Cílem práce je ukázat v průběhu 18letého sledování kognitivní, emoční a behaviorální změny u pacientky s roztroušenou sklerózou mozkomíšní (RS). Předkládaná kazuistika dokumentuje maladaptaci na diagnózu RS a dlouhodobé maladaptivní zvládání, prolínání a vliv afektivních složek na inhibici kognitivních schopností a psychického stavu, vliv stresu na vznik „pseudoatak“ RS, psychosociální problematiku a kvalitu života. Diagnóza RS měla negativní vliv na pacientku a rodinu. Nepředvídatelný průběh ovlivňoval potíže v adaptaci a v plánování podpory. Vliv stresu souvisejícího s onemocněním a zároveň vedlejší negativní efekt léčby vedli k mnoha psychosociálním potížím, které snížily kvalitu života pacientky a měly negativní vliv na její spolupráci při léčbě a rehabilitaci. Součástí komprehensivní diagnostiky i léčby RS byla opakovaná neuropsychologická vyšetření a v závěru sledování, ve fázi chronicko‑progresivní RS, i kognitivně‑behaviorální terapie cílená na snížení zátěže způsobené chronicitou nemoci a zlepšení kvality života., The aim of the paper is to show cognitive, emotional and behavioural changes in a multiple sclerosis (MS) patient over eighteen years of observation. Case study documents maladaptation to MS diagnosis and prolonged maladaptive coping, diffusion and influence of affective components on inhibition of cognitive abilities and mental health status, the contribution of stress to development of „pseudo-relapses“ of MS, psychosocial problems and quality of life. The diagnosis of MS was devastating for the patient herself and her family. Its unpredictable course complicated adjustment and created difficulties in planning for support. Effects of disease-related stress as well as a negative side-effect of treatment led to many psychosocial problems that impaired the patient’s quality of life and affected compliance with treatment and rehabilitation. Cíl: Správný pooperační režim je důležitou součástí rekonvalescence po dekompresi karpálního tunelu. Cílem této prospektivní randomizované studie je zjistit, zda pooperační dočasné dlahování má klinicky významný efekt, nebo zda není naopak zbytečné. Výsledky dosud publikovaných prací nedávají jasné, prokazatelné a jednoznačné doporučení. Soubor a metodika: Do souboru bylo zařazeno celkem 72 pacientů s klinicky a elektromyograficky prokázaným syndromem karpálního tunelu, kteří byli randomizováni do dvou skupin. V první skupině byla pacientům po operaci na dva týdny přiložena dorzální dlaha fixující zápěstí v neutrální poloze a v druhé skupině dlaha použita nebyla. Před operací i po operaci byla hodnocena distální motorická latence, síla stisku a míra subjektivních obtíží pomocí specializovaných dotazníků DASH (Disabilities of Arm, Shoulder and Hand Questionnaire) a Bostonského dotazníku (Boston Carpal Tunnel Questionnaire). Výsledky: Po operaci došlo ke zkrácení distální motorické latence a mírnému snížení síly stisku, ale mezi skupinami nebyly nalezeny statisticky významné rozdíly (p > 0,05). Po operaci došlo také k předpokládanému zmírnění subjektivních obtíží, objektivizovanému dotazníkovými koeficienty, ale mezi skupinami nebyl nalezen statisticky významný rozdíl (p > 0,05). Závěr: Podle našich zjištění dočasné dlahování po otevřené dekompresi karpálního tunelu ve srovnání s nedlahovanou skupinou nezlepšuje výsledky chirurgické léčby. Používání dlahy v této indikaci se jeví jako nadbytečné jak z lékařského, tak z ekonomického pohledu. Na druhou stranu dočasné dlahování po operaci výsledky operační léčby nezhoršuje., Aim: Correct post-operative care is an important part of recovery after carpal tunnel decompression. The aim of this prospective randomized study was to determine whether postoperative temporary splinting has a clinically significant effect, or whether it is unnecessary. The results of published data do not give clear, demonstrable and unambiguous recommendations. Material and methods: A total of 72 patients with clinically and electrodiagnostically established diagnosis of carpal tunnel syndrome were randomized into two groups. The first group included patients with a dorsal splint, fixing the wrist in a neutral position for two weeks after surgery, whereas the second group included patients, in whom the splint was not used after surgery. Distal motor latency, grip strength and degree of subjective symptoms were assessed before and after surgery using specific questionnaires, namely the DASH questionnaire (Disabilities of Arm, Shoulder and Hand) and the Boston questionnaire (Boston Carpal Tunnel Questionnaire). Results: After surgery, there was a reduction of distal motor latency and a moderate reduction of grip strength, but there were no statistically significant differences (p > 0.05) between the two groups. After surgery, the anticipated improvement of subjective symptoms was achieved, objectified by questionnaire coefficients. However, there was no statistically significant difference (p > 0.05) between the two groups either. Conclusion: According to our findings, temporary splinting after open carpal tunnel decompression does not improve the results of surgical treatment compared to the non-splinted group. Using the splint in this indication seems to be redundant from the medical and economic point of view. On the other hand, temporary splinting after surgery does not cause worsening of the results of surgical treatment. Background: Few data are available on the specific care giving-related problems of stroke patient’s caregivers and factors that influence the burden of these caregivers. Aim: To study the influences of the active rehabilitation process on anxiety, depression, care burden and perceived social support level of stroke patients caregivers. Design: A prospective clinical trial. Setting: Patients and caregivers entering a rehabilitation program at a university hospital in Turkey. Populations: Ninety patients with a first episode of stroke and 90 caregivers responsible for their care were recruited for our study. Methods: Patients and caregivers were assessed before and after the active rehabilitation process. The functional disability level of the patients was assessed by Functional Independence Measure (FIM). The Beck Anxiety Scale (BAS) and the Beck Depression Scale (BDS) were used for anxiety and depression assessment, the Zarit Care Burden Scale (ZCBS) for care burden assessment and the Multi-Dimensional Scale of Perceived Social Support (MDSPSS) for perceived social support assessment. Results: A statistically significant rise is observed in the special person sub-assessment of MDSPSS in both female and male caregivers. Also, a significant decrease in care burden, anxiety and depression levels of caregivers was noted after the rehabilitation program (p < 0.05). Conclusion: Caregivers accept the rehabilitation period as important social support in addition to the support provided by family and friends. Also, our positive results were associated with an improvement in the patients’ functional level and an increase in the acquisition of knowledge and skill required of caregivers in order to provide care during rehabilitation. Clinical Rehabilitation Impact: The rehabilitation team should be aware of the fact that the perceived care burden may be greater due to the lack of knowledge concerning available resources and due to the inability to cope with stress effectively. and A. Úvod: Naše výsledky byly zhodnoceny za účelem posouzení bezpečnosti a efektivity endoskopické endonazální techniky v léčbě kraniofaryngeomů. <p align="">Materiál a metody: Od roku 2008 bylo endoskopicky endonazálně operováno14 pacientů s typickým supraselárním extraventrikulárním kraniofaryngeomem. Soubor se skládá z 11 mužů a 3 žen (věk 17–60 roků, průměrný věk 38 let). Jedenáct pacientů mělo předoperační deficit zorného pole v rozsahu od malého deficitu ve vnějším kvadrantu až k slepotě jednoho oka a těžkému deficitu druhého oka. Byla použita endoskopická endonazální technika čtyř rukou s peroperační 3,0 T MR. <p align="">Výsledky: V sedmi případech bylo dosaženo radikální resekce. Ve čtyřech případech byla provedena resekce subtotální, ve dvou případech resekce parciální. V jednom případě se jednalo o vypuštění kraniofaryngeomové cysty. V resekci po iMR bylo pokračováno ve třech případech. K úpravě zorného pole došlo v pěti případech. Pooperační diabetes insipidus se nově objevil ve třech případech. Reoperace pro únik mozkomíšního moku byla nezbytná ve třech případech. <p align="">Závěr: Endoskopické techniky v léčbě kraniofaryngeomu jsou bezpečné. Nicméně jsou spojeny s vyšším výskytem pooperačního úniku mozkomíšního moku ve srovnání s transkraniálními přístupy., Introduction: We reviewed our results in order to evaluate safety of endoscopic endonasal technique in the treatment of craniopharyngiomas. <p align="">Material and methods: Since 2008, endoscopic endonasal approach was used in 14 patients with typical suprasellar extraventricular craniopharyngioma. This cohort consisted of 11 males and three females (age 17 to 60 years, average age 38 years). Eleven patients had preoperative visual field deficit ranging from small deficit in the outer quadrant to blindness on one eye and a severe deficit of the second eye. Endoscopic endonasal four hands technique was used with intraoperative 3.0 T MRI. <p align="">Results: In seven cases, the radical resection was achieved. Subtotal resection was performed in four cases. The resection was partial in two cases. In one case, a cyst was drained. Resection after iMRI was performed in three cases. Visual field deficit improved in five cases. Postoperative diabetes insipidus developed in three cases. Reoperation for CSF leakage was necessary in three cases. <p align="">Conclusion: Endoscopic technique in the treatment of craniopharyngioma is safe. Úvod: Je uváděno, že prevalence neurodegenerativních onemocnění v malých evropských komunitách izolovaných od okolí je vyšší ve srovnání s běžnou populací. Tentýž jev jsme pozorovali v malém, relativně izolovaném regionu jihovýchodní Moravy. Cíl: Zjištění prevalence neurodegenerativního parkinsonizmu v izolovaném regionu jihovýchodní Moravy. Metodika: Byla použita metoda třífázového vyšetření. V první fázi byly do ordinací praktických lékařů distribuovány dotazníky, které byly vyplněny všemi pacienty, kteří se z jakéhokoliv důvodu dostavili v průběhu tří měsíců do ordinace praktického lékaře. Ve druhé fázi byly u pozitivních respondentů příznaky parkinsonizmu objektivizovány vyšetřením edukovaným ambulantním neurologem. Ve třetím stupni byli respondenti, u kterých byly při skríningovém vyšetření příznaky parkinsonizmu objektivizovány, přijati k detailnímu vyšetření za hospitalizace v terciárním centru pro diagnostiku a léčbu neurodegenerativních onemocnění. Výsledky: Celková prevalence parkinsonizmu v populaci starší 50 let byla 2,8 % (95% CI: 2,2-3,4); prevalence v populaci 50-64 let byla 1,9 % (95% CI: 1,2-2,5); a v populaci nad 65 let byla 4,06 % (95% CI: 2,9-5,1). Byly získány tři velké rodokmeny s patrnou autozomálně dominantní dědičností parkinsonizmu. Závěr: Zjištěná prevalence byla překvapivě vysoká a podstatně odlišná od hodnot prevalence publikovaných v jiných evropských zemích. Vzhledem k charakteristice regionu je předpokládán vliv genetických faktorů. Současný výzkum je zaměřen na genetické pozadí a DNA analýzu probandů z rodokmenů, ve kterých byla identifikována autozomálně dominantní dědičnost parkinsonizmu. We recently observed this phenomenon in a small specific region of south-eastern Moravia. Objective: To assess the prevalence of neurodegenerative parkinsonism in an isolated region with a rural population in south-eastern Moravia. Methods: A three-stage method of data collection was used. In the first phase, originally designed questionnaires were distributed to general practitioners and completed by all patients who visited them for any reason during a three-month period. In the second phase, positive responders were examined by trained primary care neurologists. Finally, the diagnosis was confirmed or excluded by a movement disorders specialist. Results: The overall prevalence in the population older than 50 years of age was 2.8% (95% CI: 2.2-3.4); the prevalence in the population from 50 to 64 years was 1.9% (95% CI: 1.2-2.5), and it was 4.06% (95% CI: 2.9-5.1) in the population over 65 years of age. Three large families with an autosomal-dominant inheritance patterns of parkinsonism were identified. Conclusions: The prevalence rates were surprisingly high; they substantially differed from the published prevalence rates in other European countries. Due to the characteristics of the region, we expected a particular impact of genetic factors, most probably the autosomal-dominant inheritance of parkinsonism. INTRODUCTION: A previous study has employed shear-wave ultrasound elastographic imaging to assess corneal rigidity in an ex-vivo porcine eye model. This study employs the same modality in vivo in a rabbit eye model in order to assess lens, ciliary body and total ocular rigidity changes following the instillation of atropine and pilocarpine. METHODS: Ten non-pigmented female rabbits were examined. Measurements of the lens, ciliary body and total ocular rigidity as well as lens thickness and anterior chamber depth were taken with the Aixplorer system (SuperSonic Imagine, Aix-en-Provence, France) with the SuperLinear™ SL 15-4 transducer in both eyes at baseline as well as after pilocarpine and atropine instillation. The IOP was also measured with the TonoPen tonometer. RESULTS: Changes in rigidity in the examined areas following atropine instillation were statistically not significant. Ciliary body rigidity was significantly increased whereas lens and total ocular rigidity were significantly reduced following pilocarpine instillation. The decrease in lens rigidity following pilocarpine was significantly associated with the respective increase in ciliary body rigidity. CONCLUSIONS: Shear-wave ultrasound elastography can detect in vivo rigidity changes in the anterior segment of the rabbit eye model and may potentially be applied in human eyes, providing useful clinical information on conditions in which rigidity changes play an important role, such as glaucoma, pseudoexfoliation syndrome or presbyopia. and E. V současné době je věnována familiární hypercholesterolemii (FH) po letech opět zvýšená pozornost. Je tomu tak především proto, že se toto geneticky podmíněné onemocnění stává cílem velmi účinných nových hypolipidemik, PCSK-9 inhibitorů, lomitapidu a mipomersenu. Předložené sdělení se zabývá popisem 2 souborů nemocných (vždy ve své době z největších na světě), před 50 lety a nyní. I když přímé statistické srovnání není možné, lze přeci jen najít v klinickém obrazu FH jisté vývojové změny. Faktem však je, že základní charakteristika FH se zásadně nezměnila. V biochemickém obrazu dominuje těžká izolovaná hypercholesterolemie s hodnotami celkového cholesterolu 9–10 mmol/l, s hodnotami LDL-cholesterolu 7–8 mmol/l a s normálními hodnotami triglyceridů. Je zajímavé, že právě triglyceridy se při srovnání s dobou před 50 lety významně zvyšují a dosahují hraničních hodnot. Hladiny HDL-cholesterolu jsou normální. Manifestace ICHS ve věku od 40 let u mužů a od 50 let u žen není výjimkou (ojediněle se vyskytují i případy infarktu myokardu již ve 3. dekádě života). Klasickou klinickou manifestací FH je xantomatóza. Pravděpodobně v důsledku časné detekce nemocných s FH a účinné agresivní léčby se se šlachovými xantomy, s xanthelasma palpebrarum a arcus lipoides setkáváme méně často než před desítkami let. Obezita, diabetes mellitus (DM) ani hypertenze nepatří mezi typické klinické příznaky FH., Currently, the familial hypercholesterolemia (FH) rises the interest. The reason is that this genetic disorder is targeted by newly emerged and highly effective hypolipidemic agents, PCSK-9 inhibitors, lomitapid and mipomersen. Present paper discusses 2 patient study groups, before 50 years and nowadays. Although direct statistical analysis is impossible some changes in clinical features of FH might be found over the course of the time. In fact, the basic FH characteristic has not changed dramatically. Severe isolated hypercholesterolemia with total cholesterol 9–10 mmol/l, LDL-cholesterol 7–8 mmol/l and normal values of triglycerides dominates in laboratory analysis. Interestingly, the values of triglycerides increase and almost reach the pathological range in comparison to the values from the period 50 years ago. The values of HDL-cholesterol are normal. Manifestation of CHD in male patients over 40 years of age and in female patients over 50 years of age is not exceptional (rarely occur cases of myocardial infarction in third decade of age). Typical clinical manifestation of FH is xanthomatosis. The early detection and aggressive treatment in FH patients cause that xanthoma tendinosum, xanthelesma and arcus lipoides are less frequent as decades ago. V uplynulých dvou desetiletích byl zaznamenán trend provádět u pacientek podstupujících radikální cystektomii kontinentní derivaci moči. Povzbudivé výsledky derivace moči s užitím ortotopické neoveziky (ON) u mužů a výsledky prvních studií prokazující onkologickou a funkční bezpečnost ON u žen byly impulzem pro zavedení této techniky i u žen. V uplynulých 20 letech byla uveřejněna celá řada článků popisujících výsledky derivace moči prostřednictvím ON u žen. Autoři tohoto minipřehledového článku hodnotí výsledky dostupných publikací, které se zabývají problematikou a dopadem této techniky na kvalitu života pacientů. Výsledky studií ukazují nižší incidenci lokální recidivy v močové trubici u žen než u mužů podstupujících stejný výkon. Plně kontinentních bylo 57–100 % pacientek během dne a 27–92 % pacientek i během noci. Výskyt chronické močové retence a potřeba provádět čistou intermitentní katetrizaci byly mnohem častější u žen. Porucha sexuální funkce byla méně častá u pacientek s ON než u pacientek podstupujících jiné formy derivace moči. V případě absence absolutní indikace pro současné provádění uretrektomie by tedy vytvoření ortotopické neoveziky mělo představovat u pacientek s karcinomem močového měchýře první volbu., The trend over the last two decades was toward the use of continent urinary diversion after radical cystectomy in women. The encouraging results of orthotopic neobladder (ONB) in men and the proved oncological and functional safety in early studies in women were behind the application of ONB in women. Many articles have been published in the last two decades concerning the outcome of ONB in women. In this mini‑review, the results of all the available literature concerning this issue and the impact of this surgery on the quality of life are discussed. The results showed lower incidence of local urethral recurrence in women than in men with the same operation. Day- and night-time continence in women was achieved in 57–100% and 27–92%, respectively. Chronic urinary retention and the use of clean intermittent catheterization were much higher in female patients. Sexual function was less affected in women after ONB than those with other forms of urinary diversion. Endometrióza znamená přítomnost tkáně endometria mimo dutinu děložní. Hlubokou infiltrující endometriózu (deep infiltrating endometriosis – DIE) představují rektovaginální léze, infiltrující onemocnění střev, močového měchýře, močovodu a DIE se vzácně objevuje i v dalších lokalizacích. Negynekologické komplikace DIE rozdělujeme do dvou hlavních kategorií: samotné onemocnění zahrnuje pánevní a břišní bolest, střevní striktury, opakující se krvácení z konečníku, dyschezii, chronickou anémii, dysurii, hematurii, stenózu ureterů, ztrátu ledvinných funkcí a komplikace léčby, ty jsou charakterizovány vznikem fistul, dysfunkcí močového měchýře a opakujícími se záněty urinárního traktu. DIE přináší riziko pro vznik malignit zasažených orgánů. DIE může být léčena konzervativně, nebo chirurgicky. Předkládaná práce je přehledem gastrointestinálních a urologických komplikací DIE a předkládá také kazuistiku pacientky centra pro léčbu endometriózy v nemocnici ve Znojmě (ČR)., Endometriosis is the presence of endometrial tissue outside the endometrial cavity. Deep infiltrating endometriosis (DIE) includes rectovaginal lesions as well as infiltrative disease to the bowel, urinary bladder, ureter and other rare locations. Non gynecological complications of DIE can be divided into two major categories: complications of the disease itself which include pelvic and abdominal pain, bowel stricture, cyclic rectal bleeding, dyschezia, chronic anemia, dysuria, hematuria, ureteral stenosis as well as loss of kidney function and complications of the treatment which include fistula formation, bladder void dysfunction and recurrent urinary tract infections. DIE has potential for malignant transformation in the affected organ. DIE may be managed conservatively or surgically. However definitive treatment is surgical. In the current work, literature review of gastrointestinal and urological complications of DIE is presented together with an exemplary case report of a patient managed at the clinical center for treatment of endometriosis in Znojmo Czech republic., and H.
