Diagnostika sclerosis multiplex (SM) sa opiera o dôkaz diseminácie ochorenia v priestore a v čase pri vylúčení iných neurologických ochorení, ktoré môžu byť svojím klinickým priebehom a rádiologickým nálezom SM veľmi podobné. Kým vytvoreniu kritérií na splnenie daných požiadaviek diagnostiky SM sa venuje velká pozornosť, kritériá pre diferenciálnu diagnostiku SM ostávajú v pozadí. Na druhej strane MR kritériá pre disemináciu SM v priestore dosahujú vysokú špecificitu (89%) u tých pacientov, ktorí mali vylúčené iné neurologické ochorenia. Pomerne často vidíme v MR nálezoch nešpecifické poškodenie bielej hmoty mozgu, pre ktoré nemáme jednoznačné vysvetlenie. Sú diagnostickým problémom, umožňujú MR podporu velkého počtu ochorení, a takýmito nálezmi sa doposiaľ užívané kritériá nezaoberajú. Zároveň sa pri MR vyšetrení stretávame s nálezmi demyelinizačných lézií, ktoré svojou veľkosťou a uložením v bielej hmote mozgu pripomínajú SM, ale napr. vek pacienta alebo priebeh jeho klinických ťažkostí nezapadajú do vzorca niektorej z foriem SM (obr. 1A,B). V kontexte uvedeného môžu byť MR nálezy netypické pre SM alebo typické pre iné ochorenia nápomocné v procese diferenciálnej diagnostiky a manažmente pacientov s podozrením na SM. Európska skupina expertov nazvaná MAGNIMS (Magnetic resonance neťwork in multiple sclerosis) definovala na základe vedeckých výskumov a dôkazov práve takéto skupiny ochorení a pre praktickú názornosť i zdôraznenie ich významu sú označené červenou zástavkou., Diagnosis of Multiple Sclerosis (MS) relies on the evidence of disease dissemination in time and space at the exclusion of other neurological diseases that may clinically or radiologically mimic MS. While the establishment of criteria to meet the requirements of the diagnosis of MS is given múch attention, the criteria for differential diagnosis of MS remain in the background. On the other hand, MRI criteria for dissemination in space is of high specificity (89%) in those patients who had ruled out other neurological diseases. Quite often we see in the MRI findings a nonspecific white matter damage, for which we háve no clear explanation. There are a diagnostic problems with it, enabling MRI support for a large number of diseases and šuch findings háve not yet been engaged in the criteria used. Simultaneously on MRI seans we meet with the findings of demyelinating lesions suggestive of MS with their measurement and localisation in the white matter, but for example the age of a patient or his clinical course does not fit the pattern of anyforms of MS (Fig. 1A,B). In the context written above, the MRI findings atypical for MS or typical for other neurological diseases may be helpful in the process of diíferential diagnosis and management of patients with suspected MS.
