Cíl: Hodnocení použitelnosti výsledků kardiopulmonální třídy klasifi kačního systému NOC, při vybrané ošetřovatelské diagnóze 00031 NANDA I Neefektivní průchodnost dýchacích cest. A to za účelem následného testování implementování klasifi kačních systémů NNN v reálném klinickém prostředí chirurgické a intenzivní péče ČR. Metody: Všeobecné sestry hodnotily použitelnost NOC výsledků z kardiopulmonální třídy klasifi kačního systému (NOC Nursing Outocomes Classifi cation 4th, 2008, s. 130), které by reálně využívaly na svém klinickém pracovišti, při vybrané ošetřovatelské diagnóze 00031 NANDA I Neefektivní průchodnost dýchacích cest (NANDA-I 2009–2011, překlad Kudlová, s. 360). Hodnocení přisuzovaly prostřednictvím škály Likertova typu do záznamových archů. Ke komparaci hodnocení použitelnosti výsledků NOC na klinických pracovištích intenzivní ošetřovatelské péče, byl po základním statistickém zpracování (aritmetický průměr, medián, směrodatná odchylka) použit Pearsenův chí-kvadrát test nezávislosti. Výsledky: Šetření neprokázalo statisticky významné rozdíly na hodnocení použitelnosti NOC výsledků 0412; 0415; 0416; z kardiopulmonální třídy NOC klasifi kačního systému (4th, 2008, s. 130) všeobecnými sestrami (n65) z klinických pracovišť NIP, CHJIP a ARO. Závěr: Výše uvedené šetření hodnocení použitelnosti výsledků NOC klasifi kačního systému bylo přijato jako dílčí krok k řešení projektu IGA MZČR NF 12078-4/2011, který podporuje implementaci klasifi kačního systému NIC v chirurgické a anesteziologicko resuscitační ošetřovatelské péči do podmínek klinické praxe České republiky., Aim: The evaluation using of cardiopulmonary class NOC outcomes of classificatory system in the relation to 00031 NANDA-I. For NOC outcome evalution comparison by nurses from individual clinical workplaces Pearsen chi-square test for independency was used after the basic statistical processing (arithmetic mean, median, standard tolerance. Results: Research did not prove statistically significant differences of nurses (n65)evalution at outcomes 0412; 0415; 0416; of NOC cardiopulmonary class of the classificatory system. Východiska: Praxe založená na důkazech je velmi užitečná v každé profesi, v porodní asistenci nevyjímaje. Nedostatečná informovanost o využití praxe založené na důkazech může vést k nekvalitní a neplnohodnotné péči o klientku. Cíl: Hlavním cílem bylo zjistit informovanost porodních asistentek o Evidence Based Midwifery. Mezi dílčí cíle patřilo zjištění, zda porodní asistentky znají pojem Evidence Based Midwifery a zkratku EBM a zda s Evidence Based Midwifery ve své praxi pracují. Metody: Byl vytvořen nestandardizovaný dotazník. Celkem obsahoval 15 otázek. V úvodu dotazníku byly respondentky seznámeny se záměrem výzkumu a byl jim předložen informovaný souhlas. Respondentkami byly porodní asistentky z České republiky pracující na gynkologických, porodnických lůžkových a ambulantních pracovištích. Rozdáno bylo celkem 160 dotazníků. Návratnost činila 93,8 %. Celkem se pracovalo se 150 dotazníky. Výzkumné šetření probíhalo v měsících září–listopad roku 2013. Výsledky: Zkratku EBM uvedlo, že nezná celkem 100 (66,7 %) respondentek. Znalost pojmu Praxe založená na důkazech potvrdilo celkem 46 (30,7 %) respondentek. Zájem o studijní materiál věnující se praxi založené na důkazech v porodní asistenci má 93 (62,0 %) respondentek, zájem o školící kurz o EBMid uvedlo 110 (73,4 %) respondentek. Závěry: Cíle výzkumného šetření byly splněny. Úroveň informovanosti porodních asistentek o Evidence Based Midwifery jsme stanovili jako nedostatečná (méně než polovina respondentek nezná pojem EBMid). Většina respondentek s Evidence Based Midwifery ve své praxi nepracují. Bylo by vhodné informovat porodní asistentky o využití EBMid v praxi a poté provést další výzkum, díky kterému bychom mohli zhodnotit zlepšení dané situace či nikoliv., Background: Evidence-based practice is very useful in every profession, in midwifery exception. Lack of awareness about the use of evidence-based practice can lead to poor quality and fiduciary care of the client. Objective: To determine the awareness of Midwives of Evidence Based Midwifery, determine whether midwives are familiar with the concept of Evidence Based Midwifery and acronym EBM and whether they practice Evidence Based Midwifery in their work. Methods: Respondents were midwives working in the gynecological-obstetric wards and clinics in the Czech Republic selected by purposive sampling, there were 150 respondents. e research was carried out by a non-standardized questionnaire. Results: 100 respondents don´t know EBM: 93 respondents are interested in the study material about EBMid and 110 respondents are interested in a training course. Conclusions: e objectives of the research were met. e level of Midwives`awareness of Evidence Based Midwifery was detected. Cíl: Cílem práce je analyzovat literární přehled faktorů pracovní spokojenosti sester ve vztahu k setrvání sester na pracovišti. Metodika:Zdroje dat - Elektronické databáze: MEDLINE, Wiley Library Online, PubMed, Bibliographia medica Čechoslovaca. Periodika – Kontakt, Profese on-line, Ošetřovatelství a porodní asistence, Ošetrovateľstvo: teória, výskum, vzdelávanie a odborné recenzované publikace. Klíčová slova byla zadávána v databázích publikovaných od roku 1990 nebo později, které zkoumaly pracovní spokojenost sester a její vliv na setrvání sester v práci. Literární výsledky jsou prezentovány v tabulkovém formátu ke komparaci jednotlivých studií. Výsledky: Z celkového počtu 42 sdělení nebo abstraktů, bylo do tohoto přehledu zahrnuto 29 studií, převáţně kvantitativních studií. Byly zjištěny následující prediktory pracovní spokojenosti sester, které ovlivňují jejich záměr setrvat v zaměstnání: (1) pracovní vztahy, (2) pracovní odměny, (3) styl vedení, vztah a podpora manaţera, (4) organizační podpora, stav pracovního prostředí, (5) fyzická a psychická reakce na práci. Závěr: Manaţeři ošetřovatelské péče sehrávají významnou roli v udrţení pracovní spokojenosti sester. Znalosti o proměnných pracovní spokojenosti sester mohou pomoci managementu zdravotnických zařízení implementovat strategie k udrţení spokojenosti sester. Pozornost by měla být směrována zvláště k posílení interpersonálních vztahů a potřebné kapacitě sester v zařízení, k udrţení kvality péče o pacienta., Aim: The aim of this work was to perform a literature review of nurses’ job satisfaction factors with respect to their retention in the workplace. Methods: Data sources – electronic databases: MEDLINE, Wiley Online Library, PubMed, Bibliographia medica Čechoslovaca; journals: Kontakt, Profese on-line, Ošetřovatelství a porodní asistence, Ošetrovateľstvo: teória, výskum, vzdelávanie; and peer-reviewed publications. The key words were entered into the databases published from 1990 or later that have examined the job satisfaction of nurses and its impact on their retention in work. Literature results are presented in a tabular format for the comparison of individual studies. Results: Out of a total of 42 articles or abstracts, 29 – mostly quantitative – studies were included in this review. The following predictors of nurses’ job satisfaction that influence their intention to remain in the workplace were identified: (1) working relationships, (2) job rewards, (3) leadership style, relationship and support of a manager, (4) organizational support, the state of the work environment, and (5) physical and psychological reactions to work. Conclusion: Nursing care managers play an important role in maintaining job satisfaction of nurses. Awareness of the variables of nurses’ job satisfaction can help healthcare facility managers implement strategies to maintain nurses’ job satisfaction. To study defects and chromosomal abnormalities of the fetus, we retrospectively analyzed results of comprehensive dynamic survey of 26,404 pregnant women aged 1850 years old at 6 to 40 weeks of pregnancy. Of them, 25,956 (98.3%) women had physiological course of pregnancy, 448 (1.7%) women had abnormal pregnancy. For the diagnosis of fetal defects, we carried out ultrasound, biochemical, invasive and cytogenetic studies. The results of study showed that the majority of fetal defects and pathological course of pregnancy was noted in women at the age of 2125 years old, since at this age period women have the highest number of pregnancies. At the older age, we noted a gradual decrease in the number of pregnant women, as well as the number of abnormalities of the fetus. Based on the analysis of the results obtained, we have developed an algorithm for early fetal ultrasound examination. Introduction Cutaneous malignant melanoma prevalence, incidence and mortality rates are increasing in white populations worldwide more rapidly than any other cancer site (American Cancer Society, 2006). Despite the potential importance of regular skin selfexamination and promotion of selfprotection practices, little is known about the prevalence of these practices in medical students in Albanian population. Methods This is a descriptive, quantitative crosssectional study. In this study were included a sample of 150 individuals chosen among the students of Faculty of Medicine based on their family history for skin cancer. This study was started on October the 3rd and finished on November the 12th. Subjects had to fill in a structured, selfadministered questionnaire. All participants lived within Republic of Albania but at the time of the study were students in University of Tirana, Faculty of Medicine. Results In this study that we conducted we included 150 individuals supposed to have a risk for skin cancer based on their family history. 200 individuals were approached for participation. Of these, 150 individuals returned questionnaire data, yielding a response rate of 75% among eligible, successfully contacted participants. The mean age of the sample was 20.05 years (Std. Dev. = 0.925), with males (22%) and females (78%) represented unequally, because the gender configuration of the faculty itself has gender disparities with more than 80% females and only 20% male students. The majority of the sample was born in city or town (85.3%), 14.7% was born in a village. In terms of income level, 79.3% of participants had a medium income family background, 15.3% high income and 5.3% had a low income level family background. Conclusion It is important for those individuals with family history for skin cancer (which inherently indicates risk for skin cancer) to develop selfexamination and SSE behaviours and practices in order to have a protection and at least an early detection (if onset) of the different forms of Melanoma. Cieľ: Cieľom prieskumu bolo zistiť prevalenciu a rizikové faktory hypertenzie bieleho plášťa (WCH) u detí. Metodika: Formou retrospektívnej štúdie sme analyzovali zdravotné záznamy 140 detí (36 dievčat, 104 chlapcov) s vysokým krvným tlakom nameraným v kardiologickej ambulancii, u ktorých bolo vykonané 24-hodinové ambulantné monitorovanie krvného tlaku (ABPM) a určený typ hypertenzie. Hypertenzia bola diagnostikovaná, keď bol systolický a/alebo diastolický krvný tlak meraný auskultačnou technikou ≥ 95. percentil pre pohlavie, vek a výšku. WCH bola definovaná ako priemerná hodnota krvného tlaku v ambulancii nachádzajúca sa v percentilovom pásme hypertenzie a hodnota tlaku krvi počas 24-hodinového monitorovania ambulantného tlaku nachádzajúca sa v pásme normálnych hodnôt krvného tlaku. Výsledky: V skupine 140 detí bola potvrdená prítomnosť WCH u 51 detí (36,4 %) – 13 dievčat a 38 chlapcov. Z celkového súboru sa WCH vyskytovala u 36,1 % zaradených dievčat a 36,5 % zaradených chlapcov. Systolický TK sa v kardiologickej ambulancii nachádzal nad 95. percentilom, diastolický TK pod 90. percentilom. Väčšina pacientov s WCH bola vo veku 15 – 19 rokov (80,4 %) a v pásme od 3. do 90. percentilu BMI (72,5 %). U pacientov prevládala negatívna rodinná anamnéza hypertenzie (54,9 %) a zriedkavé návštevy kardiologickej ambulancie (2-4x). Krvný tlak v ambulancii merala deťom výlučne sestra. Záver: Zistenie prevalencie WCH a jej rizikových faktorov je u detí významné, pretože prevažuje názor, že v detskom veku je WCH predispozíciou k trvalej hypertenzii. Na základe zistení rizikových faktorov, môžeme vykonávať prevenciu WCH., Aim: The aim of this study was to find out the prevalence and risk factors of white coat hypertension (WCH) in children. Methods: We realized the retrospective study and analyzed records of 140 children (36 girls, 104 boys) with high blood pressure measured in cardiology outpatients' clinic, to whom a 24-hour ambulatory blood pressure monitoring (ABPM) was made and the type of hypertension was specified. Hypertension was diagnosed when systolic and/or diastolic blood pressure measurements with auscultatory technique were ≥ 95th percentile for sex, age and height. White coat hypertension (WCH) has been defined as mean blood pressure (BP) readings in outpatients' clinic being located in the hypertensive range while 24-hour ambulatory blood pressure monitoring is in the normal range. Results: In the group of 140 children the presence of WCH was confirmed in 51 children (36,4 %) - 13 girls and 38 boys. WCH occurred in 36,1 % of girls and 36,5 % of boys in the total sample of respondents. Systolic BP in cardiology outpatients' clinic reached the value above the 95th percentile, diastolic BP below the 90th percentile. The most patients with WCH were 15 – 19 years old (80,4 %) and in 3rd - 90th percentile of BMI (72,5 %). Patients prevailed negative family history of hypertension (54,9 %) and fewer visit (2-4 times) of cardiology outpatients' clinic. Blood pressure of children was measured exclusively by a nurse in this outpatients' clinic. Conclusion: Determination of the prevalence of WCH and its risk factors in children is important because of prevailing opinion that WCH in children is predisposition in permanent hypertension. Východiska: Systematické review (SR) představuje v kontextu s přístupem a filozofií Evidence-Based Healthcare (EBHC) nejvýznamnější a nejhodnotnější typ studie v rámci úrovně vědeckých důkazů. Hallux valgus (HV) patří mezi nejčastější deformity v oblasti přednoží. SR, zabývající se vlivem operace HV na chůzi, kvalitu života, spokojenost a současně i bolest, ve světovém písemnictví chybí. Cíle: Hlavním cílem tohoto propedeuticky zaměřeného článku bylo poukázat na význam SR v rámci přístupu EBHC a přiblížit rigorózní a systematickou metodiku jeho tvorby příkladem dílčích výsledků SR zaměřeného na hodnocení efektu operační korekce deformity HV. Metodika: Nejprve jsme formulovali review otázku a specifikovali kritéria pro zařazení a vyloučení primárních studií do SR. Protokol SR jsme vypracovali pomocí softwaru, vyvinutého Joanna Briggs Institute pro tvorbu SR. Systematické vyhledávání publikovaných i nepublikovaných primárních studií zahrnovalo 3 kroky: 1. Iniciální vyhledávání v Medline, Embase a Cinahl; 2. sekundární vyhledávání v 13 databázích; 3. prostudování referenčních seznamů relevantních studií. Vyhledávali jsme randomizované kontrolované studie (RCT) bez časového omezení a ve všech jazycích. Výsledky: Celkem jsme nalezli 2412 studií a odstranili 767 duplikátů. Na základě dvoufázového hodnocení relevance a kritického hodnocení metodologické kvality studií pomocí standardizovaného hodnotícího nástroje JBI-MAStARI bylo zařazeno 27 RCT. V současné době probíhá extrakce výsledků pomocí standardizovaného nástroje JBI-MAStARI a příprava dat na meta-analýzu. Dílčí výsledky ukázaly nejednotnost použitých metod k hodnocení bolesti a spokojenosti pacientů po operaci HV v zahrnutých RCT. Závěry: SR představují velmi významné pojítko mezi vědeckými důkazy a jejich implementací do klinické praxe. Evidence-Based přístup je možné použít ve všech oblastech, v nichž se tvoří kvalitní primární vědecké důkazy., Background: Systematic review (SR), as secondary research is in the context of Evidence-Based Healthcare (EBHC) classified as the most important and valuable type of publication within the level of evidence hierarchy. Hallux valgus (HV) is a common forefoot deformity. In the current literature, there is no SR which has reviewed the effect of HV surgery on gait, pain or function in the global evidence. Aim: Main aim of this propedeutically focused paper was to highlight the importance of SR within EBHC approach and explain rigorous and systematic methods of its development by example of partial results of the SR focusing on evaluation of the effect of the hallux valgus surgery. Medhods: Firstly, we formulated an answerable review question and specified inclusion and exclusion criteria. Protocol of the SR was prepared using soware developed by Joanna Briggs Institute for SR preparation. Systematic search of published and unpublished studies included three steps: 1. Initial search in Medline, Embase and Cinahl; 2. secondary search in 13 databases; 3. hand-search in the reference lists of relevant papers. We have searched randomized controlled trials (RCT) in any date and language. Results: We found 2412 papers and deleted 767 duplicates. en a paper retrieval according to inclusion and exclusion criteria and critical appraisal of methodological quality were done using standardized critical appraisal instruments JBI-MAStARI. 27 RCT were included into SR. Currently, we are extracting data using JBI MAStARI and preparing them for meta-analysis. Partial results showed heterogeneity of used method for evaluation of pain and patient satisfaction aer HV surgery in included RCT. Conclusions: SR is a very important link between scientific evidence and their implementation into clinical practice. Purpose: To investigate the protective action of Ceruloplasmin (Cp) on the lysis of erythrocytes in men with prostate tumors. Material and Methods: The blood erythrocytes of the patients with benign hyperplasia of prostate (BHP) and prostate adenocarcinoma (CaP) were studied. Patients at the age 60-75 with early stages of a cancer have been investigated. The control group was consisted of apparently healthy males with the compatible age. n=15 for each group. The clinical stage of the disease was diagnosed by means of rectal, histological and echographic examination of the prostate gland. Photometric methods were applied to register lysis dynamics in order to test out the protective action of Cp. Results: The nonspecific protective function of Cp preparation on BHP and CaP erythrocytes as well as on the control group erythrocytes was revealed. CaP erythrocytes have shown more sensitivity to the lysis provocative factor than BHP and the control group erythrocytes, that was presumably attributed to the structural and functional changes of the erythrocytes developed in the presence of malignant tumor., N. Cieľ: Práca má design prierezovej, deskriptívnej štúdie, ktorej cieľom bolo zhodnotiť, aký je vzťah medzi psychickou záťažou, výskytom depresívnej symptomatológie a subjektívnou pohodou u sestier. Metodika: Vzorku respondentov tvorilo 78 sestier pracujúcich v NsP v Levoči a Spišskej Novej Vsi. Na meranie psychickej záťaže bol použitý Meisterov dotazník neuropsychickej záťaže, na meranie výskytu depresie skríningový nástroj Patient Health Questionnaire 9 (PHQ 9) a na meranie subjektívnej pohody Personal Well-Being Index (PWI) pre zdravú dospelú populáciu. Výsledky: Bol potvrdený významný vzťah medzi psychickou záťažou, výskytom depresívnej symptomatológie a subjektívnou pohodou u sestier. Vyššia frekvencia výskytu depresívnej symptomatológie sa u nich spájala s vyššou psychickou záťažou a nižšou subjektívnou pohodou. Významným a zároveň alarmujúcim zistením bolo, že len 28 % sestier uvádzalo nevýznamnú depresívnu symptomatológiu. Až u 50 % sestier bola podľa PHQ 9 prítomná ľahká depresia. Štatisticky významné korelácie boli identifikované medzi všetkými tromi faktormi Meisterovho dotazníka, ako aj jeho hrubým skóre a PHQ 9. Najsilnejšie korelácie boli medzi Faktorom I (preťaženie) a PHQ 9. Záver: Výsledky poukazujú na skutočnosť, ţe práve aspekty pracovného preťaţenia (časová tieseň, problémy a konflikty, vysoká zodpovednosť) môžu u sestier viesť ku častejšiemu prežívaniu jednotlivých symptómov depresie (predovšetkým k únave a zníženému záujmu). V ďalšom výskume navrhujeme sa zamerať na sledovanie vzťahu troch premenných: depresia, burnout a psychická záťaž sestier., Aim: The study has a design of descriptive cross-sectional survey. The aim of this study was to investigate relationship between psychological load, depressive symptomatology and subjective well-being of nurses. Methods: Sample of respondents consisted of 78 nurses from two public hospitals (Levoča and Spišská Nová Ves). To measure the psychological load was used the Meister questionnaire of neuro-psychological load. To measure the incidence of depression was used the screening tool Patient Health Questionnaire 9 (PHQ 9). To measure subjective well-being was used Personal Well-Being Index (PWI) for a healthy adult population. Results: The results indicated that there is a significant correlation between psychological load, depressive symptomatology and subjective well-being of nurses. The higher frequency of depressive symptomatology was associated with higher level of psychological load and lower subjective well-being level. The depressive symptomatology has a significant relation with neuropsychological strain and subjective well-being. Important and at the same time alarming finding was that only 28% of nurses reported insignificant depressive symptomatology. According to PHQ 9 was present mild depression in up to 50% of nurses. Statistically significant correlations were identified between all three factors of Meister questionnaire, as well as his gross score and the PHQ 9. The strongest correlations were between Factor I (overload) a PHQ 9. Conclusion: The results show that mainly aspects of work overload (time constraints, problems and conflicts, high accountability) can lead in nurses to more frequent survival of individual symptoms of depression (especially the fatigue and reduced interests). Cíl: McCloskeyho a Muellerova škála pracovní spokojenosti sester (McCloskey/Mueller Satisfaction Scale) představuje najčastěji vyuţívaný nástroj měření pracovní spokojenosti sester. Příspěvek má charakter validační studie, jejímţ cílem bylo testovat psychometrické vlastnosti české verze nástroje. V práci jsme porovnávali strukturu české verze s její původní (Mueller, McCloskey, 1990) a později revidovanou verzí (Tourangeau et al., 2006). Metodika: Výzkumný soubor tvořilo 499 všeobecných sester ze šesti nemocnic. Psychometrická analýza nástroje v naší studii byla realizována z aspektu faktorové analýzy a reliability. Ke zjištění dimenzionality nástroje byla pouţita konfirmační a explorační faktorová analýza. V rámci reliability výzkumného nástroje byla zjišťována vnitřní konzistence nástroje prostřednictvím výpočtu Cronbachova alfa koeficientu. Výsledky: Prostřednictvím explorační faktorové analýzy bylo identifikováno sedm faktorů s vlastními hodnotami nad 1. Faktor kontrola a odpovědnost byl sloučen s faktorem uznání a pochvala. Z původní verze nástroje (Mueller, McCloskey, 1990) jsme vyloučili dvanáct poloţek. Na základě výsledků explorační faktorové analýzy české verze navrhujeme její šestifaktorovou strukturu se šesti subškálama – spokojenost se sociálními vztahy a interakcemi; spokojenost s pochvalou a uznáním; spokojenost s plánováním a harmonogramem práce; moţnostmi výzkumu; spokojenost s vnějším ohodnocením a rovnováhou mezi prací a rodinou. Nízkou vnitřní konzistenci jsme zjistili ve dvou subškálách. Závěr: Šestifaktorový model české verze MMSS je konceptuálně konzistentní s původní verzí (Mueller, McCloskey 1990). Naše výsledky faktorové analýzy se více přibliţují k závěrům faktorové analýzy kanadských autorek Tourangeau et al. (2006) neţ výsledkům analýzy původní verze (Mueller, McCloskey, 1990)., Aim: The McCloskey/Mueller Satisfaction Scale represents one of the most often used instrument for measuring satisfaction among nurses. The paper has a character of a validation study, whose aim was to test the psychometric properties of the Czech version of the tool. In the paper we compared the structure of the Czech version with its original (Mueller, McCloskey, 1990) and later revised version (Tourangeau et al., 2006). Methods: The research sample consisted of 499 general nurses from six hospitals. The psychometric analysis of the tool in our study was realized from the acpect of factor analysis and realibility. In order to determine the dimensionality of the tool was used confirmatory and exploratory factor analysis. Within the scope of reliability of research tool was ascertained inner tool consistency using the calculation of Cronbach coefficient alpha. Results: By way of exploratory factor analysis were identified seven factors with its own values above 1. The factor control and responsibility was unified with factor recognition and praise. From the original tool version (Mueller, McCloskey, 1990) we have excluded twelve items. Based on the results of exploratory factor analysis of the Czech version we propose its six-factor structure with its six sub-scales – satisfaction with social relationships a interactions; satisfaction with praise and recognition; satisfaction with planning and work time scheduling; possibilities of research; satisfaction with external assessments and balance of family and work. We have identified low inner consistency in two sub-scales. Conclusion: The six-factor model of the Czech version MMSS is conceptually consistent with the original version (Mueller, McCloskey 1990).
