Přehledové sdělení se věnuje diagnostickému zobrazení akutních aortálních syndromů s důrazem na multidetektorovou výpočetní tomografii a na její význam pro volbu léčby. Diskutovány jsou jednak techniky CT zobrazení včetně CT s EKG synchronizací a dále diagnostické nálezy u disekce aorty, intramurálního hematomu a penetrujícího vředu aorty. Syndrom renálních cyst a diabetu (RCAD), zvaný také HNF1B-MODY či MODY5, je způsoben delecí nebo bodovou mutací genu HNF1B, která vede k depleci transkripčního faktoru HNF1B. Hlavními klinickými součástmi RCAD jsou cystické onemocnění ledvin nebo jiné vývojové anomálie ledvin a diabetes mellitus, který se projeví nejčastěji ve 2. dekádě života nebo později. Poruchy ledvin mohou vést k rozvoji chronické renální insuficience už v dětství nebo mladé dospělosti. K dalším příznakům patří poruchy funkce jater – cholestatický ikterus ve středním věku, ale někdy už novorozenecká cholestáza, atrofie pankreatu s poruchou exokrinní sekrece a některé vrozené anomálie pohlavního ústrojí. Na rozdíl od ostatních forem MODY diabetu rodinná anamnéza nemusí být pozitivní, protože většina odchylek HNF1B vzniká de novo. Na RCAD pomýšlíme zejména u dospělých osob s diabetem a cystickým onemocněním ledvin nebo cholestatickým ikterem a u dětí s cystickým onemocněním ledvin nejasné etiologie, i bez přítomnosti diabetu. Podpůrným nálezem může být hypomagneziemie, která se vyskytuje až u 70 % nositelů poruchy genu HNF1B, a hyperurikemie. Klíčová slova: diabetes mellitus – cholestatický ikterus – HNF1B – MODY – RCAD, The renal cysts and diabetes syndrome (RCAD), also known as HNF1B-MODYor MODY5, is caused by the deletion or point mutation of HNF1B gene which leads to the depletion of HNF1B transcription factor. The main clinical components of RCAD include cystic kidney disease or other developmental anomalies of the kidneys and diabetes mellitus which typically manifests in the second decade of life or later. Renal disorders may lead to the development of chronic renal insufficiency already in childhood or young adulthood. The other symptoms include hepatic impairment - cholestatic jaundice in middle-aged patients, sometimes even neonatal cholestasis, atrophy of the pancreas with the impairment of exocrine pancreatic secretion and some congenital anomalies of the genital tract. As opposed to the other forms of MODY diabetes, the family history may not be positive because most of the deviations of HNF1B appear de novo. We associate RCAD in particular with adults suffering from diabetes and cystic kidney disease and/or cholestatic jaundice and children with cystic kidney disease of unclear etiology, even without the presence of diabetes. A supportive finding may be hypomagnesemia which occurs in up to 70 % of patients diagnosed with HNF1B related disease and hyperuricemia. Capsular bag distension syndrome is a very rare complication of cataract surgery. Most cases occur after uneventful phacoemulsification with continuous curvilinear capsulorhexis and implantation of an intraocular lens in the capsular bag. The entity presents with reduction of visual acuity and myopic shift in the early-to-late postoperative period. Characteristic findings include the distension of the capsular bag due to the accumulation of milky material and the forward displacement of the intraocular lens. We present two cases with an unusually delayed presentation of 6 and 8 years respectively following phacoemulsification, and describe their successful management with Nd:YAG laser posterior capsulotomy. and P. Cíl: Popsat profil nemocných s akutním srdečním selháním (ASS) z pohledu přednemocniční péče v České republice. Metodika: Data pochází z registru Zdravotnické záchranné služby (ZZS) Jihomoravského kraje z období leden 2010–prosinec 2011. Byl proveden konsekutivní sběr nemocných s podezřením na ASS. Vyloučeni byli nemocní po kardiopulmonální resuscitaci nebo zemřelí před příjezdem ZZS nebo pacienti s dominantně extrakardiální příčinou dušnosti. Srovnáni jsou nemocní transportovaní do nemocnice oproti pacientům léčeným ambulantně. Cílovým parametrem je 30denní celková mortalita. Výsledky: Z celkového počtu 131 798 nemocných bylo podezření na ASS vysloveno ve 1 964 (1,5 %) případech. Konečná diagnóza ASS byla nakonec stanovena u 1 310 pacientů, kteří byli transportováni do nemocnice s předpokladem hospitalizace. Ambulantně bylo ošetřeno 35 nemocných. Průměrný věk celé populace byl 78 let, nejčastější komorbiditou byla hypertenze (67,0 %), u téměř poloviny jsou do doby předání do nemocnice přítomny známky plicního edému (46,4 %). Ambulantně léčení nemocní jsou častěji muži (71,4 % vs 49,1 %), mají menší výskyt komorbidit a nevykazují známky těžší respirační insuficience (kyslíková saturace 96 % oproti 87 %, žádné známky plicního edému). Nejčastěji aplikovaným lékem v přednemocniční péči u pacientů s podezřením na ASS je furosemid (74,0 %). Jednoměsíční mortalita souboru dosahovala 20,7 %. Závěr: ASS představuje 1,5 % všech volání ZZS pro akutní potíže, většina nemocných je transportována do nemocnice s předpokladem hospitalizace. Časná mortalita nemocných je vysoká. Methods: Registry data of the Emergency Medical System (EMS) of the South Moravian region of Czech Republic were used. Patients with suspected AHF were enrolled consecutively during the period of January 2010–December 2011. Excluded were patients who died or had cardiac arrest before the arrival of EMS and those who presented with dyspnea of dominantly extra-cardiac etiology. We compared patients who were transported to the hospital with those treated in an outpatient setting. Overall mortality within the first 30 days after the initial EMS contact was assessed as a main endpoint. Results: From the 131,798 of patients who called EMS, 1,964 (1.5%) of cases were suspected of having AHF. The diagnosis of AHF was determined in 1,310 patients who were transported to the hospital; 35 were treated in Outpatients. The mean age of the study population was 78 years, the most common comorbidity was hypertension (67.0%) and almost one half of the study population had signs of pulmonary edema before being admitted to hospital (46.4%). Patients not transported to hospital were more frequently of male gender (71.4% vs. 49.1%), with lower incidence of comorbid conditions and did not have signs of respiratory distress (oxygen saturation 96% vs. 87% in those transported to the hospital; no signs of pulmonary edema). The most common drug used in pre‑hospital care in patients with suspected AHF was furosemide (74.0% patients). The 30-day mortality rate of the whole study population reached 20.7%. Conclusion: AHF represents 1.5% of all emergency calls due to acute symptoms. The vast majority of the patients are transported to the hospital with the assumption of hospitalization. Early mortality rates of these patients are high.
