Dideoxynucleotide DNA sequencing is one of the principal procedures in molecular biology. Loss of an initial part of nucleotides behind the 3' end of the sequencing primer limits the readability of sequenced amplicons. We present a method which extends the readability by using sequencing primers modified by polyadenylated tails attached to their 5' ends. Performing a polymerase chain reaction, we amplified eight amplicons of six human genes (AMELX, APOE, HFE, MBL2, SERPINA1 and TGFB1) ranging from 106 bp to 680 bp. Polyadenylation of the sequencing primers minimized the loss of bases in all amplicons. Complete sequences of shorter products (AMELX 106 bp, SERPINA1 121 bp, HFE 208 bp, APOE 244 bp, MBL2 317 bp) were obtained. In addition, in the case of TGFB1 products (366 bp, 432 bp, and 680 bp, respectively), the lengths of sequencing readings were significantly longer if adenylated primers were used. Úvod: Měření podílu tělesného tuku je důležitým ukazatelem výživového a zdravotního stavu člověka. Na zvýšeném obsahu tělesného tuku se podílejí vedle faktorů genetických a hormonálních především výživa a pohybová aktivita. Existují různé metody na stanovení množství tělesného tuku, jejichž výsledky se mohou navzájem výrazně lišit. Cíl práce: Cílem této studie bylo srovnání výsledků různých metod zaměřených na zjištění množství a distribuce tělesného tuku. Metodika: Zkoumaný soubor tvořilo 86 žen (21–67 let). Byly měřeny parametry: tělesná hmotnost, výška, obvod pasu v polovině vzdálenosti posledního žebra a crista illiaca, vypočítán body mass index (BMI). Tělesné složení bylo zjišťováno pomocí kaliperace kožních řas metodami podle Pařízkové a podle Matiegky, bioelektrické impedance (BIA), duální rentgenové absorpciometrie (DXA). U 27 žen bylo provedeno stanovení abdominálního tuku pomocí CT ve výši L4–5. Výsledky: Jednotlivé námi použité metodiky navzájem velmi dobře korelují. Nejlepší korelace byla zjištěna mezi metodou DXA a BIA (r = 0,929) a metodou DXA a podle Pařízkové (r = 0,925), horší korelace byla mezi metodou DXA a podle Matiegky (r = 0,870), a to jak pro váhové, tak pro procentuální vyjádření množství tuku. V absolutním množství tuku se jednotlivé metody v rámci celého souboru navzájem výrazně liší (p < 0,001). Metody DXA a BIA dávaly srovnatelné výsledky ve skupině obézních žen (p = 0,684). BMI koreloval velmi dobře s metodami DXA, BIA, metodami podle Pařízkové, Matiegky a s CT břicha (r = 0,920–0,812). Obvod pasu dobře koreluje s množstvím intraabdominálního tuku stanoveným pomocí CT (r = 0,737)., Introduction: Body composition is an important indicator of male nutrition and health state and depends on a number of factors. Nutrition and physical activity are primary determinants of body composition aside from heredity and hormonal aspects. We can use different methods for assessment of body composition. If the various methods are used the results can differ widely. Objective: The aim of this study was to compare selected methods for measurement of body fat and its distribution. Methods: Body composition was assessed in a group of 57 overweight adults (21–67 years old). The following parameters were measured: body height, body weight, circumference of waist (between the 12th rib and crista illiaca), weight/height ratio (BMI). These methods were used for determination of body composition: skin fold thickness measurement by Parizkova, skin fold thickness measurement by Matiegka, bioelectrical impedance (BIA), dual-energy X-ray absorptiometry (DXA). Abdominal computer tomography (CT) at L4–L5 was used for total abdominal fat assessment in a group of 27 women. Results: The results of these methods correlate very well. The best correlation was found with DXA and BIA methods (r = 0.929) and DXA and Parizkova method (r = 0.925). Worse correlation was found with DXA and Matiegka method (r = 0.870) for the total amount of body fat and body fat percentage. There were significant differences in body fat amount assessed by DXA and other methods (p < 0.001). The results for DXA and BIA methods were similar in the group of obese women (p = 0.684). Cíl. Cílem práce je zhodnotit výskyt pozdních komplikací a reintervencí po endovaskulární léčbě subrenálního aneuryzmatu břišní aorty stentgraftem. Materiál a metoda. Od dubna 1996 do prosince 2011 bylo na našem pracovišti léčeno 270 pacientů s aneuryzmatem břišní aorty implantaci stentgraftu, z toho bylo 233 mužů a 37 žen ve věku od 49 do 91 let. Věkový průměr byl 68,7 let. Nejméně rok po léčbě byli sledováni 204 pacienti, u nich byl hodnocen výskyt pozdních komplikací (ruptura, migrace stentgraftu, endoleak, ischemické komplikace a zánětlivé komplikace) a reintervencí. Výsledky. V souboru 204 nemocných bylo zjištěno 25 pozdních komplikací, tj. 12, 3% (25/204), a bylo provedeno 21 reintervencí, tj. 10,3 % (21/204). Z 25 komplikací byl zjištěn endoleak I. typu 5krát, endoleak II. typu se zvětšeným vakem aneuryzmatu 3krát, endoleak III. typu 4krát, endotension 3krát, migrace stentgraftu 4krát, ischemické komplikace 5krát a infekce lkrát. Endoleaky I. typu a migrace byly řešeny implantaci prodlužujícího segmentu a migrace u jednoho nemocného chirurgickou konverzí. Endoleaky III. typu byly léčeny implantaci stentgraftu a endoleaky II. typu podvazem nebo embolizací zdroje. Endotension byla řešena perkutánní punkcí vaku aneuryzmatu s aspirací jeho obsahu. U trombózy raménka byla provedena lkrát trombektomie, 2krát byl našit femoro-femorální zkřížený bypass, 1krát byla léčba konzervativní a 1krát byla provedena amputace končetiny. Infekce stentgraftu byla řešena chirurgickou konverzí. Závěr. Endovaskulární léčba aneuryzmatu břišní aorty je v našem souboru spojena s výskytem pozdních komplikací a reintervencí v 12,3 %, resp. v 10,3 %, což se shoduje s literárními údaji. Většinu těchto pozdních komplikací lze řešit endovaskulárně., Aim. To evaluate the incidence of late complications and reinterventions after endovascular treatment of subrenal abdominal aortic aneurysm with stentgraft. Material and method. From April 1996 to December 2011 we treated 270 patients with abdominal aortic aneurysm endovascularly with stentgraft implantation at our department. There were 233 men and 37 women aged from 49 to 91 years. The average age was 68.7 years. 204 patients were followed for at least a year after treatment. In these patients incidence of late complications (rupture, migration, endoleak, ischemic complications and inflammatory complications) and reintervention were evaluated. Results. In a group of 204 patients 25 late complications were found, i.e. 12.3% (25/204) and 21 reinterventions were performed, i.e. 10.3% (21/204). From 25 complications there were endoleak type I 5x, endoleak type II with sac enlargement 3x, endoleak type III 4x, endotension 3x, stentgraft migration 4x, ischemic complications 5x and infection 1x. Type I endoleaks and migrations were treated with cuff implantation and with surgical conversion in 1 patient with migration. Endoleaks III were managed by stentgraft implantation and type II endoleaks with sac enlargement by ligation or embolization of source. Endotension was solved by percutaneous puncture of the aneurysm sac with content aspiration. The stentgraft arm thromboses were managed by thrombectomy 1x, femo-femoral crossover bypass was created 2x, conservative treatment 1x and leg amputation was performed 1x. Stent graft infection was managed by surgical conversion. Conclusion. Endovascular treatment of abdominal aortic aneurysm is associated with the occurrence of late complications and reintervention in 12.3% and 10.3%, which correlates with literature. Onkologická léčba je často spojena s výskytem nežádoucích účinků, z nichž nejzávažnější jsou účinky pozdní. Mohou se vyskytovat po chemoterapii, hormonální léčbě, ale zejména po radioterapii. Na zhodnocení pozdních vedlejších efektů biologické léčby není ještě dostatečný časový odstup. Cíl: předvést praktické využití protokolu, který vydal v roce 2006 CLSI (Clinical and Laboratory Standards Institute) pod označením EP15-A2 Vol.25 No.17„User Verification of Performance for Precision and Trueness; Approved Guideline, Second Edition“ [1], dále jen EP15-A2, za účelem uživatelské verifikace preciznosti analytické metody deklarované výrobci IVD MD pro klinické laboratoře. Výsledky: verifikace preciznosti deklarovaná výrobcem se prováděla na koncentraci katalytické aktivity enzymu γ-glutamyltransferázy v lidském séru (měřená veličina), a měření se provádělo na dvou koncentračních hladinách vždy ve třech opakováních v průběhu pěti dní. Diskuse: upozornit na existenci potenciálního úskalí alternativního způsobu odhadu mezilehlé preciznosti měřicího postupu nazvaného „Varianta 1“ a uveřejněného v tomto časopise (dále jen KBM), č. 1/2011, ve sdělení českého kolektivu autorů „Doporučení k provádění validace a verifikace analytických metod v klinických laboratořích“ [2], dále jen „Doporučení“. The colorectal cancer ranks high among the malignant tumours in incidence and mortality and irinotecan is standardly used in palliative treatment of metastatic disease in every therapeutic line. Unfortunately, the treatment with irinotecan is often associated with severe toxicities, especially neutropenia and diarrhea. The majority of the toxic manifestation is caused by the insufficient deactivation (glucuronidation) of irinotecan active metabolite SN-38 by UGT1A enzyme. The elevated SN-38 plasma concentration is responsible for the hematological and gastrointestinal toxicity that can become life-threatening. The patients carrying the mutation of the gene encoding UGT1A enzyme lack the ability of bilirubin glucuronidation, and suffer from the inherited un-conjugated hyperbilirubinemia (Gilbert syndrome, Crigler-Najjar type 1 and 2 syndrome). The mutations in other enzyme systems also play role in the etiopathogenesis of the irinotecan toxicity: CYP3A (cytochrome P-450), ABC family of transmembrane transporters (adenosine-triphosphate binding cassette). Většina pacientek s časným i pokročilým stadiem maligního ovariálního nádoru udává symptomy onemocnění, přičemž mezi nejčastějšími jsou zastoupeny gastrointestinální obtíže. Fyzikální vyšetření je limitováno, vzhledem k uložení adnex a časté extraovariální manifestaci onemocnění. Předoperační stanovení nádorového markeru CA 125 je indikováno pouze při podezření na ovariální maligní nádor s cílem sledování dynamiky hladin markeru během onkologické léčby. V opačném případě vysoká falešná pozitivita CA 125 vede k nadbytečnému vyšetřování zdravých pacientek a dalším intervencím. Stanovení nádorového markeru HE4 by mohlo být přínosem u selektované skupiny pacientek (premenopauzální ženy s endometroidní benigní vs. malignizovou cystou aj.), jelikož jeho rutinní (neselektovaný) odběr v případě ovariálního tumoru nevedl k lepšímu rozlišení benigních a maligních ovariálních nádorů ve srovnání s markerem CA 125. V současné době zůstává ultrazvukové vyšetření metodou volby v diferenciální diagnostice benigních a maligních ovariálních nádorů a v rukou zkušeného sonografisty umožní i stanovení stadia a operability onemocnění. Magnetická rezonance s kontrastní látkou je využívána pouze komplementárně v případě nutnosti lepšího tkáňového rozlišení. Počítačová tomografie se většinou využívá ke zhodnocení rozsahu pokročilého nádorového onemocnění, zvláště pokud není na pracovišti dostatečná erudice v abdominálním ultrazvukovém vyšetření. Kombinace počítačové tomografie s pozitronovou emisní tomografií (PET/CT) je nejpřesnější metodou ve stanovení vzdálených metastáz, její využití je však limitováno dostupností metody. Využití PET/CT v diferenciální diagnostice benigních a maligních ovariálních nádorů je limitováno vyšším počtem falešně negativních nálezů ve srovnání s ultrazvukovým vyšetřením., The majority of patients who suffer from an early or advanced stage of ovarian cancer complain about symptoms, mainly gastrointestinal ones. The pelvic examination in ovarian cancer detection is limited by the adnexal position in the pelvis and frequent extraovarian spread of disease. The evaluation of tumor marker CA 125 is justified only in suspicion of ovarian cancer, and rather is to be used in the follow up of ovarian cancer patients during oncological treatment. Otherwise the known high false positivity of CA 125 may cause unnecessary further examinations and invasive interventions. The evaluation of new tumor marker HE4 might be worthy in a selected group of patients (e.g. with endometriomas etc.), however the HE4 or combination of HE4/CA 125 did not increase the detection of malignant disease compared with CA 125 alone. Recently, ultrasound is the method of choice in diferential diagnosis between benign and malignant ovarian tumors. The experienced examiner is also able to detect extraovarian tumor spread and to assess tumor operability. MRI is used only to complement ultrasound in cases when high tissue resolution is needed. CT is a useful method for detection of extraovarian spread, especially in cases when an ultrasound examiner experienced in abdominal scanning is not available. Similarly, PET/CT is a highly accurate method for the detection of abdominal and extraabdominal tumor spread, but its use is limited by cost and the low availability of this method. Background: To evaluate the clinical usefulness of serum levels of soluble form of endoglin in stage III colorectal adenocarcinomas (CRC) patients for detection of recurrence. Methods: The case-control study consisted of 80 stage III CRC patients who underwent surgery with curative intent and 70 age-and sex-matched healthy volunteers. Serum levels of soluble form of endoglin (sol-end) were measured in both groups. Also, predictive factors of recurrence were evaluated using multivariate analyses. Results: Serum levels of sol-end in stage III CRC patients were significantly higher than those in controls. There was not a significant association between serum levels of sol-end and clinicopathological features in CRC patients. Multivariate regression analysis showed the LN R (hazard ratio, 2.54; 95% CI , 1.46–4.34; p < 0.001), to be significant independent factors to estimate local recurrence in stage III CRC patients.
