Práce prezentuje prenatální diagnostiku, možnosti terapie a následnou péči u plodů a novorozenců s kongenitálním chylothoraxem na podkladě zhodnocení 14 případů na naší klinice v letech 1993 až 2012. Povšechný hydrops plodu byl popsán ve 4 případech (29 %), v 10 případech (71 %) byla provedena odlehčovací amniodrenáž a ve 3 případech (21 %) prenatální odlehčovací punkce hrudníku. Spontánně bylo porozeno 11 plodů (78 %), hrudní drenáž byla nezbytná v průměru 9 dnů po porodu. Mortalita dosáhla v souboru 29 % (4 případy). V článku popisujeme vyšetřovací algoritmy, terapeutické prenatální i postnatální postupy a je zdůrazněna nutnost mezioborové spolupráce., The work presents prenatal diagnosis, treatment and care options in fetuses and neonates with congenital chylothorax based on a detailed evaluation of 14 cases recorded in our department between years 1993 and 2012. Generalized fetal hydrops was described in 29% of cases, therapeutic amnio drainage performed in 71% and prenatal thoracentesis in 21% of cases. 78% of fetuses were delivered spontaneously; thoracic drainage was necessary nine days after birth in average. Cíl. Retrospektivně zhodnotit efekt a dlouhodobou průchodnost transjugulární intrahepatální portosystémové spojky (TIPS) u nemocných s trombózou jaterních žil. Metoda. V letech 1992-2011 jsme vytvořili TIPS celkem u 38 pacientů s trombózou jaterních žil nekontrolovatelnou medikamentózní léčbou. Léčeno bylo devět mužů a 29 žen ve věku 13-76 let (medián 33 let), v šesti případech se jednalo o děti. Predisponující trombofilní stav byl prokázán u 84,2 % osob. Klinická forma Buddova-Chiariho syndromu byla v šesti případech akutní, v 18 případech subakutní a ve 14 případech chronická. Indikací k výkonu byl u 31 nemocných ascites, u pěti pacientů jaterní selhání a u dvou krvácení do zažívacího traktu. Zákroky byly prováděny standardní technikou v analgosedaci nebo v celkové anestezii. U 17 pacientů jsme do kanálu TIPS implantovali nepotažený stent nebo kombinaci potaženého a nepotaženého stentu, u 21 nemocných jsme zkrat vyztužili pouze pro TIPS dedikovaným potaženým stentem (stentgraftem). Sledování průchodnosti zkratu po výkonu bylo založeno na pravidelných ultrasonografických, klinických a laboratorních kontrolách, při prokázané poruše průchodnosti byla provedena reintervence. Výsledky. Celková doba sledování souboru byla 8 dní - 207 měsíců (medián 52 měsíců). Technického úspěchu jsme dosáhli u 100% nemocných, hemodynamická úspěšnost výkonu byla 86,8 %, s poklesem mediánu portosystémového gradientu z 23 mm Hg na 8 mm Hg, klinicky bylo dosaženo ústupu obtíží u 89,5% nemocných. Významné komplikace jsme zaznamenali celkem u sedmi osob, žádný pacient nezemřel v přímé souvislosti se zákrokem. V průběhu sledování zemřelo celkem jedenáct nemocných (29 %), přežití souboru za 1 rok a za 5 let od TIPS bylo 85,3 % a 65,4 %. Během doby sledování jsme provedli celkem 50 reintervencí pro dysfunkci spojky, průměrný počet reintervencí přepočtený na jednoho nemocného za dobu 5 let byl 1,65 výkonu u pacientů se zkratem vytvořeným stentem a 0,67 výkonu u nemocných, u kterých byl primárně použit stentgraft. Ve skupině se zavedeným nekrytým stentem byla primární průchodnost 52,9% za 1 rok a 20 % za 5 let od výkonu, u skupiny s implantovaným potaženým stentem byla jednoletá primární průchodnost 80 % a pětiletá primární průchodnost 33,3%. V průběhu sledování jsme zároveň u osmi pacientů provedli angioplastiku dolní duté žíly pro její symptomatickou stenózu, u šesti z těchto nemocných byl do dolní duté žíly zaveden stent. Závěr. Vytvoření TIPS je velmi efektivní metodou léčby pacientů s trombózou jaterních žil. Použití pro TIPS dedikovaných potažených stentů vede knižšímu počtu dysfunkcí zkratu s nižším počtem reintervencí nezbytných k obnově průchodnosti, nutností je však současná důsledná antikoagulační léčba a léčba základního hematologického onemocnění., Aim. To retrospectively evaluate the efficacy and long-term patency of transjugular intrahepatic portosystemic shunt (TIPS) in patients with hepatic vein thrombosis. Method. In years 1992-2011, totally 38 patients with hepatic vein thrombosis resistant to medical therapy underwent TIPS creation in our department. We treated 9 males and 29 females, age 13-76 years (median 33 years), 6 patients were children. The underlying thrombophilic state was confirmed in 84.2% of treated patients. The Budd-Chiari syndrome was acute in 6, subacute in 18 and chronic in 14 cases. Ascites was indication for TIPS in 31 patients, liver failure in 5 and gastrointestinal bleeding in 2 persons. All the procedures were performed in anal-gosedation or under general anestesia using standard technique. In 17 patients, we implanted non-covered stent or combination of non-covered and covered stents during TIPS creation, in the other 21 patients only the TIPS dedicated covered stents had been used. TIPS patency during follow-up after the procedure was based on regular ultra-sonographic, clinical and laboratory examinations. In case of shunt dysfunction, a rein-tervention was performed. Results. The total follow-up period was 8 days - 207 months (median 52 months). The technical success rate was 100%, hemo-dynamical success rate 86.8% with median portosystemic gradient decrease from 23 mm Hg to 8 mm Hg, and clinically we were succesfull in 89.5% of patients. In 7 patients, severe complications occurred during shunt creation, but none of the patients died in direct connection with the procedure. Totally 11 patients (29%) died during follow-up period, the 1-year and 5-years survival rates were 85.3% and 65.4%, respectively. Due to TIPS dysfunction, totally 50 reinterventions were needed, the average 5-year reintervention rate per patient was 1.65 procedures in the bare stent group and 0.67 porcedures in the covered stent group. In the non-covered stent group, we achieved primary patency rates 52.9% 1 year and 20% 5 years after TIPS creation, in the covered stent group the 1-year primary patency rate was 80% and 5-year primary patency rate 33.3%. Angioplasty of symptomatic stenosis of the inferior vena cava was performed in 8 patients during follow-up, in 6 of these patients also stent implantation was necessary. Conclusion. TIPS creation is a very effective treatment of patients with hepatic vein thrombosis. Typickým patologicko-anatomickým rysem emfyzému je permanentní rozšíření dýchacích cest distálně od terminálních bronchiolů spolu s destrukcí plicního parenchymu, bez výraznější fibrózy. Chirurgická léčba nabízí tři typy intervencí: bulektomii (resekci velkých bul utlačujících zdravější část plíce), volumredukční operaci plic (LVRS) a plicní transplantaci (jedno či lépe oboustrannou). Alternativou LVRS u inoperabilních nemocných mohou být některé bronchoskopické techniky - metody bronchoskopické volumredukce (BVR). There are three types of surgical treatment: bullectomy (resection of large emphysematic bulls which oppreses healthy lung parenchyma), volumreduction and lung transplantation (once or double - faced). When surgery is not possible there are some bronchoscopic techniques - bronchoscopic volumereduction. Few studies concerning the occurrence of cow’s milk allergy with the use of double-blind, placebo controlled food challenge test in adolescents and adult patients suffering from atopic dermatitis exist. Aim: To evaluate the occurrence of cow’s milk allergy in adolescents and adults suffering from atopic dermatitis. Method: Altogether 179 persons suffering from atopic dermatitis were included in the study: 51 men and 128 women entered the study with the average age of 26.2 (s.d. 9.5 years). Complete dermatological and allergological examinations were performed. Results: The positive results in specificIgE and in skin prick tests were recorded in 12% of patients. According to the open exposure tests and double-blind, placebo controlled food challenge tests these patients are only sensitized to cow’s milk without clinical symptoms of allergy. Double-blind, placebo controlled food challenge test confirmed food allergy to cow milk only in one patient (worsening of atopic dermatitis), the oral allergy syndrome was observed in another one patient, occurrence of this allergy was altogether 1.1%. Motivation Until recently it was considered that 65 years is cutoff for defining patients as elderly, but newer reports indicate that this age limit shift to 70 years of age. Elderly patients with advanced non small cell lung cancer, associated comorbidities and poor performance status represent a specific population and a challenge for use of chemotherapy. Primary aim was to evaluate the impact of mono therapy with oral etoposide on overall survival in elderly patients (≥ 70 years of age) with advanced non small cell lung cancer and poor performance status (PS) ≥ 2 (clinical stage IIIb and IV ), and as well to evaluate tolerability of this therapy. Secondary aim was to evaluate response rate. Methods Retrospectively, medical records of 79 female and male patients with advanced non small cell lung cancer and poor performance status treated with oral etoposide (2x25 mg 20 days/10 days pause) in period from 2007 till 2010 were checked for relevant data. Data regarding demographics, performance status, overall survival, response rates and drug toxicity were collected. For statistical analysis we used Pearson chi-square test, T-test, Kaplan-Meier product limited method and Cox regression. Results Median overall survival (OS) was 31 weeks, in patients with PS 2 overall survival was 34 weeks, and in group with PS 3 was only 24 weeks. Partial response was registered in 20.2% of patients, stable disease in 41.85 % and disease progression in 38% of patients. Treatment was well tolerated, febrile neutropenia and toxic deaths were not registered. Toxic effects didn't have statistically significant influence on OS. Účel studie: Hlavním cílem studie bylo odhadnout dosažení patologické kompletní odpovědi nemocných trpících lokálně pokročilým adenokarcinomem rekta po předoperační (neoadjuvantní) chemoradioterapii (CHRT) s využitím individuálních farmakokinetických parametrů 5-fluorouracilu (5-FU). Sekundárním cílem bylo hodnocení bezpečnosti a snášenlivosti léčby. Tato otevřená prospektivní studie podporovaná grantem IGA NS 9693–04/2008 zahrnula 34 dospělých s lokálně pokročilým karcinomem konečníku ozařovaných do celkové dávky 50,4 Gy ve 28 frakcích po 1,8 Gy 10–15 MV paprsky v režimu 5 + 2 (5 dní radioterapie, 2 dny volno) s kontinuálně podávaným 5-FU v i. v. infuzi 200–1000 mg/m2 po dobu 4–5 týdnů kontinuálně 7denní infuzí. Chirurgická resekce následovala 4–6 týdnů po ukončení CHRT a po klinickém restagingu. Hodnocena byla klinická a patologická odpověď na chemoradioterapii pomocí zobrazovacího (MR) a histopatologického vyšetření, vyjádřená v % jako reziduální choroba. Výsledky a závěr: Výsledek dokládá vztah mezi velikostí kumulativní dávky 5-FU a kumulativní AUC 5-FU (r = 0,61, p < 0,001). Podobný vztah byl prokázán mezi kumulativní AUC 5-FU a metabolickým poměrem (poměr dosažených plazmatických koncentrací neaktivního metabolitu dihydrofluorouracilu (5-FUH2) ku koncentracím 5-FU, r = -0,80, p < 0,0001). Kumulativní AUC korelovala se stupněm odpovědi na léčbu (r = – 0,53, p < 0,005) a určovala také stupeň toxicity léčby. Pokud bychom chtěli dosáhnout kompletní patologickou odpověď, pak by denní dávka 5-FU měla být u středně rychlého metabolizéra >350 mg/m2 a kumulativní AUC1–39 dní > 50 mg/L*h. U žádného z nemocných nebyla nalezena mutace v genu pro dihydropyrimidindehydrogenázu (DPD) a multidrug resistance-1 protein (MDR-1), přesto byla nalezena vysoká interindividuální variabilita v dosažených plazmatických koncentracích 5-FU, a to i s ohledem na cirkadiální rytmus kinetiky léčiva., Background and Purpose: The main goal of the present study was to estimate the early patients´response following neoadjuvant chemoradiotherapy (CHRT) based on 5-fluorouracil (5-FU) with curative aim in relation to plasma concentrations and pharmacokinetic parameters of 5-FU. Secondary objectives included evaluation of the safety and tolerability of the regimen. Patients and Methods: This open prospective study enrolles 34 adult patients with locally advanced rectal cancer, who received 5-FU 200 -1000 mg/m2 administered as a continuous i. v. infusion over 4–5 week and radiotherapy delivered with 10–15 MV photon beams at 1.8 Gy/fraction up o 50.4 Gy in 28 daily fractions for 5 days a week. Surgical resection with curative aim followed 4–6 weeks after the completion of CHRT and clinical restaging. Pathologic response evaluation and the rate of tumor regression was evaluated using tumor downstaging by MR, histopathological staging, and expressed as residual disease (%). Results and Conclusion: The outcome evidenced the correlation between the cumulative 5-FU dose and cumulative AUC of 5-FU (r = 0.61; p < 0.001). The similar relationship was demonstrated between the cumulative AUC and metabolic ratio (the plasma concentration od inactive metabolite dihydrofluotouracile 5-FUH2 to 5-FU; r = -0.80; p < 0.0001). The cumulative AUC was correlated with tumor regression rate (r = -0.53; p < 0.005) and determined toxicity grade. To reach pCR, the daily dose of 5-FU in patient with average metabolic ratio of 5-FUH2/5-FU should be >350 mg/m2 and the cumulative AUC1–39days > 50 mg/L*h. INTRODUCTION: Autologous stem cell transplantation (ASCT) became standard of care for patients with multiple myeloma (MM) under the age of 65 years. We routinely perform ASCT for newly diagnosed MM since 1996 in our department. PATIENTS AND METHODS: We retrospectively analyzed all 285 transplants in 185 patients done for MM from January 1996 till December 2010. We analyzed overall survival (OS) and progression-free survival (PFS) regarding conditioning, stage, complete or very good partial remission (CR, VGPR) achievement, renal impairment, single vs. double transplant. RESULTS: Estimated 10-years survival of the whole set of patients is 39% (median survival 95 months). Patients with renal impairment show same OS as those without (p = 0.22). Patients show similar overall survival and event free survival regardless of type of transplant. We observed better outcome in terms of overall survival in patients treated with new drugs (p = 0.0014). Reaching CR or VGPR was not translated into better OS (p = 0.30) and EFS (p = 0.10). Also stage of the disease and whether single or double transplant was used did not make any significant difference in the outcome. CONCLUSION: Stem cell transplantation greatly improved outcome of patients with MM. Poor outcome of allogeneic transplantation in our group of patients is related to high transplant related mortality (20% vs. 0%) and unexpected high relapse rate. There is a trend towards better survival, when new drugs are incorporated at any time in the course of the disease. Cíl studie: Akutní selhání ledvin vyžadující hemodialýzu je přítomno u 10 až 20 % všech pacientů s mnohočetným myelomem (MM) a je spojeno s „násobně“ zvýšenou morbiditou a mortalitou. Podkladem ledvinného poškození je nejčastěji odlitková nefropatie jako výsledek působení a ukládání monoklonálních volných lehkých řetězců (VLŘ), jejichž vysoké koncentrace nalézáme v séru pacientů. Rychlá redukce koncentrace VLŘ hraje klíčovou úlohu ve zlepšení ledvinných funkcí. Důkazy o účinnosti dosud užívané plazmaferézy jsou kontroverzní. V současnosti se jako nejlepší možnost jeví použití hemodialýz s „high cut-off“ dialyzátory, které efektivně odstraňují VLŘ ze séra pacientů a zlepšují tak dlouhodobě ledvinné funkce, pokud jsou kombinovány s efektivní chemoterapií. Cílem naší práce bylo zhodnocení účinnosti kombinované léčby chemoterapií s bortezomibem a hemodialýzami s použitím „high cut-off“ dialyzátorů u pacientů s nově diagnostikovaným MM s akutním selháním ledvin. Materiál a metody: Soubor tvořili čtyři pacienti s nově diagnostikovaným mnohočetným myelomem (MM) s akutním selháním ledvin vyžadujícím dialyzační léčbu se sérovou hladinou „dominantního“ VLŘ nad 500 mg/l. Všichni podstoupili kromě chemoterapie režimem s bortezomibem také sérii hemodialýz s použitím „high cut-off“ membrán (Theralite™ 2100, Gambro Dialysatoren GmbH, Hechingen, Německo). Na začátku a na konci každé hemodialýzy byly změřeny hladiny volných lehkých řetězců (VLŘ) v séru a v dialyzátu turbidimetricky (Freelite™, The Binding Site,UK) a také sérové hladiny kreatininu a dalších parametrů. Ze získaných dat byla následně vypočtena celková procentuální redukce hladiny VLŘ za celou dobu léčby i při jednotlivých procedurách. Sledována byla dále efektivita terapie a výsledný stav pacientů. Výsledky: Bylo provedeno celkem 27 hemodialýz s „high cut-off“ membránami, přičemž jednotliví pacienti podstoupili od 3 do 11 procedur. Současně byla zahájena chemoterapie bortezomibovým režimem. Průměrná procentuální redukce hladiny VLŘ v séru pacientů po ukončení terapie dosáhla 93,7± 8,8 %. Průměrný pokles hladiny VLŘ při jednotlivých procedurách pak byl 55,30 ± 20,04 %. Průměrná clearance VLŘ dosáhla 62,7 ± 37,2 ml/min resp. 29,0 ± 26,1 ml/min na počátku resp. na konci terapie hemodialýzami. U třech pacientů došlo k návratu ledvinných funkcí a nadále žijí bez závislosti na dialýze, jedna pacientka zemřela v průběhu léčby v důsledku ischemické cévní mozkové příhody nesouvisející s dialýzami. Závěr: Prokázali jsme, že hemodialýza s použitím „high cut-off“ membrán je účinná metoda k rychlému odstranění VLŘ i ke zlepšení ledvinných funkcí při akutním poškození ledvin u mnohočetného myelomu v kombinaci s chemoterapií s použitím bortezomibu. Pro definitivní a precizní zhodnocení přínosu této léčebné modality a volbu optimálního počtu a trvání hemodialýz bude nutné získání dalších zkušeností., Objective: Renal failure requiring haemodialysis occurs in 10-20 % of all multiple myeloma (MM) cases and it is associated with a greatly increased morbidity and mortality. The main cause of renal injury is cast nephropathy, as a direct consequence of high concentrations of monoclonal free light chains (FLCs) in patients´ sera. Rapid reduction of FLCs levels plays a key role in renal recovery. Evidence of benefit of plasmapheresis in removing serum FLC has been controversial. According to the recent experience, haemodialysis using high cut-off dialysers reduces effectively the level of FLCs concentrations and improves renal functions in long term when combined with an effective chemotherapy. In our single centre study we assessed efficacy of parallel treatment for patients with new diagnosed MM and acute renal failure due to excess of serum FLCs with combination of bortezomib-based chemotherapy and haemodialysis using high cut-off dialysers. Material and methods: The studied group consisted of 4 patients with new diagnosed MM and acute renal failure requiring haemodialysis and with serum FLCs levels („dominant FLC“) above 500 mg/l. They all were treated with bortezomib- based chemotherapy and simultaneously they underwent haemodialysis sessions with the high cut-off membranes (Theralite™ 2100, Gambro Dialysatoren GmbH, Hechingen, Germany). At the beginning and before the end of each session, free light chain levels in serum as well as in dialysate were measured (Freelite™, The Binding Site,UK). Percentage reduction in serum free light chains in the hemodialysis session as well as at the end of treatment was determined by calculation from the reached data. The efficacy of the treatment and clinical state of the patients were observed as well. Results: 27 sessions of haemodialysis with high cut-off membranes (HCO HD) were performed, each patient underwent between 3 and 11 sessions. At the same time, patients underwent bortezomib-based chemotherapy. Free light chain levels decreased by a mean of 93.7± 8.8 % between treatment onset and completion, while the mean percentage of reduction of FLC levels per session was 55.30 ± 20.04 %. Mean clearance of FLCs was 62.7 ± 37.2 ml/min resp. 29.0 ± 26.1 ml/min at the beginning resp. at the end of HCO HD. Renal function was recovered in 3 patients, who are alive and dialysis-free. One patient died of ischemic stroke not related to haemodialysis procedure during treatment. Conclusion: We demonstrated haemodialysis using high cut-off membranes is an effective method to remove free light chains rapidly and to recover renal functions in acute renal injury in multiple myeloma in combination with chemotherapy using bortezomib. V léčbě uveálních melanomů se v některých případech používá radiochirurgie CyberKnifem. Histologickými a imunohistologickými změnami v nádorové tkáni po terapii se zabývá poměrně málo autorů. V tomto sdělení uvádíme výsledky vyšetření dvou takových případů. U obou pacientů byla provedena enukleace bulbu pro bolestivý glaukom. U prvého pacienta šlo o recidivující melanom, který byl ošetřen gama nožem před deseti lety. Pro lehkou progresi ložiska melanomu bylo provedeno ošetření CyberKnifem celkovými dávkami 45Gy ve 3 frakcích. Po necelých pěti měsících po ozáření bulbu došlo k regresi recidivujícího nádoru z původní prominence 1,4 mm v ploše 6 mm × 0,6 mm na prominenci 0,3 mm v ploše 3 mm × 0,3 mm. Histologicky byla zaznamenána rezidua řídce celulárního ložiska regresivně změněných nádorových buněk a přítomnost reaktivních změn optického nervu. Druhý pacient byl měsíc po radioterapii CyberKnifem s následnou komplikovanou operací katarakty a s enukleací bulbu bez regrese – tedy zániku nádorových buněk. Velikost tumoru 11,1 mm × 15mm s prominencí 9,3mm přetrvávala i po ozáření dávkou 40Gy/1Fr. V histologickém vyšetření byly nádorové buňky bez patologických změn. V imunohistochemickém vyšetření jsme pozorovali u prvého pacienta vysokou difuzní expresi proteinu bcl-2 až do výše 75 % a méně než 1% pozitivitu ve vyšetření s proliferačním markerem Ki-67. U druhého případu byla situace odlišná, s výraznou až 75% pozitivní expresí bcl-2 proteinu v nádorových buňkách v povrchových vrstvách tumoru a s klesajícím počtem pozitivní exprese v hlubokých vrstvách nádoru, kde dosahovala maxima do 25 %. Proliferační aktivita s pozitivní jadernou expresí v Ki-67 vzrůstala od povrchu, kde se pohybovala v rozmezí 1–3 % do hloubky nádoru a k jeho bázi, kde dosahovala až 5 %. Naše pozorování ukazuje na výraznou redistribuci genetické aktivity uvnitř nádoru. Ta se projevuje „overexpresí“ bcl-2 proteinu a u druhého případu je patrná rozdílná pozitivita bcl-2 proteinu v různých vrstvách nádoru. Vztah tohoto fenomenu k radioterapii není dosud jasný., Radiosurgery with CyberKnife is used in the treatment of uveal melanomas in some cases. Histological and immunohistological changes in tumour tissue following treatment have been addressed by relatively few authors. This paper presents the results of investigations of two such cases. Enucleation of the eye was performed for painful glaucoma in both patients. Patient 1 had a recurrent melanoma treated with a Gamma Knife ten years ago. Due to slight progression in the melanoma site, treatment with CyberKnife was used with total doses of 45 Gy in 3 fractions. Some five months following eye irradiation, there was regression of the recurrent tumour from the original height of 1.4 mm in an area of 6 mm x 0.6 mm to that of 0.3 mm in an area of 3 mm x 0.3 mm. Histologically, residua of a sparsely cellular locus of regressively altered tumour cells and the presence of reactive changes in the optic nerve were noted. Patient 2 underwent radiotherapy with CyberKnife one month prior, with subsequent complicated cataract surgery and enucleation of the eye without regression, i.e. destruction of tumour cells. The size of the tumour of 11.1 mm x 15 mm with a height of 9.3 mm remained unchanged even after irradiation with a dose of 40 Gy/1 fr. No pathological changes in the tumour cells were seen on histological examination. In patient 1, immunohistochemistry showed high diffusion expression of the bcl-2 protein of up to 75 % and less than 1 % positivity on examination with the proliferation marker Ki-67. In patient 2, the situation was different, with significant, as high as 75 % positive expression of the bcl-2 protein in tumour cells in the superficial tumour layers and a decreasing rate of positive expression in the deep tumour layers where it reached no more than 25 %. The proliferation activity with positive nuclear expression of Ki-67 increased from the surface, where it ranged from 1 % to 3 %, to the depth of the tumour and toward its base, where it reached up to 5 %. Our observation suggests significant redistribution of genetic activity within the tumour. It is manifested by overexpression of the bcl-2 protein and, in patient 2, different bcl-2 protein positivity in the various tumour layers is apparent. We report the case of a 52year old man with severe acute pancreatitis. In this case report we discuss the undesirable effects of parenteral nutrition and the importance of nutritional support at patients with severe necrotizing pancreatitis. Severe acute pancreatitis is usually accompanied by systemic inflammatory response syndrome, which results in hypermetabolism with prominent protein catabolism. Providing nutrition to these patients is of paramount importance. An adequate nutritional support is crucial in patients with severe and complicated pancreatitis.
