Stenóza cervikálního kanálu může vést ke kompresi krční míchy a představuje nejvýznamnější mechanický faktor v patofyziologii cervikální spondylogenní myelopatie. Míšní tkáň je však vůči kompresi poměrně odolná a k rozvoji manifestní myelopatie dochází až při vyšším stupni komprese a v kombinaci s dalšími patofyziologickými faktory, zejm. dynamickým faktorem a zevním traumatem. Asymptomatická spondylogenní komprese krční míchy (ASCCC) je velmi častým nálezem zvláště v populaci vyššího věku nad 50 let. Objektivizace a kvantifikace cervikální stenózy a komprese krční míchy jsou však nejednotné a nejsou jasně stanoveny prediktory progrese do manifestní myelopatie a s tím související indikace případné preventivní chirurgické dekomprese. Přehled obsahuje nejčastěji používané metody stanovení cervikální stenózy a cervikální míšní komprese pomocí zobrazovacích metod. Nativní radiogram a výpočetní tomografie mají význam v objektivizaci cervikální stenózy, zatímco dominantní metodou objektivizace míšní komprese je magnetická rezonance (MR). Z parametrů, které nejvíce korelují s klinickou manifestací komprese, jsou uváděny příčná plocha míchy v místě komprese a přítomnost intenzitních změn míšního signálu. Z novějších zobrazovacích modalit je nejcitlivější MR zobrazení tenzorů difúze. Z prognostických faktorů rozvoje manifestní myelopatie u ASCCC jsou obecně akceptovány přítomnost manifestní radikulopatie a abnormita motorických a senzomotorických evokovaných potenciálů. Význam zobrazovacích metod v predikci rozvoje manifestní myelopatie a benefit provedení preventivní dekomprese ve skupině nemocných s asymptomatickou kompresí a vyšším rizikem rozvoje manifestní myelopatie bude nutné ověřit dalšími studiemi., Cervical spinal stenosis may lead to cervical cord compression and represents the most important mechanical factor in pathophysiology of cervical spondylotic myelopathy. Medullar tissue is, however, rather resistant to compression and development of symptomatic myelopathy occurs only when higher degree stenosis is present and in combination with other pathophysiological factors, mainly dynamic compression and trauma. Asymptomatic spondylotic cervical cord compression (ASCCC) is a quite frequent finding in older population above the age of fifty. However, reliability of the methods used to verify and quantify cervical stenosis and cervical cord compression is low and clear predictors of development of symptomatic myelopathy and related indication of potential preventive surgical decompression in ASCCC have not been determined yet. The overview discusses the most frequently used methods to establish cervical spinal stenosis and cervical cord compression using imaging methods. Radiogram and especially computed tomography are important for verification of cervical spinal stenosis, while magnetic resonance imaging (MRI) is a preferable method to detect cervical cord compression. The cross‑sectional spinal cord area and T2 MRI spinal cord hyperintensity are among the parameters considered to be the most closely correlated with clinical manifestation of spinal cord compression. Among newly introduced imaging modalities, MRI diffusion tensor imaging seems to be the most promising one. The presence of symptomatic radiculopathy and abnormality of motor and somatosensory evoked potentials are among generally accepted predictors of symptomatic myelopathy. The importance of imaging methods as predictors of symptomatic myelopathy development as well as benefits of preventive surgical decompression in ASCCC individuals with high risk of developing symptomatic myelopathy is to be established in future studies. Dna je zánětlivé, metabolicky podmíněné onemocnění indukované krystaly. Prevalence dny stále vzrůstá. V klinické praxi je často špatně diagnostikována a terapie především akutního záchvatu není adekvátní. V první části publikace jsou rozebírány diagnostické možnosti dnavé artritidy. Především je zdůrazněna výhodnost analýzy synoviálního výpotku a přímého průkazu krystalů v polarizačním mikroskopu. Při nedostupnosti materiálu na krystalografickou analýzu je nutné použít kombinaci klinických kritérií tak, jak např. uvádí ve svých doporučeních Evropská liga proti revmatizmu (EULAR). V druhé části je rozebírána terapie dny, která je rozdělena na období asymptomatické hyperurikemie, akutního dnavého záchvatu, interkritického období a chronické tofózní dny. Asymptomatickou hyperurikemii léčíme pouze při opakovaně vyšších urikemiích než 540 ?mol/l nebo při přítomnosti dalších rizikových faktorů a komorbidit. Pro léčbu akutního dnavého záchvatu je důležitější včasné zahájení léčby než výběr preparátu. Alternativou jsou NSA, kolchicin nebo glukokortikoidy. Nově registrovaným lékem pro léčbu refrakterního akutního zánětu je inhibitor IL1 kanakinumab. Při léčbě hyperurikemie se uplatňují režimová opatření, dietní opatření, abstinence a hypourikemická léčba. K dispozici jsou inhibitory xantinoxidázy alopurinol a febuxostat, který je vhodnější u pacientů se středně závažnou renální insuficiencí. Novým lékem pro léčbu těžké refrakterní tofózní dny je peglotikáza. Klíčové slova: dnavá artritida – kolchicin – nesteroidní antirevmatika, Gout is an inflammatory, metabolically conditioned crystal-induced disease. Prevalence of gout is on the increase. In clinical practice it is frequently wrongly diagnosed and the therapy of acute attacks in particular is not adequate. The first part of the publication discusses diagnostic possibilities of gouty arthritis. First of all the advantage of the analysis of synovial exudate and of direct evidence of crystals in the polarization microscope is emphasized. If the material for crystallographic analysis is not available, it is necessary to use a combination of clinical criteria as specified e.g. in the recommendations of the European League Against Rheumatism (EULAR). The second part focuses on the therapy of gout which is divided into the periods of asymptomatic hyperuricemia, acute gouty attack, intercritical and chronic tophaceous gout. Asymptomatic hyperuricemia is only treated when uricemia greater than 540 ?mol/l occur repeatedly, or when other risk factors and comorbidities are present. In the treatment of acute gouty attack its early start is more important than the choice of a preparation. Alternatives are NSA, colchicine or glucocorticoids. A newly registered medicine for the treatment of refractory acute inflammation is the IL-1 inhibitor canakinumab. The treatment of hyperuricemia involves regimen and diet measures, abstinence and hypouricemic therapy. Available are the xanthine oxidase inhibitors, allopurinol and febuxostat; the latter is better suited for patients with moderate renal insufficiency. Základní ideou dekompresivní kraniektomie (DK) je chirurgické řešení konfliktu vzrůstajícího nitrolebečního obsahu s pevným objemem kostní schránky. Zvětšený nitrolebeční obsah je většinou představován edémem mozku při nejrůznějších patologických stavech. Přestože DK zažívá v posledních letech renesanci a objevily se důkazy o její účinnosti, její použití není dosud paušálně akceptováno. Nejčastější indikací DK je kraniocerebrální poranění. Důkazy o její efektivitě jsou však limitované. Jsou očekávána data o účinnosti sekundární DK u nemocných s refrakterní nitrolebeční hypertenzí po selhání konzervativní léčby. Provedení bifrontální sekundární dekomprese ještě před vyčerpáním konzervativní léčby nepřináší nemocným s difuzním poraněním benefit. Postavení primární DK u akutního subdurálního hematomu je testováno v randomizované studii. Nejsilnější důkazy o efektivitě DK jsou u nemocných s maligním hemisferálním infarktem, přesto frekvence operace stále neodpovídá incidenci maligního hemisferálního infarktu. DK by měla být zvážena u nemocných s trombózou mozkových splavů a refrakterní nitrolebeční hypertenzí, kteří mohou z operace profitovat i při oboustranné mydriáze. DK s evakuací nebo bez evakuace hematomu může snížit mortalitu nemocných s expanzivním supratentoriálním intracerebrálním krvácením, kteří jsou v bezvědomí nebo mají refrakterní nitrolebeční hypertenzi. Dekomprese může přinést benefit selektované podskupině nemocných se subarachnoidálním krvácením, její použití však není v současných doporučeních specifikováno., The fundamental idea behind decompressive craniectomy (DC) is a surgical solution to the conflict of an increasing intracranial content within the fixed-volume intracranial cavity. The expanding intracranial content is caused by brain edema caused by a variety of pathologies. In spite of DC renaissance over the recent years and some evidence of its effectiveness, its use is not yet generally accepted. Traumatic brain injury is by far the most frequent indication for DC. However, the evidence of its effectiveness is limited. Data on the role of secondary decompression in patients with refractory intracranial hypertension after conservative treatment failure are expected to be released soon. Bifrontal early DC is not superior to medical management in patients with diffuse traumatic injury. Primary decompression in patients with acute subdural haematoma has been investigated in a randomized trial. The most conclusive evidence is in patients with malignant middle cerebral artery infarction. In spite of this, the surgery is still underutilized and the frequency of its use does not correspond with the incidence of malignant infarction. When decompression is performed within 48 hours of stroke onset in patients younger than 60 years, it reduces mortality and improves functional outcome. DC is also a lifesaving procedure in patients over 60 years of age for whom it improves chances of survival without total dependency. Decompression should be considered in patients with cerebral venous thrombosis that causes intractable intracranial hypertension. Furthermore, patients with bilateral mydriasis can also profit from the procedure. DC with or without hematoma evacuation might reduce mortality in patients with large supratentorial intracerebral hemorrhage who are in coma or have refractory intracranial hypertension. Even though the DC can be effective in selected subgroups of patients with subarachnoid hemorrhage, current guidelines, do not specify the role of decompression in these patients. Jaterní cirhóza je závěrečným morfologickým stadiem většiny jaterních onemocnění s následným rizikem vzniku dekompenzace a komplikací (portální hypertenze, HCC). V současnosti je však zjevné, že jde o dynamický obousměrný proces s možností další progrese, ale i regrese fibrózy/cirhózy v případě možnosti kauzální léčby základního hepatologického onemocnění. Určení stadia a pokročilosti jaterní fibrózy je naprosto klíčové pro další péči o nemocného, určení prognózy a eventuálně indikaci léčby. V současnosti jsou již preferovanou možností neinvazivní metody detekce jaterní fibrózy (sérové, elastografické), které v této indikaci pomalu nahrazují jaterní biopsii. Tyto metody umožňují přesný odhad prognózy nemocného a především dlouhodobé neinvazivní sledování vývoje jaterního onemocnění. Chronická hepatitida C je jednou z nejčastějších příčin jaterní cirhózy. Vyléčení hepatitidy C je klíčové pro zlepšení prognózy pacientů a snížení rizika rozvoje komplikací. V oblasti léčby tohoto onemocnění se v posledních letech odehrála farmakologická revoluce, nevídaná v jiných oborech interní medicíny. Díky přesnému zmapování a porozumění cyklu virové replikace byla do klinické praxe uvedena řada přímo působících antivirotik, která jednak umožnila po 20 letech z léčby vyřadit interferonové preparáty (řada nežádoucích účinků), ale především zvýšila účinnost léčby HCV do oblasti 95–100 % vyléčených pacientů po 12 týdnech kombinované léčby. Tyto preparáty jsou v podstatě bez nežádoucích účinků a léčba trvá 12–24 týdnů (s možností i zkrácení na 8 týdnů). Jejich hlavní nevýhodou v současne době je extrémně vysoká cena. Klíčová slova: antivirotika – chronická hepatitida C – jaterní cirhóza – léčba, Cirrhosis of the liver is the final morphological stage of most liver diseases with subsequent risk of decompensation and complications (portal hypertension, HCC). At present it is apparent, however, that a dynamic two-way process is involved with a possibility of further progression, but also regression of fibrosis/cirrhosis provided that causal treatment of the basic hepatologic disease is possible. Determining the stage and level of fibrosis progression is absolutely key to further care of the patient, establishment of the prognosis and possibly the treatment indication. At present non-invasive methods of liver fibrosis are the preferred option already, (serum, elastography), which for this indication gradually replace a liver biopsy. These methods allow for an exact estimate of the patient‘s prognosis and first of all long-term non-invasive following of the liver disease development. Chronic hepatitis C is one of the most frequent causes of liver cirrhosis. The healing of hepatitis C is essential for the improvement of patients‘ prognosis and reduction of the risk of complications development. In the field of treatment of this disease a pharmacological revolution has taken place in recent years, unprecedented in the other fields of internal medicine. Due to the exact description and understanding of the cycle of virus replication, a number of direct-acting antiviral drugs have been introduced to the clinical practice, which made it possible to remove interferon preparations (numerous adverse effects) from the treatment after 20 years, but first of all they increased the effect of HCV treatment, reaching approx. 95–100 % healed patients after 12 weeks of the combined therapy. These preparations are essentially free from adverse effects and the treatment lasts 12–24 weeks (with an option of its shortening to 8 weeks). Lumbální spinální stenóza je klinicko‑radiologický syndrom, nejnovější definice zohledňují klinickou manifestaci i anatomické změny (zúžení páteřního kanálu). Za nejvhodnější neinvazivní vyšetření k potvrzení anatomického zúžení páteřního kanálu nebo přítomnosti komprese nervových kořenů u pacientů s klinickým podezřením na lumbální spinální stenózu je považována magnetická rezonance bederní páteře. K přesnějšímu hodnocení radiologického nálezu byla definována řada kvantitativních parametrů popisujících rozměry páteřního kanálu, nicméně korelace těchto parametrů s klinickým obrazem není uspokojivá. Perspektivním postupem ke zvýšení výtěžnosti radiologického vyšetření se jeví zohlednění dynamického aspektu lumbální spinální stenózy a důraz na tíži komprese nervových a cévních struktur v páteřním kanálu s využitím morfologického popisu durálního vaku a jeho obsahu., Lumbar spinal stenosis is a clinical-radiological syndrome; a recent definition covers both the clinical manifestation and the anatomic abnormality (narrowing of the spinal canal). Magnetic resonance imaging is suggested as the most appropriate non-invasive test to confirm the presence of anatomic narrowing of the spinal canal or the presence of nerve root impingement in patients with clinical suspicion of lumbar spinal stenosis. Many numerical parameters describing the dimensions of spinal canal have been defined for more precise assessment of radiological images; correlation between these radiological parameters and clinical symptoms, however, is unsatisfactory. To improve the usefulness of radiological imaging in patients with lumbar spinal stenosis, it seems to be meaningful to take into account the dynamic aspect of lumbar spinal stenosis and to assess neural and vascular tissue impingement based on morphology of the dural sac and its content.
