Hydrocefalus po poranění míchy je popisován pouze výjimečně. Sdělení popisuje dvě kazuistiky nemocných s kompletní lézí krční míchy, u kterých v poúrazovém období došlo k rozvoji hydrocefalu. Rozbor obou případů a přehled literárních dat potvrzují rozhodující úlohu extraspinálních faktorů pro rozvoj hydrocefalu po míšním poranění. Úvod: Cílem článku je ukázat experimentální a klinické výsledky odpovědi ledvin na teplou a studenou ischemii. Porovnává různé operační postupy a možnosti ovlivnění ledvinné ischemie při resekcích ledvin. Článek je souhrnem aktuálních literárních údajů. Výsledky: Existují tři hlavní mechanizmy ischemického poškození ledvin – cévní mechanizmus s přetrvávající vazokonstrikcí a abnormální reakcí endoteliálních buněk, tubulární obstrukce se zpětným tokem moči a reperfuzní poškození. Spor trvá o maximální toleranci teplé ischemie (WI), která může být ovlivněna hlavně chirurgickou technikou. Závěr: Pokud je předpokládáno při operaci využití ischemie, nádor by měl být odstraněn v co nejkratší možné době. Obecně je doporučován čas kratší než 25 minut teplé ischemie bez ohledu na typ chirurgického přístupu. The study compared different surgical procedures and options for dealing with renal ischemia during partial nephrectomy. The article is a summary of the current literature data. Results: There are three main mechanisms of ischemic renal injury – vascular, persistent vasoconstriction with an abnormal endothelial cell compensatory response, and tubular obstruction, with backflow of urine, and injury due to reperfusion. Controversy regarding the maximal kidney tolerability to warm ischemia continues. This communication summarizes literary data regarding available surgical techniques used to diminish the effects of warm ischemia. Conclusion: If ischemia is required, the tumour should be removed within 25 minutes of warm ischemia, regardless of the surgical approach. If this longer time of resection is expected, we have to start immediately with cold ischemia. Cold ischemia, depending on the cooling method, can be tolerated for up to 2 hours (autotransplantation). The cold ischemia technique includes in situ cold arterial perfusion, ice slush placed around the kidney, retrograde caliceal perfusion or ex situ cold arterial perfusion with autotransplantation. Hydatid disease is caused by tapeworm Echinococcus granulosus. E. granulosus forms hydatid cysts in human body, mostly in the liver and the lung. However, isolated primary hydatid cyst of the pancreas is rarely seen. Hereby, we report a 40-year-old Caucasian female with a hydatid cyst in the body of the pancreas. The patient, complaining of vague abdominal pain was examined with abdominal ultrasound. The ultrasound scan of abdomen revealed an exophytic cystic lesion in the body of the pancreas, approximately 7.5x5 cm in size. The defined lesion was thought to be compatible with hydatid cyst stage 4-5. The surgical team informed the patient about all possible treatment options. The patient was put on albendazole treatment, 400 mg twice daily, for two months. Despite the medical treatment, the patient complained about abdominal pain and discomfort affecting her daily life. Therefore, she decided for surgical intervention. Following the intraoperative application of scolicidal hypertonic 20% NaCl solution, partial cystectomy with capitonnage was performed. The patient did well postoperatively. Průměrná dávka z lékařského ozáření má v populaci výrazně stoupající trend. Ve vyspělých zemích za posledních 30 let vzrostla o stovky procent a je již srovnatelná s dávkou z přírodního ozáření. Výpočetní tomografie přispívá ke kolektivní dávce z lékařského ozáření plnými dvěma třetinami. K nápravě tohoto nepříznivého vývoje způsobeného především trvale vzrůstajícím počtem CT vyšetření je nezbytné přijmout účinná opatření. Tím nejperspektivnějším se v současné době jeví technika tzv. iterativní rekonstrukce CT obrazu, která významně redukuje obrazový šum, a umožňuje tak vyšetření s dávkou záření redukovanou o desítky procent. Článek rozebírá základní principy a typy iterativní rekonstrukce, které jsou již komerčně dostupné na nejmodernějších CT tomografech včetně jejich kladů a záporů. Jsou vysvětleny rozdíly mezi iterativní rekonstrukcí a běžně používanou filtrovanou zpětnou projekcí, která je více než tři desetiletí považovaná za "zlatý standard" při rekonstrukci obrazu na všech CT přístrojích., The average radiation dose resulting from medical exposure continuously rises in the population. It has increased several times within the last three decades and reached the level of natural radiation background in some developed countries. Computed tomography is the major contributor, comprising two thirds of the entire cumulative medical exposure. To reverse this unfavourable trend attributed mainly to continuously increasing number of CT procedures, it is necessary to adopt adequate precautions. Among these, the most promising seems to be the technique of iterative reconstruction of CT image which significantly reduces image noise and therefore allows radiation exposure reduction by tens of percent. The article deals with basic principles and types of iterative reconstruction which are already available on recent CT scanners, commenting on their advantages and disadvantages.
