Erytrocytární aloimunizace (izoimunizace, hemolytické onemocnění plodu a novorozence – HON) se v současnosti díky rutinní imunoprofylaxi anti-D imunoglobulinem vyskytuje sporadicky. Výskyt atypických aloimunizací non-RhD-antigeny zůstává neměnný, poněvadž není známa profylaxe a ani prevence nezajišťuje stoprocentní účinnost. Pokud se imunoprofylaxe anti-D imunoglobulinem neprovede nebo selže, hrozí vznik hemolytického onemocnění plodu a novorozence. Každý případ je potom unikátní (specifita a typ protilátky, blokující protilátky v séru matky, antigenní výbava erytrocytů plodu, gestační stáří při začátku onemocnění atd). Pokroky v genové diagnostice, imunohematologii a dopplerovské ultrasonografii významně snižují frekvenci nutných invazivních zákroků v diagnostice a léčbě senzibilizovaného těhotenství. Intraumbilikální přísně intravenózní transfuze pod ultrasonografickou kontrolou je první metodou volby léčby případů s těžkou anémií plodu i při rozvinutém hydropsu. Konzultace a léčba se soustřeďují do center s perfektní porodnickou a neonatologickou péčí a nepřetržitou operační pohotovostí. The occurrence of atypical alloimmunizations related to non-RhD-antigens remains unchanged, as there is no prophylaxis known and even the prevention does not provide one hundred percent efficiency. When anti-D immunoglobulin immunoprophylaxis is not undertaken or fails, the threat of hemolytic disease of the fetus and newborn arises. Every case is then unique (specificity and type of antibody, blocking antibodies in maternal serum, antigenic make-up of fetal erythrocytes, gestational age at the disease onset etc). The advancements in genetic diagnostics, immunohematology and Doppler ultrasonography significantly reduce the frequency of necessary invasive interventions in the diagnostics and treatment of sensibilized pregnancy. Intraumbilical strictly intravenous transfusion under ultrasound guidance is the first-line method of therapy for cases of severe fetal anemia as well as advanced hydrops. Consultations and treatment are provided in the centres for comprehensive obstetrical and neonatal care with continuous intervention preparedness. Vrozené vývojové vady postihují dle údajů WHO přibližně 3 % novorozenců a jsou každoročně příčinou úmrtí přibližně 270 000 novorozenců na světě. Dle aktuálního doporučení ČGPS pro péči o fyziologické těhotenství by každé těhotné ženě měl být v 1. trimestru nabídnut screening nejčastějších morfologických a chromozomálních vrozených vývojových vad plodu a poskytnuta informace o metodách screeningu, poskytovatelích i formě úhrady. Preferován by měl být kombinovaný screening v 1. trimestru těhotenství, zatím však není hrazen z prostředků veřejného zdravotního pojištění. Cílem článku je poskytnout lékařům přehlednou informaci o současných možnostech screeningu vrozených vývojových vad plodu, která jim může usnadnit vysvětlení a doporučení screeningových testů těhotné ženě., Congenital anomalies affect an estimated 3 % of infants and result in approximately 270 000 newborn’s death during the first 28 days of life every year in the world. Following current recommendation of the Czech Society of Gynaecology and Obstetrics for the care for low-risk pregnancies every pregnant woman should be offered the screening for the most common congenital fetal defects in the first trimester. She should be also informed about the methods of screening, including their cost and providers. First trimester screening by combined test should be recommended the most, although its cost cannot be covered by the health insurance yet. The aim of this article is to provide an overview about the methods of screening for congenital fetal defects with comparison of their performance. Ultrazvukové měření děložního hrdla představuje dobře zavedenou metodu predikce předčasného porodu. Nicméně sensitivita a pozitivní predikční hodnota ultrazvukové cervikometrie zůstává stále relativně neuspokojivá. Proto je snaha upřesnit predikci předčasného porodu využitím dalších metod v kombinaci s ultrazvukovou cervikometrií. Jedna skupina výzkumných prací se zabývá hodnocením různých parametrů z cervikovaginálního sekretu – nejvýznamnější představuje fetální fibronektin (fFN). Pozitivní test na fFN ukazuje relativně nízkou pozitivní predikční hodnotu stran předčasného porodu, nicméně výhodou je vysoká negativní predikční hodnota – bezmála 95%. Data z posledních studií ukazují užitečnost hodnocení fFN spolu s cervikometrií. Nedávné objevy ukázaly, že T regulační lymfocyty (CD4+CD25+CD127 - dále zkráceně Treg lymfocyty) mohou hrát důležitou roli v patofyziologii předčasného porodu. Cílem výzkumu na našem pracovišti je vyhodnotit vztah mezi hladinou Treg lymfocytů a ultrazvukovou délkou děložního hrdla k riziku předčasného porodu. Pilotní data z naší práce ukazují, že kombinované hodnocení uvedených parametrů představuje mnohem lepší možnost predikce předčasného porodu než hodnocení obou parametrů samostatně. Je však potřeba dalšího výzkumu, který bude mít za cíl vyhodnotit korelaci mezi sníženými hladinami Treg lymfocytů a předčasným porodem., Ultrasound measurement of the uterine cervix represents a well-established method in the prediction of preterm labour. However, the sensitivity and positive predictive value of ultrasound cervicometry still remain rather unsatisfactory. Hence, the effort to provide more specific predictions of premature labour using other applicable methods in combination with ultrasound cervicometry. One group of studies have focused on determining various parameters from cervicovaginal secretion - the most important one being fetal fibronectin (fFN). A positive fFN test in the birth canal shows a relatively low positive predictive value in terms of premature birth; however, it does provide the advantage of a high negative predictive value, which is about 95%. Data from latest studies show the usefulness of fFN in combination with cervicometry. Recent discoveries suggest that T regulatory lymphocytes (CD4+CD25+CD127 - Tregs) may play an important role in the pathophysiology of preterm labour. The purpose of our study was to assess the relationship between maternal circulating Tregs and the sonographically measured uterine cervix and the risk of preterm labour. We can show for the first time that the combined assessment of Treg cell count and cervical length provides a much better prediction of preterm delivery that either parameter used on its own. To study defects and chromosomal abnormalities of the fetus, we retrospectively analyzed results of comprehensive dynamic survey of 26,404 pregnant women aged 1850 years old at 6 to 40 weeks of pregnancy. Of them, 25,956 (98.3%) women had physiological course of pregnancy, 448 (1.7%) women had abnormal pregnancy. For the diagnosis of fetal defects, we carried out ultrasound, biochemical, invasive and cytogenetic studies. The results of study showed that the majority of fetal defects and pathological course of pregnancy was noted in women at the age of 2125 years old, since at this age period women have the highest number of pregnancies. At the older age, we noted a gradual decrease in the number of pregnant women, as well as the number of abnormalities of the fetus. Based on the analysis of the results obtained, we have developed an algorithm for early fetal ultrasound examination. Toxoplasmóza je jedna z nejčastějších zoonóz u nás. Patří mezi onemocnění, která mohou při primoinfekci v graviditě ohrozit vývoj plodu či způsobit mnohočetná poškození, často manifestovaná až po narození dítěte. Závažnost postižení, jeho variabilita a pravděpodobnost vzniku závisí na době, kdy se těhotná žena nakazila. V ČR máme k dispozici několik diagnostických metod. Základem je sérologické vyšetření (protilátky celkové i jednotlivých tříd, avidita IgG, komparativní imunoblot), prováděné z krve či plodové vody. Možný je i přímý průkaz DNA Toxoplasma gondii pomocí PCR z krve, plodové vody či po porodu z placenty. Prenatálně se provádí izolační pokus na myši (inokulací plodové vody). Pomocnou metodou je také ultrazvuk či MRI v průběhu těhotenství, se zaměřením na případnou fetopatii. Po narození je dítě z rizikové gravdity podrobně vyšetřeno a dlouhodobě sledováno. Terapii lze podat prenatálně i postnatálně, o jejím efektu se vedou diskuze. Základními preparáty jsou spiramycin, pyrimethamin a sulfadiazin (doplněny kyselinou folinovou). V článku jsou probrány indikace k jednotlivým vyšetřením a doporučeny nejvhodnější postupy v diagnostice, léčbě a také prevenci tohoto onemocnění u těhotných. Text se podrobně věnuje i interpretaci možných výsledků a obsahuje vyjádření k preventivnímu vyšetření těhotných žen u nás., Toxoplasmosis is one of the most common zoonoses in the Czech Republic. Toxoplasma infection during pregnancy can affect fetal development and cause multiple damages that often do not manifest until after the birth. The severity of pediatric disability, its variability and the probability of occurrence depend on the time of maternal infection. Serological diagnosis of toxoplasmosis in pregnant women is performed in two steps: first, total anti-Toxoplasma antibodies are detected, most commonly by the complement fixation test (CFT) combined with IgG ELISA. In the case of a negative result, the woman can be retested 1-3 months later. Low CFT titers show latent infection with a minimal risk of primary infection during pregnancy. When CFT titers of 1:64 or higher are obtained, the second step follows. Positivity in the IgM and IgA tests and low IgG avidity can confirm a recent infection. The diagnosis of fetal infection is based on the detection of anti-Toxoplasma antibodies, and of IgM antibodies in particular, and/or on the detection of Toxoplasma in amniotic fluid by PCR or the mouse test. Other helpful methods are fetal ultrasound or MRI focusing on fetopathy. Each child born from a high-risk pregnancy is thoroughly examined and followed up on a long-term basis. The detection of neonatal anti-Toxoplasma IgM antibodies and a neonatal IgG profile differing from the maternal IgG profile in comparative Western blot are the major markers of congenital infection. Toxoplasma infection can be treated prenatally and postnatally and the therapeutic outcomes are still being considered. The first-line options for congenital toxoplasmosis are spiramycin, pyrimethamine and sulfadiazine with folinic acid. The article discusses the indications for screening and recommends best practices for the diagnosis, treatment and prevention of toxoplasmosis during pregnancy. Císařský řez je po epiziotomii nejčastější porodnickou operací. Ve vyspělých státech císařský řez dosahuje až 21 % všech porodů. Na stoupajícím trendu se mimo jiné podílí zvýšený počet indikací císařského řezu po předchozím císařském řezu. Obava z děložní ruptury mnoho porodníků i těhotných odradí od pokusu o spontánní porod. Cílem toho článku je shrnout dostupné informace o významu ultrazvukových známek hrozící ruptury děložní, které by umožnily snazší rozhodování v tom, zda přikročit při ukončení těhotenství k vaginálnímu porodu po předchozím císařském řezu (VBAC – vaginal birth after cesarean section) nebo k elektivnímu opakovanému císařskému řezu (ERCS – elective repeat cesarean section)., Cesarean section is after episiotomy the most frequent surgical procedure in obstetrics. In many countries, cesarean section procedure has reached up to 21 % of deliveries. Recently the number of women undergoing ERCS (elective repeat cesarean section) has increased due to indication of a prior cesarean section in anamnesis with persisting decrease of a trial of vaginal birth. Uterine rupture discourages most of obstetricians in their decisions considering benefits and risks of VBAC (vaginal birth after cesarean section). Cíl práce: Podat přehled o diagnostických metodách a doporučených postupech pro vcestné cévy (vasa praevia) v těhotenství. Typ studie: Souhrnný článek. Metodika: Analýza dostupných literárních zdrojů. Závěr: Vasa praevia patří mezi závažné těhotenské komplikace s nízkou incidencí. Pokud nejsou vcestné cévy diagnostikovány prenatálně, představují vysoké riziko pro plod. Včasná prenatálně stanovená diagnóza významně snižuje perinatální morbiditu a mortalitu novorozence. Diagnostickou metodou volby je ultrasonografické vyšetření s použitím barevného dopplerovského zobrazení. V případě stanovení diagnózy vasa praevia prenatálně je doporučováno ukončení těhotenství elektivním císařským řezem dříve, než dojde k nástupu porodní činnosti nebo odtoku plodové vody., Objective: To report up–to date knowledge on diagnostic methods and recommended practices for vasa praevia in pregnancy. Study design: Review. Methods: Analysis of available literature resources. Conclusion: Vasa praevia is among the major pregnancy complications with a low incidence. If they are not diagnosed prenatally, pose a high risk to the fetus. Early prenatally established diagnosis significantly decrease perinatal morbidity and mortality of the newborn. The diagnostic method of choice is ultrasound scan using colour Doppler image.
