42. Geriatrická paliativní péče a péče o umírající

V současnosti umírá v České republice ročně asi 110 000 osob. Ve více než 80 % úmrtí se jedná o osoby v geriatrickém věku. Proto je nezbytné se seriózně zabývat problematikou paliativní a terminálni péče v geriatrii. V přehledném článku autorka definuje obsah a náplň geriatrické paliativní péče a její hlavní charakteristiky. Na vlastních výsledcích demonstruje vysoký výskyt obtěžujících symptomů a jejich nedostatečné zvládnutí u geriatrických pacientů s terminálni prognózou. V závěru se zamýšU nad možnostmi zlepšení současné situace. Je třeba identifikovat bariéry v českém systému zdravotní a sociální péče a cílevědomě usilovat o jejich odstraňování na úrovni systémově, jednodivých poskytovatelů péče i konkrétních pečovatelů., Currently, the yearly death rate reaches 110 000 inhabitants in the Czech republic. More than 80 % of deaths occurs in geriatric age. Therefore it seems unavoidable to deal seriously with the palliative and end-of-life care m geriatrics. In this review article the author defines content of geriatric palliative care and its main characteristics. Based on her own results she demonstrates high occurrence of distressing symptoms and its inadequate management in geriatric patients with terminal prognosis. In conclusion she is rethinking possibilities of improvement of the current situation. Onemocnění vyvolané herpetickým virem je nejčastějším důvodem neuropatické bolesti. Až 90 % populace se podle sérologického vyšetření setkalo s virem Herpes zoster. Populace ve vyšších věkových skupinách je ohrožena tímto onemocněním téměř 2krát více než populace mladší. Ohroženými skupinami jsou lidé se sníženou buněčnou imunitou v důsledku např. maligních onemocnění, věku, ale i stresu [1]. Výzkumy prokazují více než 80 typů herpes viru, z toho 8 je humánních. Nejčastější je infekce Varicella zoster virem (VZV). V klinickém obraze se projevuje prodromálními příznaky většinou s bolestí, kožním stadiem s výsevem puchýřků, někdy s celkovými příznaky a bolestí. Po zhojení se za různě dlouhou dobu může vyvinout postherpetická neuralgie s různou intenzitou bolesti. Léčení spočívá jednak v antivirové terapii v nejčasnější fázi onemocnění (24-72 hod), v analgetické léčbě, případně s dalšími adjuvantními prostředky (kortikoidy, antibiotika, spinální blokády, lokální terapie, laser, akupunktura). Antivirová terapie není stoprocentní zárukou prevence postherpetícké neuralgie, ale urychlí hojení a zmírní bolesti. Efekt kortikoidů není jednoznačný, jeho podání nese riziko komplikací, zejmena ve vyšším věku (2) a při konkomitantních onemocněních může být tato terapie rizikem. Léčení bolesú se zaměřením na vyšší věk je nesmírně důležité a musí být zahájeno už v akutní fázi. Užití psychofarmak (amitriptylín), antikonvulziv (gabapentin, karbamazepin) je indikováno už v počáteční fázi nemoci, pokud si to vyžadují přítomně symptomy. V akutní fázi mají důležitou pozici proúzánědivé léky typu nesteroidních antirevmatik. Léčbě akutního Herpes zoster je třeba věnovat velkou pozornost, protože rychlé zvládnuú virové infekce a eliminace bolesú v akutní fázi, může zmírnit nebo předejít projevům postherpeúcké neuralgie., Disease caused by herpes-virus is the most common cause of neuropathic pain. Up to 90% of population have met the Herpes zoster virus according to serology investigation. The threat of this disease is in population at advanced age groups nearly twice higher than in younger population. People with decreased cellular immunity in a consequence of e.g. malignity, age or stress are the most threatened groups [1]. Research has proven more than 80 types of herpes-virus including 8 human types. VariceUa zoster-virus (VCV) is the most frequent infection. Its clinical manifestation include prodromal symptoms mostly with pain, dermatology phase with vesicles eruption, someúmes with general symptoms and pain. Various time after consolidation postherpetic neuralgia with pain of various intensity can develop. The treatment is based on antiviral therapy in the earliest phase of the disease (24 to 72 hours), on analgesic treatment possibly with other adjuvant agents (corúcoids, antibiotics, spinal blockade, local therapy, laser acupuncture). Antiviral therapy is not a 100% guarantee of postherpetic neuralgia prevention, but accelerate healing and reduce pain. The effect of corticoids is not definite, they bring a risk of complications and this therapy can be risky especially in elderly [2] and during concomitant diseases. The treatment of pain oriented to elderly is very important and must be treated already in the acute phase. Psychopharmacological agents (amitriptyline), anticonvulsives (gabapentine, carbamazepine) are indicated in the presence of symptoms already in the initial stage of the disease. Anti-inflammatory agents, such as nonstreoid antirheumatics play important role in acute phase. Great attention must be paid to acute Herpes zoster treatment, because a rapid viral infection management and pain elimination in the acute phase may dampen or prevent symptoms of postherpetic neuralgia., Irena Pechanová, Dana Vondráčková, Lit: 12, and Souhrn: eng
