“ a „3“, tedy kombinací plastiky tepen v třísle a transluminální angioplastiky vtokového nebo výtokového traktu končetiny. Výsledky skupiny „1“ v pořadí PP, PAP a SP ve 2 letech: 80,6 %, 84,3 % a 84,3 %. Výsledky skupiny „3“ ve 2 letech ve stejném pořadí: 81,1 %, 82,8 % a 86,3 %. O poznání horších výsledků bylo dosaženo u pacientů ze skupiny „2“ a „5“. Výsledky PP, PAP a SP ve 2 letech u pacientů ze skupiny „2“ byly: 39,4 %, 59,1 % a 59,1 %. Výsledky průchodnosti pacientů z heterogenní skupiny „5“ byly ve 2 letech: 30,4 %, 49,2 % a 70,7 %. Při zohlednění vlivu indikace na průchodnost rekonstrukce ve 2 letech byly nejlepší výsledky primární asistované a sekundární průchodnosti dosaženy u pacientů ze skupiny „CLI“: 81,6 % a 86,2 %. Jenom primární průchodnost ve 2 letech byla nejlepší u pacientů s klaudikacemi (skupina „II“), a to 71,1 %. Pacienti indikováni k hybridnímu výkonu pro akutní končetinovou ischemií (ALI) dosahovali nejhorších výsledků z uvedených 3 skupin, a to PP, PAP a SP ve 2 letech: 33,7 %, 46,2 % a 45,5 %. Amputation-free survival ve 2 letech měl nejlepší hodnotu pro pacienty ze skupiny „CLI“, a to 89,1 % („II“ 81,9 %; „ALI“ 61,4 %). Overall-survival a amputation-free survival ve 2 letech za celý soubor dosahovaly hodnot 93,2 % a 82,3 %. Závěr: Hybridní revaskularizace je kombinovaný výkon, který umožňuje řešení multietážového postižení tepenného systému dolních končetin v rámci jednoho sezení. Je zatížen nízkým rizikem periprocedurální mortality a morbidity pro pacienta. Lze jím dosáhnout dobrých výsledků průchodnosti tepenného systému a záchrany končetiny bez ohledu na typ chronické ischemie (klaudikace, kritická končetinová ischemie). Výsledky u pacientů s akutní končetinovou ischemií jsou inferiorní. Jako výkon s nejlepšími výsledky nám vychází kombinace plastiky tepen v třísle spojená s transluminální angioplastikou vtokového nebo výtokového traktu končetiny., Introduction: The goal of this study was to evaluate our experience and results of single staged hybrid procedures for revascularization of lower extremities affected by multilevel arterial occlusive disease. We analysed the effect of the indication and type of reconstruction on patency. Method: Patients were retrospectively reviewed. Data collection was conducted prospectively. Patients were divided into 5 groups based on the type of hybrid reconstruction. Group “1” included patients who underwent transluminal angioplasty (TA) ? stenting of iliac arteries with endarterectomy (EA) and patch arterioplasty of the femoral bifurcation (35 patients; 27.6 %). Group “2” included patients who underwent TA ? stenting of iliac arteries with infrainguinal bypass (15 patients; 11.8 %). Group “3” consisted of patients who underwent TA ? stenting of outflow lower extremity arteries: superficial femoral artery ? popliteal artery ? crural arteries in combination with EA and patch arterioplasty of the femoral bifurcation (52 patients; 40.9 %). Group “4” represented patients who underwent infrainguinal bypass surgery in combination with TA distal to the site of open reconstruction (3 patients; 2.4%). Group “5” represented a heterogenous population of patients who underwent a hybrid reconstruction which did not belong to any of the 4 previously mentioned groups (22 patients; 17.3 %). The patients were divided into 3 groups based on the indication criteria of the intervention: Patients with acute limb ischemia (ALI) were put into group “ALI”. Patients with critical limb ischemia (CLI) were put into group “CLI”. Patients with claudications were put into group “II”. Patency analyses were performed using Kaplan-Meier life tables. Differences in patency rates between the different groups of patients were determined using the log-rank test. Statistical analysis was performed using the software IBM SPSS Statistics 21.0. Results: The study included 127 patients who underwent 127 hybrid arterial procedures. Technical and clinical success rates were 96.9% and 98.4%. 30-day perioperative mortality rate was 3.1%. The primary (PP), assisted-primary (APP) and secondary patency (SP) results at 2 years were the best amongst the patients from groups “1” and “3”. These patients underwent patch arterioplasty ? EA of the femoral bifurcation in combination with transluminal angioplasty of inflow or outflow arteries of the lower extremity. The resulting PP, APP and SP rates at 2 years in group “1” were 80.6%, 84.3% and 84.3%. Patency rates in group “3“ in the same order at 2 years were 81.1 %, 82.8% and 86.3%. Significantly lower patency rates were achieved in patients from groups “2“ and “5“. The resulting PP, APP and SP rates in group “2” at 2 years were 39.4%, 59.1% and 59.1%; in group “5“ at 2 years they were 30.4%, 49.2% and 70.7%. Taking into account the effect of the indication on patency rates, we found that patients from group “CLI” had the best APP and SP rates at 2 years: 81.6% and 86.2%. Only the PP rate at 2 years was the best in group “II” (patients with claudications): 71.1%. Patients who underwent their operations because of acute limb ischemia (group “ALI”) achieved the worst results among these 3 groups. Their PP, APP and SP rates at 2 years were 33.7%, 46.2% and 45.5%. Patients from the group „CLI“ had the best amputation-free survival at 2 years: 89.1% amongst the 3 groups subdivided according to the indication for the intervention (“II“ 81.9%; “ALI“ 61.4%). Overall survival and amputation-free survival at 2 years were 93.2% and 82.3%, respectively, for the whole studied population. Conclusion: The hybrid procedure is an intervention which combines endovascular and open reconstruction performed at the same time. Hybrid procedures provide effective therapy for patients with multilevel lower extremity arterial disease. They have low complication rates, periprocedural morbidity and mortality. Using these procedures, we are able to achieve good limb salvage rates and patency rates irrespective of the form of chronic limb ischemia (claudications, critical limb ischemia). Results for patients with acute limb ischemia are inferior. The combination of patch arterioplasty ? endarterectomy of the femoral bifurcation with transluminal angioplasty of inflow or outflow limb arteries is our hybrid procedure of choice with the most favourable results., and E. Comparative assessment of clinical efficiency of omeprazole and rabeprazole in treating gastroesophageal reflux disease (GERD). 65 patients with a verified diagnosis GERD were examined. Comparative clinical investigations of using of omeprazole and rabeprazole have revealed effectiveness of both drugs in the therapy of GERD. Práce prezentuje prenatální diagnostiku, možnosti terapie a následnou péči u plodů a novorozenců s kongenitálním chylothoraxem na podkladě zhodnocení 14 případů na naší klinice v letech 1993 až 2012. Povšechný hydrops plodu byl popsán ve 4 případech (29 %), v 10 případech (71 %) byla provedena odlehčovací amniodrenáž a ve 3 případech (21 %) prenatální odlehčovací punkce hrudníku. Spontánně bylo porozeno 11 plodů (78 %), hrudní drenáž byla nezbytná v průměru 9 dnů po porodu. Mortalita dosáhla v souboru 29 % (4 případy). V článku popisujeme vyšetřovací algoritmy, terapeutické prenatální i postnatální postupy a je zdůrazněna nutnost mezioborové spolupráce., The work presents prenatal diagnosis, treatment and care options in fetuses and neonates with congenital chylothorax based on a detailed evaluation of 14 cases recorded in our department between years 1993 and 2012. Generalized fetal hydrops was described in 29% of cases, therapeutic amnio drainage performed in 71% and prenatal thoracentesis in 21% of cases. 78% of fetuses were delivered spontaneously; thoracic drainage was necessary nine days after birth in average. Mortality reached 29%. Cíl. Retrospektivně zhodnotit efekt a dlouhodobou průchodnost transjugulární intrahepatální portosystémové spojky (TIPS) u nemocných s trombózou jaterních žil. Metoda. V letech 1992-2011 jsme vytvořili TIPS celkem u 38 pacientů s trombózou jaterních žil nekontrolovatelnou medikamentózní léčbou. Léčeno bylo devět mužů a 29 žen ve věku 13-76 let (medián 33 let), v šesti případech se jednalo o děti. Predisponující trombofilní stav byl prokázán u 84,2 % osob. Klinická forma Buddova-Chiariho syndromu byla v šesti případech akutní, v 18 případech subakutní a ve 14 případech chronická. Indikací k výkonu byl u 31 nemocných ascites, u pěti pacientů jaterní selhání a u dvou krvácení do zažívacího traktu. Zákroky byly prováděny standardní technikou v analgosedaci nebo v celkové anestezii. U 17 pacientů jsme do kanálu TIPS implantovali nepotažený stent nebo kombinaci potaženého a nepotaženého stentu, u 21 nemocných jsme zkrat vyztužili pouze pro TIPS dedikovaným potaženým stentem (stentgraftem). Sledování průchodnosti zkratu po výkonu bylo založeno na pravidelných ultrasonografických, klinických a laboratorních kontrolách, při prokázané poruše průchodnosti byla provedena reintervence. Výsledky. Celková doba sledování souboru byla 8 dní - 207 měsíců (medián 52 měsíců). Technického úspěchu jsme dosáhli u 100% nemocných, hemodynamická úspěšnost výkonu byla 86,8 %, s poklesem mediánu portosystémového gradientu z 23 mm Hg na 8 mm Hg, klinicky bylo dosaženo ústupu obtíží u 89,5% nemocných. Významné komplikace jsme zaznamenali celkem u sedmi osob, žádný pacient nezemřel v přímé souvislosti se zákrokem. V průběhu sledování zemřelo celkem jedenáct nemocných (29 %), přežití souboru za 1 rok a za 5 let od TIPS bylo 85,3 % a 65,4 %. Během doby sledování jsme provedli celkem 50 reintervencí pro dysfunkci spojky, průměrný počet reintervencí přepočtený na jednoho nemocného za dobu 5 let byl 1,65 výkonu u pacientů se zkratem vytvořeným stentem a 0,67 výkonu u nemocných, u kterých byl primárně použit stentgraft. Ve skupině se zavedeným nekrytým stentem byla primární průchodnost 52,9% za 1 rok a 20 % za 5 let od výkonu, u skupiny s implantovaným potaženým stentem byla jednoletá primární průchodnost 80 % a pětiletá primární průchodnost 33,3%. V průběhu sledování jsme zároveň u osmi pacientů provedli angioplastiku dolní duté žíly pro její symptomatickou stenózu, u šesti z těchto nemocných byl do dolní duté žíly zaveden stent. Závěr. Vytvoření TIPS je velmi efektivní metodou léčby pacientů s trombózou jaterních žil. Použití pro TIPS dedikovaných potažených stentů vede knižšímu počtu dysfunkcí zkratu s nižším počtem reintervencí nezbytných k obnově průchodnosti, nutností je však současná důsledná antikoagulační léčba a léčba základního hematologického onemocnění., Aim. To retrospectively evaluate the efficacy and long-term patency of transjugular intrahepatic portosystemic shunt (TIPS) in patients with hepatic vein thrombosis. Method. In years 1992-2011, totally 38 patients with hepatic vein thrombosis resistant to medical therapy underwent TIPS creation in our department. We treated 9 males and 29 females, age 13-76 years (median 33 years), 6 patients were children. The underlying thrombophilic state was confirmed in 84.2% of treated patients. The Budd-Chiari syndrome was acute in 6, subacute in 18 and chronic in 14 cases. Ascites was indication for TIPS in 31 patients, liver failure in 5 and gastrointestinal bleeding in 2 persons. All the procedures were performed in anal-gosedation or under general anestesia using standard technique. In 17 patients, we implanted non-covered stent or combination of non-covered and covered stents during TIPS creation, in the other 21 patients only the TIPS dedicated covered stents had been used. TIPS patency during follow-up after the procedure was based on regular ultra-sonographic, clinical and laboratory examinations. In case of shunt dysfunction, a rein-tervention was performed. Results. The total follow-up period was 8 days - 207 months (median 52 months). The technical success rate was 100%, hemo-dynamical success rate 86.8% with median portosystemic gradient decrease from 23 mm Hg to 8 mm Hg, and clinically we were succesfull in 89.5% of patients. In 7 patients, severe complications occurred during shunt creation, but none of the patients died in direct connection with the procedure. Totally 11 patients (29%) died during follow-up period, the 1-year and 5-years survival rates were 85.3% and 65.4%, respectively. Due to TIPS dysfunction, totally 50 reinterventions were needed, the average 5-year reintervention rate per patient was 1.65 procedures in the bare stent group and 0.67 porcedures in the covered stent group. In the non-covered stent group, we achieved primary patency rates 52.9% 1 year and 20% 5 years after TIPS creation, in the covered stent group the 1-year primary patency rate was 80% and 5-year primary patency rate 33.3%. Angioplasty of symptomatic stenosis of the inferior vena cava was performed in 8 patients during follow-up, in 6 of these patients also stent implantation was necessary. Conclusion. TIPS creation is a very effective treatment of patients with hepatic vein thrombosis. Cíl: Správný pooperační režim je důležitou součástí rekonvalescence po dekompresi karpálního tunelu. Cílem této prospektivní randomizované studie je zjistit, zda pooperační dočasné dlahování má klinicky významný efekt, nebo zda není naopak zbytečné. Výsledky dosud publikovaných prací nedávají jasné, prokazatelné a jednoznačné doporučení. Soubor a metodika: Do souboru bylo zařazeno celkem 72 pacientů s klinicky a elektromyograficky prokázaným syndromem karpálního tunelu, kteří byli randomizováni do dvou skupin. V první skupině byla pacientům po operaci na dva týdny přiložena dorzální dlaha fixující zápěstí v neutrální poloze a v druhé skupině dlaha použita nebyla. Před operací i po operaci byla hodnocena distální motorická latence, síla stisku a míra subjektivních obtíží pomocí specializovaných dotazníků DASH (Disabilities of Arm, Shoulder and Hand Questionnaire) a Bostonského dotazníku (Boston Carpal Tunnel Questionnaire). Výsledky: Po operaci došlo ke zkrácení distální motorické latence a mírnému snížení síly stisku, ale mezi skupinami nebyly nalezeny statisticky významné rozdíly (p > 0,05). Po operaci došlo také k předpokládanému zmírnění subjektivních obtíží, objektivizovanému dotazníkovými koeficienty, ale mezi skupinami nebyl nalezen statisticky významný rozdíl (p > 0,05). Závěr: Podle našich zjištění dočasné dlahování po otevřené dekompresi karpálního tunelu ve srovnání s nedlahovanou skupinou nezlepšuje výsledky chirurgické léčby. Používání dlahy v této indikaci se jeví jako nadbytečné jak z lékařského, tak z ekonomického pohledu. Na druhou stranu dočasné dlahování po operaci výsledky operační léčby nezhoršuje., Aim: Correct post-operative care is an important part of recovery after carpal tunnel decompression. The aim of this prospective randomized study was to determine whether postoperative temporary splinting has a clinically significant effect, or whether it is unnecessary. The results of published data do not give clear, demonstrable and unambiguous recommendations. Material and methods: A total of 72 patients with clinically and electrodiagnostically established diagnosis of carpal tunnel syndrome were randomized into two groups. The first group included patients with a dorsal splint, fixing the wrist in a neutral position for two weeks after surgery, whereas the second group included patients, in whom the splint was not used after surgery. Distal motor latency, grip strength and degree of subjective symptoms were assessed before and after surgery using specific questionnaires, namely the DASH questionnaire (Disabilities of Arm, Shoulder and Hand) and the Boston questionnaire (Boston Carpal Tunnel Questionnaire). Results: After surgery, there was a reduction of distal motor latency and a moderate reduction of grip strength, but there were no statistically significant differences (p > 0.05) between the two groups. After surgery, the anticipated improvement of subjective symptoms was achieved, objectified by questionnaire coefficients. However, there was no statistically significant difference (p > 0.05) between the two groups either. Conclusion: According to our findings, temporary splinting after open carpal tunnel decompression does not improve the results of surgical treatment compared to the non-splinted group. Using the splint in this indication seems to be redundant from the medical and economic point of view. On the other hand, temporary splinting after surgery does not cause worsening of the results of surgical treatment. Aldosterone blocker Spironolactone has antiinflammatory, antiproliferative and antioxidative effects, that is why pathogenetically it is expedient to use it in complex therapy of rheumatoid arthritis. Material and methods: 46 patients with RA took 2550 mg/day of Spironolactone during 12 months as an addition to standard therapy, the comparison group consisted of 47 patients that got only standard therapy, all the patients were fully examined prior and post the treatment. Results: complex RA therapy leads to improved VAS, HAQ, the antioxidative potential index F; decreased blood concentrations of TNFα, ICAM1, FGF and VEGF in contrast to standard therapy. Complex therapy made reduced the DAS 28 more > 0.6. Conclusions: applying of Spironolactone in complex therapy of rheumatoid arthritis contributes to more pronounced improvement in indices of articular syndrome and patients life quality, reduce of antiinflammatory, antiproliferative and angiogenic cytokines, and more effectively reduces the activity of the disease comparing to standard therapy., Elena Komarova, Borys Rebrov, and Literatura
Few studies concerning the occurrence of cow’s milk allergy with the use of double-blind, placebo controlled food challenge test in adolescents and adult patients suffering from atopic dermatitis exist. Aim: To evaluate the occurrence of cow’s milk allergy in adolescents and adults suffering from atopic dermatitis. Method: Altogether 179 persons suffering from atopic dermatitis were included in the study: 51 men and 128 women entered the study with the average age of 26.2 (s.d. 9.5 years). Complete dermatological and allergological examinations were performed. Results: The positive results in specificIgE and in skin prick tests were recorded in 12% of patients. According to the open exposure tests and double-blind, placebo controlled food challenge tests these patients are only sensitized to cow’s milk without clinical symptoms of allergy. Double-blind, placebo controlled food challenge test confirmed food allergy to cow milk only in one patient (worsening of atopic dermatitis), the oral allergy syndrome was observed in another one patient, occurrence of this allergy was altogether 1.1%. Motivation Until recently it was considered that 65 years is cutoff for defining patients as elderly, but newer reports indicate that this age limit shift to 70 years of age. Elderly patients with advanced non small cell lung cancer, associated comorbidities and poor performance status represent a specific population and a challenge for use of chemotherapy. Primary aim was to evaluate the impact of mono therapy with oral etoposide on overall survival in elderly patients (≥ 70 years of age) with advanced non small cell lung cancer and poor performance status (PS) ≥ 2 (clinical stage IIIb and IV ), and as well to evaluate tolerability of this therapy. Secondary aim was to evaluate response rate. Methods Retrospectively, medical records of 79 female and male patients with advanced non small cell lung cancer and poor performance status treated with oral etoposide (2x25 mg 20 days/10 days pause) in period from 2007 till 2010 were checked for relevant data. Data regarding demographics, performance status, overall survival, response rates and drug toxicity were collected. For statistical analysis we used Pearson chi-square test, T-test, Kaplan-Meier product limited method and Cox regression. Results Median overall survival (OS) was 31 weeks, in patients with PS 2 overall survival was 34 weeks, and in group with PS 3 was only 24 weeks. Partial response was registered in 20.2% of patients, stable disease in 41.85 % and disease progression in 38% of patients. Treatment was well tolerated, febrile neutropenia and toxic deaths were not registered. Toxic effects didn't have statistically significant influence on OS. Úvod: Terapie ran kontrolovaným podtlakem (NPWT) je efektivní metodou lokální terapie širokého spektra nehojících se ran. V současnosti je využívána především u hospitalizovaných pacientů, pro systematické použití v ambulantním režimu nejsou v České republice dostupná klinická data. Metody: V prospektivní nerandomizované studii autor porovnává klinickou a nákladovou efektivitu léčby ulcerací na dolní končetině u diabetiků kontrolovaným podtlakem (NPWT) ambulantně versus za hospitalizace. Cílem sledování je stanovit účinnost NPWT v ambulantním režimu na základě hodnocení vývoje spodiny rány, kvantifikovat předpokládanou redukci celkových nákladů na terapii a porovnat efektivitu obou režimů navzájem. Do sledování bylo zařazeno 36 hospitalizovaných (NPWT systémy RENASYS GO nebo PICO) a 28 ambulantních pacientů (PICO). NPWT byla ukončena v momentě, kdy byla spodina rány zcela pokryta granulační tkání nebo při vzniku vážných komplikací léčby. Hodnotili jsme velikost ulcerace, charakter její spodiny, počet NPWT výměn, její celkovou délku, intenzitu bolesti, přítomnost komplikací a náklady na terapii. Výsledky: Sledování prokázalo porovnatelnou klinickou účinnost NPWT v ambulantním režimu (n=28) versus za hospitalizace (n=36, statisticky nesignifikantní rozdíly v hojení ulcerací verifikované na základě vývoje spodiny rány a redukce plochy ulcerací v čase), stejné riziko komplikací spojených s NPWT (statisticky nesignifikantní rozdíly ve funkčnosti obou technologií a ve výskytu komplikací vzniklých v průběhu sledování) i obdobné dlouhodobé výsledky (statisticky nesignifikantní rozdíly v počtu amputací). Celkové náklady na ambulantní terapii ran byly statisticky signifikantně nižší (16 700 Kč versus 36 000 Kč, p 0,001), stejně tak průměrné náklady na 1 den podtlakové terapie (817 Kč versus 3400 Kč, p 0,001). Závěr: Analýza klinických výsledků jednoznačně prokázala, že ambulantní podtlaková terapie ulcerací syndromu diabetické nohy (metoda PICO) má stejnou efektivitu jako podtlaková terapie za hospitalizace. V ambulantním provozu je NPWT levnější, převazy jsou vnímány méně bolestivě a NPWT je pacienty lépe snášena., Introduction: The negative pressure wound therapy (NPWT) is an effective local treatment method of many non-healing wounds. NPWT is routinely used for inpatient treatment in the Czech Republic; however, no clinical data is available for systematic outpatient treatment. Methods: The prospective non-randomized study was used to compare the clinical impact and cost-effectiveness of diabetic leg-ulcer and foot-ulcer negative pressure treatment in outpatient and inpatient settings. The aim of the study was to assess the effectiveness of outpatient NPWT on the basis of wound bed evaluation, to detect and quantify pre-expected reductions in the total costs of the therapy, and to compare the efficacy of both treatment options. Enrolled subjects included inpatients (n=36, NPWT systems RENASYS GO or PICO) and outpatients (n=28, PICO system). NPWT was concluded when the wound bed had been totally covered by granulation tissue, or when serious health problems occurred. We monitored the ulcer area, wound bed trait, number of NPWT dressing changes, length of NPWT, intensity of pain (VAS scale), presence of complications, and treatment costs. Results: We found comparable clinical efficacy of outpatient (n=28)/inpatient (n=36; statistically non-significant differences in healing of ulcers verified based on wound bed development and reduction of ulcer areas in time) NPWT, the same risk of complications associated with NPWT (statistically non-significant differences in the function of both technologies and in complications developed during follow-up) as well as similar long-term results (statistically non-significant differences in the count of amputations). The total costs for outpatient wound treatment were statistically significantly lower (EUR 600 versus EUR 1300, p=0.001), and so were the average one-day-NPWT costs (EUR 30 versus EUR 120, p=0.001). Conclusion: We demonstrated that outpatient diabetic-foot-ulcer NPWT provided the same clinical efficiency as inpatient NPWT, while outpatient NPWT was less expensive, less painful and better tolerated than inpatient NPWT., and J. Imunostimulace polybakteriálním lyzátem (Urivac®) v prevenci recidivujících infekcí dolních močových cest. Infekce močových cest (IMC) zaujímají v evropských zemích i u nás dlouhodobě druhé místo ve výskytu infekčních onemocnění v populaci a představují tak významný socioekonomický problém. Spolu s respiračními infekty se rozhodující měrou podílejí na preskripci antimikrobiálních přípravků. Extenzivní antimikrobiální léčba je jednou z nejvýznamnějších příčin nárůstu bakteriální rezistence. Určitou možnost prevence recidiv infekcí dolních močových cest a snížení spotřeby antimikrobiálních preparátů představuje imunostimulace. Prospektivní multicentrická studie hodnotila klinický účinek, snášenlivost a přínos k léčbě recidivujících zánětů při podávání imunostimulačního přípravku Urivac®. Počet atak infekcí dolních močových cest se po imunostimulaci významně snížil jak v jejím průběhu, tak po jejím ukončení (p = 2,2x10-16). V průběhu imunostimulace bylo bez příznaků 85 % nemocných a při dalším sledování 70,8 %. Snížil se také počet aplikovaných antimikrobiálních přípravků. Subjektivní zlepšení přetrvávalo ve více než 57 % i po ukončení léčby. Mikrobiologická odpověď dosáhla 77 %, v odstupu od ukončení léčby se snižuje. U exacerbací byla uropatogenní flóra detekována v 61 %. Nežádoucí účinky u dvou nemocných vedly k předčasnému ukončení terapeutického režimu., In European countries, including the Czech Republic, urinary tract infections (UTIs) have long occupied the second place in the incidence of infectious diseases in the population, thus representing a major socioeconomic problem. Along with respiratory infections, they are the major contributors to the prescription of antimicrobial agents. Extensive antimicrobial therapy is one of the most important causes of increased bacterial resistance. Immunostimulation is a possible way of preventing recurrent lower urinary tract infections and reducing the consumption of antimicrobial agents. A prospective multicentre study evaluated the clinical effi cacy, tolerability, and benefi t to treatment of recurrent infections with the immunostimulatory drug Urivac®. The number of attacks of lower urinary tract infections following immunostimulation reduced signifi cantly both during its course and after its cessation (p = 2.2 x 10–16). During immunostimulation, 85 % of patients were symptom-free, and during subsequent follow-up it was 70.8 %. There was associated with a reduction in the number of the antimicrobial agents administered. Subjective improvement persisted in more than 57 % even after treatment cessation. Microbiological response reached 77 %; it decreases with increased time from treatment cessation. In the case of exacerbations, uropathogenic fl ora was detected in 61 %.
