The Hronov-Poříčí Trough represents the easternmost part of the Trutnov-Náchod Depression. The NW-SE striking structure was formed due to the post-Cretaceous flexural folding and is filled with the Upper Cretaceous sediments. Both the NE and SW margins of the trough are bounded by flexures with the Upper Cretaceous strata dipping 40-60° towards the axis of the trough. The NE flexure is situated close to the parallel Hronov-Poříčí Fault Zone. Up to now, it is not fully known, in what extent the normal faulting was involved in the evolution of the structure. From the geomorphological point of view, the normal fault constraints of the trough seem to be acceptable, as the surface topography of its present margins exhibit many signs typical for fault scarps. However, the existence of a fault system bounding all round the Hronov-Poříčí Trough has not b een proved by any geological research. Hence the geophysical research was carried out on both sides of the NW part of the trough to support one of these hypothesis. Five geoelectrical profiles were measured in the area and the fault system was proved on the NE side of the trough. On the SW boundary the fault system was not found. Těhotenství žen bez domova představuje specifický sociálně-zdravotní problém, který má řadu aspektů. Prvním z nich je minimální prevence otěhotnění plynoucí jednak z menší dostupnosti antikoncepčních prostředků z ekonomických důvodů, jednak z nízkého intelektu, případně z naděje, že těhotenství je cesta k vyřešení tíživé sociální situace za pomoci státu nebo dobročinných organizací. Vážným problémem je také značné zdravotní riziko pro těhotnou i pro plod plynoucí z nedostatečné intimní hygieny, z absence pravidelné gynekologické péče, z rizika nákazy sexuálně přenosnými chorobami, ze závislosti na návykových látkách i z malnutrice. Podobně je sociálně-zdravotně problematická i perinatální péče. Tato situace je demonstrována ilustrativní kazuistikou. Klíčová slova: bezdomovkyně – pouliční medicína – těhotenství, Pregnancy of homeless women represents a specific social and health problem which involves a number of aspects. The first one is minimum prevention of pregnancy as a result of a more difficult access to contraceptives for economic reasons, because of low intelligence or maybe a hope that pregnancy offers a way to solve a precarious social situation with the support of the state or charitable organizations. A serious problem is also a considerable health risk for the pregnant woman and fetus, resulting from inadequate intimate hygiene, the absence of regular gynecological care, risk of sexually transmitted diseases, addictions and malnutrition. Similarly problematic are the social and health aspects of perinatal care. The described state is illustrated by case studies. Cíl práce: Aktuální pohled na těhotné s roztroušenou sklerózou (RS), analýza výsledků těhotenství těchto nemocných ve FN Motol v letech 2003-2011. Typ studie: Retrospektivní analýza Soubor a metodika: Analýza 76 těhotenství pacientek s relaps-remitentní formou roztroušené sklerózy – posouzení vlivu gravidity na aktivitu onemocnění (použitá analgezie a laktace ve vztahu ke vzniku akutní ataky) a naopak vlivu onemocnění na průběh těhotenství (komplikace, vedení porodu a perinatální výsledky). Výsledky: Akutní ataka v graviditě se v souboru 76 žen vyskytla u 4 z nich (5,3 %, RR 0,08), do půl roku po porodu jsme akutní ataku zaznamenali u 17 žen (22,4 %, RR 0,46). Ataku mělo 18,4 % kojících žen (9/49) ve srovnání s 33,3 % žen (8/24), které nekojily, p = 0,2375 (OR = 0,45, 95 % CI 0,15-1,37). Epidurální analgezii k porodu mělo 13,6 % žen (3/22) s akutní atakou, 25,9 % žen (14/54) s atakou po porodu bylo bez analgezie, p = 0,3648 (OR = 0,45,95 % CI 0,12-1,76). Zhoršení choroby jsme neprokázali (EDSS 1,4, resp. EDSS 1,6). 75 % těhotenství bylo ukončeno nekomplikovaným vaginálním porodem. Výskyt závažných těhotenských komplikací nebyl zvýšen a perinatální výsledky byly srovnatelné s běžnou populací. Závěr: V souladu s recentními studiemi prokazujeme v našem souboru pokles aktivity choroby v graviditě s nárůstem počtu akutních atak po porodu. Ke zhoršení či progresi onemocnění však během 12 měsíců po porodu nedochází. Kojení ani použití epidurální analgezie nemá na vznik akutní ataky vliv, těhotenství probíhají ve většině případů fyziologicky, vedení porodu a novorozenecké výsledky se neliší od běžné populace. Gravidita je tedy pro stabilizované pacientky s relaps-remitentní formou RS bezpečná., Objective: Current view on pregnant women with multiple sclerosis (MS), the analysis of pregnancy outcomes of these women with MS in the Motol Hospital in the years 2003-2011. Design: Retrospective analysis Methods: Analysis of 76 pregnant women with MS – to assess the impact of pregnancy on the activity of the disease (breastfeeding and used analgesia in relation on the relapse rate post partum) and the impact of the disease on the course of pregnancy (pregnancy complications, differences in the management of labor, and perinatal outcomes). Results: Relapse during pregnancy occurred in 4 out of the 76 women (5.3% relapse rate – RR 0.08). 17 women experienced a post partum relapse in the first six months after delivery (22.4%, RR 0.46). 18.4% breastfeeding women (9/49, 18.4%) had post partum relapse in comparison with 33.3% women, who did not breastfeed (8/24, 33.3%), p = 0.2375 (OR=0.45, 95% CI 0.15-1.37). 13.6% women (3/22, 13.6%) with post partum relapse used epidural analgesia (EDA), 25.9% women (14/54, 25.9%) with relapse did not used it, p = 0.3648 (OR = 0.45, 95% CI 0.12-1.76). Pregnancy did not influence the progress of disability (EDSS 1.4 or EDSS 1.6). 75% women had uncomplicated vaginal delivery, the incidence of serious pregnancy complications was not increased, and the perinatal outcomes are comparable with the general population. Conclusion: Consistent with recent studies, we evaluated decreasing relapse rate during pregnancy. In the post partum period the relapse rate has increased, however the pregnancy did not influence the disability progress in 12 months postpartum. Neither breastfeeding nor epidural analgesia correlated with presence of post partum relapses. In most cases, the pregnancies in patients with MS were physiological, the method of delivery and the overall perinatal outcomes are comparable with the general population. Zvířecí modely představují klíčový nástroj pro studium Huntingtonovy nemoci (HN). V posledním desetiletí byla snaha vytvářet velké zvířecí modely této choroby z důvodu zlepšení znalostí o patologických změnách v mozku, který se co nejvíce podobá lidskému. Transgenní miniprase exprimující N‑terminální část lidského mutovaného huntingtinu se 124 CAG/CAA repeticemi se zdá být velmi nadějným modelem pro studium HN. Jelikož předchozí charakterizace tohoto modelu prokázala rozličné fenotypy postihující subcelulární, buněčnou stejně tak i orgánově‑systémovou úroveň. Cílem této studie bylo detekovat a analyzovat patologický vzorec ve fyzické aktivitě transgenních (TgHD) kanců ve věku 3 let pomocí telemetrie. Do experimentu bylo zařazeno pět TgHD a pro srovnání pět netransgenních (WT) kanců. Fyzická aktivita byla u kanců měřena pomocí telemetrického systému rodentPACK2, přičemž transmiter byl umístěn do límce na krku. Na základě analýzy bylo zjištěno signifikantní snížení totálního zrychlení reprezentujícího fyzickou aktivitu u TgHD kanců mezi 4:40 a 5:30 ráno (po nočním spánku – před ranním krmením) v porovnání s WT jedinci, které může být vysvětleno narušeným energetickým metabolizmem. Telemetrická analýza bude jistě hrát v budoucnosti významnou roli ve studii fyzické aktivity a biopotenciálů důležitých pro podrobnější charakterizaci preklinické a klinické fázi HN u velkých zvířecích modelů. During the last decade large animal models of HD have been established to improve knowledge of HD under large brain conditions. Transgenic minipig expressing N‑terminal part of human huntingtin with 124 CAG/CAA repeats seems to be very promising HD model. Its previous characterization has shown various phenotypes affecting subcellular, cellular as well as organ systems level. The goal of this study was to detect and analyze a pathological pattern in physical activity of transgenic (TgHD) boars at the age of three years using telemetry approach. Into the study we have included five TgHD and five wild type (WT) animals for comparison. The physical activity was measured by the telemetric system rodentPACK2 whereas transmitter was placed into the collar. The analysis have shown significant decrease of total acceleration representing physical activity in TgHD boars between 4:40 and 5:30 a. m. (after night sleep – before morning feeding) in comparison with WT ones which could be explained with disturbed energy metabolism. Telemetry approach will play an important role in the study of physical activity and biopotentials essential for deeper characterization of large animal HD models in their preclinical and clinical phase., and M.
