A hybrid approach to elephant trunk technique for treatment of thoracic aortic aneurysms combines a conventional surgical and endovascular therapy. Compared to surgery alone, there is a presumption that mortality and morbidity is reduced. We present a case report of a 42-year-old man with a giant aneurysm of the entire thoracic aorta, significant aortic and tricuspid regurgitation and ventricular septum defect. Karotická endarterektomie byla dlouho považována za zlatý standard v léčbě a prevenci ischemické cévní mozkové příhody u pacientů s významnou stenózou vnitřních karotických arterií. Karotický stenting, i přes počáteční limitace, dosahuje v současné době výsledků srovnatelných s karotickou endarterektomií a u selektovaných pacientů je považován za rovnocennou volbu. Bezpečnost obou výkonů je dána zejména rizikem periprocedurálních komplikací, které jsou významně ovlivněny zkušenostmi operatéra. Transkraniální dopplerovská ultrasonografie je zavedenou diagnostickou a monitorovací metodou, jejíž využití přispívá ke zkvalitnění péče o pacienty podstupující karotické intervence. Tento článek shrnuje znalosti týkající se invazivní léčby karotických stenóz se zaměřením na možnosti optimalizace pomocí transkraniální dopplerovské ultrasonografie., Surgical carotid endarterectomy was for a long time considered the standard approach to the treatment of atherosclerotic carotid artery disease. Despite its initial limitations and slow acceptance, carotid artery stenting has evolved into an elaborate method currently considered to be equivalent and in some patients even preferable to carotid endarterectomy. Given the fact that the long‑term outcomes of carotid artery stenting and carotid endarterectomy are comparable, the major risk of both procedures is derived from the periprocedural complications, which are highly operator dependent. Transcranial Doppler ultrasound is a well‑established diagnostic and monitoring tool allowing optimising the care of patients with carotid artery disease. Opožděné krvácení je jednou z možných komplikací pankreatoduodenektomie. Jeho léčba je operační nebo endovaskulární (embolizace, stentgraft). V naší kazuistice uvádíme případ pacienta s krvácením z pseudoaneuryzmatu a. hepatica v odstupu 3 měsíců od pankreatoduodenektomie provedené pro karcinom choledochu, které bylo řešeno zavedením stentgraftu., Delayed bleeding is a potential complication of pancreatoduodenectomy. Cíl. Ischemické cévní mozkové příhody (iCMP) jsou stále jednou z nejčastějších příčin mortality a morbidity postihující i mladší populaci v produktivním věku. Klíčovým faktorem ovlivňujícím prognózu po iCMP je včasná rekanalizace uzavřené mozkové tepny s obnovením perfuze v postižené mozkové tkáni. Cílem práce bylo zhodnocení bezpečnosti a efektivity kombinované revaskularizační terapie sestávající z iniciální intravenózní trombolýzy a přímo navazující mechanické trombektomie u pacientů s akutní iCMP a uzávěrem mozkové tepny. Metoda. Do souboru pacientů byli zařazeni všichni po sobě jdoucí pacienti s akutní iCMP a prokázaným uzávěrem střední mozkové tepny v úseku M1-2 nebo distální části arteria carotis interna nebo uzávěrem arteria basilaris, a to v období od června 2010 do dubna 2012. Všichni nemocní byli léčeni kombinovanou revaskularizační terapií, tzn. iniciální intravenózní trombolýzou a přímo navazující mechanickou trombektomií systémem Solitaire. Iniciální stupeň neurologického deficitu byl stanoven pomocí NIHSS skóre. Výsledný klinický stav byl zhodnocen po 90 dnech pomocí modifikované Rankinovy škály (mRS). Výsledky. Do souboru bylo podle kritérií zařazeno 50 pacientů (průměrný věk 66, 8 ? 14,6 roku) se vstupním mediánem NIHSS skóre 18,0 (8-32). Rekanalizace (TICI 2A, B a 3) bylo dosaženo u 94 % pacientů, kompletní rekanalizace (TICI 3) u 72% pacientů. Průměrný rekanalizační čas (čas od začátku symptomů do dosažení maximální rekanalizace) byl v našem souboru 244,2 ? 87,9 minut. Symptomatické intracerebrální krvácení se vyskytlo u 6 % pacientů. Medián mRS po 90 dnech byl v našem souboru 1, 60 % pacientů mělo devadesátidenní klinický výsledek dle mRS hodnocen jako dobrý (mRS 0-2). Tříměsíční mortalita byla ve sledovaném souboru 14 %. Závěr. Kombinovaná revaskularizační terapie akutní iCMP sestávající z iniciální intravenózní trombolýzy a přímo navazující mechanické trombektomie je bezpečná a efektivní. Okamžité provedení mechanické tromektomie po iniciální intravenózní trombolýze vede k výraznému zkrácení rekanalizačního času, který je rozhodující pro dobrý klinický výsledek u pacientů intervenovaných pro akutní ischemickou cévní mozkovou příhodu., Aim. A key factor influencing prognosis after ischemic stroke is early recanalization of close cerebral artery and reperfusion in the affected brain tissue. The aim of our study is to evaluate the safety and effectiveness of re-vascularization combined therapy consisting of initial intravenous thrombolysis and immediately after that performed mechanical thrombectomy in the treatment of patients with acute ischemic stroke. Methods. Consecutive non-selected acute ischemic stroke patients with baseline CTA or MRA documented occlusion of middle cerebral artery (MCA) (segment M1-2) or distal part of internal carotid artery (ICA) or basilar artery (BA) and treated between June 2010 and April 2012 at our stroke center were included in the study. All patiens were treated by combined therapy consisting of initial intravenous thrombolysis and immediately after that performed mechanical thrombectomy by Solitaire stent. Stroke severity was assessed using National Institutes of Health Stroke Scale (NIHSS), 90-day clinical outcome using modified Rankin Scale (mRS). Results. Fifty enrolled patients (mean age 66.8 ? 14.6 years) had baseline median NIHSS 18.0 (8-32). Overall recanalization (TICI 2A, B and 3) was achieved in 94% of patients, complete (TICI 3) in 72% of patients. The mean time from stroke onset-to-maximal recanalization was 244.2 ? 87.9 min. Symptomatic intracerebral hemorrhage occurred in 6% of patients. The median mRS score at 90 days was 1 and 60% patients had good outcome (mRS 0-2). Overall 3-month mortality was 14%. Conclusion. Combined revascularization therapy of acute ischemic stroke consisting of initial intravenous thrombolysis and immediately after that performed mechanical thrombectomy is safe and effective. Úvod: Léčení výdutě aorty abdominální pomocí endovaskulární implantace stentgraftu je někdy komplikované nebo nemožné z důvodu nevhodné anatomické konfigurace výdutě a přilehlých úseků aorty. Příčinou obtíží bývá nevhodný proximální krček, průchodné větve ústící do vaku výdutě nebo výrazně vinuté přístupové pánevní tepny. Metoda endovaskulární výplně aortální výdutě je nový pracovní postup, jehož cílem je překonat některé nedostatky současných stentgraftů. Metoda: V roce 2014 jsme na Oddělení cévní chirurgie Nemocnice Na Homolce zavedli 51 stentgraftů do subrenální aorty a pánevních tepen. Většinou se jednalo o bifurkační stentgrafty klasické konstrukce. U dvou nemocných jsme použili nový stentgraft Nellix, v jednom případě pro krátký subrenální krček 13 mm, ve druhém pro výrazně kónický tvar krčku. Výsledky: Pooperační průběh byl u obou pacientů bez rušivých příhod a byli propuštěni do ambulantní péče 5. pooperační den. Kontrolní CT angiografie po šesti týdnech prokázala dobré těsnění SG a polymerem kompletně vyplněný vak výdutě. Závěr: Nová metoda endovaskulární výplně výdutě aorty abdominální (endovascular aneurysm sealing – EVAS) umožní léčit pacienty, u kterých by anatomická situace vedla k technicky obtížným a finančně náročným endovaskulárním postupům. Její dlouhodobé výsledky bude možné sledovat teprve na větších souborech pacientů., Introduction: Endovascular stent graft therapy of abdominal aortic aneurysms is sometimes complicated due to unusual anatomy of the aorta and adjacent arterial regions, an irregular or short proximal neck, numerous patent branches originating from the aneurysm, or tortuous iliac arteries. Endovascular aneurysm sealing is a new method designed to overcome certain limitations of current stent grafts. Method: At the Department of Vascular Surgery of Na Homolce Hospital, we implanted 51 stent grafts in the subrenal aorta and iliac arteries. Most of them were regular bifurcated stent grafts. Two patients were treated with the new Nellix stent graft, in one case due to a short subrenal neck of only 13 mm, and due to a considerably conical neck in the second case. Results: The post-operative course was uneventful in both patients and they were discharged on the 5th postoperative day. CT angiography after six weeks proved that the stent graft had sealed well. The polymer filled the aortic lumen completely. Conclusion: This new method of endovascular aneurysm sealing (EVAS) of abdominal aortic aneurysm makes it possible to treat patients whose anatomy would normally require technically complex and more expensive endovascular methods. Polytrauma je jednou z hlavních příčin mortality lidí v produktivním věku. Nejdůležitější při léčbě polytraumatu je rychlá aktivace záchranného systému. Při závažných poraněních je klíčový primární transport do specializovaného pracoviště – traumacentra. Naše kazuistické sdělení se zaměřuje na dva případy polytraumatizovaných pacientů, u kterých došlo v průběhu léčení k relativně vzácným situacím. V prvním případě se jedná o vzácné poranění slinivky břišní, které si vyžádalo pankreatoduodenektomii. V druhém případě o poranění jater ošetřené pravostrannou lobektomií se zřídkavou komplikací. Rovněž poukazuje na nezbytnost multioborové spolupráce v léčbě polytraumatizovaného pacienta., Polytrauma is one of the leading causes of mortality in people at productive age. Prompt activation of the rescue system is most important in the treatment. In cases of severe injuries, primary transport to a specialized hospital – trauma center is crucial. Our report is focused on two cases of polytraumatized patients whose treatment was associated with relatively rare situations. One of the patients suffered a pancreatic injury that required pancreaticoduodenectomy. The other patient had a liver injury, which was treated with right lobectomy with a rare complication. Bio-degradable stents are be made of different synthetic polymers (like polylactide or polyglycolide) or their co-polymers (polydioxanone). They can be used for treating benign stenoses of the small and large intestine, particularly in Crohn’s disease. Endoscopic introduction of bio-degradable stents into small and large intestinal stenoses is feasible and relatively simple. Initial results are encouraging and the complication rate is low. However, there are still some difficulties that need to be overcome. The rate of early stent migration is still rather high (up to one third of patients). This might be solved by changes in the shape or rigidity of the stents as well as by further improvement in the design.
